  • To irradiate the entire neuroaxis, bilateral parallel opposed brain fields and direct posterior spinal field have been utilized and radiation dose at the junction between abutting fields has been extensilvely studied. And several workable methods were reported to achieve uniform dose at a desired depth at the juction between abutting fields whose central axis are coplanar. But the dose distribhution at the juction of orthogonal fields has been a persistent problem in radiation oncology. Author describes a new method to solve the junction problem between abutting fields whose central axis are orthogonal. Author utilized split beam/comllimator rotation or collimator/couch rotation to avoid hot or cold spots that may arise from beam divergence. Author achieved accurate and homogeneous dose distribution by mathching the $50\%$ isodose line at the junction between orthogonal central axis beam fields.

  • This experiment was carried out to investigate the effect of gamma ray on the eggs of later stage in late fall rearing season. The results are summarized as follows: 1. The duration of larval stage was simmilar to that of control in any dose. The silkworm growth was uniform in does of 4,000 gamma ray, but in does of 8,000r 16000r was irregular. There were not malformed silkworms but many small silkworm. 2. Mortality of silkworms was very high 8,000r and over; virus diseases of 87.6 per cent, flacherie of 5.9 per cent and complication of 6.5 per cent. 3. The more the dose of gamma ray became, the lighter the weight full grown silkworm and matured silkworm became. 4. The uniformness of cocoons was registered in dose of 2,000r and the cocoons were irregular or small in dose of 16,000r

  • Intraperitoneal administration of radioisotopes is suggested to treat the metastatic ovarian cancer in the peritoneal cavity. Administering beta-emitting radioisotopes into the peritoneal cavity allows the maximum energy delivery to the cancerous cells of the peritoneal wall surface while sparing the normal cells located in deep site of the peritoneal wall. In this study, dose estimates of the peritoneal wall are provided to be used for prescribing the amount of $^{166}Ho$-chitosan complex administered. The $^{166}Ho$-chitosan complex diffused in the peritoneal fluid may attach to the peritoneal wall surface. The attachment fraction of $^{166}Ho$-chitosan complex to the peritoneal wall surface is obtained by simulating the ascites with Fischer rats. Both volume source in the peritoneal fluid and the surface source over the peritoneal wall surface are counted for the contribution to the peritoneal wall dose. The Monte Carlo code EGS4 is used to simulate the energy transfer of the beta particles emitted from $^{166}Ho$. A plane geometrical model of semi-infinite volume describes the peritoneal cavity and the peritoneal wall. A semi-infinite plane of $10{\mu}m$ in thickness at every 1 mm of depth in the peritoneal wall is taken as the target in dose estimation. Greater than 98 percents of attachment fraction has been observed from the experiments with Fischer rats. Given $1.3{\mu}Ci/cm^2$ and $2.4{\mu}Ci/ml$ of uniform activity density, absorbed dose is 123 Gy, 8.59 Gy, 3.00 Gy, 1.03 Gy, and .327 Gy at 0 mm, 1 mm, 2 mm, 3 mm, and 4 mm in depth to the peritoneal wall, respectively.

  • Monte Carlo calculations were performed to demonstrate the dose modulation with dynamic magnetic fields in phantom. The goal of this study is to obtain the uniform dose distributions at a depth region as a target on the central axis of photon beam under moving transverse magnetic field. We have calculated the depth dose curves for two cases of moving magnetic field along a depth line, constant speed and optimal speed. We introduced step-by-step shift and time factor of the position of the electromagnet as an approximations of continuous moving. The optimal time factors as a function of magnetic field position were calculated by least square methods using depth dose data for static magnetic field. We have verified that the flat depth dose is produced by varying the speed of magnetic field as a function of position as a results of Monte Carlo calculations. For 3 T magnetic field, the dose enhancement was 10.1% in comparison to without magnetic field at the center of the target.

