총의치 교합면 형태가 하악골 응력 분포에 미치는 영향에 관한 삼차원 유한요소분석적 연구 (THREE DIMENSIONAL FINITE ELEMENT ANALYSIS OF MANDIBULAR STRESSES UNDER COMPLETE DENTURES WITH VARIANT ARTIFICIAL TEETH FORMS AND OCCLUSAL PATTERNS)

  • Complete denture occlusion must be developed to function efficiently and with the least amount of trauma to the supporting tissues. For the preservation of supporting tissues, it is imperative to reduce to a minimum the functional stress induced by dentures. The magnitude of the horizontal component of functional stress contributed by various occlusal teeth forms has not been studied. This study was aimed to investigate the influence of different occlusal teeth forms on the mode of distribution of the stresses in the mandibular tissue, and the displacement of lower dentures during the variant functional movement of mandible for this study three dimensional finite element analysis was used. FEM models were created using commercial software Super Sap for IBM 32 bit computer. The model was composed of 3380 brick elements and 4346 nodes. The results were as follows. 1. The magnitude of stress was similar between two models in centric occlusion, in the case of anatomic model, the stress was concentrated on the buccal side of alveolar ridge beneath the bicuspids. 2. During the protrusive movement, the increasing of stress from the posterior to anterior part of mandible was seen in the case of anatomic model. 3. During the lateral movement, the stress of anatomic model was greater than that of nonanatomic model. 4. The stress of anatomic model was concentrated on the anterior region of residual ridge during the lateral movement. 5. In the case of anatomic model the anterior part of denture was displaced severely at the centric and lateral position, but the denture of nonanatomic model was displaced minutely at the protrusive and lateral position.

Autograft Surgery Using the Condylar Fragment for Implant Placement

  • The fracture of facial bone usually accompanies alveolar bone fracture and dislocation or fracture of teeth. Thus, aside from the reduction of fracture, the reconstruction of occlusion through the rehabilitation of lost teeth should be considered. The dislocation of tooth after trauma accompanying alveolar bone fracture needs bone grafting in case of implant treatment. Although autogenous bone graft shows good prognosis, it has the disadvantage of requiring a secondary surgery. This is a case of a mandibular condyle head fracture accompanied by alveolar bone fracture. The condylar head fragment removed during open reduction was grafted to the alveolar bone fracture site, thereby foregoing the need for secondary surgery.

치주질환시의 혈생화학적연구 (Blood Chemistry in Periodontal Disease)

  • The causes of periodontal disease have been descried as malocclusion, trauma from occlusion, local irritants and poor oral hygien. The systemic factor has also been considered as a etiologic factor of periodontal disease. On the other aspect systemic condition could be affected by periodontal disease. For the study of relationships between periodontal disease and systemic condition, twenty patients with periodontal disease and twenty persons with no periodontal involvement were evaluated for inorganic elements and organic materials in the blood. The results of the blood analysis of the two groups were as follows. : Phosphorus and alkline-phoshatase in the group with periodontal disease showed slightly increased phenomenon compared to the control group, and on the other hand calcium and calcium-phosphorus ratio decreased phenomenon. But there is no any significant alteration in the content of each element between the two groups.

임프란트 수술이나 발치 후 지각이상 환자에서의 약물치료 (Pharmacologic management for the patient with paresthesia after implant surgery or extraction)

  • The inferior alveolar nerve provides unilateral innervation to the dentition, labial mucosa and skin from about commissure to the mental protuberance. Injury to this nerve resulting in sensory impairment can be a distressing problem to some patients. The causes of this problem include trauma, extraction, implant surgery and any maxillofacial surgery and generally the altered sensation is temporary. The surgical procedure has been the most common treatment for this condition but it has some complications. The antidepressants and anticonvulsants have been effective to the treatment of trigeminal dysesthesia. This case report suggests that the use of antidepressants and anticonvulsants is an alternative method to treat the paresthesia after implant surgery or extraction.

