The Effects of Nursing Information on knowledge, physical symptoms, state anxiety and daily activity levels of patients recovering from open heart surgery (퇴원시 간호정보 제공이 개심술 환자의 지식, 신체증상, 상태불안 및 일상활동에 미치는 효과에 관한 연구)

  • This study investigated the effects of nursing in formation given verbally and a booklet on the knowledge, Physical symptoms, anxiety and daily activities of patients recovering from open heart surgery from just before discharge to six weeks after discharge. The convenience sample was of patients who had undergone open heart surgery in Seoul National University Hospital. The first 25 patients who agreed to participate in the study became the control group. They did not receive the intentional information but the usual nursing care. The next 28 patients became the experimental group who received a booklet about their post-discharge care which was discussed with them by the researcher before discharge. This study used a non equivalent control group non - synchronized quasiexperimental design. The tools included a 30 item knowledge scale, a 9 item physical symptom scale, Spielberger's state anxiety scale, and a 28 item daily activity scale. The instruments were applied before discharge and at two and six weeks after discharge. For data analysis, frequency, t-test, Pearson's Correlational Coefficient and Cronbach's $\alpha$ were used. The results were as follows : 1. Information given through the booklet was effective in increasing the knowledge of patients recovering from open heart surgery. The experimental group's knowledge was higher at 2 weeks after discharge than prior to discharge (P<0.05). 2. There was no significant difference in physical symptoms between the two groups at two and six weeks after discharge. 3. Information given through the booklet was effective in decreasing the State Anxiety two weeks after discharge (P<0.001) but at six weeks after discharge, there was no significant difference in anxiety between the control and the experimental groups. 4. There was no significant difference in daily activity between the two groups at two and six weeks after discharge. 5. There was a negative correlation between knowledge at discharge and at 2 weeks and anxiety at 2 weeks after discharge. There was a positive correlation between knowledge at discharge and daily activity at 2 weeks after discharge. There was a negative correlation between knowledge at 2 weeks and Physical symptom at 6 weeks after discharge.

Family Support and Hopelessness in Patients Admitted to Neuro-Surgical Intensive Care Unit (중환자가 지각한 가족지지와 절망감과의 관계연구)

  • This study identified correlations between perceived family support and hopelessness in patients admitted to Neuro - surgical Intensive Care Units. The purpose was to enhance theoretical understanding of the relationships of these two variables. The subjects of this study were 51 patients admitted to N-lCU, at three general hospitals in Seoul. Data were collected by researcher in structured interviews from Aug. 12 to Oct. 13, 1992. The research tools were parts of the Moos Family Environment Scale and the Beck Hopelessness Scale. The general characteristic data were analyzed for frequency and percentage ; the hypothesis was tested by the pearson product Moment Correlation Coefficient. After normality tests by using Kolmogorov - Sminorvtest, and T- test, ANOVA and Mann-Whitney U test, Kruskal -Wallis test were used on the Family Support and the Hopelessness about general charcteristics. The results of the above analysis were as follows 1) The average family support score for the group was 63.61 (tool average 51) and item average was 3.74 (tool item average 3) : the family support score of this sample was higher than average. The average family cohesion score of family support was 35.25 (tool average 27) and item average was 3.91 (tool item average 3). The average family expression score of family support was 28.35 (tool average 24) and item average was 3.57 (tool average 3). In this sample, perceived family expression was lower than family cohesion. 2) The average hopelessness score was 45.88 (tool average 60) and item average was 2.29 (tool item average 3) : the hopelessness score of this sample was low in comparison to the average. 3) The hypothesis in this study was supported. The main hypothesis that the higher the perceived family support level, the lower the level Of the hopelessness, was Supported (r=-.3869 p=.003). The sub-hypothesis that the higher the perceived family cohesion level, the lower the level of hopelessness, was supported(r=-.3688 p=.004). The sub-hypothesis that the higher the perceived family expression level, the lower the level of hopelessness, was supported (r=-.3068 p=.014). 4) General characteristics of the objects related to family support were ‘economic status’(p=.025) and ‘helping person’(P=.044) : the higher the economic status, the greater the family support. When the patient identified the helping person as a spouse, family support was rated more highly. The only general characteristic related to family cohesion was ‘helping person’(p=.041). No general characteristics were related to family expression. 5) The one general characteristic related to hopelessness was ‘education’(p=.002) : the higher their education, the lower their hopelessness. For these ICU patients, were related perceived family support and hopelessness, and family expression level was low in comparison to family cohesion level. The perceived family support of these seriously ill patients in situational crisis may have influenced the patient's emotional reaction of hopelessness. This study concluded that nurses in the ICU confirm the family support of the patient, and involve the family as the most intimate support systems in the care of the patient to help reduce the patient's hopelessness.

