Spontaneous closure of perimembranous ventricular septal defect in a cat

  • We discuss the case of a 5-month-old male British Shorthair cat referred to our hospital following the detection of a heart murmur during a routine vaccination appointment. Two-dimensional echocardiography revealed a 1.18 mm ventricular septal defect (VSD) located immediately below the aortic valve, without signs of secondary cardiac remodeling. Given the absence of cardiac dysfunction, no treatment was administered, and the cat was periodically monitored over the next 2 years. Echocardiography at 29 months of age revealed no signs of the VSD. Future studies are needed to increase the evidence base for spontaneous VSD closure in small animals.

One-point versus two-point fixation in the management of zygoma complex fractures

  • Background: The treatment of zygoma complex fractures is of crucial importance in the field of plastic surgery. However, surgical methods to correct zygoma complex fractures, including the number of fixation sites, differ among operators. Although several studies have compared two-point and three-point fixation, no comparative research has yet been conducted on one-point versus two-point fixation using computed tomography scans of surgical results. Therefore, the present study aimed to address this gap in the literature by comparing surgical results between one-point and two-point fixation procedures. Methods: In this study, we randomly selected patients to undergo surgery using one of two surgical methods. We analyzed patients with unilateral zygoma complex fractures unaccompanied by other fractures according to whether they underwent one-point fixation of the zygomaticomaxillary buttress or two-point fixation of the zygomaticomaxillary buttress and the zygomaticofrontal suture. We then made measurements at three points-the zygomaticofrontal suture, inferior orbital wall, and malar height-using 3-month postoperative computed tomography images and performed statistical analyses to compare the results of the two methods. Results: All three measurements (zygomaticofrontal suture, inferior orbital wall, and malar height) showed significant differences (p< 0.05) between one-point and two-point fixation. Highly significant differences were found for the zygomaticofrontal suture and malar height parameters. The difference in the inferior wall measurements was less meaningful, even though it also reached statistical significance. Conclusion: Using three parameters in a statistical analysis of imaging findings, this study demonstrated significant differences in treatment outcomes according to the number of fixations. The results indicate that bone alignment and continuity can be achieved to a greater extent by two-point fixation instead of one-point fixation.

Effects of Inlet Turbulence Conditions and Near-wall Treatment Methods on Heat Transfer Prediction over Gas Turbine Vanes

  • This paper investigates the effects of inlet turbulence conditions and near-wall treatment methods on the heat transfer prediction of gas turbine vanes within the range of engine relevant turbulence conditions. The two near-wall treatment methods, the wall-function and low-Reynolds number method, were combined with the SST and ${\omega}RSM$ turbulence model. Additionally, the RNG $k-{\varepsilon}$, SSG RSM, and $SST_+{\gamma}-Re_{\theta}$ transition model were adopted for the purpose of comparison. All computations were conducted using a commercial CFD code, CFX, considering a three-dimensional, steady, compressible flow. The conjugate heat transfer method was applied to all simulation cases with internally cooled NASA turbine vanes. The CFD results at mid-span were compared with the measured data under different inlet turbulence conditions. In the SST solutions, on the pressure side, both the wall-function and low-Reynolds number method exhibited a reasonable agreement with the measured data. On the suction side, however, both wall-function and low-Reynolds number method failed to predict the variations of heat transfer coefficient and temperature caused by boundary layer flow transition. In the ${\omega}RSM$ results, the wall-function showed reasonable predictions for both the heat transfer coefficient and temperature variations including flow transition onset on suction side, but, low-Reynolds methods did not properly capture the variation of the heat transfer coefficient. The $SST_+{\gamma}-Re_{\theta}$ transition model showed variation of the heat transfer coefficient on the transition regions, but did not capture the proper transition onset location, and was found to be much more sensitive to the inlet turbulence length scale. Overall, the Reynolds stress model and wall function configuration showed the reasonable predictions in presented cases.

  • Single treatment and staged treatments in vertical wells are widely applied in sandstone and mudstone thin interbedded (SMTI) reservoir to stimulate the reservoir. The keys and difficulties of stimulating this category of formations are to avoid hydraulic fracture propagating through the interface between shale and sand as well as control the fracture height. In this paper, the cohesive zone method was utilized to build the 3-dimensional fracture dynamic propagation model in shale and sand interbedded formation based on the cohesive damage element. Staged treatments and single treatment were simulated by single fracture propagation model and double fractures propagation model respectively. Study on the changes of fracture vicinity stress field during propagation is to compare and analyze the parameters which influence the interfacial induced stresses between two different fracturing methods. As a result, we can prejudge how difficult it is that the fracture propagates along its height direction. The induced stress increases as the pumping rate increasing and it changes as a parabolic function of the fluid viscosity. The optimized pump rate is $4.8m^3/min$ and fluid viscosity is $0.1Pa{\cdot}s$ to avoid the over extending of hydraulic fracture in height direction. The simulation outcomes were applied in the field to optimize the treatment parameters and the staged treatments was suggested to get a better production than single treatment.

