The heart rate variability(HRV) of the headache patients caused by Chiljungsang (칠정상(七情傷)으로 인한 두통 환자의 심박변이도(Heart Rate Variability)에 관한 고찰)

  • Objective: To treat psychogenic headache patients, doctors have to amplify on the headache caused by emotional stress to patients, and assist the patients to cope with difficulties. So, we investigated HRV of the headache patients caused by Chiljungsang and would like to apply to the clinical treatment. Method: Our study measured time and frequency domain HRV indicies(5-min resting study) of 123 headache patients caused by emotional stress. Standardized tests of HRV allow a quantitative estimation of autonomic nervous system function. Results & Conclusions: 1. The study classed as aspects of the head pain showed the differences in RMS-SD(square root of mean squared difference of successive NN intervals) band, HF(high frequency) band significantly. 2. The male headache patients showed higher all the indicies except heart rate compared to the female patients, significantly in SDNN(standard deviation of NN interval), TP(total Power), HF band. 3. As the patients grow older, SDNN, RMS-SD band was lower and LF(low frequency) band, LF/HF ratio higher significantly. The beginning age lower, SDNN, RMS-SD band was higher significantly. The duration of the disease longer, LF band, LF/HF ratio was higher significantly.

Heart Rate Variability in Cold Pattern: 3-year Follow-up Study (추적관찰을 통해 살펴본 한증 HRV지표)

  • This study aimed to investigate heart rate variability (HRV) characteristics of cold pattern with repeated measurement data. Participants were taken from a Daejeon University cohort study from 2015 to 2018. Forty-seven of the participants studied displayed cold pattern while 23 showed signs of non-cold pattern. HRV was measured in supine position for 5 minutes at each year, and an 8-item cold pattern questionnaire was used for the diagnosis of cold pattern. SDNN (standard deviation of the NN intervals) and RMSSD (the square root of the mean squared differences of successive NN intervals) were used as time domain analysis, and TP (total power), VLF (power in very low frequency range), LF (power in low frequency range), HF (power in high frequency range), LF norm (LF power in normalized units), HF norm (HF power in normalized units) and LF/HF were used as frequency domain analysis. In the Mann-Whitney U test, LF norm, HF norm, and LF/HF showed differences between the cold pattern group and non-cold pattern group at every measurement, and in the independent t-test, the differences were also observed at three points except for the baseline (2015). In the repeated measures ANOVA, the interaction effects were not observed in all HRV parameters, but the time period effects were observed in SDNN, RMSSD, TP, VLF, LF and HF. There were significant differences between those two groups in LF norm, HF norm and LF/HF. This study suggests that LF norm, HF norm and LF/HF might be a useful indicator of cold pattern properties.

A Study on Oriental Medicine Diagnostic Application through Analysis of Heart Rate Variability in Polycystic Ovary Syndrome Females (PCOS 여성의 HRV 특성 분석을 통한 한의학적 진단 활용성에 관한 연구)

  • Purpose: The aim of the study was to compare the characteristics of the autonomic innervation of the heart in polycystic ovary syndrome(PCOS) patients with regulary cycling controls. Methods: We studied 21 patients visiting $\bigcirc\bigcirc$hospital from 25th June 2009 to 25th June 2010. The subjects were categorized in two groups, 11 PCOS patients and 10 healthy regularly cycling controls. We studied the difference of Heart rate variability (HRV) between two groups by Independent samples T-test using SPSS for windows(version 17.0). Results: The Standard deviation of all normal R-R intervals (SDNN), The spuare root of the sum of the spuare of difference between adjacent normal R-R intervals (RMS-SD) of PCOS group was non-significantly lower than non-PCOS group. High frequency power (HF) and Normalized high frequency power (HF norm) of PCOS group was significantly higher than non-PCOS group. Normalized low frequency power (LF norm) of PCOS was signficantly lower than non-PCOS group. The results means increased sympathetic and decreased vagal modulation. Total power (TP), Very low frequency power (VLF) of PCOS group was non-significantly lower than non-PCOS group. Low frequency power (LF), LF/HF ratio of PCOS group was non-significantly higher than non-PCOS group. Conclusion: The results suggest that PCOS can be related to decreased activity of parasympathetic nervous system.

