발살바 기법이 몸통 굴곡과 신전에 대한 저항운동에 미치는 영향 (An effect of the Valsalva maneuver on resistance exercise of trunk flexion and extension)

  • Background: The effect of the Valsalva maneuver applied in resistance exercise has shown contradictory results. This study aims to investigate the effect of the Valsalva maneuver on resistance exercise by examining the changes that occur by applying the Valsalva maneuver during trunk flexion and extension resistance exercise in healthy adults. Design: Randomized controlled trial. Methods: 34 healthy adults were conveniently recruited. According to the with or without Valsalva maneuver, the flexion and extension of the trunk were measured and evaluated. Results: Paired t test showed that there was a statistically significant difference in maximum torque of trunk flexion in the resistance exercise with Valsalva maneuver. However, there was no statistically significant difference in trunk extension (p<.05), in the total and average work of trunk flexion and extension (p<.05). Conclusion: The results of this study in which the difference in the amount of exercise according to the application of the Valsalva maneuver was not significant, show that the practice of the Valsalva maneuver, which has a risk of injury, should be avoided in rehabilitation exercises.

Effects of trunk Muscles Endurance, Hip Joint Muscular Strength, and Pelvic Alignment on Mild Low Back Pain

  • Objective: Low back pain easily becomes chronic and has a high recurrence rate. Therefore, it is most important to prevent chronicity and reduce the risk of recurrence in the early stages of back pain or at the stage with mild pain. Therefore, this study was conducted to compare hip joint muscle strength, trunk muscle endurance, and pelvic alignment between subjects with mild low back pain and subjects without back pain. Design: Crossed-sectional study Methods: The study was conducted by recruiting 30 students in their twenties who are enrolled in K University in Gyeongsangnam-do, and classifying them into 15 patients with mild back pain and 15 patients with normal. The subjects who participated in the experiment were measured for hip flexor and extensor muscle strength, trunk flexion and extension muscle endurance, and pelvic alignment. To measure hip joint muscle strength, biodex was used, and muscle endurance of the trunk was recorded at the end range of the trunk flexion and extension. And pelvic alignment was measured using Formetric 4D. Results: There were no significant differences in hip joint muscle strength, pelvic alignment, and trunk extension muscle endurance. The retention time was found to be significantly shorter in the mild low back pain group than in the normal group for trunk flexion muscle endurance. Conclusions: In the early stages of back pain or in the mild pain stage, training to increase muscle endurance of the flexor muscles may be helpful.

시야 가림막을 활용한 자세 조절 훈련이 중등도-중증 뇌성마비 아동의 머리 조절, 몸통 조절, 앉기 능력에 미치는 효과: 예비연구 (Effects of Postural Control Training Using a Visual Blind Board on Head Control, Trunk Control, and the Sitting Abilities of Children with Moderate to Severe Cerebral Palsy: A Pilot Study)

  • PURPOSE: This study was conducted to investigate the effects of postural control training using a visual blind board, on head control, trunk control, and the sitting abilities of children with moderate-to-severe cerebral palsy. METHODS: Ten children with moderate to severe cerebral palsy participated in this study. Postural control training with a visual blind board was given for 40 minutes per session, 3 times a week for 4 weeks (12 sessions). Before and after the intervention, head control, trunk control, and sitting abilities were quantified using the head control scale, Korean version-trunk control measurement scale, and the sitting part of the Korean version-gross motor function measure-88, respectively. RESULTS: Postural control with the visual blind board significantly improved the head control ability of children with moderate to severe cerebral palsy (p = .015). Their trunk control abilities also showed significant improvement after the intervention (p = .016). However, their sitting ability did not show a significant change. CONCLUSION: These results showed that postural control training using a visual blind board is effective in improving the head and trunk control abilities of children with cerebral palsy. Further studies with suitable sample sizes and control groups are needed to reach a conclusion about the use of postural control training with visual blind boards for improvement of postural control of children with moderate to severe cerebral palsy.

