PPG 2차 미분을 이용한 정상인과 당뇨병 환자의 말초혈관 탄성도 비교 (Comparison of peripheral vascular compliance between normal and diabetic group using second derivative of photoplethysmogram)

  • Recently, the prevalence rate of diabetes in Korea has been increasing rapidly due to high growth of economy and changes in dietary lifestyle. Vascular complication is one of diabetic complications which have frequently occurred by obesity, hyperglycemia and impaired glucose metabolism. Photoplethysmogram(PPG) measured from finger and toe is very useful for evaluation of vascular aging and sclerosis level since the PPG signal represents characteristics of peripheral vascular. Several researchers have reported that second derivative of the finger PPG waveform was useful to evaluate vascular compliance and developed various analysis methods for vascular compliance. However, peripheral vascular compliance study for diabetic patient was never evaluated by using second derivative of PPG. Therefore, we aimed to objectively compare and to assess normal and diabetic group vascular compliance using the second derivative of PPG waveform in this study. The evaluated factors of second derivative of PPG are 'a', 'b', 'c', 'd', 'e' and b/a represents vascular compliance. Study found out that when vascular compliance is decreased, b/a is increased, the absolute value of b/a is decreased. The average vascular compliance of 50 diabetic patients with neuropathy is statistically lower than the normal group(p<0.05). We suggest an objective evaluation of peripheral vascular compliance for diabetic patients and prevention of vascular complication.

  • Up to the present, finger replantation patients have complained about various symptoms because of blood circulatory failure. Objective evaluation methods for the level of the blood circulatory failures in fingers were needed other than patient's subjective symptoms and complaints. Photoplethysmogram(PPG) measured from fingers is very useful for evaluation of vascular aging and sclerosis level since the PPG signal represents characteristics of peripheral vascular. Several researchers have reported that second derivative of the finger PPG waveform was useful to evaluate vascular compliance and developed various analysis methods for vascular compliance. However, vascular compliance study for finger replantation patient was never evaluated by using second derivative of PPG. 가속도 맥파를 이용한 혈관탄성 추정에 관한 연구 (A Study on Estimate Vascular Compliance using Acceleration Photoplethymogram)

  • In this study, we try to estimate vascular compliance for management vascular disease. Because It is known the vascular compliance represents the state of the blood vessels. In general, the vascular compliance is estimated using an acceleration photoplethymogram from measured by photoplethymogram-based. The acceleration photoplethymogram is come from second derivative of photoplethymogram. By using the acceleration photoplethymogram, we can estimating vascular compliance, vascular age, vascular state. In this study, for measuring acceleration photoplethymogram we have developed a measurement device having analog filters. It has the advantage of miniaturization, low-power because it is simpler than digital filters. Using the developed device, we have estimated vascular compliance and tested the reliability of our device compare with conventional device having digital filters. As a result.

  • 현재, 우리나라 당뇨병의 유병률은 빠른 속도로 증가하고 있으며 당뇨병의 유병기간이 길수록 각종 합병증이 발생하여 더욱 심각한 증상을 나타내는데, 그 중 비만 및 고혈당, 당대사장애로 인한 당뇨병성 혈관합병증과 말초 혈관 경화증이 많이 발병하고 있는 실정이다. 따라서 본 연구에서는 광혈류량 측정법(Photoplethysmography)으로 손가락 및 발가락에서 얻어진 맥파의 2차 미분분석을 통해 연령이 비슷한 정상인 50명과 당뇨병으로 확진된 50명의 말초 혈관 탄성도를 객관적으로 비교하고자 하였다. PPG 파형의 2차 미분 분석에 사용되는 평가 인자는 a, b, c, d, e 이고, b/a는 혈관의 탄성도를 의미하며 탄성도가 떨어질수록 b/a의 절대 값은 감소하게 된다. 정상인 50명의 PPG 2차 미분 b/a값은 $-1.09{\pm}0.14$, 당뇨병 환자 50명의 PPG 2차 미분 b/a값은 $-0.81{\pm}0.09$로 정상인에 비해 당뇨병 환장의 말초혈관 탄성도가 감소하였으며, Independent t-test 검정 결과 통계적으로 유의한 차이를 보였다(p<0.05). 본 연구에서는 PPG 파형의 2차 미분 분석을 통하여 정상인과 혈관합병증 발병율이 높은 당뇨병 환자의 말초혈관 탄성도를 비교하였으며, 향후 비침습적이고 간단한 방법으로 당뇨병 환자의 말초혈관 탄성도와 혈관 경화정도를 객관적으로 평가하고 진단함으로써 당뇨병 환자들의 심혈관계질환 사전예방과 치료효과 판정에 도움을 줄 것으로 기대한다.

