Identification of Potential Prognostic Biomarkers in lung cancer patients based on Pattern Identification of Traditional Korean Medicine Running title: A biomarker based on the Korean pattern identification for lung cancer

  • Objective : We studied prognostic biomarkers discovery for lung cancer based on the pattern identification for the personalized Korean medicine. Methods : Using 30 tissue samples, we performed a whole exome sequencing to examine the genetic differences among three groups. Results : The exome sequencing identified among 23,490 SNPs germline variants, 12 variants showed significant frequency differences between Xu and Stasis groups (P<0.0005). As similar, 18 and 10 variants were identified in analysis for Xu vs. Gentleness group and Stasis vs. Gentleness group, respectively (P<0.001). Our exome sequencing also found 8,792 lung cancer specific variants and among the groups identified 6, 34, and 12 variants which showed significant allele frequency differences in the comparison groups; Xu vs. Stasis, Xu vs. Gentleness group, and Stasis vs. Gentleness group. As a result of PCA analysis, in germline data set, Xu group was divided from other groups. Analysis using somatic variants also showed similar result. And in gene ontology analysis using pattern identification variants, we found genes like as FUT3, MYCBPAP, and ST5 were related to tumorigenicity, and tumor metastasis in comparison between Xu and Stasis. Other significant SNPs for two were responsible for eye morphogenesis and olfactory receptor activity. Classification of somatic pattern identification variants showed close relationship in multicellular organism reproduction, anion-anion antiporter activity, and GTPase regulator activity. Conclusions : Taken together, our study identified 40 variants in 29 genes in association with germline difference of pattern identification groups and 52 variants in 47 genes in somatic cancer tissues.

Simultaneous Quantification of Three Marker Compounds in Samultang by HPLC/DAD (HPLC-DAD를 이용한 사물탕 중 3종 성분의 동시분석법 확립)

  • Samultang is one of traditional medicine composed of Paeonia lactiflora, Angelica gigas, Rehmannia glutinosa and Cnidium officinale. To develop simultaneous determination of paeoniflorin, decursin and 5-HMF in Samultang, a high performance liquid chromatography with diode array detector was used. To separate three marker components, Dionex $C_{18}$ column (5 ${\mu}m$, 120 ${\AA}$, 4.6 mm${\times}$150 mm) was used with a gradient elution system of water and methanol. UV wavelength of detector set at 230 nm and 280 nm. This method was validated by linearity, precision test and recovery test. Calibration curves of three standard components were showed good linear regression ($R^2$>0.9973). LOD and LOQ ranged from 0.08 ${\mu}g$/ml to 0.38 ${\mu}g$/ml and 0.25 ${\mu}g$/ml to 1.16 ${\mu}g$/ml, respectively. The relative standard deviations (RSDs) of data of the inter-day and intra-day experiments were less than 0.54% and 0.89%, respectively. The measured results of recovery test were varied from 93.36 to 107.79 with RSD values 0.01~1.45%. The established method was applied for separation of bio-conversion Samultang sample and compared with control sample.

Analysis of Existing Guidelines and Randomized, Controlled, Clinical Trials for Development of [Guideline of Clinical Trial with Herbal Medicinal Product for gastric cancer] (위암 한약제제 임상시험 가이드라인 개발을 위한 한약제제 무작위배정 대조군 임상시험 고찰)

  • Objectives: This study aimed to learn what should be considered in [Guideline of Clinical Trial with Herbal Medicinal Product for Gastric Cancer)] by analyzing the existing guidelines and clinical trials. Methods: The development committee searched guidelines for herbal medicinal product or gastric cancer developed already. Then, clinical trials for gastric cancer using herbal medicine were searched. The searched trials were analyzed in terms of inclusion and exclusion of participants, intervention, comparator, outcomes and trial design. Then, we compared the results of analysis with the regulations and guidelines of Ministry of Food and Drug Safety to suggest the issue that we will have to consider when making the [Guideline of Clinical Trial with Herbal Medicinal Product for Gastric Cancer]. Results: As a result, few guidelines for anti-tumor agent and clinical trial with herbal medicinal product were searched in the national institution homepage. In addition, 10 articles were searched by using the combination following search term; 'stomach neoplasm', 'herbal medicine', 'Medicine, Korean traditional', 'Medicine, Chinese Traditional', 'TCM', 'TKM', 'trial'. Most trials included gastric cancer participants with medical history of operation. The type of intervention was various such as decoction, granules, and fluid of intravenous injection. Comparators were diverse such as placebo, conventional treatment including chemotherapy and nutritional supplement. The most frequently used outcome for efficacy was quality of life. Besides, the symptom score, tumor response, and survival rate were used. Safety was investigated by recording adverse events. Conclusion: We found out some issue by reviewing the existing guidelines and comparing it with clinical trials for gastric cancer and herbal medicinal products. These results will be utilized for developing [Guideline of Clinical Trial with Herbal Medicinal Product for Gastric Cancer].