체간 안정화 운동과 고관절 운동이 만성 요통 환자의 허리가동범위, 통증, 장애정도에 미치는 영향 (Effects of Trunk Stability Exercise and Hip Exercise on Lumbar Range of Motion, VAS, Disability Chronic Low Back Pain Patients)

  • Background: This study compared the effects of trunk stabilization exercise and hip joint exercises on the range of motion of the lumbar spine, pain severity, and severity of disability in patients with chronic lower back pain. Methods: A total of 30 participants were enrolled and divided into group 1 (n=10), group 2 (n=10), and group 3 (n=10) were performed by each group thrice a week for a total of 8 weeks. Group 1 performed warm-up exercise (15 min), trunk stabilization exercise (25 min), finish-up exercise (15mins). Group 2 performed warm-up exercise (15 min), hip exercise (25 mins, finish-up exercise (15mins). Group 3 warm-up exercise (15 min), trunk stabilization and hip exercise (25 min), Finish-up exercise (15 min). Participants were assessed for the range of motion of the lumbar spine, pain severity (visual analog scale score; VAS), and severity of disability (Oswestry disability index score; ODI) before and after the interventions. Results: All three groups showed a significant increase in the range of motion of the lumbar spine, but there was no significant difference among the groups. Moreover, the severity of pain and ODI were significantly decreased in all groups; however, the intergroup differences were non-significant. Conclusion: The results from this study confirmed the effectiveness of trunk stabilization and hip joint exercise in improving the lumbar range of motion, pain severity, and chronic lower back pain in patients. Thus, trunk and pelvic stabilization exercises and hip joint exercise can be used as clinical practices to treat and prevent chronic lower back pain.

The Relationship Between Upper Extremity, Trunk and Hip Muscle Strength and the Modified Upper Quarter Y-balance Test

  • Background: Various functional tests such as upper quarter Y-balance test (UQYBT) are used to evaluate shoulder stability and mobility in clinical or sports fields. Previous studies have been conducted to determine the correlation between the scapular or trunk muscle and UQYBT. However, the correlation between UQYBT and hip flexor, which can be considered as a core muscle, has not been confirmed. Objects: To verify the relationship between the UQYBT and scapular muscle (scapular protractor and lower trapezius [LT]), trunk muscle, and hip flexor strengths in healthy male participants. Methods: A total of 37 healthy male participants were recruited and underwent UQYBT in the push-up posture. The isometric strength of the scapular protractor, LT, trunk flexor and extensor, and hip flexors were measured using a smart KEMA strength sensor (KOREATECH Inc.). Results: The superolateral direction of the UQYBT was moderately to strongly related to trunk extensor (r = 0.443, p < 0.01), scapular protractor (r = 0.412, p < 0.05), LT (r = 0.436, p < 0.01), and both sides of the hip flexors (supporting-side: r = 0.669, p < 0.01; non-supporting-side: r = 0.641, p < 0.01). The inferolateral direction of the UQYBT was moderately related to the scapular protractor (r = 0.429, p < 0.01), LT (r = 0.511, p < 0.01), and both sides of hip flexors (supporting-side: r = 0.481, p < 0.01; non-supporting-side: r = 0.521, p < 0.01). The medial direction of the UQYBT was moderately to strongly related with the scapular protractor (r = 0.522, p < 0.01), LT (r = 0.541, p < 0.01), and both sides of hip flexors (supporting-side: r = 0.605, p < 0.01; non-supporting-side: r = 0.561, p < 0.01). Conclusion: This study showed that the strength of the scapular muscles, trunk muscles, and hip flexor muscles correlated to the UQYBT. Therefore, the strength of not only the scapular and trunk muscles but also the hip flexor muscles should be considered to improve the UQYBT.

