Analyzing the factors that contribute to the development of embryological classical type of bladder exstrophy

  • Bladder exstrophy is a rare congenital condition of the pelvis, bladder, and lower abdomen that opens the bladder against the abdominal wall, produces aberrant growth, short penis, upward curvature during erection, wide penis, and undescended testes. Exstrophy affects 1/30,000 newborns. The bladder opens against the abdominal wall in bladder exstrophy, a rare genitourinary condition. This study is vital to provide appropriate therapy choices as a basis to improve patient outcomes. This study may explain bladder exstrophy and provide treatment. Epispadias, secretory placenta, cloacal exstrophy, and other embryonic abnormalities comprise the exstrophy-spades complex. The mesenchymal layer does not migrate from the ectoderm and endoderm layers in the first trimester, affecting the cloacal membrane. Embryological problems define the exstrophy-aspidistra complex, which resembles epimedium, classic bladder, cloacal exstrophy, and other diseases. Urogenital ventral body wall anomalies expose the bladder mucosa, causing bladder exstrophy. Genetic mutations in the Hedgehog cascade pathway, Wnt signal, FGF, BMP4, Alx4, Gli3, and ISL1 cause ventral body wall closure and urinary bladder failure. External factors such as high maternal age, smoking moms, and high maternal body mass index have also been associated to bladder exstrophy. Valproic acid increases bladder exstrophy risk; chemicals and pollutants during pregnancy may increase bladder exstrophy risk. Bladder exstrophy has no identified cause despite these risk factors. Exstrophy reconstruction seals the bladder, improves bowel function, reconstructs the vaginal region, and restores urination.

Sequential treatment with transurethral resection and hormonal therapy for bladder endometriosis of vesicoureteric junction

  • Objective: To estimate the efficacy of sequential treatment of bladder endometriosis (BE) of the vesicoureteric junction using transurethral resection (TUR) and hormonal therapy. Design: Case report. Setting: Private multispecialty hospital. Patient: A multiparous woman presented with perimenstrual lower urinary tract symptoms, cyclic chronic pelvic pain, and left loin pain. Intervention[s]: Ultrasonography revealed marked left renal dilatation. Computed tomography confirmed the presence of a bladder mass. A diagnostic cystoscopy revealed compression of the left vesicoureteral junction. Complete TUR BE with release of chocolate material during resection, followed by ureteric double J stent insertion for 3 months, was performed. Histopathology confirmed the diagnosis of BE, followed by adjuvant hormonal therapy (dienogest) for 3 months. Follow-up for about 2 years revealed complete relief of the symptoms without any recurrence. Main Outcome Measure[s]. Success and recurrence rates of sequential TUR and hormonal therapy of BE of the vesicoureteric junction. Result[s]. TUR BE followed by adjuvant hormonal therapy was very effective in eradicating BE of the vesicoureteric junction in a safe manner without recurrence on follow-up for 2 years. Conclusion[s]. BE of the vesicoureteric junction can be properly treated by sequential TUR and hormonal therapy without recurrence over a 2-year follow-up.

Altered Proteome of Extracellular Vesicles Derived from Bladder Cancer Patients Urine

  • Proteomic analysis of extracellular vesicles (EVs) from biological fluid is a powerful approach to discover potential biomarkers for human diseases including cancers, as EV secreted to biological fluids are originated from the affected tissue. In order to investigate significant molecules related to the pathogenesis of bladder cancer, EVs were isolated from patient urine which was analyzed by mass spectrometry based proteomics. Comparison of the EV proteome to the whole urine proteome demonstrated an increased number of protein identification in EV. Comparative MS analyses of urinary EV from control subjects and bladder cancer patients identified a total of 1,222 proteins. Statistical analyses provided 56 proteins significantly increased in bladder cancer urine, including proteins for which expression levels varied by cancer stage (P-value < 0.05). While urine represents a valuable, non-invasive specimen for biomarker discovery in urologic cancers, there is a high degree of intra- and inter-individual variability in urine samples. The enrichment of urinary EV demonstrated its capability and applicability of providing a focused identification of biologically relevant proteins in urological diseases.

