Depressive Disorders among Hansen Disease Patients Living in a Collective Farm (한 집단 농원 한센병 환자들의 우울장애)

  • Objectives: Depression is a major health concern that can be life threatening if not recognized and treated early. However, there is few report on the depressive disorder of Hansen disease patients in Korea. Therefore, the authors executed this study in order to check factors related to a depressive disorder of a Hansen disease patients and compare with factors to reach to a depressive symptoms of ordinary people with studying their life state and the trouble that Hansen disease patients were currently experiencing Method: The authors surveyed depressive symptoms using self-reported questionnaires in 74 Hansen disease patients and 84 controls. The severity of depressive symptoms was measured using Korean Form of Geriatric Depression Scale (KGDS) score. Result: Positive rate of depressive disorders among Hansen disease patients was 70.3% and that the referents was 31.0%. There is significant difference positive rate of depressive disorders between Hansen disease group and the referents in the factors such as gender, age, frequency of going out, familial type, and familial income. Depressive disorder of Hansen disease group was associated with sex, familial income. According to the multiple logistic regression, the odds ratios of the Hansen disease group versus referents, gender, familial income, frequency of going out were significant (p<0.05). Conclusion: Hansen disease patients had statistically significant higher depressive symptom score than the referents. Also, Hansen disease patients who have lower familial income were more likely to have depressive disorders. Therefore, Economical supports and policy are required for the Hansen disease patients.

  • This study shows that the Warsaw Convention in Article 1 is not an international transport, origin, destination and all the Contracting Parties is not a purely domestic shipping does not apply to this Treaty.   • The increase of traffic over a bridge has been emerged as one of the most severe problems in view of bridge maintenance, since the load effect caused by the vehicle passage over the bridge has brought out a long-term damage to bridge structure, and it is nearly impossible to maintain operational serviceability of bridge to user's satisfactory level without any concern on bridge maintenance at the phase of completion. Moreover, bridge maintenance operation should be performed by regular inspection over the bridge to prevent structural malfunction or unexpected accidents front breaking out by monitoring on cracks or deformations during service. Therefore, technical breakthrough related to this uninterested field of bridge maintenance leading the public to the turning point of recognition is desperately needed. This study has the aim of development on automated inspection system to lower surface of bridge superstructures to replace the conventional system of bridge inspection with the naked eye, where the monitoring staff is directly on board to refractive or other type of maintenance .vehicles, with which it is expected that we can solve the problems essentially where the results of inspection are varied to change with subjective manlier from monitoring staff, increase stabilities in safety during the inspection, and make contribution to construct data base by providing objective and quantitative data and materials through image processing method over data captured by cameras. By this system it is also expected that objective estimation over the right time of maintenance and reinforcement work will lead enormous decrease in maintenance cost.

The Change of $SaO_2$, PFT and ABGA During the Bronchofiberscopy (기관지 내시경 검사에 따른 산소 포화도, 폐기능 및 동맥혈 가스의 변화)

  • Background: Bronchofiberscopy is a procedure with a chance of airway irritation and it may cause pathophysiologic changes of respiratory system. So we tried to evaluate the influence of bronchofibercopy on $O_2$ saturation, ABGA and PIT by patient's basal status and procedure type. Method: $O_2$ saturation was measured every 1 minute from the left index finger tip with percutaneous oximetry. ABGA was done before and right after the bronchofiberscopy and PIT was done before and within 10 minutes after the bronchofiberscopy. Results: The mean time for bronchofiberscopy procedure was 14.5mim and $SaO_2$ maximally fall to 89.0 below 8% of the baseline after mean time of 8.4min, which was recovered at the end of the procedure. $SaO_2$ change amount was 8.4 % on Non-$O_2$ supply group, which was lower compared to 6.4 % of the $O_2$-supply group without statistically significance. Biopsy Group and BAL group showed more $SaO_2$ fall than washing only group. The level of $PaO_2$ and FEV1 of the patient didn't influence significantly on $SaO_2$ fall during the procedure. ABGA taken before and after the bronchofiberscopy showed mild fall of $PaO_2$ and mild rise of $PaCO_2$. Whereas PFT showed decrease of FEV1(P<0.05) and increase of RV without changes in airway resistance and pulmonary diffusion capacity. Comparing before and after the bronchofiberscopy, the washing group showed no significant changes on PIT, while the biopsy group and the BAL group showed increase of RV & decrease of $FEV_1$ after the bronchofiberscopy. BAL group showed more changing tendency rather than biopsy group although not statistically significant. Conclusion: Bronchofiberscopy is considered as a relatively safe procedure, but it would be better to be done with $O_2$ supply especially in the patient with low $PaO_2$ and in the case of biopsy and BAL.

