Critical Pathway for Management of Primary Spontaneous Pneumothorax (원발성 자연기흉의 치료지침)

  • Background: With the advances of video technology, thoracoscopic surgery has been applied to various areas of the thoracic surgical fields including major surgeries. Now a days,-thoracoscopic surgery is Performed as a procedure of choice for primary spontaneous pneumothorax. But the operative indication for the primary spontaneous pneumothorax has not been changed since the last few decades, although the procedure of choice was changed from open thoracotomy to thoracoscopy. Therefore, we thought new treatment strategy will be necessary for the management of primary spontaneous pneumothorax. Material and Method: Between January 1998 and December 1999, 149 primary spontaneous pneumothorax patients were admitted to the Kyungpook National University Hospital. Result: Of these patients, 177 were first attack pneumothoraces and the number of total attacks were 250. Conclusion: Analyzing the amount of pneumothorax, methods of treatment, number of recurrences, recurrence rate and hospital stay, we propose a critical pathway for establishing new treatment strategy for the management of primary spontaneous pneumothorax.

Estimation of Carbon Emissions and Energy Self-Sufficiency in Sewage Treatment plant's Improvement by using Life Cycle Management Tool (LCM 기법을 이용한 하수처리장 개선에 따른 탄소배출량 및 에너지 자립율 평가)

  • In this study, carbon emissions and energy consumption were evaluated to establish a design strategy which has low energy consumption and carbon emission production, by using life cycle energy (LCE) and life cycle $CO_2(LCCO_2)$ calculation methods in life cycle management(LCM) tools. After improvement design projects, the energy consumption and $CO_2$ emission were calculated and compared in three sewage treatment plants (STPs), which are A STP, B STP, and C STP. The reduction of carbon emissions was 28,020.1 ton $CO_{2-}e/yr$, 37,399.6 ton $CO_{2-}e/yr$ and 8,788.3 ton $CO_{2-}e/yr$, respectively. Production of energy was 792 TOE/yr, 1,235 TOE/yr and 1,023 TOE/yr, respectively. As a result, the estimation of energy and energy self-sufficiency was 5.1 %, 14.5 % and 23.5 %, respectively. The result of this study shows the LCM can be contributed to establish strategy for energy and carbon emission reduction in sewage treatment plants.

  • Bispbosphonates are a class of pharmaceutic agents, which induce apoptosis of osteoclast as well as impair osteoclastic activity to suppress bone resorption. Thus, bisphophonates are effectively used to treat osteoporosis, multiple myeloma and to prevent bone metastases of malignant cancer. However, recently dental disease have been reported associated with Bisphosphonates. Thus, there are a number of discussions about proper prevention and treatment of bisphosphonate-related osteonecrosis of jaw(BRONJ). Marshall R. Urist in 1965 made the seminal discovery that a specific protein, BMP(bone morphogenetic protein), found in the extracellular matrix of demineralized bone could induce bone formation newly when implanted in extraosseous tissues in a host. BMPs are multi-functional growth factors which are members of the transforming growth factor-beta super family and their ability is that plays a pivotal roll in inducing bone. About 18 BMP family members have been identified and characterized. Among of them, BMP-2 and BMP-7 have significant importance in bone development. In this study, patients of BRONJ were recieved who visited Department of oral and maxillofacial surgery, school of dentistry, Wonkwang university for past 3 years from 2011 to 2013. We focused on the results of the surgical intervention. We suggest that new strategy of treatment used to rhBMP-2 and LFA(Lidocaine-Fibrinogen-Aprotinin)-collagen scaffold for patients of BRONJ. The purpose of this paper is to give a brief overview of BMPs and to critically review the clinical data currently available on rhBMP-2 and LFA collage scaffold.

Dilemma of Asthma Treatment in Mild Patients

  • Inhaled corticosteroids (ICSs) have been widely used as a key medication for asthma control. However, ICSs have been known to cause respiratory infections, such as pneumonia and pulmonary tuberculosis. Consequently, a dilemma exists regarding recommendation of persistent lifetime use of ICSs to mild asthma patients. Short-acting ${\beta}$-agonists (SABAs) have also been widely used for symptom relief. However, SABAs have been reported to increase the risk of asthma-related death, though incidences have been very rare. Consequently, a dilemma exists regarding recommendation of a SABA alone without an ICS or a controller to asthma patients even with very mild disease. In the real world, asthma patients tend to intermittently use ICS and more likely to be dependent on SABA since many patients want immediate relief of their symptoms. Consequently, a dilemma exists regarding the underuse of ICSs but the overuse of SABAs. One strategy for solving the presented dilemma would be identification of patients with asthma who require persistent use of asthma controllers. Such patients, who may be referred to as "persistent controller users," should continuously receive ICSs, even under controlled states of asthma. Another strategy would be a patient-adjusted, symptom-driven, intermittent-to-regular treatment combining low-dose ICS/rapid-onset long-acting ${\beta}$-agonists instead of using a SABA alone or with low-dose ICS for the asthma patients with mild disease. Both of these two strategies could avoid the risky treatment of a SABA alone without an ICS and could reduce the dose of ICS with the maintenance of asthma control.

