3년령 암컷 잡종개(체중 5.3 kg)가 실신, 운동불내성, 의기소침, 기면 등의 증상으로 강원대학교 동물병원에 내원하였다. 진단검사상 적혈구증가증이 관찰되고, 흉부방사선 검사상 심한 우심종대 패턴이 관찰되었으며, 심초음파상우-좌 단락의 심방중격 결손증, 심한 폐동맥 협착증(~5 m/s of peak velocity)과 우심실 비대소견이 관찰되었다. 환자는 진단영상학적 소견을 근거로 팔로삼징(trilogy of Fallot)으로 진단되었다. 환자의 임상증상을 안정화시키기 위해 diltiazem과 enalapril을 투여하였고 일주일에 한번씩 정맥사혈을 실시하고 있다. 본 증례는 국내에서 최초로 보고되는 팔로삼징의 증례이다.

131 cases of open heart surgery were performed in the Department of Thoracic and Cardiovascular Surgery, Kyungpook National University Hospital in 1985. There were 116 congenital cardiac anomaly and 15 acquired heart diseases. Out of 116 congenital cardiac anomaly, 73 cases of acyanotic group and 43 cases of cyanotic group were noted. In 73 cases of acyanotic group, 17 ASD, 52 VSD and 4 other acyanotic anomaly were noticed. In 43 cases of cyanotic group, 4 Trilogy of Fallot, 34 TOF, 1 Pentalogy, 3 DORV and 1 DCRV were included. Of the 15 acquired valvular heart disease cases, individual incidence was in mitral valve 10, double valve 3, and simple aortic valve 2 cases. Total number of valve replaced was 16, and 13 for mitral, 2 for aortic, and 1 for tricuspid in position, including 1 cases of double valve replacement. Overall operative mortality for 131 cases of open heart surgery was 4.5%, and the operative mortality was 5.5% in congenital acyanotic group, 2.3% in congenital cyanotic group, 0% in TOF group and 6.7% in acquired group.

103 cases of open heart surgery were performed in the Department of Thoracic and Cardiovascular Surgery, Kyungpook National University Hospital in 1984. There were 90 congenital cardiac anomaly and 13 acquired heart diseases. Out of 90 congenital cardiac anomaly, 63 cases of acyanotic group and 27 cases of cyanotic group were noted. In 63 cases of acyanotic group, 11 ASD, 45 VSD and 7 other acyanotic anomaly were included. In 27 cases of cyanotic group, 4 Trilogy of Fallot, 15 TOF, 3 Pentalogy of Gasul and 5 other cyanotic anomaly were found. Among 13 cases of acquired heart diseases, 12 valvular lesions and 1 atrial myxoma were noted. Two open mitral commissurotomy and ten valve replacements were performed for 12 valve lesions. The frequent complications were acute respiratory insufficiency and low cardiac output syndrome occurred in 5 cases. The perioperative mortality was 4.8% in acyanotic congenital cardiac anomaly, 7.4% in cyanotic congenital cardiac anomaly, and 0% in acquired heart diseases. Overall mortality for 103 cases of open heart surgery was 4.9%.

Up to October 31, 1975, 34 cases of atrial septal defect, secundum type, operated in this department, were presented. This is 23.2% of all congenital heart diseases, operated utilizing cardiopulmonary bypass, in this department during this period [34 out of 146]. Out of 34 cases, 32 cases are pure ostium secundum type and one case is sinus venosus and another one is ostium secundum type with partial anomalous pulmonary venous drainage. Six cases of endocardial cushion defect and 3 cases of trilogy of Fallot are excluded in this report. All 34 cases are repaired under direct vision utilizing extracorporeal circulation. Among 34 cases of atrial septal defect, 16 cases are male, and 18 cases are female. Their ages range between 3 to 48 years, but over 59% of the cases are below the ages of 20 years. Thirty-two cases are repaired by direct sutures while 2 cases are repaired with Teflon patches. The average perfusion time is 30 minutes; the shortest 12 and the longest 81 minutes. The number of the defect is single in 31 cases, double in 2 cases, triple in one case. But the associated defect except the main defect are so small as can be closed by simple direct suture. The size of the defect is average $12cm^2$; the smallest 0.7 and the largest $25cm^2$. The surgical mortality is 2 cases [5.6%] and other cases are found to be excellent in the follow up studies.

Seventeen patients of the congenital pulmenic stenosis were operated at the department of Thoracic Surgery, Seoul National University Hospital. There were thirteen male and four females, and ranging from six to thirty years of age. And it's incidence was 2.7% of congenital heart disease cases that were operated on. (Total 628) Seven case of trilogy of Fallot were excluded. Two cases of bacterial endocarditis were found. Right heart catheterization was done in all patients. Average right ventricular pressure was $121{\pm}35.8$mmHg, pulmonary artery $20{\pm}6.8$mmHg, and RV-PA pressure gradient $98{\pm}34.5$mmHg. The preoperative average time interval of A2-P2 which was checked at phonocardiography was $0.08{\pm}0.016$second, and was reduced to $0.03{\pm}0.009$second postoperatively. One was operated by Varco's procedure, another one was done by Brock's procedure, and fifteen patients were done by open heart surgery with heart-lung machine. Pure valvular stenosis was found in sixteen and infundibular stenosis in one case. And the combining anormalies were two patent ductus arteriosus, two patent foramen ovale, and one tricuspid valve hypoplasia. Two expired postoperatively at the begining stage of cardiac surgery in this Department. The remaining fifteen showed excellent operative results.
