Metabolic Profiling of Eccentric Exercise-Induced Muscle Damage in Human Urine

  • Skeletal muscle can be ultrastructurally damaged by eccentric exercise, and the damage causes metabolic disruption in muscle. This study aimed to determine changes in the metabolomic patterns in urine and metabolomic markers in muscle damage after eccentric exercise. Five men and 6 women aged 19~23 years performed 30 min of the bench step exercise at 70 steps per min at a determined step height of 110% of the lower leg length, and stepping frequency at 15 cycles per min. $^1H$ NMR spectral analysis was performed in urine collected from all participants before and after eccentric exercise-induced muscle damage conventionally determined using a visual analogue scale (VAS) and maximal voluntary contraction (MVC). Urinary metabolic profiles were built by multivariate analysis of principal component analysis (PCA) and orthogonal partial least square-discriminant analysis (OPLS-DA) using SIMCA-P. From the OPLS-DA, men and women were separated 2 hr after the eccentric exercise and the separated patterns were maintained or clarified until 96 hr after the eccentric exercise. Subsequently, urinary metabolic profiles showed distinct trajectory patterns between men and women. Finally, we found increased urinary metabolites (men: alanine, asparagine, citrate, creatine phosphate, ethanol, formate, glucose, glycine, histidine, and lactate; women: adenine) after the eccentric exercise. These results could contribute to understanding metabolic responses following eccentric exercise-induced muscle damage in humans.

Validity of Self-reported Smoking Using Urinary Cotinine among Vocational High School Students

  • Objectives : This study was conducted to validate self-reported smoking among high school students using urinary cotinine. Methods : A self report of smoking behavior was collected together with urine sample for cotinine analysis from 130 male and female students in two vocational high school students in November, 2007. Validity and agreement between self-reported smoking and urinary cotinine was analyzed with STATA 9.0 for different definitions of current smokers, and frequent and daily smokers. Urinary cotinine concentration was measured by the DRI Cotinine Assay for urine (Microgenics Corp., Fremont, CA) on Toshiba 200FR. The cut-off point of urinary cotinine was 50 ng/dl. Results : The concentrations of urinary cotinine were significantly different according to the frequency and amount of smoking. Sensitivity and specificity was 90.9% and 91.8% respectively, and the Cohen s kappa value was 0.787 among the current smokers who smoked at least one day during one month preceding the survey. The comparable high sensitivity, specificity, and kappa value were shown also among the other definitions of current smokers, that is, subjective smokers, and weekly smokers. Conclusions : The results showed the high validity of self-reported smoking among high school students. However, due to the small sample size and limitation of the participants, it is cautious to generalize the results to overall high school students.

  • Vesicoureteral reflux(VUR) is a state that urine regurge from bladder to ureter and kidney because of congenital, structural, functional abnormality of ureterovesical junction and lower urinary tract than bladder. It may be the primary cause of recurrent urinary tract infection(UTI) in chindhood, If urine regurge with UTI, it can cause renal damage, leading to scar formation, hypertension, chronic renal failure, But upper complications can be prevented by early diagnosis and proper treatment of VUR and UTI, so clinician must focus on them in treatment of VUR. We had experienced a case of recurrent UTI with VUR regardless of consistent antibiotics therapy in 7 years old boy, Chief complain was urinary frequency, The symptom of urinary frequency was successfully treated by herb medicine(Gamijihwag-tang), So, we report this case with a brief review of related literatures.

사상체질에 따른 여대생의 건강 특성 분석과 BMI에 영향을 미치는 요인 (Study of health characteristics of female college students according to sasang constitution and factors affecting BMI)

