Traditional Chinese Medicine in the Treatment of Mental Retardation;A Review Study (정신지체에 대한 한의학 치료와 연구 동향;2003-2007년 중국 임상 논문에서)

  • Objectives This study was designed to analyze the clinical studies on Mental retardation(MR) in traditional Chinese medicine(TCM). Methods For this study, we searched the clinical studies on MR, which had been published from 2003 to 2007, through web site CNKI(中國知識基礎設施工 http://www.cnki.net). There were 17 clinical studies and we focused on those studies. Results 1. In those Chinese studies, they used following words to describe Mental retardation; 小人弱智($xi\check{a}o\acute{e}rru\grave{o}zh\grave{i}$), 智能發育不全($zh\grave{i}n\acute{e}ngf\bar{a}y\grave{u}buqu\acute{a}n$), 智力低下($zh\grave{i}l\grave{i}d\bar{i}xi\grave{a}$), 精神發育遲滯($j\bar{i}ngsh\acute{e}f\bar{a}y\grave{u}ch\acute{i}zh\grave{i}$), 智能落后($zh\grave{i}n\acute{e}nglu\grave{o}h\grave{o}u$), 智能落后($zh\grave{i}n\acute{e}ngch\acute{i}hu\check{a}n$), 失天愚型患人($xi\bar{a}nti\bar{a}ny\acute{u}x\acute{i}nghu\grave{a}n'\acute{e}r$). 2. There were many kinds of TCM treatment methods for MR, such as herbal medicine, acupuncture, electroacupunture, acupoint injection, Chuna therapy, and special education. And those TCM treatments methods showed higher efficacies in the treatment of MR compared with Western medicine. 3. Mental retardation was related with the deficiency of heart, kidney, liver, spleen(心虛, 腎虛, 肝虛, 脾虛) and the pathological mechanism of Phlegm(絹) and Blood stasis(慫沂)in the studies about the Bian Zheng-the types of differential diagnosis- of MR. 4. Most of studies used Intelligence Quotient(IQ) to assess the efficacy of TCM treatment of MR. And the duration of treatment, the degree of illness, the age of patient, and the cause of illness affected the prognosis of MR. Conclusions These results suggest that traditional medicine could be one of the useful treatments on MR. And these results could be used in the clinical practices and studies on MR in Korea.

A Clinical Case Report of a Lung Cancer Patient with Shoulder Pain after Lobectomy (폐엽절제술 후 견배부 통증을 호소한 폐암환자 치험 1례)

  • Objectives : Quality of life(QoL) in patients with lung cancer usually goes down after pneumoectomy, and the pain is the most common cause. Uncontrolled postoperative pain can restrict the movements of shoulders resulting in frozen shoulder. This case report is to report the effect of herb medicine, Ohjuksan on a 49-year-old female lung cancer patient with shoulder pain after lobectomy. Methods : The patient was treated with Ohjuksan for 10 days. The patient's symptoms were assessed by range of motion(ROM) of shoulder, Visual analogue scale(VAS) of shoulder pain and abdominal diagnosis. Results & Conclusions : The patient showed improvement of ROM, VAS and abdominal diagnosis. As the shoulder pain is relieved from VAS6 to VAS3, ROM of abduction, adduction and internal rotation were increased. Also discomfort of abdomen was disapeared to a certain degree. This suggests that herb medicine, Ohjuksan is effective for treating postoperative pain of shoulder for a Lung cancer patient.

  • Objectives: The basis research of aged people's healthcare technology is very important, because our nation goes into an aged society rapidly. Especially in traditional medicine, pulse shapes of aged people were supposed to weak and empty pulse until now, but any objective measurement and compare research do not taken. Methods and Results: Therefore in this study we design observation study to find characteristic of pulse between younger group (age $20{\sim}30$) and elder group (age $43{\sim}80$) in Busan area. We used 3-dimensional pulse analyzer '3-D MAC' (Daeyomedi Co., Ltd.). And major parameters for comparison are 1) pulse height (h1), 2) applied pressure, 3) pulse power (energy), 4) high pressure duration (w) and 5) R-AI. Conclusions: According to analysis result, elder people's pulse has more large value in pulse height, applied pressure and pulse power than younger's that. It reveals that elder people in modernized lifestyle has Yang symptoms as well Yin symptoms. Consequently we need new method to care elder people's health, and this means more important direction than old text book. From now on verification research with objective diagnosis device like 3-D MAC, must be activated to build of evidence in traditional medicine. Cause traditional medicine's spirit of care and prevention is suitable to care elder people's cardiac and cerebral vascular disease those are have close relation with elder people.

