Effect of Treatment Setting of the Medical Services on the Patient Participation : Focusing on Moderating Effect of Negative Emotion (진료환경이 환자참여에 미치는 영향: 부정적 감정의 조절효과를 중심으로)

  • The main purposes of this study is to examine the effect of treatment setting on patient participation, and the moderating effect of negative emotion between treatment setting and patient participation. For the purposes of this study's goals, the 320 samples for this empirical study were collected from the general hospital and medical clinic outpatients in C and carried out hierarchial regression by SPSS 19.0. The results of this study are as follows. There is positive effect of cleanness of the treatment setting on behavior factor in patient participation. The higher aging of patient have influences on emotional factor in patient participation. There are positive effects of the third person in treatment settings on emotional and informational factors in patient participation. There is negative effect of negative emotion on emotional and informational factors in patient participation. There are negative effects of time pressure in treatment settings on emotional and informational factors in Patient Participation. On interaction effect, there are positive effects of cleanness in treatment setting and negative emotion on emotional and informational factors in patient participation. Implications for theoretical and practical patient participation are discussed.

  • Priority setting in national health insurances in major advanced countries and the nation was investigated to draw the criteria for priority setting and suggest the most rational criteria for dental insurance so as to help secure the efficiency of medicare financing and individual's health right and also elevate medical consumers' satisfaction with health insurance. 1. Priorities in national health insurance are different from country to country, depending on the medical security systems, priority introducing conditions, and social environment, but have many common factors. 2. The priority setting criteria for national health insurance in those countries include the following in common: the efficiency, equity, and cost effect of treatment, emergency of treatment, consumption of expense, efficacy of treatment, patient's receptiveness, patient's demand, severity of disease, and patient's responsibility for the disease. 3. In oral diseases, severe diseases including oral cavity cancer are low in rate, and in-hospital treatments are few. From the above findings, it is suggested that dental insurance should establish discriminative criteria for priority setting by reflecting the aspects of dental diseases and system difference between dental and other health insurances and taking account of efficiency of treatment through prevention, cost effect, prevalence and incidence of generalized diseases, and individual's financing burden.

The Effect of Fertigation Setting Point on the Growth and Fruit Quality of Sweet Pepper (Capsicum annuum L.) (관비재배에서 토양수분이 착색단고추의 생육과 품질에 미치는 영향)

  • Objective of this research was to investigate the effect of fertigation setting point such as -5, -10, -20, and -30 ㎪ on the growth and fruit quality of sweet pepper (Capsicum annuum L.) in greenhouse culture. The net $CO_2$ assimilation and transpiration rate were the lowest in the treatment of -30㎪. The pH and EC in soil solution were not severly affected by irrigation setting point and no statistical differences were observed among treatments of irrigation setting point tested. The N content of above ground plant tissue was the lowest in the treatment of -30 ㎪ and those of K, Ca, and Mg were the highest in the treatment of -10 ㎪. But that of P did not show statistical differences among treatments tested. As the fertigation setting point was getting low, the growth decreased at 60th day after planting, while there were no differences among treatments at 210th day after planting. The fruit quality except sugar contents did not show differences among treatments, but sugar contents was the highest in the treatment of -30 ㎪ with $8.0^{\circ}$Brix. Above results indicated that fertigation setting point should be in the range from -10 ㎪ to -20 ㎪ to ensure good crop growth and fruit quality in sweet pepper production.

Review of Traditional Chinese Medicine Clinical Studies on Tuina Treatment of Children's Autism Spectrum Disorder (소아 자폐스펙트럼 장애의 추나 치료에 대한 중의학 임상 연구 고찰)

  • Objectives The purpose of this study is to investigate the effect of Tuina treatment for children with Autism Spectrum Disorder (ASD), and to propose guidance to future studies or application in clinical setting. Methods The articles published from year 2000 to May 2021 by China National Knowledge Infrastructure (CNKI) were searched by setting up field as '中醫學', '中藥', '中西醫結合' and by setting up keyword as '小儿自閉症', '儿童自閉症', '自閉症譜系障碍', 'Autism', 'Autism spectrum disorder' that corresponds to participant and '推拿', '導引', '按摩', 'Tuina', 'massage'. Results Nine selected articles were analyzed and summarized by authors and year published, characteristics, diagnostic criteria, treatment methods and contents, treatment duration, evaluation criteria and results of the study. The articles were also reviewed by reported side effects, and evaluated the quality of the study according to Cochrane Handbook's Risk of Bias 2 (RoB 2) assessment of the Randomized Control Trial (RCT) articles. All studies used Tuina treatment to achieve effective results for pediatric autism spectrum disorder. Conclusion Pediatric Tuina treatment (TM) is a non-invasive treatment that does not stimulate patients with unstable ASD but increase patient adherence, and can be safely and simply operated. Therefore, pediatric Tuina treatment is worth utilizing as a clinical basis for further development of ASD treatment.

