Foot Ulcer Risk, Foot Care Knowledge, and Foot Care Practice in Patients with Type 2 Diabetics (제2형 당뇨병 환자의 발궤양 위험, 발관리 지식 및 발관리 수행)

  • Purpose: The purpose of this study was to investigate foot ulcer risk factors, foot care knowledge, and foot care practice in patients with type 2 diabetes. Method: One hundred fifty type 2 diabetic patients were in and out-patients in a large urban hospital. The data were collected using a self-report questionnaire, chart review and foot examination. The questionnaires were developed by the researchers through the experts consultation and literature review. High risk for foot ulcer was evaluated by peripheral neuropathy(PN), peripheral vascular disease(PVD), and prior foot ulcer. Foot risk scores(FRS) means numbers of present risk factors. Results: 31.3% of subjects show 1 FRS, and 13.3% showed 2 FRS. Mean foot care frequency was 3.5 times per week. There were significant differences in foot care knowledge according to DM education (t=2.96, p=.004) and foot care education (t=3.65, p=.001). There were significant differences in the foot care practice activities according to duration of DM (t=3.48, p=.010) and educational levels. Conclusion: There were high proportion of foot ulcer risk among the patients. It is necessary to screen high risk foot ulcer patients and provide practical education for foot care practice of diabetic patients.

Development of Experimental Animal Model with Duodenal Ulcer for Nutritional Study and its Nutritional Characteristics (영양실험을 위한십이지장 궤양 흰쥐 실험 모델 개발과 그 영양생리적특성)

  • This study aimed to develop an experimental animal model of duodenal ulcer for application to nutrition, and to verify the nutritional characteristics. The 7 different doses and 8 different injections of cysteamine and 2 kinds of diets with different casein levels(10%, 20%) were tested to examine the incidence and the process of duodenal ulcer in female Sprague-Dawley rats. The result showed that the duodenal ulcer was induced when rats fed 10% casein diet(for 9 days) were injected 6 times with 13mg/100g BW cysteamine. The first injection was conducted after 24 hours of fasting, followed by the 2nd shot after 3 hours. The following shots were repeated every 3 day(4th and 7th days). Duodenal ulcer was observed 3 days after the last injection by optical microscope and the naked eye. The characteristics of nitrogen bioavailability of duodenal ulcer model were as followings : 1) gastric emptying rate was faster, 2) trypsin activity at duodenum was higher, 3) total protein concentration at serum and nitrogen retention rate were lower than the control. Free Tissue Transfer in the Treatment of Infected Diabetic Foot Ulcers (유리 조직 이식술을 이용한 당뇨병성 족부 궤양의 치료)

  • Diabetic foot ulcer is a serious complication which result from long-standing diabetes. Especially, severe infected diabetic foot ulcer results in unwanted lower extremity amputation. The diabetic patient is considered the relative contraindication for microsurgery because of the severe peripheral vascular disease. Recently, microvascular free tissue transfer technique applied to diabetic foot ulcer. It is well known that free tissue transfer provides immediate soft tissue coverage and control of infection. So it is possible that preservation of the lower extremity through free tissue transfer. A retrospective study of diabetic patients who had infected foot ulcer from 1999 to 2000 with foot defects reconstructed with free tissue transfer were reviewed. Thirteen patients were studied with mean follow-up of 12.7 months. There were two deaths during follow-up period. There were two failures after free flap surgery. All eleven survived patients were ambulatory. There was no recurrence of ulcer. No patient need amputation above the ankle joint. We have found that free tissue transfer for infected diabetic foot ulcer is very effective surgical technique. Careful patient selection and regular follow-up is important.

