응급의료센터에서 수혈을 시행한 성인 외상환자에서 사망 예측 인자 (Predictive Factors for Mortality among Adult Trauma Victims Transfused in an Emergency Department)

  • Purpose: The most common cuase of transfusion for trauma victims in an emergency department is hypovolemic shock due to injury. After an injury to an internal organ of the chest or the abdomen, transfusion is needed to supply blood products and to compensate tissue oxygen transport and bleeding. From the 1990's, there have been some reports that transfusion is one of the major factors causing multiple-organ failure. Thus, as much as possible, tranfusion has been minimized in the clinical setting. This study aims to analyze the prognostic factors for mortality among trauma victims transfused with blood products in an emergency department. Methods: We conducted this study for the year of 2010 retrospectively. The study group included adult trauma victims tranfused with blood products in our ED. The exclusion criteria were discharge against medical advice, and missing follow-up due to transfer to another facility. During the study period, 34 adult trauma victims were enrolled. We compared the clinical variables between survivors and non-survivors. Results: the mean age of the 34 victims was 58.06 years, and males account for 58.5% of the study group. The most-frequently used form transportation was ambulance(119, 55.9%), and the most common injury mechanism was mobile vehicle accidents(67.6%). The mean revised trauma score (RTS) was 5.9, and the mean injury severity score (ISS) was 47.76. The mortality rate in the ED was 58.5%, Comparison of survivors with non-survivors showed statistical differences in injury mechanism, initial SBP, DBP, RTS, ISS, and some laboratory data such as AST, ALT, pH, PO2, HCO3, glucose (p<0.05). Regression analyses showed that mortality among adult trauma victims transfused in the ED correlated with RTS. Conclusion: When an adult trauma victim is transported to the ED and needs a tranfusion, the emergency physician carefully assess the victim by using physiologic data.

Internal Changes of Blood Compartment and Heat Distribution in Swamp Buffaloes under Hot Conditions : Comparative Study of Thermo-Regulation in Buffaloes and Friesian Cows

  • From previous studies, there is a strong possibility in buffaloes that the marked increase in blood volume (BV) under hot conditions contributes to heat transportation from the rectum to the skin. The present study was done to clarify changes with environmental temperature on water-shift between blood and extracellular fluid (ECF), heat distribution between the rectum and the skin, and blood flow rates (BFR) at the hind legs (reflecting the skin surface). Four buffaloes and four Friesian cows were successively exposed to three different temperatures of $20^{\circ}C$, $30^{\circ}C$ and $35^{\circ}C$. BV and ECF volume were measured with Evans' blue and sodium-thiocyanate dilution methods, respectively. Rectal and subcutaneous (as the skin) temperatures were measured by copper-constantan thermocouples. BFR were measured by a supersonic blood flow meter. With an increase in environmental temperature, skin temperature in buffaloes increased significantly than cows, but rectal temperature was not significantly different between two species. BV, especially plasma compartment, increased significantly in only buffaloes, while ECF volume did not change in both species. BFR increased significantly in buffaloes, but not in cows. From these results, the increased of BV may be caused by water flowing from ECF compartment. The water-shift may induce the increase of BFR and skin temperature. It is suggested in the present study that internal changes of blood compartment in buffaloes contribute to transfer of heat to the skin surface.

저온의 고-액상변화 모듈 용기의 배열에 따른 축냉시스템의 수치해석 (Numerical Analysis of Cold Storage System with Array of Solid-Liquid Phase Change Module)

  • 본 논문은 육해상의 운송장치에 축냉시스템을 적용시키기 위한 기초 연구이다. 또한, 축냉재의 고액상변화에 대한 수치해석을 수행한 연구이다. 수치해석법으로는 유한차분법(Finite-Difference Method)을 이용하였으며, 1차원 비정상의 상태를 가정하여 계산하였다. 또한 용기는 직사각형의 구형용기로 가정하여 대칭의 조건을 이용하였다. 축냉을 목적으로 사용하는 열매체는 염화칼슘 수용액($CaCl_2$) 30wt%의 물성치를 사용하여 계산을 수행하였다. 계산에 영향을 미치는 요소로는 냉동고의 냉기 온도 및 냉기 유속이 있으며, 축냉재를 싸고 있는 용기는 플라스틱으로 가정하였다. 본 수치해석에서 경계층의 두께는 냉기의 속도 증가와 함께 얇게 되고 축열시간도 짧아지는 것을 확인할 수 있었다. 그리고 냉기의 유속이 빨라질수록 열전달이 촉진되어 축냉용기 전면부에서의 온도가 낮아짐을 알았다. 축냉용기의 후면부에서는 경계층이 두꺼워져 열전달이 전면부에 비해 작아짐을 알았다.