Economic Evaluation of Gemcitabine-cisplatin Chemotherapy for Non Small-Cell Lung Cancer Patient in an Outpatient Setting (비용-효과 분석 기법을 이용한 Gemcitabine 외래 항암 치료의 경제성 평가)

  • Purpose: This analysis was conducted to evaluate the cost-effectiveness of gemcitabine-cisplatin chemotherapy for non small-cell lung cancer patients in an outpatient setting compared with the traditional inpatient setting. Methods: A cost-effective analysis was conducted from a societal perspective. The effects of treatment, which was measured as an adverse event rate, were abstracted from a published literature search and empirical data from one university hospital. The costs included both direct and indirect costs. Direct costs included hospitalizations, outpatient visits, and lab tests. Pharmaceutical costs were excluded in analysis because they were same for both options. Indirect costs included productivity loss of patients as well as care-givers. In order to determine the robustness of the results, sensitivity analysis on treatment protocol was conducted. Results: Literature search showed no difference in adverse effect rates between inpatient treatment protocol and outpatient treatment protocol. Therefore, this analysis is a cost-minimization analysis. Cost-savings in the outpatient setting was 555,936 won for one treatment cycle. Our sensitivity analysis indicated that the outpatient chemotherapy still showed cost-savings, regardless of changes in treatment protocol. Conclusion: The outpatient gemcitabine-cisplatin chemotherapy for non small-cell lung cancer resulted in cost savings compared to inpatient chemotherapy. More importantly, outpatient chemotherapy could improve the utilization of health service resources in terms of available beds.

  • Generally, hair treatments by complementary actions to give a sense of activity are permanent setting, dyeing, and bleaching, etc. In this study, we investigated the wave formation ability and hair damage occuring in permanent setting and bleaching process. The wave formation ability was evaluated by the differences of length and curl diameter after permanent setting. And the hair damage was also evaluated by the protein release ability and SEM of hair. The bleached hair immediately after permanent setting treatment has better wave formation ability, but much more damaged than the bleached hair after rinsing thoroughly with warm water. It was considered that the chain of hair keratin broke down easily by the bleaching action in the existence of permanent setting agents.

  • The large forgings used in chemical plants or nuclear power plants are produced by complex heat treatment. because of thickness up to 200~300 mm and weight up to 200~300 ton, setting proper heat treatment cycle is so difficult. In addition, defects of products make companies wasting large money and valuable time. In this study, to reduce try & err, when setting heat treatment of reactor pressure vessel steel SA508Gr.3, carrying out the basic mechanical property test of SA508 Gr.3 and testing hardness of SA508Gr.3 in various tempering temperature. and calculating temper curve with Hollomon-Jaffe parameter.

Effects of heat setting temperature conditions on the mechanical properties of Polybutylene succinate (PBS) monofilament yarn after net-making (편망 후 열처리 온도가 PBS 모노필라멘트사의 물리적 특성에 미치는 영향)

  • The monofilament with 0.304mm of diameter was produced using a polybutylene succinate (PBS) resin, and a gill net was made by it. We investigated the impact of heat setting temperature on the mechanical properties, knot state and height of gill net. Heat treatment was carried out using the high pressure steam machine for 20 minutes at temperature of $55^{\circ}C$, $60^{\circ}C$, $70^{\circ}C$ and $75^{\circ}C$. Before heat treatment, the strength and elongation of PBS monofilament were estimated to be $48.1kg/mm^2$, 23.8% at unknot, $37.6kg/mm^2$, 18.8% at single knot, $26.6kg/mm^2$, 22.9% at double knot in dry condition, respectively. The strength and elongation of PBS monofilament with double knot were decreased as heat setting temperature increased, and the decreasing rate of strength was showed to be higher than that of elongation. It was not found any differences in strength and elongation of PBS monofilament yarn with double knot at the $65^{\circ}C$ and $70^{\circ}C$ of heat setting temperature by 5% significance of T-test, but there was a significant difference at the $70^{\circ}C$ and $75^{\circ}C$ of heat setting temperature. The net's height and length from leg to leg appeared no differences at the $70^{\circ}C$ and $75^{\circ}C$ of heat setting temperature. In results, it was investigated that the PBS monofilament gill net with the maximized physical properties could be manufactured at $70^{\circ}C$ of heat setting temperature using a high pressure steam machine for 20 minutes.

The Influence of Pore Water Contamination on the Cement Treated Sandy Soil (공극수 오염이 시멘트 고화처리된 사질토에 미치는 영향)

  • Laboratory experiments were carried out to investigate the influence of pore water contamination on the treatment effect of sandy soil which was solidified by Portland cement. In the experiments, setting time of hydraulic cement that was mixed with contaminated mixing water was measured using Vicat equipment and observed the tendency of setting process with the kind of contaminants, organic or inorganic components. It was shown that organic contaminants of the mixing water affect largely on the initial setting process of hydraulic cement and inorganics, expecially heavy metals, did not affect on the initial setting process, otherwise it was appeared that setting time of the sandy soil that was contaminated with inorganic components was apparently faster than the sandy soil that did not include inorganic components even though organic concentrations was relatively low level (COD=200∼300) in the mixing water. The results of unconfined compression strength test (UCST) were well consistent with the results of Vicat equipment test.

A Comparison between the Thermomechanical and Structural Changes in Textured PET Yarns after Superheated Steam and Dry Heat Treatment

  • PET yarns textured at different texturing conditions were treated with superheated steam or dry heat at different temperatures for different times. The effects of the treatment conditions on the thermomechanical and structural changes of the yarn were examined by shrinkage, X-ray diffraction and birefringence measurements. With increase in superheated steam temperature, the crystalline orientation factor and birefringence decreased, whereas crystal size increased. Dry heat treatment had a smaller effect on shrinkage and structural properties in comparison with superheated steam treatment. The additional shrinkage after texturing process was investigated. The effect of heat-setting in both media was more significant at $200^{\circ}C$. The time dependence of the properties was not linear.