Breeding Site Characteristics of Styan's Grasshopper Warblers(Locustella pleskei) on Mara Islet, Jeju Province, Korea (마라도에 번식하는 섬개개비의 번식지 특성)

  • Styan's Grasshopper Warblers (Locustella pleskei) are vulnerable species distributed in East-Asia only. Its known breeding sites in Korea are remote islets including Hongdo Islet, Chilbal Islet, Mara Islet, and Chuja Islets, and, therefore, the breeding biology of this species is still poorly known. This study was conducted on Mara Islet (N $33^{\circ}$ 06', E $126^{\circ}$ 16') from May to September 2008 to investigate the breeding status and breeding site characteristics of the grasshopper warblers. A total of 11 breeding pairs and their nests were found on trees and shrubs at artificially planted forests and hedges of Pinus thunbergii. The grasshopper warblers preferentially utilized the shrub trees for nesting places, and major nesting trees were Camellia japonica, Pittosporum tobira and Pinus thunbergii as nesting trees. Average heights of nesting trees and nests were $2.77{\pm}1.10m$ and $1.75{\pm}0.56m$, respectively. The grasshopper warblers selected lower shrubs and trees for nesting than randomly selected ones around them, probably to avoid strong and prevailed winds in flat and un vegetated environments on Mara Islet. The shape of nests was a round bowl-type, and measurements of nests were $11.9{\pm}0.5cm$ in exterior nest diameter, $11.1{\pm}1.1cm$ in height of exterior nest, $5.8{\pm}0.4cm$ in interior nest depth, and 6.0cm in interior nest diameter. It incubated eggs until the early August on Mara islet, and incubation periods of Mara Islet was possibly later than that of other areas. Furthermore, the clutch size in the study area was three, and they laid smaller number of eggs than normal clutch size (4~5 eggs) reported in other areas. Although we could not observe any nest predator on this species in the study area, selective cutting and pruning of trees will diminish dense shrub layer of forests. Therefore, it may affect the breeding of this threatened species which prefers dense shrubs of artificially planted forest of Pinus thunbergii. This study suggests that detailed and consistent further research on breeding biology and habitats of the grasshopper warblers are needed to conserve and manage of Pinus thunbergii forests on Mara Islet as an important breeding site of Styan's Grasshopper Warblers.

The Comparison of Video Assisted Thoracic Surgery (VATS) with 10 mm Thoracoscopy to 2 mm Thoracoscopy for Primary Spontaneous Pneumothorax (원발성 자연 기흉에서 10 mm와 2 mm 비디오 흉강경 수술의 비교)

  • Background: The video-assisted thoracic surgery (VATS) with 2 mm thoracoscopy in primary spontaneous pneumothorax (PSP) was known to be unreliable in its accuracy and recurrence rate. We compared 10 mm VATS with 2 mm VATS in the results of operation. Material and Method: From Sept. 1998 to Dec. 2002, 176 cases (10 mm VATS; 73 cases, 2 mm VATS; 103 cases) of PSP were treated by VATS blob resection at Korea University Ansan Hospital. 10 mm thoracoscope, 5 mm port, and 5 mm instruments were used in 10 mm VATS group, and 2 mm thoracoscope, 2 mm ports and 2 mm instruments used in 2 mn VATS group. In the two groups, staples were inserted through 11.5 mm port for chest tube. Result: The mean follow-up duration was 20,8$\pm$16.1 months in 10 mm VATS group, and 13.9 $\pm$8.2 months in 2 mm VATS. The most common indication of operation was a recurrent pneumothorax ($34\%$) in 10 mm VATS and patient's desire ($40\%$) in 2 mm VATS, respectively. The operation time, number of staples used in operation, postoperative chest tube keeping days, postoperative total amount of drainage, and postoperative hospitalization days were statistically lower in 2 mm VATS. Other significant variables affecting the operation time in linear regression analysis were the number of staples that used in operation, the presence of pleural adhesion, and type of pleurodesis and thoracoscope used in operation. However, $R^2$ values were lower than 0.1. The postoperative recurrence rate was $2.7\%$ in 10 mm VATS and $2.9\%$ in 2 mm VATS. It was not significant statistically. Recurrent cases developed within 1 year in both groups but the difference was statistically insignificant. Conclusion: Although there were differences in follow-up duration between two groups, the operation time, number of staples that used in operation, postoperative chest tube keeping days, postoperative total amount of drainage, and postoperative hospitalization days were statistically lower in 2 mm VATS. And in 2 mm VATS, there were no technical difficulties during operation and no differences in recurrence rate from 10 mm VATS. As a result, we suggest that 2 mm VATS can be used in the treatment of PSP.

