  • Background: With the enactment of the Hospice, Palliative, Care, and Life-sustaining Treatment Decision-Making Act in February 2018, legal guidelines for physician orders for life-sustaining treatment (POLST) were presented. This study was conducted to analyze the association of writing POLST on the use of health care before death. Methods: The study analyzed the electronic medical records and POLSTs of 1,003 adult patients who died at a tertiary hospital located in Seoul from February 4, 2018 to February 4, 2019. Results: Of the deaths, 80% (n=804) completed POLST. Among patients who completed POLST before death, 51% (n=412) were written 1-7 days before death, and only 31% (n=246) were completed by patients themselves. 99% (n=799) decided to withdraw or withhold cardiopulmonary resuscitation. As a result of analyzing the effect of POLST on medical use before death, it was found that POLST and inpatient cost had a significant negative correlation, and POLST completion significantly reduced death in the intensive care unit (ICU). However, both inpatient costs and death at ICU increased when the POLST was completed by surrogate decision-makers rather than patients themselves. Conclusion: The enactment of the Hospice, Palliative, Care, and Life-sustaining Treatment Decision-Making Act provided a legal basis for withdrawing and withholding meaningless life-sustaining treatment. By specifying the treatment to be received at the end of one's life through the POLST, inpatient treatment costs and death at the ICU were decreased. However, the frequent decision-making by the surrogates and completion of POLST close to death may hinder the original purpose of the law.

  • Two experiments were conducted to study the effect of using Egyptian mallow leaf meal (EMLM) on growth performance and feed utilization of common carp (Cyprinus carpio L.) in experiment 1. Four diets containing 0, 5, 15 and 25% EMLM were included at the expense of berseem leaf meal and fed to fingerlings of common carp for 98 days. The results showed that the average daily gain, feed intake and feed coefficient ratio (FCR) were improved (p<0.05) with increasing the level of EMLM in the diet. Fish fed 25% EMLM were similar to control fish (30% berseem leaf meal) in the productive value (PPV%). In experiment 2, five diets were prepared to contain: 1) 30% berseem leaf meal, 2) 25% untreated EMLM, 3) 25% cooked EMLM 4) 25% treated EMLM with 0.5% NaOH and 5) 25% treated EMLM with 1% NaOH. The results showed that diet containing EMLM gave the best growth performance and feed utilization. However, diet containing 1% NaOH treated EMLM superior to the other diets in PPV% and energy utilization. Diets containing 0.5% NaOH-treated EMLM or cooked EMLM decreased the protein utilization compared to those containing EMLM.

  • This paper provides a case study of landfill gas (LFG) utilization fer direct use as process fuel, and for electrical power generation at restored landfills in the Hong Kong Special Administrative Region of China (HKSAR). The paper specifically covers the LFG utilization schemes, which are required under landfill restoration contracts at the Shuen Wan and Urban Landfills. These contracts provide for the restoration and aftercare of six landfills, and are administered by the Environmental Protection Department (EPD) of the Hong Kong Government. The LFG utilization scheme at the Shuen Wan Landfill incorporates the direct use of LFG by compressing and dehumidifying the LFG prior to conveyance through a 1.6-kilometer (1-mile) pipeline. The pipeline provides an alternate fuel source to naphtha during process heating for gas production at the Tai Po Gas Production Plant of the Hong Kong and China Gas Limited (HKCC). The LFG utilization scheme at the Jordan Valley Landfill (one of the Urban Landfills) beneficially uses the LFG as fuel for electrical power generation with reciprocating internal combustion engines. The LFG is compressed, cooled, and filtered prior to delivery to two engine/generator sets. This system provides power to operate the leachate pre-treatment plant, which processes leachate from all of the Urban Landfill sites. The case study will examine the technical and non-technical considerations, including harriers, for developing, designing and implementing the LFG utilization projects in Hong Kong. Specific regulatory considerations and external governmental agency approvals are discussed, including the requirement to register as a gas-producing utility. While the paper focuses on LFG utilization applications in Hong Kong, many of the considerations discussed are also applicable to development of LFG utilization in other regions of Asia.

The objectives of this study were to reveal the cause of low utilization of hospitals in rural area, and to increase utilization of those hospitals. In this study the improvement methods of rural hospital's utilization were presented by examining were placed in difficult management condition, in respect of hospital's management conditions, manpower input, patient medical treatment record, financial record, and actual output. The causes of rural hospital's low utilization were as follows; 1) changes in number and structure of population 2) rural people's preference for large hospitals and hospitals which located in urban area 3) rural hospitals lacking in hospital management skill. Consequently rural hospital's operation condition got more and more deteriorated. To raise rural hospital's utilization, method for social policy, method for health policy, and intrinsic method of hospitals were presented in turn. For rural residents to utilize medical service conveniently, it is necessary for rural hospitals operated normally. So government must insist that rural hospitals solve the problems which come out from their internal management problems. And also these rural hospitals should be supported and nurtured by the government until their management is operated normally.