Comparison of Bleeding Tendency Between Selective Serotonin Reuptake Inhibitors and Serotonin Norepinephrine Reuptake Inhibitors Using Platelet Function Analyzer (혈소판기능분석기를 이용한 선택적 세로토닌 재흡수 억제제와 세로토닌 노르에피네프린 재흡수 억제제의 출혈 경향성 비교)

  • Objectives : The purpose of this study is to compare bleeding tendency of selective serotonin reuptake inhibitor (SSRI) and serotonin norepinephrine reuptake inhibitors (SNRI) using platelet function analyzer (PFA-100) in patients with major depressive disorder. Methods : This study is a prospective open-label study conducted by a single institution. A total of 41 subjects diagnosed with major depressive disorder under the DSM-5 diagnostic criteria participated in this study. The subjects were classified into SSRI (escitalopram) groups and SNRI (duloxetine) groups, respectively, according to random assignments. The closure time (CT) was measured using a platelet function analyzer (PFA-100) before each antidepressant was administered and after 6 weeks. Paired-sample t-test was conducted within each group to determine whether a specific antidepressant had an effect on closure time. In order to confirm the relative change in platelet function between the two groups, an independent sample t-test was conducted to compare and analyze the change in closure time between the two groups. Results : There was no significant changes in closure time (CEPI-CT, CADP-CT) before and 6 weeks after drug administration in the SSRI and SNRI groups, and there was no difference in the amount of changes in closure time between the two groups. Conclusions : Our results showed no difference in bleeding tendency between SSRI and SNRI. This study suggests that further large-scale studies on bleeding tendency for various antidepressants are needed in the future.

A Study on Social Support and the Quality of Life in the Elderly(The comparative analysis between home residents and institution residents) (노인의 사회적 지지와 삶의 질에 관한 연구 -일반가정노인과 양로원노인을 대상으로-)

  • Recent improvements in the standard of living, national income and medical care, and a decline in the infant death rate which have occurred related to economic growth and modernization, have led to a longer average life-span and a higher ratio of elderly people in the total population, Therefore, not only in the field of nursing science, but also in the field of many other discipline, issues concerning the elderly have been given increasing interest. A great deal of effort has been spent on increasing the quality of life for elderly people. This study was conducted to analyze the correlation between social support and quality of life. The sample consisted of III subjects residing at home and 107 subjects residing in institutions all of whom were over 65 years of age. The data collection period was from October 23, 1990 to January 26, 1991. Social support was measured using the Norbeck social Support Questionnaire developed by Norbeck, translated by Oh, Ka Sil and quality of life was measured using the QOL scale developed by No, You Ja. Data were analyzed using pereentages, t-test, Pearson Correlation Coefficient and ANOVA. The results of this study are as follows : 1. There was a statistically signifivant difference in the level of social support between the two groups (t=-8.83, p<.001), The elderly at home reported a much higher level of social support. 2. There was a statistically significant difference in the level of QOL between the two groups (t=-5.77, p<.001) . The elderly at home reported a much more positive quality of life. 3. There was a positive correlation between social support and QOL for the elderly at home ard it was statistically significant (r=.32, p<.001). 4. There was a postitive collealtion between social support and QOL for the elderly in institutions and it was also statistically significant (r=.19, p<.05). 5. The relationship between the general characteristics of the elderly at home and the variables of social support and of QOL were as follows ; 1) according to sex(t=10.57, p<.01) and the number of offspring(F=6.19, p<.01), there was a statistically significant difference in social support. 2) according to amount of Pocket money, there was a statistically significant difference in QOL(F=2.98, p<.05). 6. The relationship between the general characteristics of the elderly in institutions and the two variables were as follows ; 1) according to sex(t=6.24, p<.05), the number of offspring(F=6.16, p<.001) and religion (F=2.58, p<.05), there was a statistically significant difference in social support. 2) according to religion, there was a statistically significant difference in QOL(F=2.90, p<.05). In conclusion, it can be said that social support is an important variable related to QOL in the elderly and that social support levels are higher for the elderly residing at home. Therefore, more specific and objective approaches and efforts are needed to effectively use resources to maintain the elderly at home and to enhance social support available to the elderly in institutions and thereby increase QOL regardless of residence.

