Subaortic Stenosis in Dogs. (개의 대동맥협착증)

  • Subaortic stenosis in a 2.9kg, 3-month-old pug dog and a 11 kg, 4-month-old Rott -weiler dog showing signs of dyspnea, tachypnea, anorexia, exercise intolerance and depression was diagnosed with electrocardiography, thoracic radiography and ultrasonographic evaluation. In electrocardiography, PR interval widening, R wave amplitude increase in lead II in case 2, ST segment depression and left axis deviation in case 1 and 2 were identified. In thoracic radiographs, enlarged left ventricle and atrium, caudal displacement of left crus of diaphragm and caudal part of cranial mediastinum widening were identified in ventrodorsal view. In lateral view, left ventricular enlargement, secondary dorsal deviation of trachea and post-stenotic dilation of aorta were seen. Also, edematous pulmonary changes were shown in peri-hilar area. In ultrasonographic examination, left ventricular dilatation, pericardial effusion and subaortic valvular ridge were seen with real time B-mode in right parasternal long axis view. Aortic valve regurgitation was identified in color-Doppler mode.

  • Ultrasound is a first line examination of the knee and useful diagnostic tool in assessing the various knee lesions. The advantages of ultrasound are that it is non-invasive, easily available at bed side, cheap, well accepted by patients, and that it has technical benefits including dynamic evaluation. Its limitations include reliability, which is largely operator dependent and its inability to evaluate deep structures. Technical guidelines of the ultrasound released by European Society of MusculoSkeletal Radiology (ESSR) and ultrasonographic findings of various knee lesions are described.

Hemangiosarcoma in a German Shepherd Dog (German Shepherd견의 혈관육종례)

  • A hemangiosarcoma in a 30 kg, 6-year-old German Shepherd dog showing signs of abdominal distension, anorexia and depression was diagnosed with clinicopathologic examination, abdominal radiographs and ultrasonographic assessmint. In abdominal radiographs, overall abdominal distension was seen. Stomach and some parts of small intestines were deviated caudo-ventrally by enlarged liver. The splenomegaly was also identified and descending colon was dislocated laterally by splenic mass. In ultrasonographic findings, abdominal fluid was identified. Hepatomegaly was seen and it was consisted of multiple, anechoic cysts. The spleen was enlarged and a large round mass with mixed echo pattern at the mid to tail portion of spleen was identified. Grossly, at necropsy, multi-sized cysts in which contained blood and fibrpus materials and some blood were seen in the cross section and it showed hard consistency. Also, lots of small red tumor nodules were dispersed on the serosal surface of the bladder, omentum, mesentery, diaphragm and peritoneum. In abdominal paracentesis, fluid having almost the same properties as circulating blood was identified. The hemoperitoneum was thought to be resulted from the bleeding into peritoneum owing to the rupture of cystic lesions located on the superficial liver area.

  • Ultrasonography is a powerful and useful method for the examination of the various shoulder diseases. The use of high-resolution transducer and technical evolution allowed the improvement of the accuracy of detection of the rotator cuff disease. However, its limitation is that there is marked disparity in the interpretation according to the operators' experience. This article describes the ultrasonographic findings of rotator cuff pathology compared to the MR (magnetic resonance) imaging and arthroscopic findings for helping the interpretation of ultrasonographic picture.

Radiographic and Ultrasonographic Diagnosis of Unilateral Ovarian Cysts (개에서 편측성 난소낭종의 방사선학적 및 초음파학적 진단 2례)

  • A female, 14-year-old mixed dog weighing 4 kg and a female,8-year-old schnauzer dog weighing 6 kg showing vaginal discharges and vomiting for a few weeks were examined for the preoperative check of anterior cruciate ligament rupture and the recheck after the surgery of nephrectomy. There were no distinct abnormal signs except for the increased CBC at schnauzer dog on blood chemistry analysis. Radiographic and ultrasonographic findings were a large abdominal mass and a cyst-like hypoechogenic or anechogenic cavitary lesion on the ovary region. These findings were consistent with an ovarian cyst. Thereafter highly suspected ovarian cysts by diagnostic imaging were supported by high estrogen concentration on hormonal assay and the results of Pathologic examination as the unilateral ovarian cysts with secondary pyometra.

Radiogyaphic and Ultrasonographic Diagnosis of Pyelonephritis in 2 dogs (개에서 신우신염의 방사선학적 및 초음파학적 진단 2례)

  • A female 8-year-old Pug weighing 7.3 kg and a female 10-year-old Maltese dog weighing 3.5 kg showing anorexia and vomiting for a few weeks were referred to Veterinary Medical leaching Hospital, Seoul National University. Radiographic findings were an enlarged right kidney in a pug dog and a radiopaque material on the right ureteral region lateral to the third lumbar vertebrae with indefinite right kidney contour in a Maltese dog, repectively. Excretory urography performed in a Pug dog revealed a poor opacified enlarged right kidney with absent of pelvic recesses and pelvic dilation with proximal ureteral dilation on contralateral kidney. Ultrasonographic findings were enlarged kidney with dilated pelvis and echogenic sediment within the medulla in both dogs and especially an engorged proximal ureter and a thin rim of functional renal tissue remains in a Maltese dog. Those diagnostic findings indicated high possibility of pyelonephritis and these were confirmed by pathologic examination. Radiography and ultrasonography, although not giving final diagnosis for pyelonephritis, are useful for assessment and diagnosis of pyelonephritis.