고령 장애인의 골다공증 유무에 따른 일상생활수행능력과 삶의 질 비교 (Comparison of ADL and QoL for the Osteoporosis and Non-osteoporosis in elderly people with disabilities)

  • Objective: This study examines the prevalence of osteoporosis, and compares with activity of daily living(ADL), instrumental activity of daily living(IADL) and health-related quality of life(QoL) among the elderly people with disabilities. Methods: This study analyzed the data of 3,113 persons with disabilities over 65 years of age who responded to the questionnaire using data from the National Survey of People with Disabilities in 2017 on the people with disabilities (PWD). Descriptive statistics, X2-test, and independent sample t-test were conducted using the SPSS win 21.0 program. Results: The prevalence of osteoporosis in the elderly PWD was 18.7%. There were significant differences in sex, age, type of disability, and disability severity according to the presence or absence of osteoporosis (p<0.05). Walking and Transfer of ADL were related to osteoporosis in elderly PWD. Financial management and Transportation use of IADL were related to osteoporosis (p<0.05). The PWD with osteoporosis were analyzed to have lower health-related quality of life compared to the disabled elderly without osteoporosis (p<0.05). Conclusion: Based on the results of this study, a strategy for developing a program for managing osteoporosis. Strengthening health management in the elderly PWD is required.

An ICI Canceling 5G System Receiver for 500km/h Linear Motor Car

  • This paper proposed an Inter-Carrier-Interference (ICI) Canceling Orthogonal Frequency Division Multiplexing (OFDM) receiver for 5G mobile system to support 500 km/h linear motor high speed terrestrial transportation service. A receiver in such high-speed train sees the transmission channel which is composed of multiple Doppler-shifted propagation paths. Then, a loss of sub-carrier orthogonality due to Doppler-spread channels causes ICI. The ICI Canceler is realized by the following three steps. First, using the Demodulation Reference Symbol (DMRS) pilot signals, it analyzes three parameters such as attenuation, relative delay, and Doppler-shift of each multi-path component. Secondly, based on the sets of three parameters, Channel Transfer Function (CTF) of sender sub-carrier number 𝒏 to receiver sub-carrier number 𝒍 is generated. In case of 𝒏≠𝒍, the CTF corresponds to ICI factor. Thirdly, since ICI factor is obtained, by applying ICI reverse operation by Multi-Tap Equalizer, ICI canceling can be realized. ICI canceling performance has been simulated assuming severe channel condition such as 500 km/h, 2 path reverse Doppler Shift for QPSK, 16QAM, 64QAM and 256QAM modulations. In particular, for modulation schemes below 16QAM, we confirmed that the difference between BER in a 2 path reverse Doppler shift environment and stationary environment at a moving speed of 500 km/h was very small when the number of taps in the multi-tap equalizer was set to 31 taps or more. We also confirmed that the BER performance in high-speed mobile communications for multi-level modulation schemes above 64QAM is dramatically improved by the use of a multi-tap equalizer.

외상으로 인한 사망 환자의 임상적 분석 (Clinical Analysis of Death in Trauma Patients)

  • Purpose: Trauma is the 5th most common leading cause of death in Korea, but there has been no appropriate management system for patients until now. We analyzed the main causes of death in trauma patients by comparing the characteristics of those patients with the characteristics of patients who survived. We feel this analysis should have a positive effect on the development of an appropriate trauma management system in Korea. Methods: We retrospectively reviewed trauma patients who had been admitted to the Department of General Surgery from February 2002 to February 2007. We compared several expected risk factors between the mortality and the survival group. Data on the transportation, arrival time at the emergency center, amount of transfusion, initial shock index, cause of death, and initial physical condition according to RTS (Revised trauma score), ISS (Injury severity score) and TRISS (Trauma and Injury Severity Score) were collected. Patients with ISS lower than 12 were excluded. Results: Three hundred sixty-six(366) patients with multiple injuries were included. There were 40 patients in the mortality group and 326 patients in the survival group. The mean arrival time (minutes) to emergency center was longer in the mortality group (137.6 vs 93.6 p 0.04). The total amount of transfusion (ml) was larger in the mortality group (7139 vs 2470 p 0.01). The initial shock index was higher in the mortality group (1.45 vs 1.17 p<0.01). The RTS, ISS, and TRISS were not statistically different between the groups. In the multivariate analysis, mean arrival time and initial shock index were important factors for survival. Conclusion: If the mortality rate of trauma patients is to be reduced, the arrival time at the emergency center should be minimized. Improvement of the emergency medical transfer service system is very important for achieving that.