A Study on the Structural Characteristics and Estimation of Refrigerating. Load for the Fruit Storage (청과물저장고의 구조특성 및 냉각부하량 산정에 관한 연구)

  • This study was intended to provide the basic design creteria for the refrigerated storage, and to estimate the required optimum capacity of refrigerator for the different sizes and kinds of the existing fruit storage. The structural characteristics of the existing fruit storages in Pyungtaek-khun of Kyungki-do were surveyed. The average out-door air temperature during the expected storage life after harvesting, was obtained by analyzing the weather information. The heat transfer rates through the different models of storage walls were estimated. The refrigerating load required for different models of fruit storage was analyzed in the basis of out-door air temperature. The results obtained in this study are summarized as follows: 1. The fruit storages surveyed were constructed on-ground, under-ground and sub-ground type buildings. The majority of them being the on-ground buildings are mostly made of earth bricks with double walls. Rice hull was mostly used as the insulating materials for their walls and ceilings. About 42% of the buildings were with the horizontal ceiling, 22% with sloped ceiling, and about 36% without ceiling. About 60% of the storage buildings had floor without using insulated material. They were made of compacted earth. 2. There is no difference in heat transfer among six different types of double walls. The double wall, however, gives much less heat transfer than the single wall. Therefore, the double wall is recommended as the walls of the fruit storage on the point of heat transfer. Especially, in case of the single wall using concrete, the heat transfer is about five time of the double walls. It is evident that concrete is not proper wall material for the fruit storage without using special insulating material. 3. The heat transfer through the storage walls is in inverse proportion to the thickness of rice hull which is mostly used as the insulating material in the surveyed area. It is recommended that the thickness of rice hull used as the insulating material far storage wall is about 20cm in consideration of the decreasing rate of heat transfer and the available storage area. 4. The design refrigerating load for the on-ground storages having 20 pyung area is estimated in 4.07 to 4.16 ton refrigeration for double walls, and 5.23 to 6.97 ton refrigeration for single walls. During the long storage life, however, the average daily refrigerating load is ranged from 0.93 to 0.95 ton refrigeration for double walls, and from 1.15 to 1.47 ton refrigeration for single walls, respectively. 5. In case of single walls, 50.8 to 61.4 percent to total refrigerating load during the long storage life is caused by the heat transferred into the room space through walls, ceiling and floor. On the other hand, 39.1 to 40.7 percent is for the double walls. 6. The design and average daily refrigerating load increases in linear proportion to the size of storage area. As the size increases, the increasing rate of the refrigerating load is raised in proportion to the heat transfer rate of the wall. 7. The refrigerating load during the long storage life has close relationship to the out-door air temperature. The maximum refrigeration load is shown in later May, which is amounted to about 50 percent to the design refrigerating load. 8. It is noted that when the wall material having high heat transfer rate, such as the single wall made of concrete, is used, heating facilities are required for the period of later December to early February.

Effect of Thresher Drum-Speed on the Quality of the Milled Rice (탈곡기의 급동 속도가 도정 손실에 미치는 영향)

