Clinical Guidelines for Hwabyung II. (Research on the Status of Hwabyung in Korea) (화병 임상진료지침 II. (화병의 실태))

  • Objectives : The Hwabyung Research Center of The Korean Society Of Oriental Neuropsychiatry attempted to produce clinical guidelines for doctors of Korean medicine for the treatment of Hwabyung. Methods : A standard guideline development process was followed. Relevant literature was identified by a review of bibliographies. The operational criteria were used to rate the quality of scientific evidence, and the line of treatment recommendations included a consensus clinical opinion. This section of "the status of Hwabyung in Korea" is 2 out of 5 articles drafted and reviewed by clinicians. Results : Hwabyung is changed depending on the time and have a risk of relapse. Like the previous studies, Female and lower socio-economic and educational level people suffers Hwabyung frequently. But recently number of Male Hwabyung patients increases. Recovery of Hwabyung is involved in depression, anxiety, stress. Hwabyung is related to Major depressive disorder, generalized anxiety disorder, somatoform disorder and diseases of the digestive system. Conclusions : This study was observed for the progression of Hwabyung. Hwabyung is a long-term disease associated with depression, anxiety and stress. Hwabyung is accompanied by physical and psychological symptoms and degrades the quality of life.

Current update on allergic rhinitis for Korean Medicine management (알레르기 비염의 한의학적 관리를 위한 최신 지견)

  • Objectives : The purpose of this review is to introduce the recent advance in allergic rhinitis and to provide help in establishing strategy and selection of drugs for Korean medical treatment of allergic rhinitis. Methods : We searched articles about allergic rhinitis comprehensively in PubMed, CNKI, JStage, KISTI. And in order to reflect clinical situation, we also reviewed some profession's writing for practitioners. Results : This review discussed allergic rhinitis's epidemiology, pathophysiology, diagnosis, treatment, prognosis. We found many standardized clinical practice guideline have been published in this field. And some guideline reflected accumulation of medical evidence on interventions in Korean medicine. It suggested that acupuncture, herbal medicine, herbal patch are useful to prevention and alleviate allergic symptoms. But some interventions have heterogeneity due to each nation's medical background. Conclusions : Acupuncture therapy is now recommended world widely for treating allergic rhinitis. But other interventions of Korean medicine are not well recognized in the same manner. We need more research to identify mechanism and rigorous clinical trials to clarify efficacy and safety of Korean medicine intervention.

  • Medical disputes are increasing year by year in korean medical institutions as the frequency of patient's visiting korean medical institutions goes high and the reliability relations between korean medical doctors and patients are getting worse than the past. Frequently occurring accidents in korean medical acts fall into some categories such as medical treatment of acupuncture${\cdot}$moxibustion${\cdot}$cupping, korean herbal medication, korean medical physical therapy, wrong diagnosis. Frequently occurring accidents include these cases. Infection due to improper treatment process of acupunctrue or wet-cupping ; skin-burns caused by moxibustion, infrared therapy or haet therapy ;toxic hepatitis or kidney dysfunction induced by herbal medication ; progression of disease or missing of opportunity of timely transfer because of wrong diagnosis. It will prevent medical accidents to a great extent for korean medical doctors to have scientific diagnostic techniques and lab. test results at hand in korean medical institutions.

Need for an Update for the Guideline for the Management of Mild Cognitive Impairment

  • Attention is being paid to diagnosis and treatment of mild cognitive impairment (MCI) because early diagnosis and preventive management can slow down the progression of Alzheimer's disease. In particular, in the present era, the use of biomarkers for predicting conversion into dementia is permitted in medical practice. Therefore, authors aimed to propose additional considerations when updating guidelines for the management of MCI, including predictable biomarkers, revising treatment option after additional clinical trials for cholinesterase inhibitors, and detailed regimes for lifestyle interventions. After reviewing 3 patients with MCI by detailed evaluation, we realized that cholinesterase inhibitors were not recommended. In addition, regular exercise and cognitive training were only possible recommendations for patients according to current guidelines, although all 3 patients had evidence of β-amyloid accumulation and related neurodegeneration. Furthermore, caregivers for all 3 patients were worried whether patients could keep doing regular exercise and cognitive training by themselves and asked about the economic training system which monitors patients so that they can keep training. Therefore, we propose that guidelines for managing MCI need to be updated in the present era when the use of biomarkers for predicting conversion into dementia is permitted in medical practice.

  • Industrial wastewater management guideline and evaluation model of Best Available Technologies for the leather tanning and finishing industry was developed as an economical evaluation model using evaluation of BAT including economical evaluation combined with cost analysis model and cost annualization model in considering of economical factors and non-water environmental factors. It was verified that approximately 10% will be increased annually to modify conventional treatment process ($3,700m^3/d$) of J leather wastewater treatment plant to advanced process of K leather wastewater treatment plant.

