위암 한약제제 임상시험 가이드라인 개발을 위한 한약제제 무작위배정 대조군 임상시험 고찰 (Analysis of Existing Guidelines and Randomized, Controlled, Clinical Trials for Development of [Guideline of Clinical Trial with Herbal Medicinal Product for gastric cancer])

  • Objectives: This study aimed to learn what should be considered in [Guideline of Clinical Trial with Herbal Medicinal Product for Gastric Cancer)] by analyzing the existing guidelines and clinical trials. Methods: The development committee searched guidelines for herbal medicinal product or gastric cancer developed already. Then, clinical trials for gastric cancer using herbal medicine were searched. The searched trials were analyzed in terms of inclusion and exclusion of participants, intervention, comparator, outcomes and trial design. Then, we compared the results of analysis with the regulations and guidelines of Ministry of Food and Drug Safety to suggest the issue that we will have to consider when making the [Guideline of Clinical Trial with Herbal Medicinal Product for Gastric Cancer]. Results: As a result, few guidelines for anti-tumor agent and clinical trial with herbal medicinal product were searched in the national institution homepage. In addition, 10 articles were searched by using the combination following search term; 'stomach neoplasm', 'herbal medicine', 'Medicine, Korean traditional', 'Medicine, Chinese Traditional', 'TCM', 'TKM', 'trial'. Most trials included gastric cancer participants with medical history of operation. The type of intervention was various such as decoction, granules, and fluid of intravenous injection. Comparators were diverse such as placebo, conventional treatment including chemotherapy and nutritional supplement. The most frequently used outcome for efficacy was quality of life. Besides, the symptom score, tumor response, and survival rate were used. Safety was investigated by recording adverse events. Conclusion: We found out some issue by reviewing the existing guidelines and comparing it with clinical trials for gastric cancer and herbal medicinal products. These results will be utilized for developing [Guideline of Clinical Trial with Herbal Medicinal Product for Gastric Cancer].

A molecular genetic study on the fruiting-body formation of Cordyceps militaris.

  • In the fungal genus Cordyceps, the type species Cordyceps militaris produces bioactive ingredients and exhibits medicinal effects as a Traditional Chinese Medicine (TCM), The fruiting bodies of C.militaris have now been mass-produced artificially and used as functional food and medicine in China. The unstable variation in forming fruiting body is however a key restrictive factor in industrial production. The genetic study on in vitro stromata formation of C. militaris has rarely been carried out. Here, we report the effects of genetic variation including the mating system on perithecial stromata formation of C. militaris. Monoconidial isolates which have both MAT1-1-1 and MAT1-2-1(genotype MAT1-1/2) could produce stromata. While the isolates only have either MAT1-1-1 or MAT1-2-1 (genotype MAT1-1 or MAT1-2) failed to produce stromata. Despite obvious heterothallism, homothallism was occasionally observed in a few isolates of C. militaris. High genetic variation was observed amongst the different monoconidial isolates of C. militaris. The unstable variation or lose of fruiting body formation was found to be caused by the inner-species high genetic variation of C. militaris. These results also indicated that C. militaris sexually behaved as both heterothallic and homothallic and required two mating type compatible in the same culture in order to produce regular clubshaped perithecial stromata.

갑상선기능항진증의 한약 치료에 대한 무작위 대조군 연구의 문헌 고찰 (A Review of Randomized Controlled Trials of Herbal Medicine Treatment for Hyperthyroidism)

