The Role of Negative-Pressure Wound Therapy in Latissimus Dorsi Flap Donor Site Seroma Prevention: A Cohort Study

  • Background Donor site seroma is the most common complication after latissimus dorsi (LD) flap harvest. This study aimed to evaluate the efficacy of negative-pressure wound therapy (NPWT) in preventing donor site seroma formation after the harvest of an LD flap for breast reconstruction. Methods In this prospective matched-pair study, 40 patients in whom an LD flap was harvested for breast reconstruction were enrolled. NPWT was used in 20 patients, and in a control group composed of another 20 patients, the conventional donor site dressing technique was used. Information was collected regarding postoperative complications, the incidence of seroma, total drainage volume, the number of percutaneous seroma aspirations, and the volume aspirated. Results In the NPWT group, the incidence of seroma formation after drain removal was significantly lower than in the control group (15% vs. 70%; odds ratio=0.07; relative risk, 0.24). Both the mean percutaneous aspirated volume (P =0.004) and the number of percutaneous aspirations (P=0.001) were also significantly lower in the NPWT group. There were no significant differences in the total drainage volume or the duration of wound drainage between the NPWT dressing group and the control group (P>0.05). Conclusions This study showed that NPWT is a promising tool for reducing the incidence of seroma formation after removing the drain at the donor site after LD flap harvesting. It is a simple and safe technique.

Maxillary sinus volumetric changes in jet aircraft pilots: A multislice computed tomography pilot study

  • Purpose: This study evaluated maxillary sinus volume changes in military jet aircraft pilot candidates before and after the training program, in comparison with a control group, considering the effects of pressurization, altitude, and total flight hours, through multislice computed tomography. Materials and Methods: Fifteen fighter pilots were evaluated before initiating the training program and after the final approval. The control group consisted of 41 young adults who had not flown during their military career. The volumes of each maxillary sinus were measured individually before and at the end of the training program. Results: When comparing the initial and final volumes in the pilots, a statistically significant increase was observed both in the left and right maxillary sinuses. When evaluating the average total volume of the maxillary sinuses(i.e., the average volume of the right and left maxillary sinuses together), a significant increase in the volume of the maxillary sinuses was observed in the pilot group when compared to the control group. Conclusion: The maxillary sinus volumes in aircraft pilot candidates increased after the 8-month training program. This may be explained by changes in the gravitational force, the expansion of gas, and positive pressure from oxygen masks. This unprecedented investigation among pilots might lead to other investigations considering paranasal sinus alterations in this singular population.

Affecting Water Supply Capacity Followed by Allocating Flood Control Volume in Heightening Reservoir (홍수조절용량 설정에 따른 증고저수지의 용수공급능력 변화)

  • This study was performed to analyze the affect of water supply capacity followed by allocating flood control volume in heightening reservoir, of which Baekgog reservoir was selected as a case study in here. Baekgog reservoir is located in Jincheon county, Chungbuk province, of which full water level will be heightened from EL. 100. 1m to EL. 102.1m, and total storage from 21.75M $m^3$ to 26.67M $m^3$. Flood inflow with 200year frequency was estimated to 997 $m^3$/s in peak flow and 22.54M $m^3$ in total volume. Reservoir flood routing was conducted to determine flood limited water levels, which was determined to have scenarios such as EL 97-98-99m in periods of 6.21.-7.20., 7.21.-8.20., and 8.21.-9.20., respectively, EL 97-97-97m, EL 98-98-98m in present reservoir, and EL 99-100-101m, EL 99-99-99m, and EL 100-100-100m in heightened reservoir. Reservoir inflow was simulated by DAWAST model. Annual paddy irrigation requirement was estimated to 33.19M $m^3$ to 2,975ha. Instream flow was allocated to 0.14mm/d from October to April. Operation rule curve was drawn using inflow, irrigation and instream flow requirements data. In case of withdrawal limit reservoir operation using operation rule curve, reduction rates of annual irrigation supply before and after flood control by reservoir were 2.0~4.3% in present size and 1.5~3.6% in heightened size. Reliability on water supply was decreased from 77.3% to 63.6~68.2% in present size and from 81.6% to 72.7~79.5% in heightened size. And reduction rates of water storage at the end of year before and after flood control by reservoir were 7.3~16.5% in present size and 7.7~16.9% in heightened size. But water supplies were done without any water deficiency through withdrawal limit reservoir operation in spite of low flood regulating water level.