Threshing under an increased drum speed would give a high performance rate, which is the general practice in custom work threshing in association with the use of semiauto-t hreshers. In the connection, however, it may result in the promotion of grain cracks and brokens of the rice product after milling. No reference or determination for an opti mum drum speed of the thresher is made available for various grain moisture contents at the time of the threshing operation and for different rice varieties especially for the Tongil rice varieties. This study was Conducted to find out and determine effects of the drum speeds on grain losses. The grain loss was quantified in terms of recovery rates of rice grains after treatments. Samples of each of all treatments were taken from the grain sampling plate placed in the grain conveyor of threshers. The grain sample plate was specially provided for this experiment. The brown-rice, milling, and head-rice recJveries were tes ted in the laboratory mill, respectively. Two rice varieties, Akibare and Suweon 251, each with five levels of different moist\ulcornerure contents at harvest and six levels of different drum speeds of threshers, were selected and used for treatments in this experiment. Two conditions of materials were tested in the thresher. One condition was to thresh the experimental material immediately after cutting, referred to as the wet-material thr eshing in this study. The other was to thresh the experimental :material, dried to contain about 15-16 percent of the grain moisture under the shocking operation. This is referred to as the dry-material threshing in this study. In additioon, field measurements for the grain moistures and drum-sdeeds under actual operation practices of the traditional field threshing, were conducted with a view to comparing with results of the experimental treatments. The results of the study may be summarized as follows: 1. For threshing treatments of Japonica-type rice variety (Akibare) , the effect of drum speeds and levels of grain moisture at cutting upon brown-rice, milling, and head-rice recoveries were found statistically significant. No significant difference in these recovery rates was noticed regardless of whether the material was threshed right after cutting or after drying by the shocking operation. 2. For the Tongil-sister rice variety(Suweon 251), milling recovery for the varied drum-speed and the grain~moisture level at cutting was found statististically significant. Th milling recovery was much significant when associated with the wet-material thres\ulcornerhing compared to the dry-material threshing. 3. The optimum peripheral velocity to be maintained at the edge of teeth on the thr\ulcorneresher drum was determined and may be recommanded as that of about 12 to 13 meters per second in view of the maximum recovery rate of the milled rice. 4. The effect of the drum speed on the qualitative loss of the milled rice was much greater in the case of the Tongil variety than Japonica. This effect was also greater by the wet-material threshing than by the dry-material threshing. Therefore, to apply the wet-material threshing operation for the Tongil variety, in particular, it should be very important to introduce the kind of threshing technology which would maintain the drum speed at optimum. 5. A field survey for the actual drum speed of threshing operations for 50 threshers indicated that average peripheral velccity was 12.76m/sec., and that the range was from 10.50 to 14.90m/sec. Approximately, more than 30% of the experimented and measured threshers were being operated at speeds which exceeded the optimum speed determined and assessed in this study. Accordingly, it should be highly desirable and important to take counter-measures against these threshing practices of operational overspeed.

The Formation of the Cenozoic Volcanic Edifice in the Goseong-Ganseong Area, Gangwondo, Korea (강원도 고성-간성일대의 신생대 화산체의 형성과정)

  • In the Obongri-Goseong area of Gangwondo, South Korea, there are six densely distributed volcanic edifices i.e., Duibaejae, Oeumsan, Galmibong, 249 m height, 166 m height, and 102 m height, and two other volcanic edifices including Goseongsan and Unbongsan volcanic edifice that are separately located from a distance. A previously undiscovered 249m volcanic edifice in Obongri was found in this investigation, and the six volcanic edifices distributed in Obongri will be referred to as the Obongri volcanic edifice group. Volcanic edifices in this area were interpreted by other researchers as being volcanic plug, plug dome, and cylindrical volcanic pipe type edifices. The aim of this study is to investigate the aspect of volcanic activity in the Obongri-Goseong area and the formation of volcanic edifices by examining of the shape of volcanic edifices, stratigraphy, and characterization of volcanic products. All the volcanic edifices in the area are composed of basaltic rocks on the Mesozoic granite basement, and the prevalence of the dome shape increased towards the upper part of the mountain. Three volcanic edifices (Duibaejae, 166 m height, 102 m height) include intercalated pyroclastic deposits between the basaltic rocks and the basement. The pyroclastic deposit in the Duibaejae volcanic edifice is composed of quartz, feldspar, granite fragments originated from the basement, and scoria fragments originated from the volcanic eruption. In addition to angular olivine, plagioclase, and pyroxene xenocrysts, all the basaltic rocks contained mantle xenolith, gabbroic xenolith originated from the lower crust, and granitic xenolith originated from the basement. This fact indicates that magma rapidly rose to the surface and that the volcanic activity was explosive. It is also interpreted that, as the basaltic magma became highly viscous due to the large amount of xenocrysts, the erupted magma formed a dome structure on the surface. The original dome structure was then severely eroded out leaving a plug dome formation on the basement.