Effect of extraction treatment on upper airway dimensions in patients with bimaxillary skeletal protrusion relative to their vertical skeletal pattern

  • Objective: To investigate dimensional changes in regional pharyngeal airway spaces after premolar extraction in bimaxillary skeletal protrusion (BSP) patients according to vertical skeletal pattern, and to further identify dentoskeletal risk factors to predict posttreatment pharyngeal changes. Methods: Fifty-five adults showing BSP treated with microimplant anchorage after four premolar extractions were included in this retrospective study. The subjects were divided into two groups according to the mandibular plane steepness: hyperdivergent (Frankfort horizontal plane to mandibular plane [FH-MP] ≥ 30) and nonhyperdivergent groups (FH-MP < 30). The control group consisted of 20 untreated adults with skeletal Class I normodivergent pattern and favorable profile. Treatment changes in cephalometric variables were evaluated and compared. The association between posttreatment changes in the dentoskeletal and upper airway variables were analyzed using linear regression analysis. Results: The BSP patients showed no significant decrease in the pharyngeal dimensions to the lower level in comparison with controls, except for middle airway space (MAS, p < 0.01). The upper airway variable representing greater decrease in the hyperdivergent group than in the nonhyperdivergent group was the MAS (p < 0.01). Posttreatment changes in FH-MP had negative correlation with changes in MAS (β = -0.42, p < 0.01) and inferior airway space (β = -0.52, p < 0.01) as a result of multivariable regression analysis adjusted for sagittal skeletal relationship. Conclusions: Decreased pharyngeal dimensions after treatment in BSP patients showed no significant difference from the normal range of pharyngeal dimensions. However, the glossopharyngeal airway space may be susceptible to treatment when vertical dimension increased in hyperdivergent BSP patients.

Proteomic Analysis in ob/ob Mice Before and After Hypoglycemic Polysaccharide Treatments

  • In an attempt to discover novel biomarker proteins in type 2 diabetes prognosis, we investigated the influence of hypoglycemic extracellular polysaccharides (EPS) obtained from the macrofungus Tremella fuciformis on the differential levels of plasma proteins in ob/ob mice using two-dimensional gel electrophoresis (2-DE). The 2-DE analysis demonstrated that 92 spots from about 900 visualized spots were differentially regulated, of which 40 spots were identified as principal diabetes-associated proteins. By comparing control with EPS-fed mice, we found that at least six proteins were significantly altered in ob/ob mice, including Apo A-I, IV, C-III, E, retinol-binding protein 4, and transferrin, and their levels were interestingly normalized after EPS treatment. Western blot analysis revealed that the altered levels of the two regulatory molecules highlighted in diabetes and obesity (e.g., resistin and adiponectin) were also normalized in response to EPS. The Mouse Diabetes PCR Array profiles showed that the expression of 84 genes related to the onset, development, and progression of diabetes were significantly downregulated in liver, adipocyte, and muscle of ob/ob mice. EPS might act as a potent regulator of gene expression for a wide variety of genes in ob/ob mice, particularly in obesity, insulin resistance, and complications from diabetes mellitus.

  • Organ preservation surgery and minimally invasive surgery have been developed during the past 20 years with major focus on transoral laser surgery, endoscopic surgery, and robotic surgery. Two major robotic surgeries in head and neck area are transoral robotic surgery (TORS) and robotic thyroidectomy. Transoral robotic surgery is a safe and efficacious method of surgical treatment of oropharyngeal. hypopharyngeal and laryngeal neoplasm. Advantages of the technique include adequate ability to visualize and manipulate lesions with two hands. TORS can provide magnified three dimensional views and overcome the limitation resulting from the "line of sight" which hinders transoral laser procedure. The swallowing function following transoral robotic surgery show superior and patients were able to retain or rapidly regain swallowing function in the majority of cases. Recently, robotic thyroidectomy has also been developed to overcome the [imitation of endoscopic thyroidectomy. Robotic thyroidectomy by a gasless unilateral axillo-breast or axillary approach using a da Vinci S Surgical Robot is a feasible and cosmetically excellent procedure. It can be a promising alternative to endoscopic thyroidectomy or conventional open thyroidectomy.