  • This case report is to show the effects of Traditional Korean medicine (TKM) on the patient with delayed encephalopathy after carbon monoxide poisoning. A patient with delayed encephalopathy after carbon monoxide poisoning occurs rarely and only a few cases have been reported. We treated a patient with cognitive deficit, gait disturbance was treated with herbal medicine(Boshiniknoe-tang-gami), acupuncture, moxibustion. We evaluated the improvements of symptoms by Korean Version of Mini Mental Status Examination score(K-MMSE), Modified Barthel Index(MBI) and Glasgow Coma Scale(GCS). After 3 weeks, total score of K-MMSE was increased from 12 to 17 and it was maintained to week 6. After 6 weeks, total score of MBI was increased from 30 to 86. And GCS was increased from 11 to 14. This report proved effect of Traditional Korean medicine on the improvement of delayed encephalopathy after carbon monoxide poisoning

  • Pulse is one of the basic diagnostic information of TKM(Traditional Korean Medicine). To quantify and standardize pulse diagnosis, we had collected an amount of clinical data from May 2005 by using newly developed pulse analyzer. But there were many noises in pulse wave according to measuring method, environment, operator and condition of patient. So some data can’t be included for analyzing diagnosis. To reduce noises from measuring pulse and to collect reliable pulse wave data, we made the process map of measuring method and applied six sigma project. With this we can improved the method of measuring pulse wave in collecting clinical data. The project follows a disciplined process of five macro phases: define, measure, analyze, improve and control (DMAIC). A process map and C-E diagram are used to identify process input and output variables. The major input variables are selected by using C&E matrix, and process map is developed by analyzing input variables. And the optimum process conditions are going to be controled to avoid in increasing loss of collecting pulse wave data.

  • Nutrient and Blood are really common conceptions in Traditional Korean Medicine(TKM). However, those are used in mixed. not only because of their common points but users who doesn't have clear classifications between them. Therefore, I will explain those owns by explaining origins, places of origons, pathway, circulations, organs related to them and color. In addition, I will try to explain thi things in common without conceptions of their own. As a result, the first, nutrient is what is originated from water and food at middle energizer and circulated in meridian vessel by control of spleen. The second, blood is what is originated from qi of water and food and circulated in blood vessel by control of heart and its color is red. The third, nutrient blood is what is originated from water and food or qi of water and food and circulated in meridian or blood vessel by control of spleen and heart. Also it is red and its function is nourishing human body. The name of Nutrient blood is a new term that I made and it is different from nutrient or blood. Also it is different from nutrient and blood. Nutrient blood is combination by conceptions, nutrient and blood is combination by word itself.

  • Purpose: The purpose of this study is to report the effect of Takrisodok-eum gami(托裏消毒飮加味: TRSDEGM) on Breast Abscess. Methods: One patient was a 35-year-old woman who complains pain, induration, burning sensation and skin flare of left breast, the other patient was a 49-year-old woman who complains pain, burning sensation and edema of right breast. Patients were treated by Herb medicine(TRSDEGM Water Extract). And the progress of symptoms was evaluated by checking the change of VAS, and inspecting the DITI. Results: After taking TRSDEKM, pain, induration, burning sensation, skin flare and edema were alleviated or resolved. And they haven#t recurred. Conclusion: TRSDEKM is expected to have positive effect on Breast Abscess.

  • It is the well-known truth that processing of raw information is needed to a certain extent during information search. Especially for Oriental Medical information, it becomes much clearer that even more complex processing is necessary. As a means of reducing such complexity, this study suggests a way to understand effectively the organic relationships among information found on the interface. In this process, 'knowledge-based archaeological' method has been used. A new concept of interface observed by this research is the study of a system which contains realistically considered knowledge-based archaeological and historical specificity. These models are organized so that search results could be materialized in different tree-structured interface models, which can help one understand the relationships among wanted search results at one glance and confirm the details of those results via mouse click. Strength of the vertical tree structure resides in its capability of suggesting its users clear historical relationship between separate Oriental medical information. The horizontal tree structure enables deeper understanding of sectional interrelationship of searched information. The strength of the prescription tree structure is that it helps one understand the lineage of prescriptions, as Oriental medicinal treatment is often summarized into changes in prescriptions.

  • The aim of this study is to evaluate the effect of cosmetic products containing concentrated ginseng using novel methods based on Traditional Korean medicine (TKM). In TKM, inspection of facial skin was an important method to diagnose symptoms and body condition of patients. Doctors in oriental medicine examined a patient as observing symptoms on the face and skin related to inner organs. This was called "mang-jin. In this study, eleven female, who have been using the cosmetic products containing concentrated ginseng for the last 5 years, and eleven healthy controls, who have never used the same product, were recruited. Three doctors in oriental medicine evaluated 17 inspection factors on facial skin. And 7 non-invasive skin bioengineering factors were assessed by various instruments at the same time. In results, 7 factors of the inspection were better in using the product than in controls. And sebum, roughness, pigmentation and wrinkle depth were significantly lower in using the product than in controls. So, this results propose that the cosmetic product containing concentrated ginseng has a good anti-aging effect on skin. And it is suggested that the novel evaluation method combined, Traditional Korean medicine and modern bioengineering technique, has to be developed for holistic concept of Hanbang cosmetics.

A Study of Application of Exercise Treatments according to Sasangin (사상인의 운동요법 적용에 관한 연구)

  • Sasang Constitutional Medicine (SCM) is based on the theory of Sasang (Taeyang, Soyang, Taeum, Soeum) in Traditional Korean Medicine (TKM). It is hypothesized that all of the things in the material world can be divided into the four categories of SCM. We made a new attempt to combine nutritive components, and patterns in physical activities with SCM. At first, we hypothetically divided the nutrients into 4 compartments according to SCM theory, and human body composition as well. These factors could be basically related to Sasangin Exercise Treatment (SET). Therefore, we applied sasangin theory to exercise prescription. As a research method, we hypothesized that human beings' types can be distinguished by mind and body. Mind was focused on exercise psychological aspects, and body also handled by nutrients, body compositions, body shapes, and exercise physiological aspects. For application of sasang theory to the exercise prescriptions, Taeumin as a sample sasangin was selected in this study. We concluded that in perspective of general traits, Taeumin have to reduce their carbohydrate intake volume, but greatly increase their intake of vitamins, minerals, and water in daily diet. They are also recommended exercises that can increase cardiorespiratory function and make them sweat, like endurance running, hiking, swimming, cycling, etc.