  • Despite progress made in tuberculosis control worldwide, the disease burden and treatment outcome of multidrug-resistant tuberculosis (MDR-TB) patients have remained virtually unchanged. In 2016, the World Health Organization released new guidelines for the management of MDR-TB. The guidelines are intended to improve detection rate and treatment outcome for MDR-TB through novel, rapid molecular testing and shorter treatment regimens. Key changes include the introduction of a new, shorter MDR-TB treatment regimen, a new classification of medicines and updated recommendations for the conventional MDR-TB regimen. This paper will review these key changes and discuss the potential issues with regard to the implementation of these guidelines in South Korea.

Optimization of Inpatient Management of Radioiodine Treatment in Korea (우리나라에서 방사성옥소입원치료 관리 최적화)

  • We established a model to calculate radioactive waste from sewage disposal tank of hospitals to optimize the number of patients receiving inpatient radioiodine therapy within the safety guideline in our country. According to this model and calculation of radioactivity concentration using the number of patients per week, the treatment dose of radioiodine, the capacity and the number of sewage tanks and the daily amount of water waste per patient, estimated concentration of radioactivity in sewage waste upon disposal from disposal tanks after longterm retention were within the safety guideline (30 Bq/L) in all the hospitals examined. In addition to the fact that we could increase the number of patients in two thirds of hospitals, we found that the daily amount of waste water was the most important variable to allow the increase of the number of patients within the safety margin of disposed radioactivity. We propose that saving the water amount be led to increase the number of patients and they allow two patients in an already furnished hospital inpatient room to meet the increasing need of inpatient radioiodine treatment for thyroid cancer.

  • Objective : The purpose of this study was to investigate consensus relative to treatment strategies for psychosocial treatment in panic disorder, that represents one subject addressed by the Korean guidelines for treatment of panic disorder 2018. Methods : The executive committee developed questionnaires relative to treatment strategies for patients with panic disorder based on guidelines, algorithms, and clinical trials previously published in foreign countries and Korea. Seventy-two (61.0%) of 112 experts on a committee reviewing panic disorder responded to the questionnaires. We classified the consensus of expert opinions into three categories (first-line, second-line, and third-line treatment strategies), and identified treatment of choice using the Chi-square test and 95% confidence intervals. Results : For psychosocial treatment of panic disorder, individual and group cognitive behavior therapy (CBT) were recommended treatments of choice, and mindfulness based cognitive therapy (MBCT) was recommended as first line strategy. There was statistically significant consensus among experts regarding usefulness of each component of CBT and MBCT, for treatment of patients with panic disorder. Conclusion : Results, that reflect recent studies and clinical experiences, may provide the guideline for psychosocial treatment strategies for panic disorder.

Optimum Dosage of Fenton's Reagent for the Dyeing Wastewater by the Different Conditions of Biological Treatment as the Pre-treatment Process (염색폐수의 생물학적 전처리 조건변화에 의한 최적 펜톤시약 투입량 결정에 관한 연구)

  • The consecutive combination process of a biological process as the pre-treatment and a chemical process as the post-treatment is applied for the dyeing wastewater. The poor efficiency of biological treatment using pure oxygen makes the chemical treatment cost high. It is necessary to improve the efficiency of biological treatment in order to reduce the cost of chemical treatment. The purpose of this paper is to find the minimum dose of chemical reagent to fit the Discharged Water Quality Standards for the different biological treatment effluents. Results revealed that the minimum dosage of Fenton's reagent lead to save the cost of chemical treatment based on the guideline dose in the treatment plant. The possible maximum saving reagents was up to $70\%$ for the effluent of the pilot plant packed with the carrier imbedded microorganisms which were selected from the present treatment plant.

Acute Toxicity Test for Wastewater from Several Drainage Canals and Discharges Using Daphnia Magna (생태독성도를 이용한 공단배수 및 공장배출수의 독성도 조사)

  • Daphnia Magna Stratus has been widely accepted as useful species for estimating the toxicity of chemicals to aquatic invertebrate and recommended as species for the testing chemicals from the international guideline as well as Korean guideline. The study was performed for the acute toxicity test by using water flea(D. Magna) for effluents from several wastewater treatment plants and drainage canals in GyeongBuk area. Five heavy metals, 1,4-Dioxane and Perchlorate were tested. Most Toxicity Units(TU) of Industrial wastewater effluents were less than 1 which means effluent was not toxic to D. Magna. However, effluents containing 1,4-Dioxane and Perchlorate were significantly toxic to D. Magna. Therefore, facilities should reduce the 1,4-dioxane since new regulations will force them after the year of 2011.