Effect of Exogenous Hormones on Spermiation in the Starry Flounder Platichthys stellatus (외인성 호르몬 처리가 강도다리 (Platichthys stellatus)의 배정에 미치는 영향)

  • Two experiments were designed to examine short-term effects of human chorionic gonadotropin (hCG), and long-term effects of gonadotropin-releasing hormone agonist (GnRHa), $17{\alpha}-hydroxyprogesterone$ (17P), and $17{\alpha},20{\beta}-dihydroxy-4-pregnen-3-one\;(17,20{\beta}P)$, alone or in combination, on milt production of the starry flounder Platichthys stellatus. In the first experiment, fish were injected with either 200 IU hCG/kg body weight or the same volume of marine fish Ringer's solution (MFRS). In the second experiment, each fish was implanted with a blank cholesterol pellet (control), $200\;{\mu}g$ GnRHa, $500\;{\mu}g$ 17P, or $100\;{\mu}g\;17,20{\beta}P/kg$ body weight alone or in combination. In the first experiment, hCG injection resulted in an increase in the expressible milt volume and a decrease in the spermatocrit (Sct). After pellet implantation in the second experiment, the milt volume was increased in males treated with GnRHa, GnRHa+17P, or $GnRHa+17,20{\beta}P$. On day 7 after hormone pellet implantation, the milt volume began to increase, and on day 14, the milt volume in the $GnRHa+500\;{\mu}g$ 17P group was significantly higher than that in the control group. Compared with the control group, the hormone pellet-treated groups had a significant reduction in the mean Sct and sperm concentration (Sc) at day 7 after pellet implantation, while there were no differences in total sperm number. The results suggest that increases in milt volume are generally associated with decreases in Sct and SC, suggesting that the main mechanism for the increase in milt volume was milt hydration.

Regional Grey and White Matter Changes in the Brain Reward System Among Patients with Alcohol Dependency

  • The purpose of the study was to find grey matter (GM) and white matter (WM) volume reduction in the brain reward system among patients with alcohol dependency. This study investigated regional GM and WM in chronic alcoholic patients, focusing primarily on the reward system, including principal components of the mesocorticolimbic reward circuit as well as cortical areas with modulating and oversight functions. Sixteen abstinent long-term chronic alcoholic men and demographically matched 16 healthy control men participated in the study. Morphometric analysis was performed on magnetic resonance brain scans using voxel-based morphometry (VBM)-diffeomorphic Anatomical Registration through Exponentiated Liealgebra (DARTEL). We derived GM and WM volumes from total brain and cortical and subcortical reward-related structures. Morphometric analyses that revealed the total volume of GM and WM was reduced and cerebrospinal fluid (CSF) was increased in the alcohol group compared to control group. The pronounced volume reduction in the reward system was observed in the GM and WM of the nucleus accumbens (NAc), GM of the amygdala, GM and WM of the hippocampus, WM of the thalamus, GM and WM of the insula, GM of the dorsolateral prefrontal cortex (DLPFC), GM of the orbitofrontal cortex (OFC), GM of the cingulate cortex (CC), GM and WM of the parahippocampal gyrus in the alcohol group. We identified volume reductions in WM as well as GM of reward system in the patients with alcohol dependency. These structural deficits in the reward system elucidate underlying impairment in the emotional and cognitive processing in alcoholism.

Organic Water Additive on Growth Performances, Hematological Parameters and Cost Effectiveness in Broiler Production

  • The experiment was conducted with 144 broiler chicks from day-old to 5 weeks of age to investigate the efficacy of a water additive in broiler production. The chicks were randomly distributed into four different treatments namely T1 (control), T2 (water additive as per recommendation level), T3 (25% less than recommendation) and T4 (25% more than recommendation). Body weight of control group was higher in 2nd week of age, but at the end of the experiment additive groups showed higher values compare to control (p<0.05). Body weight gain was increased and feed conversion ratio was improved in the additives groups during the finishing and total period, although feed intake was different among the additive groups (p<0.05). When the hematological parameters were evaluated, packed cell volume and total erythrocytes counts were increased in the additive group that received 25% more than recommendation, and hemoglobin in 25% less than recommendation group. Mean cell volume and mean cell hemoglobin of the additive groups showed lower (p<0.05) values compare to the control, but other parameters were not affected. Sales price and profit were significantly higher in the additive groups compare to the control, although total production cost was increased in the additive groups (p<0.05). All levels of water additive increased profit in comparison with the control but 25% less than recommendation level appeared to be most profitable and cost effective. It also suggests that any additive considered for poultry, must undergo trial for determining efficacy as well as its cost effectiveness for application.