  • This study was conducted with the purpose of identifying the effects of behavioral psychosomatic symptoms, cognitive functions, ability to perform daily life, and depression among the elderly with dementia. The sample for this study comprised 40 people in the experimental group and 20 in the contrastive group from the Dementia Supporting Center, located in NoWon-Gu of Seoul city. Data were collected from November 1, 2016 until March 31, 2017. The collected data were analyzed using SPSS 23.0, and descriptive statistics, chi-square test, independent t-test two-way repeated measures ANOVA, and ANCOVA were calculated. An 8-week brain activation intervention program including lectures was administered to elderly people with dementia. The results show improved cognitive function and reduced depressed feelings. Based on the results of this study, brain activation intervention programs can be applied to elderly people with dementia at the community level to help improve their cognitive function and relieve them from depression.

Effects of Maternal Role Education Program on the Mother-Infant Interaction and Infant Development (영아기 어머니역할 교육 프로그램이 모아상호작용과 영아발달에 미치는 효과)

  • The impact of childhood experience has lifelong significance on subsequent health and development. Especially, the experience of infant is mostly affected by the quality of parental care and rearing environment. But the new mothers usually do not know what to do because of the lack of experience in these days. Therefore, an educational program regarding maternal role would be necessary. This study was conducted to evaluate the effectiveness of the maternal role education program for mother-infant interaction, child-rearing environment, and infant development. Non-equivalent control group time-series design was used, and Barnard's mother-infant interaction model was used as a conceptual framework of this study. The subjects were the healthy infants weighing over 2,500gm at birth, whose gestational age was more than 37 weeks, and their mothers. The final sample consisted of 19 mother-infant dyads for intervention group and 18 dyads for control group. Data were collected from March 15th to September 3rd in 1999. For the intervention group, programmed education which focused on mother-infant interaction, breast feeding, and infant care was provided before discharge. Telephone counselling was provided within one week after discharge. Home visiting for maternal role education was provided twice, one month and three months postpartum. For the control group, home visiting was also conducted but only for data collection. The data were analyzed using chi-square test and t-test to test the equivalence of two groups, and the effectiveness of intervention program was determined with repeated measure ANCOVA and t-test. The results were as follows: 1. Significant differences were found in mother-infant interaction between two groups(p=.000). It indicates that intervention program was effective in improving mother- infant interaction. In subscale analysis, four out of six subscale showed significant differences between the groups: sensitivity to cues (p=.000), social-emotional growth fostering (p=.000), cognitive growth fostering(p=.000) in mothers, and responsiveness to caregiver (p=.019) in infants. 2. The difference in the mean score of childrearing environment (HOME) between the intervention group and control group was significant(p=.003). When each subscale of HOME was examined individually, intervention group showed significantly higher scores in the diversity of stimulation(p=.000), and mother's involvement(p=.001). 3. Three-month-Infants of the intervention group showed higher GQ in the Griffiths mental development scale(p=.026). In subscale analysis, significant differences were found in the personal-social(p=.005), and the hearing and speech(p=.003). In conclusion, the maternal role education program proved to be effective in promoting the mother-infant interaction, organizing the childrearing environment, and fostering the infant development. These results are very meaningful that we found maternal role education necessary for normal infants' mothers, and that nurses can make a great contribution in promoting health of infants and mothers.

Development of Instrument for Korean Osteoarthritis Impact Measurement Scale (KAIMS) in Hip and Knee Osteoarthritis (한국형 골관절염영향측정척도(KAIMS) 도구개발 - 무릎 및 엉덩관절의 골관절염환자 중심 -)

  • Purpose: This study was conducted in order to develop an instrument for the Korean Osteoarthritis Impact Measurement scale (KAIMS) in hip and knee osteoarthritis patients. Methods: A sample of 426 subjects was recruited in Ulsan metropolitan city (six hospitals and clinics) and Andong city (five hospitals and clinics) from June 2010 to May 2011. Item internal consistency and item discriminant validity were analyzed on the item-level, and floor (%) and ceiling (%) effect were analyzed on the scale-level. Exploratory factor analysis was performed for construction of items in the KAIMS instrument; confirmative factor analysis was also performed to test the fit of the model. Results: In 426 respondents, age was 64.3 years. The reliability for r coefficient 0.90 (mobility) and 0.89 (pain and stiffness) (Cronbach's alpha 0.95, 0.95 respectively) was high in the test-retest, and there was no significant difference in paired t-test (p>0.05). Item internal consistency (${\alpha}=0.92$, ${\alpha}=0.87$ respectively) was also high. It was constructed of two factors (mobility, pain and stiffness dimension) and nine items in explanatory factor analysis; results of confirmatory factor analysis also indicated that the dimensional structure model was fitted well in the test of model fit (${\chi}^2=83.83$, df=26, p=0.000; RMR=0.05; GFI=0.96; AGFI=0.83). Conclusion: In conclusion, the results of this study showed that the KAIMS that was developed was a reliable and valid instrument for measurement of osteoarthritis (OA) patients. We recommend further research for additional development of the instrument on OA in Korea.