  • In order to minimize radiation exposure and secure diagnostic value images during CT examination of the head of children, the usefulness of volume axial mode is evaluated through comparison and analysis of exposure dose and images of volume axial mode, high pitch mode, and helical mode. Image evaluation and dose evaluation were performed in CT high pitch mode, helical mode, and volume axial mode for infants under the age of 1 according to the voltages of 70, 80, and 100 kVp tubes. The image evaluation was conducted by comparing image quality by setting ROI for each image, calculating SNR and CNR, using ONE-WAY (ANOVA) to evaluated statistical significance, and cross-examining the dose evaluation using DLP values displayed in the Dose Report. When inspected using volume axial mode, DLP values were generally low, and SNR and CNR values differed by ROI and kVp. When volume axial mode evaluated the quality of the image compared to other scan modes, the difference is not uniform. For the reason, certain modes are not considered excellent, but the exposure dose was reduced the most in terms of dose. In addition, the point that the volume axial mode can be examined at its original location, short scanning time and needless of table movement is useful for CT tests for children under 1 year of age with high radiation sensitivity.

Low-dose Chest CT in Evaluation of Coronary Artery Calcification: Correlation with Coronary Artery Calcium Score CT (관상동맥 석회화 평가에서 저선량 흉부 CT와 관상동맥 석회화검사의 일치도)

  • Low-dose chest CT, which is used as a lung screening test, also includes information on coronary artery calcification within the scan range. The purpose of this study was to investigate the usefulness of determining coronary artery calcification using Low-dose chest CT. Those who underwent low-dose chest CT and coronary artery calcification score CT on the same day were eligible. Coronary artery calcium score CT results were divided into 4 groups (Low: 1〈CACS〈10, Mild: 10〈CACS〈100, Moderate: 100〈CACS〈400, High: 400〈CACS) by referring to the Coronary artery calcium score categories and risks. After selecting 30 people each group, five radiotechnologists with more than 15 years of experience in coronary artery calcium measurement retrospectively analyzed the presence or absence of coronary artery calcification in low-dose chest CT images. The results of the five observers' uniform interpretation of the low-dose chest CT image were consistent with the coronary artery calcium score CT results in Low group: 56%, Mild group: 96.6%, Moderate group: 100%, and High group: 100%. appeared. In the Low group, all 5 observers observed calcification in 17 out of 30 cases, and in 7 cases all 5 observers decided that calcification could not be identified. Coronary artery calcification could be observed in 100% of asymptomatic adults with a calcium score of 15 or higher in low-dose chest CT scans. The minimum calcium score that can be identified is 1, and it was found that even very small calcifications can be identified when the subject's body size is small or the scan is performed at a time when heart movement is minimal.

The Study of Normal Tissue Complication Probability(NTCP) for Radiation Pneumonitis by Effective Volume Method (유효체적 방법과 임상분석을 통한 방사선에 의한 정상 폐조직의 부작용 확률에 관한 연구)

  • Purpose : In radiation therapy, NTCF is very importart indicator of selecting the optimal treatment plan. In our study, we tried to find out usefullness of NTCP in lung cancer by comparng the incidence of radiation pneumonitis with NTCP. Materials and Methods : From August 1993 to December 1994, thirty six patients with locally advanced non=small cell lung cancer were treated by concurrent chemoradiation therapy. Total dose of radiation therapy was 6480cGy (120cGy, bid) and chemotherapeutlc agents were mitomycin C. vinblastion, cisplatin (2 cycles, 4 weeks interval). We evaluated the development of raniation pneumonitis by CT scan, chest x-rar and clinical symptoms. We used grading system of South Western Oncology Group (SWOG) for radiation pneumanitis. Dose Volume Histograms (DVH) were analyzed for ipsilateral and whole lung, Non uniform DVH was translated to uniform DVH by effective volume method. With these data, we calculated NTCP for ipsilateral and whole lung. Finally we compared the clinical results to NTCP. Results : Eight of thrity six patients developed radiation pneumonitis. Of these 8 patients , 6 had grade I severity and 2 had grade II. The average NTCP value cf the patients who showed radiation pneumonitis was significantly higher than that uf the patients without pneumonitis $(66\%\;vs.\;26.4\%)$. But the results of pulmonary function test was not correlated with NTCP. Conclusion : NTCP of lung is very good indicator for selecting rival treatment planning in lung cancer. According to the results of NTCP, it may be possible to adjust target volume and optimize target dose. In the near future, we are going to anaiyze the effect of hyperfractionation and concurrent chemotherapy in addition to NTCP.

