Study on Heart Rate Variability Characteristics of Cold Hypersensitivity of Hands and Feet Patients (수족냉증 환자의 HRV 특성연구)

  • Objectives: Heart Rate Variability (HRV) is a diagnostic tool for assessing the function of the autonomic nervous system. Although studies on the HRV characteristics of the cold hypersensitivity of hands and feet have been done, no study has compared HRV characteristics of cold hypersensitivity of hands and feet patients to that of normal Korean women. For this reason, this study compared the HRV characteristics of cold hypersensitivity of hands and feet patients and normal Korean women. Methods: We studied 130 cold hypersensitivity of hands and feet patients who visited Kyung Hee University Hospital at Gangdong from 01 March 2011 to 11 August 2013. We measured HRV of Each patient. Patients are divided into under 29 years old, 30~39 years old, 40~49 years old and over 50 years old with age. Patient's HRV results are compared to that of normal Korean women in each age group. Results: In each age group, cold hypersensitivity of hands and feet patients' LF/HF was statistically lower than that of normal Korean women. Conclusions: HRV characteristics and symptoms of cold hypersensitivity of hands and feet is similar to that of cold induced vasodilation (CIVD). So, cold hypersensitivity of hands and feet can be considered as abnormal condition that CIVD is continued. More studies on measuring of blood flow of cold hypersensitivity of hands and feet patients are necessary.

A study on sprouting of a young merchantable pitch pine stand (장령기(壯令期)에 가까운 리기다소나무 임분(林分)의 맹아(萌芽) 갱신(更新)에 대(對)한 연구(硏究))

  • (1) The objects of this study are to observe the possibility of regenerating a young merchantable pitch pine stand by sprouts and to compare the growth trend of sprouts with that of seedlings of same age grown under the almost same circumstances. (2) A plot of 20 year old pitch pine plantation, i.e. 200 trees on 0.1 ha of average D.B.H. 14 cm was clearcut at 20 cm above ground in April, 1945. By the late spring of that year sixty per cent of the cut stumps had sprouted. (3) Fourty to eighty sprouts were found on each stump (maximum:412 sprouts) at the first, but many of them had gradually died out leaving only four to five sprouts per stump by the time of three years after cutting. At that time only one vigorous sprout was left per stump by eliminating the weaker ones. (4) The sprouts, as they grew, started to cover the old stumps with new tissues developed from lower part of sprouts;consequently forming new root systems from the base of new tissues, and they appeared to be seedlings. When the age of sprouts was thirteen years old, the old stumps were completely decayed away and the reproduced stand from sprouts was averaged at 9.7 cm in D.B.H. and at 5.5m in height. (5) When the age of sprouts was thirteen years old, the sprouts exceeded the seedlings in both of total present growth and mean annual increment in height, volume, D.B.H. and basal area, but the seedlings began to exceed the sprouts in current annual increment of height, volume, D.B.H. and basal area at about ten years of age. The rates of increment of the seedling in height, volume, ect. were larger than those of sprouts except when they were one to four years old. From above facts, the following may be concluded: (1) In regenerating a pitch pine stand by sprouts, the lower the stump height, the better the result. (2) If no light limit exists, regenerating a pitch pine stand by sprouts is well possible even at the age of 20 year. (3) Pitch pine reproduction started from sprouts exceeds the seedlings of same age in growth under the almost same circumstances until they get ten years of age.

A Study on the Eating Habits and Its Related Factors of Residents in Urban Area (도시지역주민의 식습관과 연관요인 연구)