  • Objectives: The purpose of this study is to analyze the effectiveness and safety of herbal medicine treatment for treating hyperthyroidism. Methods: We searched for randomized controlled trials of Hyperthyroidism treated with herbal medicine, published from January 2016 to May 2021, through electronic database such as Pubmed, EMBASE, China Academic Journal (CAJ). Interventions and results of the selected clinical studies were analyzed. Results: The 25 randomized controlled trials were finally included according to the inclusion and exclusion criteria. Treatment group was treated with herbal medicine alone in 2 studies, and with herbal medicine and conventional medicine in 23 studies. Control group was treated with conventional medicine. Outcome measures are total efficacy rate, thyroid function test, Traditional Chinese medicine (TCM) syndrome scores, liver function test, recurrence rate, etc. Prunellae Spica (夏枯草) (60%) was the most frequently used herb in herbal medicine treatment. In all of 25 studies, treatment group was more effective and safer for hyperthyroidism than the control group. Conclusions: This study indicates that herbal medicine treatment alone or combined with conventional medicine treatment could be helpful in improving the therapeutic effect on hyperthyroidism and reducing side effects as well.

한약과 양약의 개념 설정 어떻게 할 것인가? (How to establish the definition of Conventional medicine and Korean herbal medicine?)

  • Objectives : The purpose of this study is to find a reasonable solution to a current status of drug classification between the drugs of western conventional (allopathic) medicine and Korean medicine. A clear and distinct concept on the drugs of allopathic medicine and Korean medicine based on reasonable concepts and broad consensus is a pressing issue in Korea, and will facilitate the development of herbal medicinal products and pharmaceutical industry. Methods : Considering the issue of drug classification from domestic and international regulations, we reviewed the current Drug Law of Korea and China, Guidance for Industry, Botanical Drug Products of USA, Directive 2004/24/EC of the European parliament and of the council. Results : In Korea, the drug classification of allopathic medicine and Korean medicine is quite vague even though differential licensure system is enforced for the clinicians of allopathic medicine and Korean medicine field. According to the definition in the Drug Law, the scope of Korean medicine drug is so broad that even a drug made of single-compound material, as well as herbal extract of crude mixture, is regarded as a drug of Korean medicine, as long as the material may be separated from medicinal herbs, animal tissues, or mineral resources. Only new compound not found in natural resources are outside of the scope of Korean medicine drug. In USA and EU, medicinal products manufactured from herbs are approved by separate regulations for the herbs with special waivers. In China, the category of new medicine and the definition of allopathic medicine and traditional chinese medicine are clearly specified and classified. Conclusions : As medicines are validated therapeutic materials for efficacy and toxicity, we suggest that generally the concept of conventional medicines is based on a single compound that has been synthesized and individually validated and that of Korean medicines is based on a compound extracted from natural materials or a complex of compounds that has been validated as a whole in its totality.

Evaluation of Women with Myofascial Abdominal Syndrome Based on Traditional Chinese Medicine

  • Objectives: This study used semiology based on traditional Chinese medicine (TCM) to investigate vital energy (Qi) behavior in women with abdominal myofascial pain syndrome (AMPS). Methods: Fifty women diagnosed with chronic pelvic pain (CPP) secondary to AMPS were evaluated by using a questionnaire based on the theories of "yin-yang," "zang-fu", and "five elements". We assessed the following aspects of the illness: symptomatology; specific location of myofascial trigger points (MTrPs); onset, cause, duration and frequency of symptoms; and patient and family history. The patients tongues, lips, skin colors, and tones of speech were examined. Patients were questioned on various aspects related to breathing, sweating, sleep quality, emotions, and preferences related to color, food, flavors, and weather or seasons. Thirst, gastrointestinal dysfunction, excreta (feces and urine), menstrual cycle, the five senses, and characteristic pain symptoms related to headache, musculoskeletal pain, abdomen, and chest were also investigated. Results: Patients were between 22 and 56 years old, and most were married (78%), possessed a elementary school (66%), and had one or two children (76%). The mean body mass index and body fat were 26.86 kg/cm2 (range: 17.7 - 39.0) and 32.4% (range: 10.7 - 45.7), respectively. A large majority of women (96%) exhibited alterations in the kidney meridian, and 98% had an altered gallbladder meridian. We observed major changes in the kidney and the gallbladder Qi meridians in 76% and 62% of patients, respectively. Five of the twelve meridians analyzed exhibited Qi patterns similar to pelvic innervation Qi and meridians, indicating that the paths of some of these meridians were directly related to innervation of the pelvic floor and abdominal region. Conclusion: The women in this study showed changes in the behavior of the energy meridians, and the paths of some of the meridians were directly related to innervation of the pelvic floor and abdominal region.