  • Purpose: This study was conducted to examine the effects of an individualized cardiac rehabilitation education program on knowledge about coronary artery disease (CAD), compliance of sick role and vascular health status in patients with percutaneous coronary intervention (PCI). Methods: Quasi-experimental design-based nonequivalent control group pretest-posttest design was employed. Using convenience sampling, data were collected from 60 patients who underwent PCI at S Hospital in S City from September 2014 to February 2015. For examining the effects of an individualized cardiac rehabilitation education program, knowledge about CAD, compliance of sick role and vascular health status were measured. Results: The experimental group showed statistically significant differences in knowledge about CAD (t=24.21, p<.001), compliance with sick role (t=20.81, p<.001) and vascular health status (t=15.07, p<.001) compared to the control group. Conclusion: The individualized cardiac rehabilitation education program is effective in improving knowledge about CAD, compliance of sick role and vascular health status in patients who underwent PCI. Based on the findings of this study, nursing intervention programs focusing on individualized approach will be useful for patients undergoing PCI.

The Wall Shear Rate Distribution Near an End-to-End Anastomosis : Effects of Graft Compliance and Size

  • The patency rates of small diameter vascular grafts are disappointing because of the formation of thrombus and intimal hyperplasia. Among the various factors influencing the success of graft surgery, the compliance and the size of a graft are believed to be the most important physical properties of a vascular graft. Mismatch of compliance and size between an artery and a graft alters anastomotic flow characteristics, which may affect the formation of intimal hyperplasia. Among the hemodynamic factors influencing the development of intimal hyperplasia, the wall shear stress is suspected as the most important one. The wall shear stress distributions are experimentally measured near the end-to-end anastomosis models in order to clarify the effects of compliance and diameter mismatch on the hemodynamics near the anastomosis. The effects of radial wall motion, diameter mismatch and impedance phase angle on the wall shear rate distributions near the anastomosis are considered. Compliance mismatch generates both different radial wall motion and instantaneous diameter mismatch between the arterial portion and the graft portion during a flow cycle. Mismatch in diameter seems to be affecting the wall shear rate distribution more significantly compared to radial wall motion. The impedance phase angle also affects the wall shear rate distribution.

Effect of Dilation on the Mechanical Characterization of Vascular Prostheses

  • The purpose of this study has been to investigate the effect of dilation on the some mechanical properties of several types of warp-knitted vascular grafts. The structures of warp knit vascular grafts used in the experiments were reverse locknit, locknit, and Tricot. Various mechanical properties of these grafts were determined using devices developed for the purpose. Clinical data obtained were compared with experimental results of warp knit vascular grafts. The most important mechanical properties are found to be creep extension, bursting strengths, and compliance. Preliminary results indicate that vascular grafts are non-compliant and exhibit creep which is predictive of the long term dilation that has been noted in the clinical results. It is found that there is a positive correlation between experimental data and clinical results for at least the grafts tested.

신체계측치 및 혈관탄성도에 따른 최대산소섭취량에 관한 연구 (TA Study on Maximum Oxygen Uptake according to Body Measurement and Vascular Compliance)

  • 본 연구는 40~50대 중년 남녀를 대상으로 신체계측을 통해 얻어진 신체계측치 및 혈관탄성도와 최대산소섭 취량의 관련성을 규명하기 위하여 일개 군 보건소 운동처방실을 방문한 남자 43명, 여자 53명을 대상으로 신체계측치 및 혈관탄성도, 최대산소섭취량 등을 측정하였다. 결과를 보면 1.남자의 신체계측치와 혈관탄성도의 상관관계에서는 비만도가, 여자에서는 허리-엉덩이비율이 유의한 상관관계를 보였다. 2.남자의 신체계측치와 최대산소섭취량의 상관관계에서는 비만도, 체질량지수, 허리-엉덩이비율, 체지방율이 유의한 상관관계를 보였으며, 여자에서는 허리-엉덩이 비율, 체지방율이 유의한 상관 관계를 보였다. 3.남자의 혈관탄성도 및 심혈관계지표와 최대산소섭취량의 상관관계에서는 최대심박수가 유의한 상관관계를 보였고, 여자에서는 혈관탄성도, 최대심박수가 유의한 상관관계를 보였다. 4.최대 산소섭취량을 종속변수로 한 다중회귀분석에서는 남녀 모두 체지방율, 혈관탄성도, 최대심박수가 유의한 변수로 나타났다. 즉 체지방율이 낮을수록, 혈관탄성도가 높을수록, 최대심박수가 높을수록 최대산소섭취량이 높았다. 따라서 40~50대 중년의 최대산소섭취량에 영향을 주는 요인에는 신체계측에 의한 비만관련지수, 혈관탄성도, 심혈관계지표등 여러 요인들이 복합적으로 관련되는 것으로 판단되어진다.