  • Background and Purpose : The concept of $Wux{\'{\i}}ng$(五行) is central to all elements of the Koran traditional medicine. And $Wux{\'{\i}}ng$ is considered to one of the constitution theories in some of oriental medicine fields in South Korea. The aim of this study is to find necessary and concrete estimation factors for distributing patterns of $Wux{\'{\i}}ngr{\'{e}}n$(五行人) respectively. Methods : We translated and summarized the records about distinguishing characteristics and distributing points of Wu-Xing Ren described in <$L{\'{\i}}ngshu$(靈樞)${\cdot}$Yin-Y${\'{a}}$ng ${\`{e}}rsh{\'{\i}}wur{\'{e}}n$(陰陽二十五人)>. Some review articles were identified through searches of KISS and KERIS databases. Results and Conclusion : Concrete distinguishing characteristics and distributing points of Wu-Xing Ren were described in <$Lingshu{\cdot}Yin-Y{\'{a}}ng\;{\`{e}}rsh{\'{\i}}wur{\'{e}}n$>. According to <$Lingshu{\cdot}Yin-Y{\'{a}}ng\;{\`{e}}rsh{\'{\i}}wur{\'{e}}n$>, the most notable characteristics and distributing points are the skin color and shape of face. but the skin color and shape of face are too ambiguous to be used as distributing criteria in a practical manner. In stead size of head, longitudinal length of back, disposition, and width between both shoulders may be used as distributing criteria in practical clinic fields.

Analysis of Western-Korean cooperative treatment in hospital-care of patients with dementia

  • Objectives : The purpose of this study is to provide data to guide dementia health-care policy in Korea and to establish the position of Korean medical specialists in long-term care hospitals by analyzing the data of dementia inpatients. We analyzed the actual condition of dementia patients in care hospital and the effect of Western-Korean cooperative medicine on the progress of dementia. Methods : From January 1, 2016 to December 31, 2016, inpatients who were diagnosed with dementia at Mungyeong municipal long-term care hospital and admitted for more than 3 months were enrolled. Their medical records and simple tests were analyzed retrospectively. Results : We examined the detailed diagnosis, including both main and sub diagnosis, and Alzheimer disease dementia, at 97%, was the most common. At the time of admission, Korean Version of the Mini-Mental State Exam (K-MMSE) analysis showed that severe dementia affected 52%, and most were rated as Geriatric Depression Scale (GDS) 6. Based on the admission date, the results of a simplified test applied to the dementia patients every 6 months showed an maintain in the K-MMSE and GDS scores in 83%. Conclusion : The results of this study show that the rate of progression of dementia is somewhat lower in inpatients with moderate to severe Alzheimer's who have received Western-Korean cooperative treatment. However, due to institutional limitations, long-term inpatients such as those with dementia do not receive active traditional Korean medical treatment; hence, it is necessary to improve the national institution of traditional Korean medicine in long-term care hospitals.

Cold sensitivity classification using facial image based on convolutional neural network

  • Objectives: Facial diagnosis is an important part of clinical diagnosis in traditional East Asian Medicine. In this paper, we proposed a model to quantitatively classify cold sensitivity using a fully automated facial image analysis system. Methods: We investigated cold sensitivity in 452 subjects. Cold sensitivity was determined using a questionnaire and the Cold Pattern Score (CPS) was used for analysis. Subjects with a CPS score below the first quartile (low CPS group) belonged to the cold non-sensitivity group, and subjects with a CPS score above the third quartile (high CPS group) belonged to the cold sensitivity group. After splitting the facial images into train/validation/test sets, the train and validation set were input into a convolutional neural network to learn the model, and then the classification accuracy was calculated for the test set. Results: The classification accuracy of the low CPS group and high CPS group using facial images in all subjects was 76.17%. The classification accuracy by sex was 69.91% for female and 62.86% for male. It is presumed that the deep learning model used facial color or facial shape to classify the low CPS group and the high CPS group, but it is difficult to specifically determine which feature was more important. Conclusions: The experimental results of this study showed that the low CPS group and the high CPS group can be classified with a modest level of accuracy using only facial images. There was a need to develop more advanced models to increase classification accuracy.

  • Hyungsang Medicine is a medical theory a Korean traditional medical doctor Park In Kyu established based on the contents of "Donguibogam" that is currently practiced commonly today in Korean TKM clinics. Although it is based on "Donguibogam", the most basic text of TKM, it differs from "Donguibogam" in that its diagnosis, treatment, and treatment methods of illnesses are based on the person's appearance. This study organizes the medical theory by considering its relation to other medical theories and organizing the theoretical backgrounds through examining documents that it is based on.