  • Transitional cell carcinoma(TCC) was identified by cytological and ultrasonographic findings in an 11-year-old, intact female Yorkshire terrier with intermittent hematuria for three years. In color-doppler ultrasound images there was marked irregular wall thickness of the bladder surface and well-defined hyperechoic focal lesions within the mass. Active vascular response was observed in the surroundings of hyperechoic lesions.

  • We report the cytologic features of a case of primary small cell carcinoma of the urinary bladder with high grade transitional cell and signet ring cell carcinomatous components. A 64-year-old male presented with gross hematuria for one week. Computed tomography revealed an ill-defined mass in the left lateral wall of the urinary bladder. Urinary cytology showed hypercellularity with predominantly isolated single cells and clustered cells. They have scanty cytoplasm and naked hyperchromatic nuclei with finely granular nuclear chromatin and rare nucleoli. The tumor cells occurred predominantly singe cells, but a few in clusters. Nuclear molding was prominent. No glandular formation or nesting was noted. The second tumor cells had high nuclear/cytoplasmic ratio, irregular nuclear membrane, and coarse granular chromatin. The background was inflamed and necrotic. The histoiogic findings of transurethral resection were mainly composed of small cell carcinoma, and partly transitional cell and signet ring cell carcinomatous components. Small cell neuroendocrine carcinoma have distinctive cytologic features to make a proper diagnosis.

Predictors of outcomes after the trans-obturator tape procedure in females with equal severity for stress and urge mixed urinary incontinence

  • Mixed urinary incontinence (UI) is common in women. This study aimed to assess the efficacy of anti-incontinence surgery in female patients with equally severe stress UI (SUI) and urge UI (UUI). All patients had equal severity of SUI and UUI. The postoperative cure rate was categorized into the cure group (CG) and failure group (FG). Postoperative satisfaction was categorized into the satisfaction group (SG) and the dissatisfaction group (DG). Statistical significance was set at P<0.05. Ninety patients (SG, 73.3%; DG, 26.7%; CG, 93.3%; FG, 6.7%) were included in the study. In the univariate analysis, body mass index (BMI), total bladder capacity, and overactive bladder symptom score (OABSS) were significantly different between the SG and DG groups. Peak urinary flow, Valsalva leak point pressure (VLPP), and OABSS were significantly different between the CG and FG groups. In the multivariate analysis, OABSS (P=0.001) and BMI (P=0.032) were independent predictors of postoperative satisfaction. VLPP (P=0.023) was the only independent factor associated with the postoperative cure rate. In equal severity of SUI and UUI, VLPP was found to be the only independent factor associated with postoperative cure rates. Higher VLPP values were associated with higher cure rates. BMI and OABSS were identified as independent predictors of postoperative satisfaction, with lower BMI and OABSS associated with higher postoperative satisfaction.

방광의 일차 악성 섬유성 조직구종의 영상 및 임상 소견: 증례 보고 (Imaging and Clinical Findings of Primary Malignant Fibrous Histiocytoma of the Urinary Bladder: A Case Report)

  • 원발성 악성 섬유성 조직구종은 요로, 특히 방광에서 흔하게 발생하지 않는 중간엽 유래의 악성 종양이다. 방광암의 대부분을 차지하는 요로 상피암과는 달리, 악성 섬유성 조직구종은 점막 고유층, 고유근층, 장막층으로 구성된 방광 벽의 요로 상피 하부에서 발생한다. 조직학적 기원은 섬유아세포와 조직구 세포가 부분적으로 분화된 저분화 중간엽 줄기세포에서 발생하는 것으로 추정된다. 영상의학적으로는 요로 상피암에서 흔히 볼 수 있는 유두상 성장 패턴을 보이지 않아 '비유두상 종양'으로 알려져 있으며, 흔히 고유근층 이상의 침범을 보이고 괴사를 동반하지 않는 거대 종괴의 형태로 진단된다. 이 드문 악성 종양의 예후는 다양한 병리학적 인자에 의해 결정되지만, 일반적으로 불량한 예후와 높은 국소 재발률을 보인다. 또한 무통성 혈뇨를 주로 호소하는 요로 상피암 환자와 달리 주변 장기를 압박하여 비특이적인 하복부 통증이 나타날 수 있어 진단에 어려움이 있을 수 있다. 저자들은 혈뇨 없이 하복부 통증을 주 증상으로 내원하여 방광의 원발성 악성 섬유성 조직구종으로 진단된 후 빠른 치료를 시작하였음에도 매우 공격적인 임상 경과를 보였던 증례를 보고하고자 한다.