편두통 환자의 한약치료 임상연구에 대한 고찰 (Review of Clinical Research on Herbal Medicine Treatment of Migraine)

  • Objectives: The purpose of this study was to investigate the effects of herbal medicine treatment for patients with migraine and to suggest research methods for herbal medicine treatment on migraine. Methods: In this study, a search was conducted through several academic sites using a combination of terms '편두통', 'Migraine', '한약', '한약치료', 'Herbal medicine', 'Herbal medicines', 'Herb', 'Traditional Chinese Medicine', and 'TCM'. Randomized controlled trials using herbal medicine treatments for adult patients with migraine were selected. Results: Ultimately, 46 papers were selected and analyzed. A statistically significant improvement was noted in the treatment group in terms of clinical migraine symptoms and other migraine evaluation tools before and after the herbal medicine treatment. Herbal medicines were administered in decoction, pill, and granule formulations. Many kinds of medicinal herbs, such as 解表藥類, 補益藥類, 淸熱藥類, 活血祛瘀藥類, and 平肝藥類, have been used for migraine. Among them, 川芎, belonging to 活血祛瘀藥類, is mentioned 36 times and is the most frequently used medicine. Herbal medicine was used safely for migraine treatment, without major adverse reactions, and the recurrence rate was significantly lower in the treatment group than in the control group. Conclusions: In conclusion, the herbal medicine treatment for patients with migraine showed a statistically significant improvement in 46 papers. Future studies should utilize standardized and objective evaluation tools, along with appropriate experimental design. The relevant articles should be increased to a significant level to verify the effect of herbal medicine treatments on migraine.

한국 참구의학의 체질관점과 실용적 통합치료 의학으로서의 특징 (Constitutionally individualized and practically integrated characteristics of Korean Acupuncture)

  • Objectives: Although Korean Acupuncture (KA) in Korean Medicine (KM) shares its origins with Traditional Chinese Medicine (TCM), it has made its own contribution to the field of acupuncture and general medicine. This study reviews characteristics of KA. Methods: Representative theories of KA were reviewed in light of a constitutionally individualized and practically integrated approach. Individualized approach in KA is based on constitutional energy traits recognized in Saam, Taegeuk, and Eight Constitution acupuncture theories. New practically integrated therapeutic modalities, such as herbal, and bee venom acupunctures, have also been developed in KA. Results: As a form of natural medicine, KA along with KM put emphasis on the vitality and self-correcting mechanism of an individual, which led to the refinement of constitutional approach. KA along with KM theories features a summarized understanding and practical application of integrated therapeutics, which produced creative development of new modalities. Conclusions: KA is expected to make significant contributions to the individualized and practical medicine, based on basic research and clinical trials.

Quantitative study of acupuncture manipulation of lifting-thrusting using an needle insertion-measurement system in phantom tissue

  • Objectives: Quantification, objectification, and standardization of lifting-thrusting manipulation are important issues in traditional Chinese medicine (TCM). The purpose of this study was to quantitatively investigate the difference in the amount of stimulation according to range and frequency among parameters of lifting-thrusting manipulation with the use of a needle insertion-measurement system. Methods: For quantification of lifting-thrusting manipulation, an acupuncture needle insertion-measurement system was used in phantom tissue. The motor and force sensors of the needle insertion device were connected to the control software. This enabled operation of the lifting-thrusting manipulation and measurement of the acupuncture needle force. The measurement of the acupuncture needle force according to various frequencies (0.25, 0.50, 0.75, and 1 Hz) and ranges of movement (2, 4, 6, 8, and 10 mm) was repeated 10 times. Results: At a constant frequency of movement, acupuncture needle force according to range of movement (2, 4, 6, 8, and 10 mm) increased with increasing range of movement (p < 0.05). At a constant range of movement, acupuncture needle force according to frequency of movement (0.25, 0.50, 0.75, and 1.0 Hz) increased with increasing frequency of movement (p < 0.05). Conclusion: In this study, we conducted a quantitative comparison of the amount of stimulation according to range and frequency, the main parameters of lifting-thrusting manipulation, by using an acupuncture needle insertion-measurement system. Future studies on various manipulations and parameters are warranted to quantify and objectify the amounts of stimulation by acupuncture manipulation.