  • This study was conducted to investigate the awareness and practice level of health food intake and use the information for the development of Public Health Policy and Health Education Program in order to help healthy dietary life. The interview was done by trained surveyors on 1, 143 city dwellers who were selected randomly from Oct. 2, to Oct. 21, 1992. The results are summarized as follows: 1) General characteristics of subjects; Among 1, 143 respondents, there were more female(53.8%) than males(46.2%) and the age group of 20-29 account for the hightest portion, 46.7%. As for the education levels, 45.3% of the subjects were college graduates. And 38.9% of the subjects were students. 2) The percentage of subjects who assess themselves ‘Healthy’ was 46.7% and ‘Not Healthy’ was 17.4%. 3) The subjects who have irregular meals(3 times a day) was 48.7%, the rate of females was higher than males. The proportion of unmarried subjects, people with low economic level, high education level, and students was high. 4) As for the amount of 1 meal, 72.4% was ‘moderate’, 16.3% was ‘too much’ amount, which was higher in 19 years old, single, low economic level, elementary school graduate, than other groups. 5) As for the eating speed, 40.2% was fast, which was higher in single(44.3%), in low economic level(50.0%). college graduate(44.9%), student(44.7%) than other groups. 6) As for the level of preference to hot and salty food, 25.7% preferred to have hot and salty food. The difference in the degree of preference to hot and salty food by age and economic level was significant. 7) With regard to the individual food taste, the subjects who prefer to meat and fried food were 12.3%. Which was higher in the group under 19 years old, single, and college graduates. 8) With regard to having a snack, 38.7% have a snack. By characteristics, the subjects who have a snack were higher in female, under 19 years old, single, high economic level, apartment dwellers, college graduate, and student than other groups. 9) As for the coffee intake amount per day, 39.8% of subjects was more than 3 cups daily. 10) With regard to the self-assessment of body weight, the subjects who assess themselves over body weight had late dinner time. 11) As for the correlation among related variables, the age and taking breakfast was positive correlation. And sex and drinking alcohol was positively associated while age had negative correlation.

The Effect of Safety Education on Accident Proneness Prospect in Preschooler (안전교육이 학령전기 아동의 사고경향 예측에 미치는 효과)

  • The purpose of this study was to provide basic data on preschool children for accident prevention and improve their health through sound, safe living environment with safety education established through more systematic method. Data were collected from 300 preschoolers(150 preschooler are assigned to experimental group and 150 preschoolers are assigned to control group) from 4 to 6 years old using APP paper test which consists of questions and drawings. To experimental group, safety education were done 4 times within the time of 30 minutes per 1 time using education books, video, OHP, slide. The findings of this study are as follows: 1. There were significant difference in behavioral character between experimental group and control group(χ2=11.690, p= 0.003). So, safety education have effect on the behavioral character of preschooler. 2. In the accident proneness on preschooler between experimental group and control group according to general characteristics, it proved significant difference in the case of accident prevention education were done, movement stability(χ2=8.844, p=0.012) and behavioral character(χ2=6.699, p=0.035), in the case of housing pattern is mixed-type, behavioral character(χ2=10.37, p=0.006), in the case of subjects' age is 4 years old, watchfulness(χ2=9.525, p=0.009), in the case of subjects' age is 5 years old, behavioral character(χ2=7.324, p=0.026), in the case of children's order is second, behavioral character(χ2=14.31, p=0.001), in the case of children's sex is boy, living safety(χ2=7.981, p=0.018), movement speed (χ2=6.661, p=0.036), bihavioral character(χ2=8.837, p=0.012), in the case of children's sex is girl, reasoning power(χ2 =9.78, p=0.008), in the case of childrens have no past accidental experience, behavioral character(χ2=9.862, p=0.007), in the case of nuclear family, movement speed(χ2=6.341, p=0.042) and behavioral character(χ2=9.326, p=0.009), in the case of mothers' age is under thirty behavioral character(χ2=16.40, p=0.000), in the case of mothers' school career is under high school graduate, behavioral character(χ2 =8.375, p=0.015), in the case of mothers' school career is beyond college graduate, reasoning power(χ2=9.803, p=0.007) and behavioral character(χ2=6.205, p=0.045), in the case of mothers' job is part time, movement speed(χ2=10.99, p=0.004), in the case of mothers have no job, movement stability(χ2=8.490, p=0.014) and behavioral character(χ2=10.11, p=0.006). The difference of accident proneness between experimental group and control group according to general characteristics, it also showed that there were significant difference in behavioral character compared to other area.. From this findings, we can guess that safety education change and guide preschoolers' behavioral character to desirable direction.

