Celiac Disease in South Jordan

  • Purpose: Celiac disease, an autoimmune enteropathy triggered by exposure to gluten, is not uncommon in South Jordan. However, its prevalence is underestimated due to lack of physician awareness of the diversity of disease presentation. The clinical spectrum includes classic gastrointestinal manifestations, as well as rickets, iron-deficiency anemia, short stature, elevated liver enzymes, and edema. Our goal was to evaluate celiac disease presentation in clinically diagnosed children. Methods: Retrospective study included all children diagnosed with celiac disease between September 2009 and September 2015. Hospital charts were reviewed. Demographic data, clinical characteristics, and follow-up were recorded. Results: Thirty-five children were diagnosed with celiac disease during the study period. Mean age${\pm}$standard deviation was $6.7{\pm}3.8$ years (range, 2.0-14 years). There were 17 (48.6%) female patients. The average duration between onset of symptoms and diagnosis was $16.3{\pm}18.7$ months. Fifteen (42.9%) patients presented with classic malabsorption symptoms, whereas 7 (20.0%) patients presented with short stature. Positive tissue transglutaminase antibodies (tTg)-immunoglobulin A (IgA) was seen in 34 (97.1%) patients. The one patient with negative tTg-IgA had IgA deficiency. Although tTG-IgA values were not available for objective documentation of compliance, clinical data (resolution of presenting abnormalities and growth improvement) assured acceptable compliance in 22 (62.9%) patients. Conclusion: CD in children may present with diverse picture. Although of the small number, the non-classical presentations are not uncommon in our rural community. Gluten-free diet is the main strategy for treatment and associated with usually correction of laboratory abnormalities and improvement of growth.

Impact of Serological and Histological Factors on Neurological Manifestations in Children and Adults with Celiac Disease

  • Purpose: Celiac disease (CD) is a common autoimmune disease with extra-intestinal manifestations, including neurological disorders. There are few reports to assess various factors in increasing the chances of developing neurological disorders in CD, so we designed this study. Methods: All patients with CD at any age who had been referred to the Celiac Clinic were evaluated for neurological problems. CD was defined as IgA anti-transglutaminase antibodies (anti-tTG) of 18 IU/mL or higher in serology and Marsh type I or more severe in histopathological evaluation. Logistic regression analysis was used to evaluate the impact of various independent variables on the neurological manifestations. Results: A total of 540 patients enrolled in this study. A 360 (66.7%) of patients were children. A 64.8% and 35.2% were female and male, respectively. Overall, 34.1% of patients had neurological manifestation, including headache, neuropathy, epilepsy, and ataxia. The odds of developing neurological manifestations in children were significantly lower than in adults (odds ratio [OR], 0.66; 95% confidence interval [CI], 0.45-0.96; p=0.03) and in patients with gastrointestinal (GI) symptoms significantly higher than in the group without GI manifestations (OR, 1.77; 95% CI, 1.18-2.63; p=0.005). Other variables, including Marsh classification (OR, 0.44; 95% CI, 0.18-1.11; p=0.08) and anti-tTG levels (OR, 1.00; 95% CI, 0.999-1.001; p=0.59) did not significantly increase the chances of developing neurological disorders. Conclusion: Our study showed that increasing age and the presence of GI symptoms, but not serological and histological findings, could increase the chances of developing neurological diseases in CD patients.

Enhancement of skin barrier and hydration-related molecules by protopanaxatriol in human keratinocytes

  • Background: Protopanaxatriol (PPT) is a secondary intestinal metabolite of ginsenoside in ginseng. Although the effects of PPT have been reported in various diseases including cancer, diabetes and inflammatory diseases, the skin protective effects of PPT are poorly understood. Methods: HaCaT cells were treated with PPT in a dose-dependent manner. mRNA and protein levels which related to skin barrier and hydration were detected compared with retinol. Luciferase assay was performed to explore the relative signaling pathway. Western blot was conducted to confirm these pathways and excavated further signals. Results: PPT enhanced the expression of filaggrin (FLG), transglutaminase (TGM)-1, claudin, occludin and hyaluronic acid synthase (HAS) -1, -2 and -3. The mRNA expression levels of FLG, TGM-1, HAS-1 and HAS-2 were suppressed under NF-κB inhibition. PPT significantly augmented NF-κB-luc activity and upregulated Src/AKT/NF-κB signaling. In addition, PPT also increased phosphorylation of the mitogen-activated protein kinases (MAPKs) ERK, JNK and p38 and upstream MAPK activators (MEK and MKK). Furthermore, transcriptional activity of AP-1 and CREB, which are downstream signaling targets of MAPK, was enhanced by PPT. Conclusion: PPT improves skin barrier function and hydration through Src/AKT/NF-κB and MAPK signaling. Therefore, PPT may be a valuable component for cosmetics or treating skin disorders.