  • PET/CT combines the functional information from a positron emission tomography (PET) exam with the anatomical information from a computed tomography (CT) exam into one single exam. A CT scan uses a combination of x-rays and computers to give the radiologist a non-invasive way to see inside your body. One advantage of CT is its ability to rapidly acquire two-dimensional pictures of your anatomy. Using a computer these 2-D images can be presented in 3-D for in-depth clinical evaluation. A PET scan detects changes in the cellular function - how your cells are utilizing nutrients like sugar and oxygen. Since these functional changes take place before physical changes occur, PET can provide information that enables your physician to make an early diagnosis. The PET exam pinpoints metabolic activity in cells and the CT exam provides an anatomical reference. When these two scans are fused together, your physician can view metabolic changes in the proper anatomical context of your body. PET/CT offers significant advantages including more accurate localization of functional abnormalities, and the distinction of pathological from normal physiological uptake, and improvements in monitoring treatment. A PET/CT scan allows physicians to measure the body's abnormal molecular cell activity to detect cancer (such as breast cancer, lung cancer, colorectal cancer, lymphoma, melanoma and other skin cancers), brain disorders (such as Alzheimer's disease, Parkinson's disease, and epilepsy), and heart disease (such as coronary artery disease).

2차원 자유표면파 문제에서의 방사조건 처리에 관한 고찰 (A Study on the Treatment of Open Boundary in the Two-Dimensional Free-Surface Wave Problems)

  • 자유표면파 문제에서의 방사조건을 위해 두가지 기법을 적용하여 보았다. 우선, 가상감쇠의 개념을 적용하여 일정한 구간에서 파를 감쇠시킴으로써 열린경계면(open boundary)에서 반사파를 제거하는 방법을 적용하여 보았다. 또 다른 방법으로서는 Orlanski의 방법을 변형하여 적용함으로써 단방향 파동에 대한 방사조건 처리기법을 다루었다. 몇가지의 전형적인 자유표면파문제에 대해 이들 기법을 적용하여 그 유용성을 고찰하였는데, 이들 기법은 그 적용방법이 간단하고 비선형문제에서도 사용될 수 있을 것으로 사료된다. 문제의 해법으로 기본 쏘오스 분포법이 사용되었고 비선형의 자유표면 경계조건이 적용되었다.

Two-Cell Spheroid Angiogenesis Assay System Using Both Endothelial Colony Forming Cells and Mesenchymal Stem Cells

  • Most angiogenesis assays are performed using endothelial cells. However, blood vessels are composed of two cell types: endothelial cells and pericytes. Thus, co-culture of two vascular cells should be employed to evaluate angiogenic properties. Here, we developed an in vitro 3-dimensional angiogenesis assay system using spheroids formed by two human vascular precursors: endothelial colony forming cells (ECFCs) and mesenchymal stem cells (MSCs). ECFCs, MSCs, or ECFCs+MSCs were cultured to form spheroids. Sprout formation from each spheroid was observed for 24 h by real-time cell recorder. Sprout number and length were higher in ECFC+MSC spheroids than ECFC-only spheroids. No sprouts were observed in MSC-only spheroids. Sprout formation by ECFC spheroids was increased by treatment with vascular endothelial growth factor (VEGF) or combination of VEGF and fibroblast growth factor-2 (FGF-2). Interestingly, there was no further increase in sprout formation by ECFC+MSC spheroids in response to VEGF or VEGF+FGF-2, suggesting that MSCs stimulate sprout formation by ECFCs. Immuno-fluorescent labeling technique revealed that MSCs surrounded ECFC-mediated sprout structures. We tested vatalanib, VEGF inhibitor, using ECFC and ECFC+MSC spheroids. Vatalanib significantly inhibited sprout formation in both spheroids. Of note, the $IC_{50}$ of vatalanib in ECFC+MSC spheroids at 24 h was $4.0{\pm}0.40{\mu}M$, which are more correlated with the data of previous animal studies when compared with ECFC spheroids ($0.2{\pm}0.03{\mu}M$). These results suggest that ECFC+MSC spheroids generate physiologically relevant sprout structures composed of two types of vascular cells, and will be an effective pre-clinical in vitro assay model to evaluate pro- or anti-angiogenic property.