Collective review of pancreatic carcinosarcoma, a very rare pancreatic malignancy

  • Pancreatic carcinosarcoma is a very rare malignancy with a poor prognosis. Because of these characteristics, a treatment strategy for it has not been established yet. The aim of this study was to establish a therapeutic strategy for pancreatic carcinosarcoma. We reviewed data of a 65-year-old female patient who was diagnosed with pancreatic carcinosarcoma through endoscopic ultrasound-guided fine needle aspiration biopsy before surgery. For literature review, we searched PubMed using terms of "Pancreatic" or "Pancreas" and "carcinosarcoma" or "carcinosarcomatous". The patient received 11 cycles of neoadjuvant treatment with leucovorin, fluorouracil, irinotecan, oxaliplatin and pembrolizumab because the tumor was borderline resectable. She underwent stereotactic ablative body radiotherapy (SABR) with 35 Gy in 5 fractions, followed by robotic pylorus-preserving pancreaticoduodenectomy. After surgery, the patient received adjuvant chemotherapy in the same regimen as before surgery. She is alive without any recurrence. Among 48 patients within 33 available papers, the median survival time was 15 months. The survival rate of patients who received adjuvant chemotherapy tended to be higher than that of those who did not receive adjuvant chemotherapy, although the difference was not statistically significant (median survival, 47 vs. 15 months; p = 0.485). Three patients who received neoadjuvant chemotherapy had a survival period of 13-23.5 months. Surgery with lymphadenectomy, adjuvant therapy, and neoadjuvant therapy are thought to help improve survival outcomes. Modern treatment approaches for conventional pancreatic ductal adenocarcinoma could be applied to pancreatic carcinosarcoma.

  • This paper studies electrostatically suspended induction motors (ESIM). The ESIM possesses the rotating ability of an ordinary electrostatic induction motor, in addition to providing contactless support by electrostatic suspension. To accomplish these two functions, a feedback control strategy and the operating principle of an ordinary electrostatic induction motor are used. The stator possesses electrodes which exert the electrostatic forces to the rotor and are divided into a part responsible for suspension and one for rotation. Two rotor types are utilized: a polished glass disk without any surface treatment, and a polished glass disk covered with a thin layer of conductive material (ITO layer) on only one side. In this paper, the principle of the ESIM is described, followed by stator electrode design, experimental apparatus, control strategy for stable suspension. Experimental results show that the glass disk has been rotated with a speed of approximately 70 rpm while being suspended stably at a gap length of 0.3 mm.

Sedation in the Critically Ill Patients (중환자에서의 진정 진통 치료)

  • Optimal level of sedation and analgesia is important for the comfort and safety of critically ill patients. However, suboptimal sedation is relatively common in the intensive care unit (ICU) and it could cause prolonged mechanical ventilation and ICU stay, also increase delirium and ICU acquired weakness and resultant decreased survival. Therefore, accurate assessment of the level of sedation and analgesia, maintaining adequate level of sedation, and daily evaluation of each patient and following adjustment could be important treatment strategy in critically ill patients. Recently, the strategy for sedation in the ICU is changing toward the direction of lowering sedation level or even "no sedation" with concurrent use of analgesics and the use of ultra short acting analgesics could be helpful in some patients. Clinicians should be aware of the importance of algorithmic approach including daily interruption of sedative and assessment of sedation level and especially in the patients under mechanical ventilation, organizational approaches such as the 'ABCDE' bundle could improve the management of critically ill patients.

  • Small hydropower, one of renewable energies, is the clean energy which emits no carbonic acid gas. It has noting to do with global warming. It is a useful resource to contribute to the dispersion of power. Because of ballooning oil prices, the need for small hydropower is increasing. Recently small hydropower development using dams, small rivers, reservoirs, water pipes, sewage treatment plants and cool ing water of thermal power plants is conducting.   • Production Information System make it possible to check and control the information of manufacturing processes. Based on this information companies are able to act to the price strategy and marketing strategy timely. In this study we will examine developing the MES(Manufacturing Execution System) as Production Information System in the Automobile Shock Absorber Piston rod Manufacturing Process of which processes are composed of heat treatment, lathing, grooving, milling, rolling, inspection and packing processes in order to control its production information efficiently.

  • The starting point of knowledge generation and management is the enhancement of learning capability and capacity of organizational members. Organizational change for learning environment should be aligned with the change of organizational strategy, structure and processes. The study employed action learning methodology to constitute learning organization processes. The treatment effect to institute learning organization has been successful thanks to the members' zeal and consensus to change the processes. However, not every learning team has been so successful. Some cases complained time consuming where others expect to be helpful for their incentives. The researchers concluded that the most important point for success of the learning organization project should be the support of top management.