An Economic Design of the Integrated Process Control Procedure with Repeated Adjustments and EWMA Monitoring

  • Statistical process control (SPC) and engineering process control (EPC) are based on different strategies for process quality improvement. SPC reduces process variability by detecting and eliminating special causes of process variation, while EPC reduces process variability by adjusting compensatory variables to keep the quality variable close to target. Recently there has been need for an integrated process control (IPC) procedure which combines the two strategies. This article considers a scheme that simultaneously applies SPC and EPC techniques to reduce the variation of a process. The process disturbance model under consideration is an IMA(1,1) model with a location shift. The EPC part of the scheme adjusts the process, while the SPC part of the scheme detects the occurrence of a special cause. For adjusting the process repeated adjustment is applied by compensating the predicted deviation from target. For detecting special causes the two kinds of exponentially weighted moving average (EWMA) control chart are applied to the observed deviations: One for detecting location shift and the other for detecting increment of variability. It was assumed that the adjustment of the process under the presence of a special cause may change any of the process parameters as well as the system gain. The effectiveness of the IPC scheme is evaluated in the context of the average cost per unit time (ACU) during the operation of the scheme. One major objective of this article is to investigate the effects of the process parameters to the ACU. Another major objective is to give a practical guide for the efficient selection of the parameters of the two EWMA control charts.

Characteristics of Heart Rate Variability Among Adults with Metabolic Syndrome (대사증후군 집단의 심박동수 변이의 특징)

  • Objectives : The authors tried to investigate the characteristics of Heart Rate Variability(HRV) among the subjects with metabolic syndrome for testing the possibility of clinical application of HRV in evaluating metabolic syndrome. Methods : We examined the difference of the means of HRV between 122 subjects of metabolic syndrome and 1057 healthy controls using T-test, and the change of means in HRV according to increasing risk factors of metabolic syndrome. Using multiple regression analysis, we examined the association of HRV with the risk factors of metabolic syndrome. Results : The HRV indices including SDNN(Standard Deviation of all normal NN intervals), RMSSD(the square Root of the Mean Squared Differences of successive all normal NN interval), TP(Total Power), LF(Low Freuency), and HF(High Frequency) were significantly lower in the metabolic syndrome group than in the normal control group, and the means of all HRV indices except LF/HF were decreased consistently according to increasing risk factors of metabolic syndrome. The fasting glucose, triglyceride, and waist circumference were dependent factors that contributed significantly to the change of HRV. Conclusion : These findings suggest metabolic syndrome adversely affects cardiac autonomic system and HRV could be a useful method for evaluating metabolic syndrome.

The Effects of Moxibustion on Heart Rate Variability in Cancer Patients (쑥뜸치료가 암환자의 심박변이도에 미치는 영향)

  • Objective : The study aims to investigate the effect of moxibustion treatments on autonomic nervous system function of cancer patients through the evaluation of heart rate variability (HRV) biofeedback testing. Materials and Methods : Six cancer patients from inpatient care unit of Dunsan Oriental Hospital, Daejeon University were given three moxibustion treatment sessions every other day over one week period on five Oriental Medicine meridian points CV4, CV6, CV12, KD1, and PC8. HRV biofeedback was conducted before and after each treatment sessions. Three areas of analyses were done from the test conducted; Time Domain Analysis, Frequency Domain Analysis and Autonomic Nervous System (ANS) balance analysis. Results : Time Domain Analysis has shown increased Standard Deviation of all Normal R-R Intervals (SDNN), and decreased Mean Heart Rate and Physical Stress Index (PSI) levels, with statistical significance (P<0.05). In Frequency Domain Analysis, series of moxa treatments have increased Total Power (TP), Very Low Frequency Oscillation Power (VLF), High Frequency Oscillation Power (HF), normalized HF values while decreasing Low Frequency Oscillation Power (LF), normalized LF and LF/HF ratio with statistical significance (P<0.05). The values of ANS activity, ANS balance, Stress resistance, Stress index, have also shown significant changes. For cardiac stability stroke volume power (SP) and Blood Vessel Tension (BVT) were followed, which were both increased after treatment. All changes were statistically significant (P<0.05). Conclusion : The results have shown a positive correlation between the moxibustion treatments and autonomic nervous system responses on cancer patients through the HRV biofeedback testing. This study suggests possible application of moxibustion treatments for managing ANS functions of cancer patients, although additional studies with larger population are necessary to confirm the data.