  • Objectives The purpose of this study was to investigate the ordinary symptoms, special symptoms and Body Mass Index (BMI) according to Sasang constitution in female college students. Also, we aimed to analyze the factors affecting BMI. Methods Forty-four female college students participated in this study. Sasang Constitution was determined by Questionnaire for Sasang Constitution Classification (QSCC) II. BMI was measured and ordinary symptoms were acqired through the questionnaire. Special symptoms was determined by Fatigue Severity Index (FSS), Premenstrual Symptom Screening Tool (PSST), ROME III, Ocular Surface Disease Index (OSDI), respectively. For statistical analysis, t-test, analysis of variance and correlation test has been used. Results There existed significant differences in ordinary symptoms and special symptoms between sasang constitutions. Taeumin had higher urine frequency than soyangin, soeumin had higher gap of feces than taeumin. Taeumin had higher BMI and ROME III score than soyangin and soeumin. They showed significant differences in ordinary symptoms and special symptoms according to BMI. Overweight and obese group is higher in water intake than low and normal group. Low weight and normal group is higher in gap of feces than overweight and obese group. High score group in PSST and ROME III showed high BMI than low score group. We analyzed the factors that affect BMI. BMI are highly correlated with systolic and diastolic blood pressure. Also, FSS, PSST, ROME III and OSDI showed high correlation with each other. Conclusion Urine frequency and gap of feces are different among sasang constitutional types. The obese group and normal group showed significant differences in water intake, gap of feces, PSST and ROME III score. It is found that factors that affecting BMI are systolic and diastolic blood pressure. These results may lead to identifying the causes and factors of obesity in female college students related to Sasang constitution.

Effect of Synbiotic on the Treatment of Jaundice in Full Term Neonates: A Randomized Clinical Trial

  • Purpose: Jaundice accounts for most hospital admissions in the neonatal period. Nowadays, in addition to phototherapy, other auxiliary methods are used to reduce jaundice and the length of hospitalization. This study aimed to investigate the effect of probiotics on the treatment of hyper-bilirubinemia in full-term neonates. Methods: In this randomized clinical trial, 83 full-term neonates, who were admitted to the hospital to receive phototherapy in the first 6 months of 2015, were randomly divided into two groups: synbiotic (SG, n=40) and control (CG, n=43). Both groups received phototherapy but the SG also received 5 drops/day of synbiotics. Serum bilirubin, urine, stool, feeding frequency, and weight were measured daily until hospital discharge. A p-value<0.05 was considered statistically significant. Results: The mean total serum bilirubin in the SG was lower than that in the CG ($9.38{\pm}2.37$ and $11.17{\pm}2.60mg/dL$, respectively). The urine and stool frequency in the SG was significantly higher than that in the CG (p<0.05). The duration of hospitalization in the SG was shorter than that in the CG. Conclusion: Use of synbiotics as an adjuvant therapy had a significant treatment effect on jaundice in full-term neonates. Further studies including larger samples with long follow-up periods are essential to confirm the benefits of routine use of synbiotics in neonatal patients with jaundice.

섭취량과 배설량에 근거한 대구지역 성인여성의 아연 영양상태 (Zine Status of Adult Female in the Taegu Region as Assessed by Dietary Intake and Urinary Excretion)

  • This study was intended to examine the zinc status of free-living adult women living in the Taegu region. Zine intake of 102 female subjects was measured by food frequency method for 3 consecutive days. Urinary zine was assessed from urine samples collected fir twenty four hours. Average dietary zinc intake of adult female subjects using food record was 5.9$\pm$1.8mg which was 49$\%$of Korean RDA. When zine intake by frequency method was higher than by food record. Average urinary zine intake by frequency method was higher than that by food record. Thus, it appeared that zinc intake by frequency method was higher than that by food record. Average urinary zinc excretion of 102 adult female subjects was 0.28$\pm$0.16mg, which belonged to marginal zinc deficiency range. Fifty nine of 102 subjects showed marginal zinc deficiency as assessed by urinary zinc excretion. If we compare the zinc status of adult female subjects by age group, zinc intake in the 50s was significantly higher than any other age groups. However, there were no significant differences in zinc status according to BMI groups and BMI groups. Significant correlations were found between zinc intake and energy, protein, carbohydrate and fat intake. Based on dietary zinc intake and urinary zinc, we concluded that zinc status of adult female living in the Taegu region is marginally deficient.

대학생의 저염식이 지식, 태도, 행위에 대한 지역사회 중재효과 (Effect of Community Intervention on Knowledge, Attitude and Behavior of Low-salt Diet among College Students)