Effect of the Dose Reduction Applied Low Dose for PET/CT According to CT Attenuation Correction Method (PET/CT 저선량 적용 시 CT 감쇠보정법에 따른 피폭선량 저감효과)

  • Purpose: Low dose of PET/CT is important because of Patient's X-ray exposure. The aim of this study was to evaluate the effectiveness of low-dose PET/ CT image through the CTAC and QAC of patient study and phantom study. Materials and Methods: We used the discovery 710 PET/CT (GE). We used the NEMA IEC body phantom for evaluating the PET data corrected by ultra-low dose CT attenuation correction method and NU2-94 phantom for uniformity. After injection of 70.78 MBq and 22.2 MBq of 18 F-FDG were done to each of phantom, PET/CT scans were obtained. PET data were reconstructed by using of CTAC of which dose was for the diagnosis CT and Q. AC of which was only for attenuation correction. Quantitative analysis was performed by use of horizontal profile and vertical profile. Reference data which were corrected by CTAC were compared to PET data which was corrected by the ultra-low dose. The relative error was assessed. Patients with over weighted and normal weight also underwent a PET/CT scans according to low dose protocol and standard dose protocol. Relative error and signal to noise ratio of SUV were analyzed. Results: In the results of phantom test, phantom PET data were corrected by CTAC and Q.AC and they were compared each other. The relative error of Q.AC profile was been calculated, and it was shown in graph. In patient studies, PET data for overweight patient and normal weight patient were reconstructed by CTAC and Q.AC under routine dose and ultra-low dose. When routine dose was used, the relative error was small. When high dose was used, the result of overweight patient was effectively corrected by Q.AC. Conclusion: In phantom study, CTAC method with 80 kVp and 10 mA was resulted in bead hardening artifact. PET data corrected by ultra- low dose CTAC was not quantified, but those by the same dose were quantified properly. In patients' cases, PET data of over weighted patient could be quantified by Q.AC method. Its relative difference was not significant. Q.AC method was proper attenuation correction method when ultra-low dose was used. As a result, it is expected that Q.AC is a good method in order to reduce patient's exposure dose.

Absorbed Dose for the Endovascular Ho-166-DTPA Brachytherapy Using a Balloon Angio Catheter (풍선도자관의 Ho-166-DTPA 흡수선량)

  • The purpose of this study was to evaluate the absorbed dose to the coronary artery segment from various sized balloon angio catheters. The liquid form of Ho-166 was produced at the KAERI by (n, ${\gamma}$ ) reaction. We used GafChromic film for the estimation of the absorbed dose by beta particles. The exposed films were read using a videodensitometer. Several film exposures were made with varying irradiation times and activities. A modified micrometer was used for the measurement of the absorbed dose distribution near the balloon surface. Four balloons of coronary catheters evaluated were 30 m long and 2.5, 3.0, 3.5 and 4.0 mm in diameter. All doses are plotted in units of Gy/min/GBq/ml as a function of radial distance in mm from the surface of balloon. The absorbed dose rate was 0.86, 1.01, 1.11 and 1.24 Gy/min/GBq/ml at a balloon surface for various balloon diameter 2.5, 3.0, 3.5 and 4.0 mm respectively. Using a vacuum pump, the air in the balloon was evacuated prior to instillation of the Ho-166 source. By removing air bubbles in the balloon, the absorbed dose distribution was more uniform.

  • PURPOSE : To investigate the effect of asymmetric jaws for delivering a uniform accurate dose of radiation to the junctions. METHODS & MATERIALS : A linear accelerator with a set of asymmetric jaws(varian 600C, 2100C, 2100CD with 4mev, 10mev, 10mev). Dose disribution was measured at the junctions with films in phantom. Total $10{\times}20cm^2$ with each $10{\times}10cm^2$ in deviation of ${\pm}1mm$ jaws. RESULTS : Film dosimetry showed the accuracy of asymmetric jaws depending on the machine. CONCLUSION : Understanding the mechanical characteristics of the use of half-beam at the junctions, without hot or cold regions.