개에서 요도를 포매한 평활근육종의 수술적 처치 및 예후 (Prognosis and Surgical Treatment of the Urethra Embedding Leiomyosarcoma in a Dog)

  • 충북대학교 동물의료센터에 16년령 암컷 시츄견이 복부팽만과 배뇨, 배변곤란을 주증으로 내원하였다. 신체검사 시 복부에서 10 cm 가량의 종양이 촉진되었으며 방사선 상에서 확인되었고, 방광은 종양에 의해 배꼽 쪽으로 변위되어 있었다. 초음파 상에서 종양은 불균질한 실질과 국소적으로 무에코성 영역을 가지고 있었다. 혈액검사 소견상 혈소판증가증과 약한 호중구증가증이 나타났으며, 혈액화학치 검사 결과 ALP 상승을 확인할 수 있었다. 개복술을 시행하여 자궁경과 방광 사이의 $10.5{\times}9.6cm$ 크기의 단단한 종양이 요도를 포매하고 있는 것을 확인하고 주위조직과 둔성분리 후 절제하였다. 조직병리학적 검사와 면역화학 적 검사 결과 종양은 평활근육종으로 진단되었다. 수술 후 일주일 뒤 배뇨곤란을 주증으로 재 내원하였다. 배뇨곤란의 원인을 찾기 위하여 요도조영술을 실시하였으나 물리적인 폐색 등 특별한 원인을 찾을 수 없었다. 방광근과 방광조임근에 작용하는 몇몇 약물요법 등을 시행하였지만 반응이 없어 중장기의 보존요법이 지시되었으며 배뇨곤란 증상은 술 후 27일차에 갑작스럽게 호전되어 환자는 원활한 배뇨를 하게 되었다. 본 증례는 요도를 포매하고 있는 복강 내 종양의 수술 후 예후와 배뇨곤란과 같은 후유증을 나타낼 수 있음을 시사하고 있다.

개에서 골화생을 동반한 전립선 주위종의 영상의학 증례 (Diagnostic Imaging of Paraprostatic Cyst with Osseous Metaplasia in a Dog)

  • A 6-year-old intact male Doberman pinscher presented with a thin soft stool and urinary signs. On radiography, three masses were observed in the caudal abdomen. One mass was described as a large, round, mineralized mass, with an "eggshell" appearance. The second mass was located caudal to the mineralized mass, and the third mass was located between the two masses. The second and the third masses had a soft tissue density. Ultrasonography was performed to identify the features and the origins of the masses. The first mass contained a large amount of anechoic fluid and had a thin wall; it was determined to be a cyst. Strong acoustic shadowing artifact was created by the mineralized cystic wall. Caudal to the cyst, the second mass was shown to be the prostate. A hypoechoic stalk connected the prostate and the cyst, which represented the cyst as a paraprostatic cyst with mineralization. The third mass was the normal urinary bladder. The cyst was removed surgically and confirmed by histopathologic examination. This report described typical clinical findings, diagnostic imaging, and treatment of a paraprostatic cyst.

Prostate spindle cell neoplasm associated with early voiding difficulty after transurethral resection of the prostate

  • This report presents the case of 75-year-old men with spindle cell neoplasm. The patient underwent percutaneous nephrolithotomy and transurethral resection of the prostate (TURP) for renal stones and benign prostatic hyperplasia. One month postoperatively, the patient was able to void without any difficulty. Five months later, the patient experienced difficulty voiding and presented to the emergency room with severe pelvic pain. Computed tomography (CT) showed regrowth of the prostate mass into the posterior bladder and penile root. The prostate-specific antigen level remained constant at 1.14 ng/mL during the pre-and postoperative periods. Five months before the TURP operation, the patient's CT scan showed a soft and mildly enlarged prostate with no protrusion into the bladder. Biopsy of the prostate, however, showed a protruding mass, indicative of a spindle cell neoplasm. The patient was subsequently treated with the chemotherapeutic drug adriamycin. Unfortunately, treatment was unsuccessful, and the patient died 18 months later.