  • Objectives To investigate the relationship between Heart Rate Variability and menopause among obese middle-aged women in Korea. Subjects and Methods Forty middle-aged women (age 49.05 $\pm$ 2.64 years) with BMI > $23kg/m^2$ were recruited by local advertisement. Blood profiles of estrogen, FSH, LH, glucose, total cholesterol, HDL-cholesterol, and triglyceride were measured. Autonomic Nervous System (ANS) was estimated by short-term spectral analysis of heart rate variability (HRV). Visceral adipose tissue (VAT) and subcutaneous adipose tissue (SAT) were accessed using Computerized Tomography (CT), Bioimpedance Analysis (BIA), and anthropometry. To scrutinize the influence of ANS on obesity-related factors, we divided the subjects into pre- and post-menopausal women. Results The total cholesterol, FSH and LH were significantly higher in post-menopausal obese women group. The estradiol and standard deviation of all normal R-R intervals (SDNN), high-frequency components (HF) of the HRV were significantly lower in postmenopausal obese women group (P<0.05). Adipose tissue distribution and blood profiles were not significantly different between two groups. Conclusion Menopause induced a decrease in SDNN and HF values and increase in total cholesterol level in obese women although the VAT itself was not related with ANS in obese women.

Short Time Effect of Caffeine on Heart Rate Variability and the Effect of Acupuncture at Neiguan (PC6): A Randomized Double Blind Pilot Study (카페인이 HRV에 미치는 영향과 내관 자침의 효과에 대한 예비연구 : 무작위 이중맹검시험)

  • Objectives : This study was to investigate the short time effect of caffeine on heart rate variability(HRV) and the effect of Neiguan(PC6) acupuncture stimulation on HRV. Methods : 27 healthy adult volunteers were randomly allocated to two groups: Neiguan group (N=13) or placebo group (N=14). The study was carried out under a randomized double-blinded placebo-controlled trial method. Each group orally received the same tablets with 200 mg caffeine. After 1 hour, acupuncture was applied to the Neiguan(PC6) points for the Neiguan group, and for the placebo group was applied to a non-acupuncture point. Both groups were estimated with HRV 3 times, before and after caffeine ingestion, 20 minutes after acupuncture stimulation. Results : After taking caffeine, pulse rate, mean-HRV, and pNN50(the proportion derived by dividing NN50 by the total number of NN intervals) decreased, SDNN(standard deviation of all normal-to-normal (NN) intervals), RMSSD (the root square of successive differences), TP log, HF(high frequency), and HRV index was increased. There were significant changes to the autonomic nervous system after taking caffeine. There were no significant differences between the two groups after acupuncture at Neiguan. Conclusion : Caffeine could induce general activation of the autonomic nervous system. Neiguan acupuncture stimulation may not have significant influence on the autonomic nervous system.

  • Objective : Heart rate variability (HRV) is known to reflect autonomic nervous system activity. Individuals with posttraumatic stress disorder (PTSD) are reported to have lower HRVs. We attempted to find HRV indices with head up tilt position that reflect the symptoms well in order to evaluate PTSD symptoms. Methods : Sixty-seven patients with PTSD and 72 patients without PTSD were assessed using the PTSD Checklist for DSM-5 (PCL-5), the Beck Depression Inventory, the Beck Anxiety Inventory and the Pittsburgh Sleep Quality Index. HRV was measured in the head-up tilt position. We collected data regarding heart rate (HR), standard deviation of the NN intervals (SDNN), the square root of the mean squared differences of successive NN intervals (RMSSD), log low-frequency (LNLF) and log high-frequency (LNHF). Results : The value of LNHF was different according to presence or absence of PTSD after head-up tilt position. In the findings of the association between PTSD symptoms and HRV indices as based on head-up tilt, LNHF had a significant correlation with the total score of PCL-5. Conclusion : The reduction of the high-frequency component of HRVs in the PTSD group might reflect more PTSD symptoms.

Environment variability predictive analysis of Myeongri Study and Yookhyo Study in Oriental studies (동양학에서 명리학과 육효학의 환경변화도 예측 분석연구)

  • Most of the previous environment change predictive studies had applied Oriental studies. Only Myeongri study were especially applied in studying environment change prediction. When these two studies are applied separately in analysis, accurate predictive analysis is insufficient. Therefore the purpose of this study is to study the accurateness of environment change prediction by applying Myeongri study and Yookhyo study. Research methods, August to December 2012 were surveyed workers. In the analyzed result, environment change prediction is more accurate when the two studies are done together compared to separating them. The mean and standard deviation of relation according to the general characteristics of respondents were high. Good results followed as the mean was higher by similar difference in each part of environment variability. Therefore the efficiency of prediction for environment variability seems to have increased.