  • Objectives: The purpose of the study was to evaluate the effect of community intervention on reducing salt intake among college students on their knowledge, attitude and practice of low-salt diet. Methods: A community intervention planned to increase college students' knowledge, attitude and practice of low-salt diet. The intervention comprised of 1 lecture on salt and health, 4 campaigns on low-salt diet, and 2 taste testing was carried out from March 24 to May 19, 2015. All the intervention was open to entire university while data was collected from a panel of convenient sample. We used the baseline data of 251 students, 226 students completed post-test. Post-test data on knowledge, attitude, and practice, blood pressure measurement, and urine salt test were collected on May 25 and 26 by trained nursing students and graduates. Baseline data were collected on March 17 and 18. Data were analyzed using IBM-SPSS 21.0 for t-test and ANCOVA. Results: Regarding a general characteristic and health behavior, there was statistically significant difference in living arrangement(p<0.001), amount of monthly allowance(p=0.005) and frequency of eating-out(p<0.001) between the two regions in this study. However, there was no statistically significant difference in gender, major, smoking, drinking and frequency of breakfast. Regarding a characteristic related to low-salt diet, there was statistically significant difference in diastolic blood pressure level(p=0.002), urine test(p=0.001). But there was no statistically significant difference in knowledge, attitude, practice of low-salt diet and systolic blood pressure. There was no statistically significant difference in their knowledge of low-salt diet(F=1.588, p=0.209), attitude(F=2.182, p=0.141), practice(F=3.507, p=0.062) and systolic blood pressure(F=1.723, p=0.191), diastolic blood pressure(F=1.552, p=0.214), urine test after a community intervention. Conclusions: Community intervention that does not have concrete target group doesn't seem to be effective on increasing knowledge, attitude and practice of the entire community, not even in university-a somewhat closed community. It is suggested to apply a target specific intervention in order to have efficient and effective outcome from a intervention.

수은 고노출 지역 초등학생의 수은노출수준과 노출요인 연구 (A Study on Mercury Exposure Levels among Schoolchildren and Related Factors in High Mercury Exposure Areas in Korea)

  • Objectives: Blood mercury levels among adults living in certain areas of the Gyeongsang Provinces have been shown to be very high (Kunwee County $29.6{\mu}g/L$, Yeongcheon-city $26.7{\mu}g/L$). The purpose of this project was to determine mercury exposure levels in schoolchildren and factors related with their mercury levels in high mercury exposure areas identifyed by the 2007 Korea National Environmental Health Survey. Methods: From June to September 2010, 1,097 students from grades 3 to 6 at 19 elementary schools participated in this study, including 294 students from 10 elementary schools in Kunwee County, 529 students from Yeongcheon City, 122 students from two elementary schools in Pohang City, North Gyeongsang Province, and 152 students from two elementary schools in Ulsan Metropolitan City. Biological samples from schoolchildren, including whole blood, urine and hair, were collected to measure total mercury at the time of a health check up. Information about children was collected by questionnaire. Total mercury concentrations in blood were measured using the Direct Mercury Analyzer 80 with the gold-amalgam collection method. Results: The mean mercury levels were $2.70{\mu}g/L$ in 1,091 blood samples, $2.25{\mu}g/g-creat.$ in 820 urine samples and $1.03{\mu}g/g$ in 1,064 hair samples. Blood mercury levels in the schoolchildren was slightly higher than the result of $2.4{\mu}g/L$ from a 2006 survey of elementary school children on exposure and health effects of mercury by the National Institute of Environmental Research. However, 0.3% and 4.5% of participants exceeded the reference level of blood mercury by CHBMII ($15{\mu}g/L$) and the US EPA ($5.8{\mu}g/L$), respectively. The reference level of urine by CHBMII ($20{\mu}g/L$) was exceeded by 0.4% of participants. As factors, residence period in the study areas, residence type, father's education level and income all showed significant associations with mercury level in the biological samples. The number of dental amalgam sides showed an association with urine mercury. Fish intake preference and fish intake frequency were important factors in mercury levels. In particular, intake of shark meat and recent intake of shark meat were associated with higher mercury levels. In this regard, participation in the performance of an ancestral rite showed a relation with higher mercury levels. Conclusion: The intake of shark meat was very important factor to high mercury exposure level. It is recommended to monitor and manage students with high mercury exposures who exceeded CHBM II and EPA guidelines, and include blood mercury testing in the Children's Health check up for this province.