Effects of Protein and Protein Hydrolysate on Nitrogen Metabolism in Rats with Cysteamine-induced Duodenal Ulcer (Cysteamine에 의해 유도된 십이지장 궤양 흰쥐의 체내질소대사에 대한 단백질과 단백질 가수분해물의 섭취효과)

  • This study aimed to verify the nutritional and curative effects of protein hydrolysate in rats with cysteamine-induced duodenal uncer. Duodenal ulcer rat model was established by intraperitoneal injections of cysteamine. Sprague-Dawley, female rats weighing approximately 200g were intraperitoneally injected twice cysteamine(13mg/100g BW) at intervals of 3h per day. This procedure was repeated 3$\times$at intervals of 3d. Animals fed on 10% casein diet for infection periods. After last injection, 4 kinds of diets(10% casein, 20% casein, 10% casein hydrolysate, 20% casein hydrolysate) were given. Gastric montility, trypsin activity in gastrointestinal content, retention rate of nitrogen, plasma total protein, albumin, amino-N, urinary urea nitrogen, creatinine and hydroxyproline were analyzed for nutritional effects of dietary nitrogen levels(10%, 20%) and sources(casein, casein hydrolysate). In duodenal ulcer rat model, there was no differences between 20% casein diet and 20% casein hydrolysate in the view of severeness of ulcer, gastric emptying rate, serum total protein, serum albumin, plasma $\alpha$-amino-N, UUN, creatinine excretion, GFR, nitrogen retention. On the other hand, rats on 10% casein hydrolysate diet group had more curative effect of the ulcer, higher plasma albumin concentration and nitrogen retention than 10% casein diet group. The casein hydrolysate diet group was lower trypsin activity in small intestinal content than the casein diet group, at both nitrogen levels(10%, 20%). The results suggest that protein hydrolysate be applied in diet therapy for the patients with gastrointestinal ulcer.

Effect of Mugwort on Inhibition of the Duodenal Ulcer Induced by Cysteamine Administration (Cysteamine에 의한 십이지장 궤양 발생에 쑥 첨가 식이가 미치는 영향)

  • This study was performed to investigate the influcence of mugwort on the duodenal ulcer induced by cysteamine administration in rats. Five groups of rats were fed each experimental diet containing 0%, 5%, 15%, 30% of mugwork powder for 10 weeks. Duodenal ulcer was induced by cysteamine injection (400mg/100g body weight) after 10 weeks of feeding experimental diets (C-0, C-5, C-15, C-30). Control animal that fed 0% mugwork powder added diet were injected saline (S-0) to compare with cysteamine injected groups. When the duodenal ulcer induced by cysteamine-HCI administration, all animals in the C-0 group formed erosion and perforating ulcer was found in 44% of animals. Higher the added mugwork ratio, more inhibited of the duodenal ulcer induced by cysteamine administration (C-5, C-15). But when the ratio of added mugwort is 30%, the inhibition effect disappeared (C-30). The alkaline phospatase activities were lower at the duodenal mucosa and small intestinal mucosa in the cysteamine treated groups(p<0.05). The acid phophatase activities were higher at the stomach, small intestine and large intestine of the cysteamine treated groups. But in mugwort added diet group, the changes of enzyme activites were lessended. The maltast activities were lower at the duodental mucosa and small intestinal mucosa of cysteamine treated groups. But in mugwort added diet group, maltase activites were recovered.

Ginsenoside $Rb_1$: the Anti-Ulcer Constituent from the Head of Panax ginseng

  • We previously reported that the butanol (BuOH) fraction of the head of Panax ginseng exhibited gastroprotective activity in peptic and chronic ulcer models. In order to identify the active constituent, an activity-guided isolation of the BuOH faction was conducted with a HCI$.$ethanol-induced gastric lesion model. The BuOH fraction was passed through a silica-gel column using a chloroform-methanol gradient solvent system, and six fractions (frs. 1-6) were obtained. The active fr. 5 was further separated by silica-gel column, to yield 6 subfractions (subfrs. a-f). Subfr. d was composed of ginsenosides Re, Rc and $Rb_1$. The most active constituent was ginsenoside $Rb_1$ ($GRb_1$), a protopanaxadiol glycoside, which was investigated for its anti-ulcer effect. Gastric injury induced by HCI$.$ethanol, indomethacin and pyloric ligation (Shay ulcer) was apparently reduced with oral $GRb_1$ doses of 150 and 300 mg/kg. $GRb_1$ at these dosage significantly increased the amount of mucus secretion in an ethanol-induced model. The anti-ulcer effects were consistent with the result of histological examination. These results suggest that the major active constituent in the head of Panax ginseng is $GRb_1$ and that anti-ulcer effect is produced through an increase in mucus secretion.

