Survey on Utilization of Ultrasonographic Machine in Small Animal Clinics in South Korea (전국 소동물 병원의 초음파 진단기기 및 활용도 현황조사)

  • This study was carried out to analyze the utilization of ultrasound machines in animal hospitals and focused on surveying the present condition of diagnostic ultrasound on veterinary medicine in South Korea. Total 279 veterinary hospitals were surveyed with e-mail questionnaires or telephone survey. E-mail questionnaires consist of 17 items of questions including existence of ultrasound machine, types of ultrasound machine, ultrasound examination costs, frequency, purposes, other diagnostic imaging equipments, and referring system of ultrasound. Telephone surveys asked about the existence of the ultrasound machine and types of the ultrasound machine to 279 animal hospitals. Two hundred and seventy-one out of 279 animal hospitals holds ultrasound machine. Seventy-two percents clinics purchased used ultrasonographic machines and mean years after the date of manufacture is 7.5 years and the proportion of superannuated machines are relatively high. Also many clinicians prefer single organ scanning rather than general scan technique and more than 60% of clinics perform ultrasonographic examination less than 5 times a week. Clinics located in Seoul area tend to have more expensive and brand-new ultrasonographic machines and the distribution of radiology specialist are higher in this area. Problems associated with the present condition were oversupply of machines, unequal distribution of the medical equipment in different localities, ineffective use of the medical equipment, and high percentage of old poor-quality medical equipments. There should be a viable alternative proposal to control amount and quality of the ultrasound machines. Also, the improved management system for the ultrasound machine is required.

Clinical Characteristics and Ultrasonographic Findings of Acute Bacterial Enterocolitis in Children

  • Purpose: This study clarified the bacterial pathogens currently causing acute infectious enterocolitis (AIE) in children and evaluated the clinical characteristics and ultrasonographic findings according to the different pathogens. Methods: Medical records regarding age, sex, clinical symptoms, laboratory data, identified enteropathogens, ultrasonographic findings, treatment, and outcome of 34 patients who were diagnosed with AIE via stool examination using multiplex polymerase chain reaction (PCR) or culture, were retrospectively reviewed. Results: Twenty-four patients (70.6%) were male. The mean age of the patients was $8.5{\pm}6.2$ (range, 1.1-17.1) years. Six bacterial pathogens were isolated: Salmonella species (spp.) (32.4%), Campylobacter spp. (20.6%), verotoxin-producing Escherichia coli (14.7%), Staphylococcus aureus (11.8%), Clostridium difficile (8.8%), and Shigella spp. (2.9%). Abdominal pain occurred in all patients regardless of pathogen. The patients infected with Salmonella were older than those infected with verotoxin-producing E. coli (p<0.05). C-reactive protein levels were higher in patients with Salmonella and Campylobacter infections than in those with verotoxin-producing E. coli infection (p< 0.05), the other clinical and laboratory data were indistinguishable between pathogens. Ultrasonography demonstrated diverse involvement of bowel segments according to pathogen. Wall thickening of both the ileum and the entire colon was the most common lesion site regardless of pathogen. Conclusion: Various bacterial agents cause AIE and the symptoms are diverse symptoms, however, all most children recovered spontaneously. Use of multiplex PCR on stool samples warrants improvement of its sensitivity for diagnosis of enteropathogenic bacteria. Ultrasonographic examination is useful for diagnosis of AIE; it can also detect the disease extent and severity.

  • The anatomical structures of foot and ankle are more superficial than other body structures. The ultrasonographic examinations are easy and useful diagnostic tools in the foot and ankle field. The interpretation of ultrasonographic findings depends on the observer's experiences of sonographical anatomy. Repeated ultrasonographic examination of the foot and ankle can improved the skills in diagnosing the disease of the foot and ankle.