  • Background: Liu-Yuan-Lei(陸淵雷) said that a medical record is both the marks of treatments and arts made by a excellent practitioner and the essence of TCM(Traditional Chinese Medicine). Jiang-Guan(江瓘) also said that reading medical records is one of the best way to develop one’s abilities If curing a disease without perfect clinical practice. Objectives: study on the special treatment about metrostaxis(崩漏) based on the Yi-Da-Gan(易大艮)’s medical records. and study on the differentiation of abnormal symptoms and signs about cold syndrome with pesudo-heat(眞寒假熱) based on the Yu-Chang(喩昌)'s medical records. Methods: First, read and study the medical records on metrostaxis(崩漏) of that made a profound study by Yi-Da-Gan(易大艮) and cold syndrome with pesudo-heat(眞寒假熱) of that made a profound study by Yu-Chang(喩昌). The next, write a paper on results and conclusions. Results and Conclusions: First, Yi-Da-Gan(易大艮) insist that must control the Qi under the blood disease conditions, taking the case of metrostaxis(崩漏). Secondly, we must study more on estimating the changing condition of Qi and the blood as time goes by, also study on the pulse and pulse condition in the four seasons(四時脈). Thirdly, Yu-Chang(喩昌) insist that be more careful in differentiation of symptoms and signs, taking the case of cold syndrome with pesudo-heat(眞寒假熱). Fourthly, Yu-Chang(喩昌) give an example that in condition of cold syndrome with pesudo-heat(眞寒假熱), sometimes, the pulse and pulse condition can be strong.

양성 발작성 자세변환성 현훈에 대한 반하백출천마탕(半夏白朮天麻湯)의 효과: 체계적 문헌고찰과 메타분석 (Banhabaeckchulchunma-tang for Benign Paroxysmal Positional Vertigo: A Systematic Review and Meta-analysis)

  • Objectives : The purpose of this study is to evaluate the effect of Banhabaeckchulchunm a-tang(BBCT) for Benign Paroxysmal Positional Vertigo(BPPV) Methods : We searched randomized controlled trials(RCTs) which assess the effect of BB CT for BPPV through 8 electronic databases from their inception to July 2021. RevMan 5.4 was used to evaluate the risk of bias. Results : 12 RCTs with 901 subjects were included. The BBCT treatment group had significantly higher total effetive rate(TER) than the western medicine treatment group(P=0.0001), and the Dizziness Handicap Inventory(DHI) score(P=0.003), traditional chinese medicine syndrome(TCM syn.) score(P<0.00001), and Visual analog scale(VAS)(P=0.0006) were significantly lower than the western medicine treatment group. The combined treatment of BBCT and canalith repositioning procedure(CRP) group had significantly higher TER than only CRP treatment group(P=0.02), and there was no significant difference in DHI score(P=0.12). TG(P=0.006) and TC(P=0.04) were significantly lower, and ApoA1 was significantly higher(P=0.0001). There was no significant difference in LDL(P=0.24). Conclusions : These results demonstrate that BBCT could be effective for BPPV especially after CRP. However, because of limits of included studies such as high heterogeneity between the literatures, unclear risk of bias, insufficient reports of adverse events(AEs), a well-designed RCTs with a low risk of bias in more diverse countries are needed in the future.