Perceived Importance, Educational Needs, Knowledge and Performance Concerning Pressure Ulcer Care by Clinical Nurses (임상간호사가 지각한 욕창 간호의 중요도, 교육요구도와 지식 및 수행과의 관계)

  • Purpose: To examine pressure ulcer care from the standpoints of perceived importance, educational needs, knowledge and performance among clinical nurses. Methods: This descriptive study was conducted with a convenient sample of 289 nurses from four hospitals. Questionnaire data collected from June to August 2008 was analyzed using descriptive statistics, t-test, ANOVA, and Pearson's correlation coefficients. Results: The mean scores of importance and educational needs concerning pressure ulcer care were high, performance was moderate, and knowledge was low. Knowledge level differed significantly according to career, performance differed significantly according to pressure ulcer care education and the main healer(the person primarily responsible for treatment). Perceived importance differed significantly according to the main healer, and educational needs differed significantly according to working area and the main healer. Significant correlations were evident between performance and importance, performance and educational needs, importance and knowledge, and importance and educational needs. Conclusion: Focus on perceived importance and educational needs to clinical nurses about pressure ulcer care may be beneficial to improve knowledge and practice of pressure ulcer care, and should be reflected in the development of effective programs to enhance relevant nursing skills.

  • Anti-ulcer activity of licorice alkaloid fraction, separated from Glycyrrhiza glabra L. by fractionation according to general alkaloid purification procedure, were studied with various experimental gastric ulcers. In Shay ulcer, licorice alkaloid fraction inhibit significantly response rate and severity of ulcer. But there was no significant changes in volume and pH of gastric contents. A male rat was fixed on a board and immersed up to breast in water of $25^{\circ}C$ for 20 hrs. Hemorrhage and erosion develop in all the animals on the mucosa of the glandular portion of stomach. Licorice alkaloid fraction (5 mg/kg, 10 mg/kg) inhibit significantly in the severity of stress ulcer but there was no changes in the response rate. Chlorpromazine produce marked inhibition in the severity and response rate of ulcer It was suggested that at least the anti-ulcer activity of licorice alkaloid fraction was not mediated by central depression.

  • Because of advancement of medical treatment, surgical management of gastric or duodenal ulcer was indicated for treatment of perforation, massive hemorrhage and obstruction. The distal gastrectomy including ulcer was known as principle method of duodenal ulcer obstruction, but actually many surgeons have performed only bypass surgery for the difficulty of formation of duodenal stump. In our case, 61-year-old male with repetitive duodenal ulcer obstruction transferred with obstruction due to deformities and inflammations of duodenal ulcer. We had performed totally laparoscopic distal gastrectomy with ROUX-EN-Y reconstruction using the clear visibility of laparoscopy and fine dissections of harmonic scalpel. The patient started soft diet on postoperative day 5 and discharged on postoperative day 8. He returned to work after discharging immediately.

A study of dietetic on the gastric & duodenal ulcer (위.십이지장궤양의 식요방법(食療方法)에 관한 연구)

  • Objectives : This research examined about dietary therapy on the gastric & duodenal ulcer based on the literary findings, for clinical treatment and prevention of the gastric & duodenal ulcer. Methods : This Research examined on histories, compositions, applications, and effects of dietetic on about 200 cases of dietary therapy of the gastric & duodenal ulcer from recent Chinese literatures Results : 1. Various vegetables, animals and mineral materials including herbs, grains, vegetables, fruits, food and drink were used for the dietary therapy. 2. Methods of the preparation for use as therapeutics were decoction, pulvis, gruel, medicinal wine, cake, tea, paste and gelatin and etc. 3. Frequently used materials were cuttle fish bone, Bletillae rhizoma, oyster shell, egg shell, sugar, aloe, licorice, lily, red jujube, and pig stomach. 4. Four properties of cold, warm, cool and hot were equal in frequencies. Most common tastes were sweet and bitter. Conclusion : Though dietary therapy for the gastric & duodenal ulcer is not based on clinical or experimental data, but through experience. It is mostly based on Yin-Yang and five elements, visceral manifestation, channels and their collateral channels and chinese herbal medicine theories. If we use them properly according to oriental medicine method, it will be effective on treating and preventing the gastric & duodenal ulcer.