  • This article reviews the papers published in the Korean Journal of Air-Conditioning and Refrigeration Engineering during 2015. It is intended to understand the status of current research in the areas of heating, cooling, ventilation, sanitation, and indoor environments of buildings and plant facilities. Conclusions are as follows. (1) The research works on the thermal and fluid engineering were carried out in the areas of flow, heat and mass transfer, cooling and heating, and air-conditioning, the renewable energy system and the flow inside building rooms. Research issues dealing with air-conditioning machines and fire and exhausting smoke were reduced. CFD seems to be spreading to more research areas. (2) Research works on heat transfer area were carried out in the categories of heat transfer characteristics, pool boiling and condensing heat transfer and industrial heat exchangers. Researches on heat transfer characteristics included the economic analysis of GHG emission, micro channel heat exchanger, effect of rib angle on thermal performance, the airside performance of fin-and-tube heat exchangers, theoretical analysis of a rotary heat exchanger, heat exchanger in a cryogenic environment, the performance of a cross-flow-type, indirect evaporative cooler made of paper/plastic film. In the area of pool boiling and condensing, the bubble jet loop heat pipe was studied. In the area of industrial heat exchangers, researches were performed on fin-tube heat exchanger, KSTAR PFC and vacuum vessel at baking phase, the performance of small-sized dehumidification rotor, design of gas-injection port of an asymmetric scroll compressor, effect of slot discharge-angle change on exhaust efficiency of range hood system with air curtain. (3) In the field of refrigeration, various studies were carried in the categories of refrigeration cycle, alternative refrigeration/energy system, system control. In the refrigeration cycle category, a cold-climate heat pump system, $CO_2$ cascade systems, ejector cycles and a PCM-based continuous heating system were investigated. In the alternative refrigeration/energy system category, a polymer adsorption heat pump, an alcohol absorption heat pump and a desiccant-based hybrid refrigeration system were investigated. In the system control category, turbo-refrigerator capacity controls and an absorption chiller fault diagnostics were investigated. (4) In building mechanical system research fields, eighteen studies were reported for achieving effective design of the mechanical systems, and also for maximizing the energy efficiency of buildings. The topics of the studies included energy performance, HVAC system, ventilation, and renewable energies, piping in the buildings. Proposed designs, performance tests using numerical methods and experiments provide useful information and key data which can improve the energy efficiency of the buildings. (5) The field of architectural environment was mostly focused on indoor environment and building energy. The main researches of indoor environment were related to the user and location awareness technology applied dimming lighting control system, the lighting performance evaluation for light-shelves, the improvement evaluation of air quality through analysis of ventilation efficiency and the evaluation of airtightness of sliding and LS window systems. The subjects of building energy were worked on the energy saving estimation of existing buildings, the developing model to predict heating energy usage in domestic city area and the performance evaluation of cooling applied with economizer control. The studies were also performed related to the experimental measurement of weight variation and thermal conductivity in polyurethane foam, the development of flame spread prevention system for sandwich panels, the utilization of heat from waste-incineration facility in large-scale horticultural facilities.

CPFD를 이용한 태양열 유동층 흡열기의 수력학적 특성 해석 (Analysis of Hydrodynamics in a Directly-Irradiated Fluidized Bed Solar Receiver Using CPFD Simulation)