  • 5 years Poodle was referred to the Veterinary Teaching Hospital, Chonnam National University in January 1997, with progressive swelling on the rear xiphoid process after cesarean section last year in local-vet clinic. Physical, radiographic, ultrasonographic and intraoperative findings that were obtained from this case were as follow; Physical findings were known as no pain, no fever in the hernia] sal at palpation. Radiographic findings revealed the decreased density of swelling contents more than peritoneum and did not identify the abdominal wall defect line. Ultrasonographic findings showed abdominal wall defect line of 3mm and intra-abdominal originated fat contents with mixed echo(hypoechoic). During the operation, falciform ligament into the hernial sac was observed and not excised, manipulated back into the cavity. The dog was given an herniorraphy with no recurrence and infection.

Ultrasonographic and radiographic study on experimental cystitis in rabbit (실험적 방광염 유발 토끼에서의 초음파 및 방사선학적 연구)

  • To investigate diagnostic imaging of cystitis and to apply it to the small animal practice, ultrasonogram of urinary bladder with moderate distension(4ml/kg) and with complete distension(9ml/kg) to evaluate the irregularity and thickness of bladder, radiography and histopathological examination were performed after experimental cystitis induction. On double contrast cystography, mucosal membrane of the urinary bladder was smooth and thickening urinary bladder wall was not found before cystitis induction. At 3rd day post induction(PI), mucosal irregularity was noted at the cranioventral region of the urinary bladder. Thickening of the urinary bladder wall and filling defect was observed as well. Cystographic findings of 7, 10, 15, 21 day PI were similar to that of 3rd day PI. On ultrasonographic findings of the mural thickness in induction group, it was ascertain that the mural thickness with moderate distention was more thickened than with complete distention at transverse scan. Ultrasonographic findings at longitudinal scan were similar to those of transverse scan. On ultrasonographic findings of mucosal irregularity in induction group, from PI to 7 day PI, mucosal irregularity with moderate distention was more irregular than mucosal irregularity with complete distention. At 10 day PI, there was similarity between moderate distention and complete distention. On histopathologic examination of a section of urinary bladder taken 3 day PI, the mucosal and submucosa were infiltrated by a mixture of thick inflammatory exudate which was composed of neutrophil, plasma protein, bacterial colony and necrotic cells. Congestion, hemorrhage and edema were also observed in the submucosa. At 7th day PI, the mucosal change was similar to that of 3rd day PI, but neovascularization and fibroplasia were observed in the submucosa. At 15th and 21th day PI, mild hyperplasia of mucosal epithelium was observed in the mucosa. Deposition of collagen, neovascularization and severe diffuse infiltration of lymphocyte were observed. These results suggest that ultrasonographic examination with moderate distention is considered to be a more simple, rapid, noninvasive, sensitive and useful diagnostic method than other diagnostic methods for the diagnosis of the cystitis and the differential diagnosis of urinary tract infection.

Diagnostic Efficacy of Fine Needle Aspiration Cytology, Frozen section Biopsy and Ultrasonography for a Palpable Thyroid Mass (갑상선 촉지 종괴에서 시행한 세침흡인세포검사법, 동결절편검사법 및 초음파검사법의 진단적 유용성)

  • Objectives: This study was done to determine the diagnostic efficacy of fine needle aspiration cytology (FNA), frozen section biopsy (FSx) and ultrasonography in a palpable thyroid mass. Materials and Methods: During the period from July 1999 to March 2002, the medical records of 98 patients who underwent a thyroidectomy for thyroid mass, at the Department of Surgery, Chonbuk National University Hospital, were reviewed retrospectively. Ninety eight cases were classified according to whether the FNA cytological diagnosis inadequate, benign, suspicious, or malignant and the FSx diagnosis benign or malignant and the ultrasonographic examination was benign, suspicious, or malignant. The diagnostic correlations of ultrasonography, FNA cytology, frozen section, and both FNA cytology and frozen section with definite histologic diagnosis were evaluated. Results: According to the FNA cytological interpretation, 76 cases were diagnosed as benign, 6 cases suspicious, 13 cases malignant, and 3 cases inadequate. The sensitivity and specificity for FNA were 86.3% and 95.9% and for FSx 95.5% and 100% respectively. The diagnostic accuracy of FNA and FSx were 93.5% and 98.9% respectively. Based on a definite histologic diagnosis, the ultrasonographic examination had a sensitivity and a specificity of 77.2% and 78.9%. Conclusion: The results confirm that FNA cytology is a reliable and useful tool in the initial evaluation of a palpable thyroid mass compared to the ultrasonographic examination. Intraoperative FSx is a valuable diagnostic procedure to confirm the cytological diagnosis with undetermined or unsatisfactory cytological diagnosis.