A Comparative Study on the Health Promoting Behavior between Average Internet Users and Excessive Internet Users in Middle School Students (일부 중학생의 인터넷 사용 수준에 따른 건강증진행위 비교 연구)

  • Purpose: This study was conducted to identify the difference of the health promoting behaviors between average Internet users and excessive Internet users. Method: Data were collected from a convenient sample of 465 middle school students from June 7th to 17th, 2002, based on a self reported questionnaire. The instruments included were the Health Promotion Behaviors reconstructed by the author based on the Health Promoting Lifestyle Profile (Walker, Sechrist & Pender, 1987) and Internet addiction test translated by Center for Internet Addiction Prevention and Counseling based on Young's test. Data were analyzed with N, %, x2 test, t-test and Pearson's correlation coefficient. Result : 1. There were no significant differences in general characteristics between average Internet users and excessive Internet users. 2. There were significant differences between two groups in diet behavior(p=.030), even though there were no differences in general health promoting behaviors(p=.109). 3. There were significant negative correlations between Internet use and diet behavior (r=-.193, p=.000). Therefore, average Internet users had more desirable diet behavior than excessive Internet users. Conclusion: Internet addiction prevention program should be conducted as part of a comprehensive school health promotion program. In addition, the results of this study should be considered in developing the school health education curriculum to rear students' responsibility on their health behaviors.

The Research of Corelation between Sasang Constitution and Pattern Identification through Clinical Research of the Patients of Menstrual Pain (임상연구를 통한 월경통 환자의 사상체질, 변증간의 상관성 연구)

  • Objective We researched to analyze correlation between menstrual pain and Korean medical diagnosis - Sasang constitution and Pattern Identification - through clinical research of the patients of menstrual pain. Method Trial gruop who consists of women with menstrual pain is 193 people, and control group who consists of women without menstrual pain is 101 people. We producted analyzing of their Sasang Constitution and symptom with Diagnosis System of Oriental Medicine(DSOM). This study used chi-square test, two sample t-test, analysis of variance, Wilcoxon's rank sum test, Kruskal-Wallis test, correlation analysis. Results 1) In trial and control group, Soeumin was the largest and Soyangin was the least. There was'nt statistical significance between trial and control group. 2) In DSOM, deficiency of blood, stagnation of qi, blood stasis, coolness, spleen, kidney and phlegm pathogenic factors showed significant difference. Conclusion There were significant corelation between menstrual pain and Korean mdical diagnosis - Sasang constitution and Pattern Identification.

Development of Sexual Autonomy Measurement for College Students (대학생의 성적 자율성 측정도구 개발)

  • Little is known about sexual autonomy in the light of human sexual health or sexual behavior. The purpose of this study was to increase understanding of sexual autonomy by developing a scale to measure sexual autonomy in college students. The measure was based on the perspective of human beings as having autonomy. A methodological developmental design was used. The study was done in three steps and included concept development through content analysis of relevant literature individual interviews using open-ended questions with 19 college students, item analysis of data from self report questionnaires for item to total correlation, content validity index by professionals and college students, and factor analysis with principal component analysis. Three groups participated in testing the measurement scale; Group A in 1998, a nationwide convenience sample of college students (n=1,163), Group B, college students in 1999 (n=233), and Group C, college students in 2000 who responded to the measurement scale before and after a sex education program (n=216). A series of exploratory and confirmatory factor analyses were conducted to test validity. For analysis of the questionnaires, statistical methods including reliability (test-retest), item to total analysis, content validity, factor analysis for construct validity and t-test for mean difference according to gender, sexual coitus, previous sex education, and pre-post experiment sex education were used with SPSS 10.0. Results: 1. Two stable factors were extracted and these contributed 48.05% of the variance in the total score. All 13 items loaded above .40 on each factor. 2. Factor I with 10 items was named 'control' and factor II with 3 items and was named 'coping'. 3. Differences in sexual autonomy scores for college students according to their experience showed that there were significantly higher scores for sexual autonomy in the post sex education group(t=-10.841, p=.000), but there were no differences according to whether or not they had previous sex education. Female college students had higher sexual autonomy scores than male college students in A or B group (t=-5.933, p=.000/ t=-1.992, p=.048). 4. Cronbach's $\alpha$ was .8603 for the total 13 items with 1,632 college students, .8739 with 232 students, .8333$\sim$.7647 with pre-post sex education with 216 students. This measurement was found to have a high validity and reliability for measuring sexual autonomy in college students. Therefore, the author recommends that it be used to measure the effects of sex education. It is suggested that further study is needed to analyze the relationship between sexual autonomy and sexual behavior.