오가피(五加皮)의 형태(形態)에 관한 연구(硏究) (A Study on a Morphological Identification of Acanthopanacis Cortex)

  • Objectives : In order to distinguish morphological characteristics of trunk bark and root bark of Ulmus davidiana var. japonica (Rehder) Nakai and the trunk bark and root bark of Hemiptelea davidii Planchon were sampled and compared in terms of their external and internal features with flour states according to their medical use, through microscopic examination. Methods : The slice of the tested material made by paraffin section technique was colored with Safranine Malachite Green contrast methods, and the flour of it was mounted by the liquid made by the same ratio of each of glycerin, acetic acid, and water, and then observed and photographed by olympus-BHT. Results : 1. Internal Features 1) A large parenchymatous cell was observed in the phloem of the slice of both trunk bark and root bark of Ulmi Cortex. However, both of the trunk bark and root bark of Hemipteleae Cortex did not have parenchymatous cell in the phloem; instead, stone cells including much square crystal of calcium oxalate were distributed around fiber bundle, and the parenchymatous cell included much druse crystal of calcium oxalate. 2) In both the Ulmi Cortex and Hemipteleae Cortex, rhytidome was observed in trunk bark, but not in root bark, but in the parenchymatous cell of the root bark of the Ulmi Cortex contained starch grain. 2. Flour States 1) In the flour of root bark of the Ulmi Cortex, a large parenchymatous cell was observed. However, in the flour of trunk bark and root bark of Hemipteleae Cortex, no parenchymatous eel was found; instead, stone cell including square crystal of calcium oxalate and druse crystal of calcium oxalate were observed. 2) There was no remarkable difference between the trunk bark and root bark of Hemipteleae Cortex. However, starch grain was contained in the parenchymatous cell of the root bark of Ulmi Cortex but not in the trunk bark of it. Conclusions : There were some morphological differences in external, internal, and flour parts of Ulmi Cortex and Hemipteleae Cortex. In particular, there was a morphological difference in flour states between the trunk bark and root bark of Ulmi Cortex, it is possible to use microscope to distinguish their flour states.

들기 작업시 중량물의 비대칭 무게중심 및 상체 옆으로 기울임에 따른 허리근육의 Peak EMG 진폭 비교 (Comparison of Peak EMG Amplitude on Low Back Muscles according to Asymmetric Load Center of Gravity and Trunk Lateral Bending while Lifting)

  • 본 연구의 목적은 중량물 들기 작업시 비대칭 무게중심과 상체 옆으로 기울임 여부가 어떻게 L5/S1 주변 허리 근육의 Peak EMG 진폭에 영향을 미치는지를 알아보는 것이다. 요통을 포함한 근골격계질환은 무겁지 않은 물체의 지속적인 반복 취급뿐만 아니라, 일회성의 커다란 부하도 신체 조직의 부담으로 작용하여 발생할 수 있다. 11명의 20대 대학생 남성은 15.8kg의 물체를 들어올리는 Task를 3회 실행하였다. 이 때 대칭 무게중심의 물체를 상체를 기울이지 않고 들어 올리는 경우, 비대칭 무게중심의 물체를 상체를 기울이지 않고 들어 올리는 조건, 그리고 비대칭 무게중심의 물체를 상체를 기울여서 들어 올리는 경우에 따라 6개의 L5/S1 주변의 근육들로부터 Peak EMG 진폭이 수집되어 분석되었다. 결과는 비대칭 무게중심을 지닌 물체 취급시 무게중심에서 먼 쪽의 허리 근육에서 Peak EMG 진폭이 발생하고, 그 진폭의 크기는 대칭 무게중심 물체를 취급하는 경우보다 크게 나타났다. 또한, 비대칭 무게중심의 물체를 취급할 경우 물체나 상체를 물체의 무게중심 방향으로 기울이는 것은 기울이지 않을 때보다 대측성 등근육에서 Peak EMG 진폭 증가시켰다. 이러한 연구 결과는 비대칭 무게중심의 중량물을 들어 올리는 작업에서 요통 발생을 예방하기 위한 행정적인 조치로써 가능하면 상체를 전두면 기준으로 기울이지 않는 작업자 자세 조언이 필요함을 말해준다.