  • Objectives: The abdominal examination is a diagnosis method for determining a morbid condition of the chest and abdomen by touching and pressing. This study reviewed research trends concerning the abdominal examination and suggested of future research directions for quantification of abdominal examination. Methods: A systematic literature search was carried out for relevant articles published between 2000 and 2016 in five databases such as Korean studies Information Service System(KISS), Oriental Medicine Advanced Searching Integrated System(OASIS), DataBase Periodical Information Academic (DBpia), National Digital Science Library and PubMed based on the Keywords 'abdominal examination'. Results: 128 articles were collected for analysis. Overview on the abdominal examination based on selected 17 articles. Then selected articles have been reclassified by diseases of clinical research and parameters for diagnosis of abdominal examination. Conclusions: Taken together, instrument for quantitative evaluation of abdomen characteristic fit to the traditional Korean Medicine have not yet developed. It is suggested that in order to dominate the market about abdominal examination in advance, more concern should be paid to establishing appropriate development of quantitative criteria and instrument. This review will help researcher to systematically understand and to develop Korean medical device as globally competitive device.

An expert survey for developing pattern diagnosis instrument of persistent allergic rhinitis (통년성 알레르기 비염의 변증 도구 개발을 위한 전문가 집단 설문조사 연구)

  • Background : According to the rising cases of clinical researches with Traditional Korean Medicine for persistent allergic rhinitis patients, the need for developing and applying Pattern diagnosis instrument has been increasing. Objective : This study was done to investigate and pragmatize the Pattern diagnosis instrument for persistent allergic rhinitis patients launched by KFDA in 2008. Methods : Data was collected by structured survey papers from Korean Medicine doctors participate in the Conference of The Korean Oriental Medical Ophthalmology & Otolaryngology & Dermatology Society in October 2012. We investigated their representative Patterns(변증) of every questions and used the mean results for assuming agreement rates. The importance order of 8 symptom domains for considering Pattern diagnostic steps were also asked and analyzed for inducing importance rates. Finally based on above two results, we developed weight points of each questions by multiplying agrement percents with importance rates. Results : 50 Korean medicine doctors described the informed consent and survey papers. Response agreement rates of each questions of Pattern diagnosis instruments were analyzed and described in Table 2. Comparing the survey results with diagnostic references, matched percent of 肺胃熱 was the 87.9%, 肺氣虛寒 was 62.5%, 肺脾氣虛 was 62.5% and 腎元虧虛 was 50%. Mean importance rates of rhinorrhea was 7.28, otoscope diagnosis 5.12, obstruction 5.04, sneeze 4.82, symptom duration 4.63, other body condition 3.54, tongue diagnosis 3.02, nasal pruritus 2.86, accordingly. Final importance rates of each questions were assumed, and the range of them was from 1.60 to 4.72, which were listed in Table 5. Conclusion : These results might provide the rational backgrounds and practical methods for developing and utilizing methods of Pattern diagnosis questionnaire for perennial allergic rhinitis.

A Review on Tibetan Traditional Medicine (티벳 전통의학(傳統醫學)에 관(關)한 고찰(考察))

  • Objectives : There has been little known about the Tibetan medicine in the society for Korean medicine. The aim of this study is to review the system of Tibetan medicine and compare with Korean medicine. Methods : The authors investigated several literatures that mentioned Tibetan medicine and organized in physiology, pathology, diagnostics, and treatment. And then, we interpreted the characteristics of Tibetan medicine as well as compared Tibetan medicine with Korean medicine. After that, we analyzed the commons and the differences, and also found out the meaning of Tibetan medicine. Results : The theory of Tibetan medicine is basically constituted of three elements of Lung, Tripa, and Peken in every parts of physiology, pathology, diagnostics, and treatment. Many organs of human body are compared to the fabrications of building. There is a detail explanation about the process of the development of fetus in Tibetan medicine. Tibetan medicine uses taking pulse in wrist, analysis of urine, watching of tongue, sperm, and menstruation, and etc. for diagnosis. In Tibetan medicine, regimen is prior to other treatments such as surgical treatment and medications. Conclusions : There is the oriental thought of '3' in Tibetan medicine, and esoteric buddhism is solved in Tibetan medicine. The anatomy and the diagnostics using urine, sperm, and menstruation have especially been developed in Tibetan medicine. Tibetan medicine emphasizes the feature of preventive medicine.