The Elderly Health and Dietary Management in Gyeonggi Province II - Comparison with Younger Old and Older Old - (경기지역 노인의 건강과 식생활관리 II - 75세 미만의 젊은 노인과 75세 이상 고령 노인 비교 -)

  • The purpose of this study is to provide the information of the aged olds for which basic data are almost no available. The subjects were divided on the base of age 75. The elderly under 75 were named 'the young olds' and over 75 were 'the aged olds'. The aim of this research is to promote health and to improve nutrition, and the survey was made for health promotion behaviors, habits against health risk, dietary management status and diet intake. And it was conducted by 24 hr-recall method and analyzed by nutrients intake. 242 subjects were collected in 12 cities or counties in Gyeonggi Province and 20 elderly people (10 male and 10 female) were selected out of 1 village in each district. Questionnaire about health behavior and dietary management was carried out by interview method through regional home extension workers. Statistical analyses were made by SAS (version 8.1) and Chi-square tests and General Linear Models were used. Out of the subjects the aged olds over 75 was 31.9%, elementary school educated (93.5%), with spouse (40.3%), with adult children(28.6%), monthly living cost of 500-1,000 thousand won(40.3%). Mean age was 78.82 years compared with 69.75 years of the young olds. 46.8% of the aged olds used monthly pocket money over 1000 won and it was lower than 63.3% of the young olds. Only half of the aged olds had regular exercise of walking (77.8%) or with athletic equipment (17.8%). However, the young olds did more frequent walking (82.1 %) and less exercise with athletic equipment (4.8 %), which was significantly different. Kinds of disease were different with the young or the aged olds, as more proportion of cardiovascular disease(37.9%) for the young olds and joint lumbago neuralgia(41.6%) for the aged olds. Dietary management was good (3 meals per day: 93.4%, fixed mealtime: 72.4%, and regular amount: 79.9%). But there was significant difference in side dish varieties and kinds of snacks; for the aged olds only 8% had over 5 sorts (compared with 18.8% of the young olds) and the kinds of snacks were cookie, candy, juice, carbonated beverage for the aged olds (compared with noodle, milk, soybean-milk for young olds). The ratio of nutrients intake (energy, riboflavin and niacin) with RDA was significantly higher for the aged olds than that of the young olds. The surveyed subjects had no difficulties in Activities of Daily Living (ADL), but some of the aged olds had difficulties in Instrumental Activities of Daily Living (IADL) like housekeeping, using transportation, going shopping and making phone calls. These results suggest that low quality of life is linked with low economic status of the rural elderly and congregate meal at village hall would be required because of the lack of side dishes variety for the aged olds. And nutrition education program about good snacks and exercise practice would be needed for the aged olds. By operating nutrition education program the aged olds would enjoy better quality life maintaining or ameliorating IADL abilities.

An Experience of The Microwave Tissue Coagulation Therapy in The Restenosis of The Esophagogastrostomy (식도위 문합술후 재협착증에 대한 microwave 조직응고법적 치험 례)

  • The microwave tissue coagulator was originally used for hemostasis in the hepatic surgery, which is effectively applied in the endoscopic surgery such as the hemostasis of gastric ulcer or tumor bleeding, stenosis relieving of esophageal or rectal stenosis and tumor reduction in inoperable early cancer cases. We experienced the good result of the microwave tissue coagulation therapy in the patient with the restenosis of esophagogastrostomy. The patient was 67 year-old female, who was admitted due to the lye stricture of esophagus for 40 years. We made the lower esophagectomy and the esophagogastrostomy with the upper intact esophagus in the right thorax. But the restenosis occurred at the esophagogastrostomy site because of the polypoid mucosal protrusion at one month after operation. We applied the microwave tissue coagulator 3 times with 6 day interval under esophagoscopy and the good symptomatic and endoscopic relief was alleviated. We think that the microwave tissue coagulation is a very convenient and advisable method in the case of restenosis after esophageal surgery.

T-cell Non-Hodgkin`s Lymphoma originating in the wall of Chronic Tuberculous Empyema - One Case Report - (결핵성 만성 농흉벽에 원발한 T 세포형 악성 임피종;1례 보고)

  • A 66-year-old male patient was admitted due to chest pain and dyspnea of 1 year duration, He had history of treatment for tuberculosis and pleurisy, about 40 years ago. At another hospital the patient underwnt closed thoracostomy drainage for six months, and thereafter the symptom gradually aggrevated. Under the diagnosis of chronic tuberculous empyema, decortication was performed. The peel attached to the posterolateral aspect of chest wall, there was an area of soft tissues with pale-brown discoloration, extending to intercostal muscles, but sparing overlying muscles. The biopsy specimen of the lesion was pathologically diagnosed as diffuse, large T-cell non-Hodgkin`s lymphoma. A review of the literature showed that malignant lymphoma of this type have been reported exclusively from Japan but never from other countries including Korea. After 1 year of followup period, the patient is well and stable.