유산소성운동과 반신욕처치가 비만 중년남성의 혈액성분, 호흡순환기능 및 혈관탄성에 미치는 영향 (Influences of Aerobic Exercise Training and Half Bath on Blood Component, Cardiorespiratory Function, and Vascular Compliance of Middle-aged Obese Men)

  • 본 연구의 목적은 유산소성운동과 반신욕 처치의 차이가 비만 중년남성의 혈액성분, 호흡순환기능 및 혈관탄성에 어떠한 영향을 미치는가를 규명하기 위한 목적으로 수행되었다. 본 연구의 대상자는 비만 중년남성 30명을 선정하여 유산소성운동집단(n=10), 반신욕 처치집단(n=10), 통제집단(n=10)으로 구분하여 3집단으로 구성하였다. 이들은 12주간 유산소성운동프로그램 및 반신욕 처치 전 후 각 혈액성분, 호흡순환기능 및 혈관탄성의 변화를 살펴보기 위하여 반복이 있는 이원변량분석(repeated 2-way ANOVA)을 실시하였으며, Scheffe 방법을 이용하여 사후분석을 실시하였으며 다음과 같은 결과를 얻었다. 첫째, 12주간 처치 후 유산소성운동 참여집단의 모든 혈액성분은 반신욕집단과 통제집단에 비해 유의하게 감소 및 증가하였으며, 반신욕집단은 Triglyceride, LDL-C, HDL-C에서 통제집단에 비해 유의한 차이를 나타내었다. 특히, HDL-C의 수준은 유산소성 운동집단과 반신욕 집단간에 유의한 차이를 보이지 않았다. 둘째, 12주간 처치 후 최대산소섭취량, 최대심박수, 최대환기량은 유산소성 운동집단이 반신욕과 통제집단에 비해 유의하게 증가하였다. 그러나 반신욕집단과 통제집단간에는 유의한 차이가 나타나지 않았다. 셋째, 혈관탄성은 유산소 운동집단이 반신욕집단과 통제집단에 비해 왼팔,오른팔 및 왼다리, 오른다리에서 유의하게 향상된 것으로 나타났으며, 반신욕집단 역시 통제집단보다 혈관탄성이 향상된 것으로 나타났다. 이상의 결과를 종합하여 볼 때 유산소성 운동과 반신욕 참여는 비만 중년 남성의 혈액성분 개선과 혈류순환 및 혈관기능의 개선에 효과적인 것으로 나타났다.

걷기 운동이 성인 제 2형 당뇨병 환자들의 당뇨지표 및 혈관탄성도에 미치는 영향 (Effects of Walking Exercise on Diabetic Parameters and Vascular Compliance in Type II Diabetes Mellitus Patients)

  • Objectives The purpose of this study was to investigate the effects of walking exercise on diabetes mellitus indicator and vascular compliance of type 2 diabetes mellitus adults. Methods The subjects were 2 groups; exercise group(n=11, $age=57.36{\pm}8.74$), non-exercise group(n=10, $age=59.20{\pm}5.81$). Walking exercise five or more times a week, and more than 10,000 steps per day with writing the walking diary ordered to the exercise group. For date analysis, mean and standard deviation scores were calculated, and independent t-test was performed. Result After 45 days of walking exercise, weight (p <.01), abdominal obesity rate (p <.05), body fat mass (p <.05) showed significant difference between the groups. but diabetes indicators and vascular compliance tend to decreased in the exercise group, there was no significant difference between the groups. Conclusions Walking exercise is effective in improving body composition.