  • Crown fractures are relatively common trauma to anterior teeth, and should be restored immediately in most cases. For those who suffer from unfortunate traumatic episode, the best treatment option should be minimally invasive approach. In the presence of fractured tooth fragment, reattachment procedure creates positive emotional response in the patient and simplifies the procedure and maintenance of the patient's original tooth anatomy and occlusion. Without fractured tooth fragment, next conservative option could be direct composite restoration which is based on minimal invasion concept. This article proposes simple and very conservative techniques that anyone can do in daily practice.

교합외상(咬合外傷)이 치주조직(齒周組織)에 미치는 영향(影響)에 관(關)한 실험적(實驗的) 연구(硏究) (THE EFFECTS OF THE OCCLUSAL TRAUMA ON THE PERIODONTAL TISSUES)

  • The author attempted to observe the histological changes of the periodontal structures induced by trauma from occlusion. Eighteen healthy rabbits were devided into two groups; control and experimental group. Three rabbits were kept as control group, while metal crowns were seated on unilateral lower molar teeth of fifteen rabbits were kept as experimental group. And the interocc1usal distance of the incisal edge was kept 1.5mm from begining to the end of the experimental period. Rabbits of each group consisting with three rabbits were killed at the intervals of three days, one week, two weeks, four weeks, eight weeks. The antagonistic teeth of maxilla including periodontal teeth were excised and decalcified for histologic preparation. The results obtained were as follows. 1. Destructions and ulcer formation of the sulcular epithelium and gingival epithelium occurred and persisted from the beginning of the experiment to the four weeks after experiment. The epithelal attachment were proliferated apically. 2. The pressure site were observed at the apical protion, where they showed compression of the periodontal ligament, thrombosis and congestion of blood vessels and hemorrhages. 3. At the pressure site, there appeared osteoclasts and bone resorption from the first week of experiment and it became more prominent at the second week with the extend into the marrow spaces adjacent to the periodontal membrane. 4. The phenomenon of bone apposition and resorption occurred at the fourth week of experiment. The reverse line of bone trabecular were more prominent. And the reactions were ceased at the eighth week of experiment.

Result of Extracranial-Intracranial Bypass Surgery in the Treatment of Complex Intracranial Aneurysms : Outcomes in 15 Cases

  • Objective : The standard treatment strategy of intracranial aneurysms includes either endovascular coiling or microsurgical clipping. In certain situations such as in giant or dissecting aneurysms, bypass surgery followed by proximal occlusion or trapping of parent artery is required. Methods : The authors assessed the result of extracranial-intracranial (EC-IC) bypass surgery in the treatment of complex intracranial aneurysms in one institute between 2003 and 2007 retrospectively to propose its role as treatment modality. The outcomes of 15 patients with complex aneurysms treated during the last 5 years were reviewed. Six male and 9 female patients, aged 14 to 76 years, presented with symptoms related to hemorrhage in 6 cases, transient ischemic attack (TIA) in 2 un ruptured cases, and permanent infarction in one, and compressive symptoms in 3 cases. Aneurysms were mainly in the internal carotid artery (ICA) in 11 cases, middle cerebral artery (MCA) in 2, posterior cerebral artery (PCA) in one and posterior inferior cerebellar artery (PICA) in one case. Results : The types of aneurysms were 8 cases of large to giant size aneurysms, 5 cases of ICA blood blister-like aneurysms, one dissecting aneurysm, and one pseudoaneurysm related to trauma. High-flow bypass surgery was done in 6 cases with radial artery graft (RAG) in five and saphenous vein graft (SVG) in one. Low-flow bypass was done in nine cases using superficial temporal artery (STA) in eight and occipital artery (OA) in one case. Parent artery occlusion was performed with clipping in 9 patients, with coiling in 4, and with balloon plus coil in 1. Direct aneurysm clip was done in one case. The follow up period ranged from 2 to 48 months (mean 15.0 months). There was no mortality case. The long-term clinical outcome measured by Glasgow outcome scale (GOS) showed good or excellent outcome in 13/15. The overall surgery related morbidity was 20% (3/15) including 2 emergency bypass surgeries due to unexpected parent artery occlusion during direct clipping procedure. The short-term postoperative bypass graft patency rates were 100% but the long-term bypass patency rates were 86.7% (13/15). Nonetheless, there was no bypass surgery related morbidity due to occlusion of the graft. Conclusion : Revascularization technique is a pivotal armament in managing complex aneurysms and scrupulous prior planning is essential to successful outcomes.