  • 실리콘 카바이드 입자(평균 입도 123 ㎛)의 유동층 태양열 흡열기의 성능 및 효율에 영향을 미치는 입자 거동 해석을 위해 MP-PIC 모델을 이용하여 전산모사를 수행하였고, 기존 실험결과와의 비교를 통해 검증하였다. 특히, 본 연구에서는 실험적으로 접근하기 어려운 유동층 표면 부근에서의 거동을 모사함으로써 흡열 성능과 입자 거동과의 상호 영향을 분석하였다. CPFD 모사결과는 입자층 및 프리보드에서의 평균 고체체류량과 압력요동 등 수력학적 특성 실험결과를 잘 예측하였다. 입자 흡열기에서 1차적으로 태양열 에너지를 흡수하여 층 내부로 전달하는 층 표면 부근에서의 국부 고체체류량은 입자층 내 기포거동에 따라 중심부에서 상대적으로 낮은 값을 나타내는 불균일 분포를 나타내었다. 프리보드 영역에서 국부 고체체류량은 기체속도가 증가할수록 축방향과 각 높이에서의 횡방향에서 불균일성이 증가하였고, 이는 입자 흡열기의 프리보드 영역 내 비산된 입자에 의해 반사된 태양광 에너지 손실과 연관된 압력강하 상대표준편차 증가의 원인임을 나타내었다. 입자 흡열기 내 기체속도 증가에 따른 국부적인 기체 및 입자 속도의 변화에 대한 고찰을 통해, 유동층 내 국부적인 입자거동 특성은 Geldart B 입자 물성과 관련된 입자층 내 기포 거동과 밀접하게 연관됨을 확인하였다. 유동층 입자 흡열기의 성능 척도인 일사량 당 유동기체의 출입구 온도차(∆T/IDNI)는 입자 층 표면 및 표면 상부 프리보드 영역 내 압력요동 RSD와 상관관계가 매우 높음을 확인하였고, 이 결과는 흡열기 성능 개선에 활용할 수 있을 것으로 판단되었다.

집중 태양열에 의한 온도구배가 열전발전모듈의 출력 성능에 미치는 영향 (Influence of temperature gradient induced by concentrated solar thermal energy on the power generation performance of a thermoelectric module)

  • 일반적으로 열전발전 소자를 사용하여 에너지 하베스팅을 하는 경우, 시스템의 작동환경에 의해 주어지는 온도구배를 활용하게 된다. 따라서 열전소자의 특성상 큰 온도구배를 기대하기 어려운 작동환경에서는 원하는 출력을 얻을 수 없으며, 작동 온도가 높을 때 얻어지게 되는 최적의 발전효율을 기대하기 힘들다. 자연환경에서 얻을 수 있는 태양에너지를 활용한 신재생 에너지의 활용은 그 동안 태양광발전이나 태양열발전에 국한되어 왔다. 태양광발전은 태양광의 일정 파장대만 사용하고 빛의 산란에 의해 발전효율이 낮아지는 단점이 있으며, 태양열발전은 일반적으로 대규모 설비를 갖춰야 하는 공간상의 제약이 있다. 본 연구에서는 태양열을 집광하여 열전소자에 조사함으로서 큰 온도구배를 형성하여 상용 열전소자의 출력을 향상시킬 수 있는 간단한 소형 발전시스템을 설계 및 제작하였다. 장시간 태양열 집중을 위해 태양 추적 장치를 설치하였으며, 열전소자 하부에 고온의 태양열이 전달되어 온도 편차가 줄어드는 현상을 막기 위해 액체 순환식 냉각기를 설치하여 큰 온도구배를 유지할 수 있도록 설계한 후, 일련의 실험으로 시험하여 그 유용성과 타당성을 검증하였다.

철도차량용 지능형 변압기 손실 저감을 위한 Dual Active Bridge 컨버터의 Modulation 기법 연구 (Modulation Technique of Dual Active Bridge Converter to Improve Efficiency of Smart Transformers in Railroad Traction System)

  • 철도차량용 구동 시스템의 주 변압기를 지능형 변압기로 대체하는 것은 철도차량의 효율을 높이기 위한 효과적인 방법이다. 지능형 변압기는 고전압 입력단을 저전압 직류단으로 변환하는 정류단 모듈과, 변환된 복수의 저전압 직류단 전압을 직/병렬로 연결하여 저전압 직류 출력 전압을 제어하는 Dual Active Bridge (DAB) 컨버터 모듈로 구성된다. DAB 컨버터는 고주파 변압기를 사이에 두고 양 측 Full-Bridge 컨버터의 전압 합성 방법에 따라 전력이 양방향으로 전달될 수 있다. 이 때, 양 측 Full-Bridge 컨버터의 전압 합성 방법에 따라 고주파 변압기로 흐르는 전류의 형태가 달라지게 되며, 이는 효율에 직접적인 영향을 미치게 된다. 본 논문은 중/저 전력 조건에서 DAB 컨버터의 효율을 개선하기 위한 새로운 modulation 방법을 제안한다. 제안하는 modulation 방법은 DAB 컨버터 내에 발생할 수 있는 무효 전력을 0으로 제어하도록 하며, 출력단 컨버터를 다이오드 정류기와 유사한 동작을 하도록 함으로써, 스위칭 손실을 최소화하도록 하였다. 제안된 modulation 방법을 통한 손실 저감은 900Vdc의 직류단 전압을 갖는 DAB 컨버터에 대한 시뮬레이션 결과를 통해 검증하였다.