중화반응으로 증명된 Anti-Sda 1예 (A Case of Anti-Sda Proven by Urine Neutralization Test)

  • 항-Sda는 용혈성 수혈 부작용 발생이 적은 항체이므로, 항체 선별검사에서 의심이 되어도, 항체 동정이 보통은 생략된다. 저자들은 73세 남자 환자에서 수혈 전 비예기 항체 검사에서 혼합시야 반응을 보이면서 항체 동정검사에서 확인되지 않는 항체를 확인하였다. 항-Sda 의심하에 추가적인 소변 중화반응으로 동정하였다. 비록 항-Sda는 임상적 중요성이 떨어지지만, 고빈도 항원에 대한 항체이기 때문에 교차시험 검사에서 문제가 발생할 수 있다. 항-Sda로 추정되는 경우에는 소변으로 중화된 혈청으로 검사하는 것이 부적합 교차시험에 대한 해결책이 될 수 있다.

수면장애와 동반된 임상증상에 따른 심박변이도와 수면 관련 설문간의 상관성 연구 (The Relation of HRV, PSQI and IQ with Symptoms of Insomnia Patients)

  • Objective: This study was to investigate the relation of what insomnia patients' symptoms with Heart Rate Variability(HRV), Pittsburgh Sleep Quality Index (PSQI) and Insomnia Questionaire(IQ). Methods: For this study, we carried out HRV, PSQI and IQ of 43 patients with insomnia who have come to Donguei oriental hospital of Donguei university from September 2007 to May 2008. We studied the relationship HRV with PSQI and IQ. For every symptom, this study classified the patients as none-symptom, symptom groups and analyzed each group's HRV, PSQI and IQ. Result : 1. The rate of xerostomia in female group is significantly higher than male group. 2. P1 (hypnagogic time), P3(awakening time), LF has negative correlation with age. The score of PI (hypnagogic time), P3(awakening time), P4(time of recoverable sleep) in the group whose age is more than 50 are higher than the group whose age is lesser than 50. 3. The frequency of using hypnotics(P6) in the group whose duration of onset are over 1 year was significantly higher than the score in the group whose duration of onset were below 1 year. 4. The accompanied symptoms in the group who started sleep disorder after stressor are higher than the number in the group whose duration of onset were poor-defined. 5. The drinking group have lower rate of anorexia, higher score of P1 (hypnagogic time) and TP than non-drinking group. 6. The score of P-total(general quality of sleep) have negative correlation with NNSO, HF, TP and positive correlation with LF/HF. 7. The score of P1(hypnagogic time) have negative correlation with LF. 8. The score of P5a(frequency of delayed hypnagogue) have positive correlation with LF/HF. 9. The score of P5e(frequency of cough or snoring) have negative correlation with RMSSD. 10. The score of I1a(degree of onset insomnia), I1b(degree of maintenance insomnia) have negative correlation with NN50, pNN50, HF. 11. The score of I1c(degree of termination insomnia) have negative correlation with RMSSD, NN50, pNN50, HF, TP and positive correlation with MHR, LF/HF. 12.. The score of P5i(frequency of night pain), LF/HF in the group with headache are higher than the group without headache. 13. The score of P5a(frequency of delayed hypnagogue), P5c(frequency of awakening for urine of stool) in the group with xerostomia are higher than the group without xerostomia. 14. The score of P9(degree of unrecoverable sleep) and I5a(degree of onset insomnia) in the group with chest discomfort are lower than the group without chest discomfort. 15. The score of P7(frequency of drowsiness for daytime) in the group with palpitation are lower than the group without palpitation. 16. The score of P5c(frequency of awakening for urine of stool), P5f(frequency of burning sensation), P5h(frequency of nightmare), I1c(degree of termination insomnia) in the group with burning sensation of upper trunk are higher than the group without burning sensation. 17. The score of NN50, pNN50, LF, TP in the group with anorexia are lower than the group without anorexia. 18. The score of P-total(general quality of sleep), P2(duration for sleep start) in the group with constipation are higher than the group without constipation. 19. The score of P4(recoverable sleep time) in the group with depression are higher than the group without depression. 20. In the group with anxiety, the score of P1(time of hypnagogue), P5a(frequency of onset insomnia) are lower and the score of I1b(degree of maintenance insomnia), I4(objective degree of impairment by insomnia) are higher than the group without anxiety. 21. The score of NN50, pNN50, HF, TP in the group with inertia are lower than the group without inertia. 22. The accompanied symptoms haver posirive correlation with P5f(frequency of burning sensation), P5h(frequency of nightmare), P5i(frequency of night pain) and LF/HF. Conclusion: This study provides insights into the complicated relations of the pattern of sleep disorder with many symptoms such as headache, chest discomfort, depression and any others. And this study showed that autonomic nervous system have important function in the regulation of sleep.