굴 가수분해물이 SD-Rat의 혈청과 간 균질물에 미치는 항산화 효과 (Anti-oxidative Effect of Oyster Hydrolysate on the Serum and Hepatic Homogenate in SD-rats)

  • Sprague Dawley계 rat을 이용하여 굴 효소 가수분해물 함유식이가 혈청과 간 균질액에 미치는 항산화 효과를 조사하였다. 식이 그룹은 대조군과 실험군으로써 TGPN 굴 효소 가수분해물 100 mg/kg BW/day 함유 식이를 섭취한 실험군(TGPN-100FG), TGPN 200 mg/kg BW/day 함유 식이를 섭취한 실험군(TGPN-200FG), PN 가수분해물 시료 100 mg/kg BW/day 함유 식이를 섭취한 실험군(PN-100 FG), 200 mg/kg BW/day 함유 식이를 섭취한 실험군(PN-200FG) 5 그룹으로 나누어 6주간 식이를 제공하여 실험하였다. 총 콜레스테롤은 TGPN-200FG 식이군이 혈청과 간 균질액에서 각각 28.9%, 26.1%까지 유의적으로 감소하였으며, 간 균질물의 중성지질은 TGPN-100FG 식이군에서 6.3%까지 유의적으로 감소하였다. 수퍼옥사이드 라디칼은 TGPN-200FG에서 유의적으로 34.7% 감소하였다. 굴 가수분해물 TGPN-200FG는 수퍼옥사이드 및 히드록실 라디칼을 소거함으로써 라디칼 소거에 관여하는 효소의 활성 수준을 감소시키며, 단백질 산화를 억제하는 것으로 나타났다.

감압 알칼리 수세하여 제조한 고등어 Surimi의 품질 특성 (Quality Characteristics of Mackerel Surimi Prepared by Alkaline Washing under Reduced Pressure)

  • 고등어 surimi를 효율적으로 생산하기 위한 연구의 일환으로 감압수세장치를 설계 제작하고 이 장치를 이용하여 각각 상압과, 560 및 660 mmHg의 감압 하에서 각각 1, 3, 5 및 7회 알칼리 수세하고 압력 및 수세횟수에 따른 고등어 surimi의 품질 특성을 검토하였다. 설계 제작한 감압수세장치는 연속수세가 가능하였으며 양질의 surimi를 효율적으로 제조할 수 있었다. 전체의 수세조건을 통하여 surimi의 수분함량은 $72.0{\sim}72.9%$, 조지방 $4.8{\sim}5.7%$ (수세하지 않은 것, 7.0%), pH $6.9{\sim}7.0$ (수세하지 않은 것, 6.0), VBN $6.9{\sim}7.0\;mg/100\;g$ 및 가압드립 $6.7{\sim}8.3%$이었으며, 단백질 추출성은 560 mmHg, 5회 수세 시 염용성, 수용성 및 기질 단백질추출성이 각각 3,694, 6,036 및 1,424 mg/100 g으로써 각각 가장 높았다. $Mg^{2+}-$$Ca^{2+}-ATPase$ 활성은 560 mmHg, 5회 수세 시 각각 0.25 및 $0.17\;{\mu}mol\;Pi/min/mg$ actomyosin으로써 가장 높았다. Setting gel의 TGase 활성은 560 mmHg, 5회 수세 시 3.932 nmol/mg이었으며 gel 강도는 setting gel 및 cooked gel에서 각각 420 g cm (상압, 320 g cm) 및 485 g cm (상압, 412 g cm)로써 각각 가장 높았다. 전반적으로 보아 고등어 surimi 제조를 위한 가장 적합한 수세조건은 760 mmHg, 660 mmHg 및 560 mmHg 압력 중 560 mmHg의 감압 하에서 5회 반복 수세하는 것이었다.