변형적 경부청소술 후 내경정맥의 유지 (Internal Jugular Vein Patency after Modified Radical Neck Dissection)

  • 저자들은 변형적 경부청소술을 시행받은 두경부암환자 23례를 대상으로 술 후 내경정맥의 유지여부을 후향적으로 연구한 결과 총 34측 경부청소술 중 28측(82.4%)에서 내경정맥이 유지됨을 알 수 있었다. 이는 기존의 연구결과와 비슷한 개존율임을 알 수 있었다. 내경정맥의 폐쇄요인으로 술중 정맥의 손상과 혈류정체, 장기간 수술로 인한 정맥벽의 건조, 수술시 제거되는 근막의 소실이 정맥의 유착과 압박을 초래하는 것으로 알려져 있다. 또한 다양한 치료방법과 술 후 발생하는 여러 요인들에 의해 내경정맥이 영향을 받는데 본 연구에서는 국소재발과 창상합병증등이 주된 영향을 준 것으로 판단된다. 방사선치료는 다른 복합적인 요소가 결부되어 단독적인 영향으로 판단하기엔 충분치 않으며 양측 경부청소술은 내경정맥의 폐색과는 무관한 것으로 생각된다. 본 연구는 증례수가 작고 추적기간이 짧은 한계점이 있으므로 향후 좀 더 많은 증례와 충분한 추적기간을 통한 연구가 필요하리라 판단된다.

  • The advent of osseointegration and advances in biomaterials and techniques have contributed to increased application of dental implants in the restoration of maxillary partial edentulous patients. Often, in these patients, soft and hard tissue defects result from a variety of causes, such as infection, trauma, and tooth loss. These create an anatomically less favorable foundation for ideal implant placement. Reconstruction of the atrophic maxillary alveolar bone through a variety of regenerative surgical procedures has become predictable; it may be necessary prior to implant placement or simultaneously at the time of implant surgery to provide a restoration with a good long-term prognosis. Regenerative procedures are used for horizontal and vertical ridge augmentation. Many different techniques exist for effective bone augmentation. The approach is largely dependent on the extent of the defect and specific procedures to be performed for the implant reconstruction. It is most appropriate to use an evidenced-based approach when a treatment plan is being developed for bone augmentation cases. The cases presented in this article clinically demonstrate the efficacy of using a autogenous block graft, guided bone regeneration, ridge split, immediated implant placement technique on the atrophic maxillary area.

심하게 감소된 치주지지를 갖는 Cross-Arch Fixed Partial Denture에 대한 고찰 및 증례보고 (A Case Report and Review on Cross-Arch Fixed Partial Denture with Severely Reduced Periodontal Tissue Support)

  • 치주질환이 중증도 이상으로 진행되면 대개 많은 량의 치주조직지지가 상실된다. 일부 치아 주변으로 발생되는 이러한 조직 파괴는 그들의 심한 동요 때문에 일부 치아들은 스스로 탈락되거나 발치가 요구되는 단계까지 진행된다. 이러한 경우에 적절한 치주치료와 유지 그리고 교합평형을 고려한 치주-보철 치료를 포함하는 포괄적 치료계획이 그들의 건강, 기능, 심미 등의 수복을 위해 필요하게 되고, 이러한 치주-보철치료 중의 하나인 크로스-아치 고정성국소의치(cross-arch fixed partial dentures, CAFPDs)가 심하게 감소된 치주조직지지를 갖는 치아들의 안정을 위해 사용될 수 있다. 그러나 불행하게 이러한 보철물의 디자인에 따른 생역학적 개념과 교합양식에 대해 아직 많이 알려져 있지 않은 상태이다. 본 논고에서는 교합성외상 그리고 CAFPDs를 통해서 본 Ante's law의 재조명, 치료 원칙, 교합부여, 장기적 예후 등에 대한 정리를 보여줄 것이며 증례를 통해 그 가능성을 제시해 보고자 한다.