Dissecting Aneurysm of Aorta: report of a case (해리성 대동맥류 1례 보고)

  • Dissecting aortic aneurysm is a disease which is characterized by hemorrhagic intramural seperation of aortic wall and extension for varlng distances proximally, distally, or both from the site of the intimal tear. Most aortas show some type of medial degeneration most commonly described as cystic medial necrosis. DeBackey classified this disease according to involved aorta and site of intimal tear to 3 basic types, such as type I, II and III. Type III is defined that dissecting process arrises in the descending thoracic aorta just distal to origin of the left subclavian artery and extends distally for a varing distance. We expirienced a case of dissecting aneurysm, type III of DeBackey's classification which dissecting process is limited to the descending thoracic aorta in the Department of Thoracic and Cardiovascular Surgery, Kyungpook National University Hospital. This patient was 40 year old woman and she had suffered from intermittent sharp back pain for 3 years .before admission. Excision of the aneurysm and Dacron graft were placed successfully under the left atrio-femoral bypass with artificial pump. The hospital course was uneventful.

Relationship Between Presence of Dental Clinics in Public Health Facilities and Oral Health Conditions of Local Elderly People (농촌지역 보건기관 치과진료실 유무와 인근 노인의 구강보건실태 관련성)

  • Objectives: This study was conducted to find the relationship between oral health conditions of elderly people and closures of public dental health clinics in rural areas. Methods: Oral examinations and surveys were conducted in 2011 from May 11 through November 4 on 383 seniors over 65-years and under 74 years old. Results: The results of this study were as follows: 1. The results of comparative analysis of the usage behaviors of health clinics of subjects in relation to the closures of public dental clinics within the area showed that the usage level and frequency of public health agencies in areas with public dental clinics were high, and that the trend of influence on personal oral health conditions and improvement in prevention was high. When compared to 3 years ago, there was an 11.6 percentage point reduction in areas without public dental clinics (24.4%) compared to areas with public dental clinics (12.8%). 2. The results of comparative analysis of the oral health conditions and behaviors of subjects in relation to the closures of public dental clinics showed that the level of dental caries was high in areas with no public dental clinics, and the number of toothbrush usage and oral health supplemental product usage were shown to be high in areas with public dental clinics. Conclusion: The closures of public dental clinics were found to affect oral health behavior and conditions of elderly people in rural areas.

Does Age Influence the Poor Prognosis after Aneurysmal SAH Surgery in Elderly Patients?

  • Objective : The incidence of aneurysmal subarachnoid hemorrhage has been increasing. At the same time, surgical results for elderly patients are unsatisfactory and no guidelines of treatment are available. We carried out a study comparing variable factors and surgical results between young and old age groups to find ways to improve prognosis. Methods : A retrospective study was carried out on 754 patients who were operated on between 1990 and 2004 by the same surgeon in our hospital. The patients were divided into seven groups according to age : 93 patients below 40 years of age [Group I], 419 patients aged $40{\sim}59$ [Group II], 115 patients aged $60{\sim}64$ [Group III], 82 patients aged $65{\sim}69$ [Group IV], 28 patients aged $70{\sim}74$ [Group V], 12 patients aged $75{\sim}79$ [Group VI] and 5 patients over the age of 80 [Group VII]. We then checked their medical history, Fisher's grade, Hunt-Hess grade, postoperative complications, and Glasgow Outcome Scale. Results : Age was not a statistically significant factor among patients below 70 years of age [P $value{\ge}0.05$]. But for those aged 70 and older, the age factor had a statistical value [P $value{\le}0.001$]. In addition, there was a close correlation between Hunt-Hess grade IV and V patients, and those with vasospasm, and hydrocephalus after surgery, with poor prognosis in elderly patients as well as young patients [P $value{\le}0.001$]. Conclusion : Advanced age [under the age of 70] dose not precluded adequate surgical treatment in patient with AN SAH, and we should be also alert to preventable causes of delayed neurological deterioration for improving the outcome in all elderly groups.