Studies on the Ultrasonographic Images for the Early Pregnancy Diagnosis in Horses (말에서 조기 임신진단을 위한 동음파영상 이용 방법에 관한 연구)

  • To investigate the estrual changes of ovaries, appropriate time of mating, diagnosis of early pregnancy after mating, and diagnosis of twin fetuses, ultrasonography was performed in 61 thorough-bred mares (5-12 years old) which had been raised in Chrju island. The followings are the results obtained: 1. The average size of a follicle of mares was $3.2{\times} 3.8cm$ at the beginning of estrus and $4.2{\times} 3.5 cm$ from the middle of estrus to ovulations respectively, and the average size of a follicle and a corpus luteum was $1.8{\times} 1.4 cm and 3.1{\times}2.3 cm$ in the middle of diestrus, respectively. 2. In the result of pregnancy according to mating time after ultrasonographic examination of follicle size, the mares having a follicle of 4.6-5.0 cm (diameter) were mated on the day of ultrasonographic examination or the next day rind all were conceived (3 mares) and another mares having a follicle of 4.1- 4.5cm (diameter) on the 1st or 2nd day after examination and all were conceived (4 mares). 3. By ultrasonographyi early pregnancy in the mares was diagnosed from 12 days of pregnancy and the equine fetus was directly detectable from 23 days of pregnancy, whereas the heartbeat of a fetus was detectable from 28 days of pregnancy. 4. Diagnosis of earl pregnancy of twin was possible from 16 days of pregnancy. These result indicated that ultrasonography is applicable in mares to diagnose the estrual changes of ovaries and the time of ovulation or mating, and to diagnose early pregnancy from 12 days of pregnancy and early pregnancy of twin fetuses.

Ultrasonographic Evaluation of the Bone Beating of the Experimentally Induced Bone Defect in Dogs (개에서 실험적 골결손 치유 반응에 대한 초음파 평가)

  • This study was performed to evaluate the usefulness of gray-scale and power Doppler ultrasonography, and to compare with radiography for detection of the repairing in experimentally induced bone defects in dogs. In 4 adult beagle dogs bilateral bone defects were created in 8 canine femurs as sized as 5 mm diameter. Mean detection time of the ultrasonographic endosteal callus formations(mean $14.25{\pm}2.31$ days) was significantly shorter than that of the radiographic opacity chanees(mean $23.50{\pm}2.27$ days) in the defected region. Mean time of the neovascularizd flow signal(mean $6.00{\pm}3.59$ days) from the power Doppler ultrasonographic examination was significantly shorter than that of gray-scale ultrasonographic findings. With these results, gray-scale ultransonography and power Doppler ultrasonography can be used for an early detection modality for bone healing.

Prenatal ultrasonographic diagnosis of cleft lip with or without cleft palate; pitfalls and considerations

  • Ultrasonographic examination is widely used for screening of abnormal findings on prenatal screening. Cleft lip with or without cleft palate of the fetus can also be screened by using ultrasonography. Presence of abnormal findings of the fetal lip or palate can be detected by the imaging professionals. However, such findings may not be familiar to oral and maxillofacial surgeons. Oral and maxillofacial surgeons can use ultrasonographic imaging of fetal cleft lip with or without cleft palate to provide information regarding treatment protocols and outcomes to the parent. Therefore, surgeons should also be able to identify the abnormal details from the images, in order to setup proper treatment planning after the birth of the fetus. We report two cases of cleft lip with or without cleft palate that the official readings of prenatal ultrasonography were inconsistent with the actual facial structure identified after birth. Also, critical and practical points in fetal ultrasonographic diagnosis are to be discussed.