편마비 환자에서 슬링을 이용한 체간-골반 안정성 집중 훈련이 근활성도 및 균형능력에 미치는 영향 (Effect of Intensive Trunk-Pelvic Stabilization Training Using Sling on Muscle Activity and Balance in Hemiplegia)

  • 본 연구의 목적은 슬링을 이용한 편마비 환자의 체간-골반 안정성 집중훈련 시 체간의 근활성도 및 균형능력에 미치는 효과를 알아보고자 하였다. 6개월 이상에서 1년 된 편마비로 진단 받은 입원 환자 20명을 대상으로 무작위 추출하여 두 그룹으로 나누었다. 실험은 4주간, 주 5회, 1회당 30분간 운동치료를 시행하였고, 대조군(n=10)은 일반적 운동훈련만 적용하였고, 실험군(n=10)은 일반적 운동훈련 및 슬링을 이용한 체간-골반 안전성 집중훈련을 실시하였다. 훈련 효과를 알아보기 위해 훈련 전 후에 체간조절능력, 근활성도, 균형능력을 각각 측정하였다. 훈련 후, 유의한 차이는 실험군에서 체간조절능력(TIS)(p<.001), 특히, 정적조절능력(p<.05), 동적조절능력(p<.01), 협응능력(p<.05), 근 활성도는 RA(p<.001), EO(p<.001), ES(p<.001), Multifidus(p<.05), FMA (Balance)(p<.01), MTD-Balance에서는 정적(p<.001), 동적균형능력(p<.001) 모두 각각 유의한 차이를 보였다. 이로써 슬링을 이용한 체간-골반 안정성 집중훈련은 체간의 근활성도와 균형능력 향상에 효과적인 치료임을 증명하였고, 균형에 있어서 체간-골반 안정화 및 조절의 중요성을 확인할 수 있었다.

  • 본 연구는 요가, 필라테스, 코어운동 등 체간 안정성 운동프로그램 적용 전후의 만성 요통 환자의 기능적 움직임, 동적 균형능력 및 체간 안정성의 변화를 확인하고, 3가지 체간 안정성 운동의 효과를 비교하고자 하였다. 만성 요통을 가진 성인 43명을 요가 집단 (n=15)과 필라테스 집단 (n=15) 및 코어 운동 집단 (n=13)으로 분류하였고, Functional Movement Screen (FMS)와 Lower Quater Y-Balance Test (YBT-LQ) 및 Trunk Stability Test (TST)를 통해 기능적 움직임, 동적 균형능력 및 체간 안정성을 각각 측정하였다. 이후 8주간 운동을 적용한 후 모든 변인을 재측정하였으며, 운동 적용 전후 차이를 비교 분석하였다. 그 결과, 세 집단 모두 운동 적용 후 FMS, YBT-LQ 및 TST의 측정값이 적용 전보다 유의하게 증가하였으나, 각 운동 적용에 따른 집단 간의 차이는 나타나지 않았다. 이런 결과는 요가, 필라테스 및 코어 운동이 만성 요통 환자의 기능적 움직임, 동적 균형능력 및 체간 안정성을 개선하는 데 도움이 된다는 것을 보여주었다.

체간의 안정화운동이 뇌졸중 환자의 동적 균형에 미치는 효과 (The Effects of Trunk Stability Exercise on Dynamic Balance in the Persons with Chronic Stroke.)

  • 본 연구는 만성 뇌졸중 환자를 대상으로 체간 안정화운동을 실시하여 동적 균형능력의 변화를 알아보고자 하였다. 연구의 대상자는 만성 뇌졸중 환자 38명으로 무작위로 두 그룹으로 나누어 체간 안정화운동(n=20)과 기존의 일반적 균형운동(n=18)을 주 3회씩 7주간 시행하였다. 운동의 시작 전과 후에 동적 균형능력을 측정하여 운동의 효과를 알아보았고 전과 후의 변화량을 가지고 운동방법간의 차이를 비교하였다. 연구 결과 체간 안정화운동으로 뇌졸중 환자의 동적 균형능력(BBS, TUG)이 유의하게 증가하였다(p<.001). 두 그룹간의 치료 전 후차 변화량을 비교한 결과, BBS 측정에서 체간 안정화운동이 일반적 균형운동보다 효과적인 것으로 나타났다(p<.05). 이와 같은 결과를 바탕으로 앞으로 만성 뇌졸중 환자의 재활 훈련 방법으로 체간의 안정화운동이 유용하게 적용될 것으로 기대되고 임상적 적용에서도 누운 자세에서 시행하는 체간 안정화운동이 뇌졸중 급성기 이후의 조기 적용에도 활용할 수 있는 효과적인 방법이라고 생각된다.