Ovarian Cancers in 2 Dogs (개에서의 난소종양 2예)

  • Two female Yorkshire terrier was referred to Veterinary Medical Teaching Hospital, Seoul National University. In case 1, clinical signs were vomiting, anorexia and weight loss. Cystic intraabdominal mass was identified in radiographic and ultrasonographic examination. In case 2, clinical sign was abdominal distension. In radiographic and ultrasonographic examination, cellular ascites and bilateral intraabdominal masses next to kidneys were observed. Many clusters of glandular epithelial cells with anisocytosis, anisokaryosis, high N:C ratio coarse chromatin and prominent nucleoli were shown in cytologic examination of sanguineous ascites. In all two cases exploratory laparotomy was performed and enlarged ovaries were observed (Rt: 6$\times$5$\times$5 cm and Lt: 3$\times$2$\times$1 cm in case 1 and 3$\times$2$\times$1 cm bilaterally in case 2). After ovariohysterectomy histopathologic examinations were performed. Histopathologically the masses were diagnosed as ovarian adenocarcinoma and ovarian cystadenocarcinoma in case 1 and case 2, respectively. In both dogs no further chemotherapy was carried out. These two dogs had no gross evidence of tumor recurrence at the time of 3 months after tumor resection, but long-term follow-up might be needed.

Usefulness of Ultrasonography in Diagnosis of Small Foreign Bodies (체내 소형 이물질 진단에서 초음파 검사의 유용성)

  • Purpose: The purpose of this study is to evaluate the usefulness of ultrasonographic examination for diagnosis and removal of small sized foreign bodies, which invaded extremity but difficulty to find on physical examination and/or plane radiographs. Materials and Methods: Since March, 2009 to February, 2012, we performed preoperatively ultrasonography and operation in 9 cases of foreign bodies of hand or foot. Mean symptom duration periods was 32 months. The location were fingers in 5, palms in 2, hand dorsum in 1, heel in 1 case. In 2 cases, foreign bodies were seen on plane radiographs. In the remaining 7 cases, foreign bodies could be diagnosed by ultrasonography only. Using high resolution stick probe, we performed evaluation on size, location, character of foreign bodies and compared preoperative ultrasonographic findings to intraoperative ones. Results: The site of foreign bodies in preoperative ultrasonography corresponded well with intraoperative findings. Foreign bodies were glass particles in 5, plant thorns in 4, pencil lead in 1. Mean size was 3.9 mm (2~7 mm). Conclusion: Ultrasonography is usefull evaluation tool for diagnosis and treatment in whom residual foreign bodies were suspected.

Usefulness of Ultrasonographic Examination in Diagnosis and Follow-up of the Frozen Shoulder (동결견의 진단과 치료 경과 판정에 대한 초음파 검사의 유용성)

  • Purpose: In order to know the effectiveness of ultrasonographic examination in diagnosis and follow-up evaluation for the frozen shoulder, we compared those with contralateral side in initial and after steroid injection. Materials and Methods: For the 20 unilateral frozen shoulder from July 2012 to July 2013, ultrasonographic examination was tried using reference line 1 (CBT: distance from coracoid process to LHB tendon) and line 2 (CBG: distance from coracoid process to bicipital groove). We tried 1 time steroid injection and compared the improvement in gross motion and reference line with 2 month, 4 month, and 6 month's serial ultrasonographic examination. Results: The differences on CBT line between lesion side and normal side were -5.6 mm, -5.0 mm, and -4.3 mm in neutral (Neut), external rotation (ER) and internal rotation (IR), respectively. The differences on CBG line were -6.1 mm, -4.7 mm, and -5.0 mm respectively (p<0.05). The changes in the reference line after steroid injection were evaluated at 2 month (CBT: -4.8 mm, -3.5 mm, -2.6 mm / CBG: -4.7 mm, -4.0 mm, -3.6 mm), 4 month (CBT: -4.7 mm, -3.2 mm, -1.7 mm / CBG: -4.3 mm, -3.7 mm, -1.2 mm), and 6 month (CBT: -1.1 mm, -2.9 mm, -0.5 mm / CBG: -1.2 mm, -0.7 mm, -0.9 mm). The gross motion was improved at 4 month after injection, from elevation $108^{\circ}$, ER $32^{\circ}$, IR L5 in initial to $133^{\circ}$, $42^{\circ}$, L3 respectively (p<0.05). Pain improved from 7.5 in initial to 3.0 at 2 month on visual analog scale (VAS). Conclusion: The serial examination after steroid injection revealed that the delayed improvement on reference line (6 month) compared with pain (2 month) or gross motion (4 month). Dynamic ultrasonogram was useful in diagnosis and follow-up evaluation of frozen shoulder.

  • Ultrasonographic examination in the obstetrics not only makes easy of the diagnosis of Hydrocephalus, placenta previa, anencephalus and ectopic pregnancy, etc, but also guaranteed its excellence in the diagnosis of women gynecology disease. Especially in case that it doesn't affect radiation hazard on the embryo, its efficiency keeps increasing and it is spotlighted to many clinic doctors. Even though in general hospital we often observe congenital hydrocephalus in Med. Dept. which entirly undergoes the medical examination to the insured, we don't observe even the women delivered with child and it is very difficult to find a rare congenital hydrocephlus throughout other present insurance companies. We inspected a congenital hydrocephalus, which other medical equipments of present insurance companies have many difficulties in the medical examination, accompanied by meningoencephalocele by ultrasonic diagnosis. We analyzed and compared the data by ultrasonic diagnosis with that of surgical diagnosis and the results are follows; 1) We could correctly describe meningoencephalocele by ultrasonic diagnosis. 2) It was able to be diagnosed in LMP 22 weeks. 3) BPD has about 10 mm higher than normal value in LMP 22 weeks. 4) Dilatation of lateral ventricle was observed in LMP 22 weeks. 5) As paralled with ultrasonic diagnosis in pregnancy medical examination, accepting declines including the risk rate can be restrained.