Influence of Age and Type 1 Diabetes Mellitus on Serological Test for Celiac Disease in Children

  • Purpose: Serological tests of tissue transglutaminase (TTG) and deamidated gliadin (DGP) antibodies for celiac disease diagnosis show conflicting correlation with histology in young children and in type 1 diabetes mellitus (T1DM). Tests' ability to predict histology and cutoff values based on age and T1DM was evaluated. Methods: A retrospective study of children who had celiac serological tests between 6/1/2002 and 12/31/2014 at a pediatric hospital. Results: TTG IgA displayed similar results in predicting histology between <4.0 and ≥4.0 years age groups with sensitivity 98% and 93%, and specificity 88% and 86%, respectively. In children <4.0 years old, sensitivity for DGP antibodies was 100% and specificity 94%; in ≥4.0 years age groups, sensitivity was 60%, 88% for DGP IgA and IgG and specificity 95%, 96%, respectively. TTG IgA had low specificity in patients with T1DM compared with non-T1DM, 42% vs. 91%. Positive TTG IgA with normal histology was associated with higher T1DM prevalence at 36% compared with negative tests at 4%. Finally, the TTG IgA cutoff value was higher in T1DM at 36 vs. 16.3 units in non-T1DM. DGP IgG cutoff showed similar values between age groups; TTG IgA and DGP IgA cutoffs were lower in <4.0 years at 8.3 and 11.9 units than ≥4.0 years at 23.4 and 19.9, respectively. Conclusion: TTG IgA is sufficient for the <4.0 years age group and DGP antibodies had no advantage over TTG IgA in older children. The cutoff value to determine a positive TTG IgA should be higher for children with T1DM.

아세트아미노펜 유도 HepG-2 세포주 손상에 대한 굴 효소 가수분해물의 보호 효과 (Protective Effects of Enzymatic Oyster Hydrolysate on Acetaminophen-induced HepG-2 Cell Damage)

  • 본 연구는 굴 가수분해물이 아세트아미노펜에 의한 간독성의 무독화 효과를 HepG-2 세포를 사용하여 조사하였다. 굴 가수분해물은 굴 단백질의 가교연결을 위해 TGase로 전처리하거나(TGPN) 혹은 하지 않고(PN), 1% Protamex와 1% Neutrase 단백질 분해효소로 2단 가수분해하였다. 두 종류의 굴 가수분해물은 아세트아미노펜으로 간 손상을 유도한 세포에 각각 처리하여 세포 생존율을 측정하였으며, 세포 배양 시 배양액으로 유출된 GOT와 GPT 활성을 측정하였다. TGPN 가수분해물의 경우 아세트아미노펜만을 처리한 negative 대조군($60.7{\pm}3.2%$)에 비하여 $100{\mu}g/mL$$200{\mu}g/mL$의 농도에서 각각 $136.2{\pm}1.4%$$179.6{\pm}3.8%$의 높은 세포 생존율을 보였다. PN 가수분해물은 $100{\mu}g/mL$$200{\mu}g/mL$의 농도에서 각각 $107.9{\pm}8.8%$$130.6{\pm}7.6%$의 세포 생존율을 보였다. GOT 활성은 negative 대조군의 경우에 $38.3{\pm}0.2$ Karmen/mL이었으며, TGPN($200{\mu}g/mL$)과 PN($200{\mu}g/mL$)에서는 $19.9{\pm}0.5$$22.0{\pm}2.4$ Karmen/mL로 농도에 따라 유의적인 활성 감소를 확인할 수 있었다. 그리고 GPT 활성도 GOT와 같은 경향의 활성을 나타내었다. 이 같은 결과에 미루어 굴 유래 가수분해물의 간 보호 건강 기능 식품 혹은 약물 개발의 가능성을 확인하였으며 앞으로 가수분해 펩티드 중의 유효성분에 대한 구조동정과 작용 기전에 관한 연구가 필요할 것으로 보인다.