The Effect of Stability Exercise For Pain Level of Cervical and Lumbar Region, Muscle Volume and Fat Mass Composition in Body of High School in Man volleyball Player (고등학교 남자배구선수들의 안정화운동에 대한 경.요부의 통증수준과 근육량, 체지방에 미치는 영향)

  • Purpose: to program introduction the effects of stability sling exercise and common exercise of high school in man volleyball player. Methods: This study divided the subjects into a sling-exercise group and a common exercise group to compare after practicing exercises twice per week for 8 weeks (total 16 times). To compare the means of the experimental group and the control group on each variable, the ANOVA for repeated measure was used and if there was any significant difference across the measurement times, post-hoc comparisons were conducted for the difference. Results: The T-tests for group difference according to exercise type on each of the variables showed the results as follows. The pain in cervical region is experimental and control groups pain correlation were not significant(p>0.05). But lumbar region experimental and control groups were significant(p<0.05). Muscle volume was increased during 8weeks in experimental and control groups. But experimental and control groups muscle volume correlation were not significant(p>0.05). Conclusion: Fat Mass composition was decreased during 8 weeks in experimental and control groups. But experimental and control groups mass composition correlation were not significant(p>0.05).

  • An experimental study has been conducted to investigate the suction performance of a designed inlet. Total pressure distortion in the inlet S-duct was measured and the effect of lip thickness and throat area was analyzed. The volume flow rate of air into the turbojet engine was controlled to observe the effect of engine RPM to the total pressure distortion. The pressure data from 36 air tubes were obtained using scanivalve, pressure transducer, and visicoder system.

Effect of Cingulate Cortical Ablation upon Gastric Secretion in Rats (흰쥐에서 대상회전 제거가 위액분비에 미치는 영향)

  • This study was undertaken to investigate the effect of cingulate cortical ablation upon gastric secretion and its components in rats. 23 male rats were divided into the cingulate(N=9) and the operated control(N= 14) groups. Cingulate cortex was ablated through a slit-shaped opening(1 mm in width, 5 mm in length) which was made symmetrically on both sides of, and parallel to, the sagittal suture by removing a bone flap from frontal bone on each side. In the operated control group, the surgical procedure was ended by the removal of the bone flap. Under light ether anesthesia, experimental animals were placed in a restraining jacket of fine mesh wire and gastric juice was collected for 5 hours via a canula which had been inserted through the anterior abdominal wall into the antral portion of the lumen of the stomach. Volume of the gastric juice was measured, and total acid output and free acid output were titrated with 0.04 N NaOH solution by using phenolphthalein and Topfer's reagent at indicator, and chloride ion output was estimated by means of chloridometer. Results obtained were that volume, total acid output, free acid output and chloride ion output of the gastric juice were higher significantly in the cingulate group than in the operated control group. It is inferred from the above results that the cingulate cortex exerts a fascilitatory influence upon gastric secretion and acid output in rats.

Quality Characteristics of White Bread Mixed with Blanched Seomcho (Spinacia oleracea L.) Powder (Blanched 섬초(시금치) 분말을 첨가한 식빵의 품질 특성)

  • This research is about the quality characteristics of white bread mixed with varying ratios of Seomcho powder, such as 1%, 3%, 5%, and 7% (Spinacia oleracea L.). The pH level of dough decreased as more Seomcho powder (Spinacia oleracea L.) was added, and 7% addition recorded the lowest pH. The fermentation power of dough expansion decreased as the additional ratio increased in both the first and second proofing, and it was negatively correlated to the fermentation time factor. The moisture content of bread decreased as the addition ratio increased. The bread volume became smaller as the addition ratio increased, but it correlated negatively with its specific volume. The color of the crumb, the lightness and redness decreased in comparison to the control, but the yellowness increased. The total amino acid contents increased as the addition ratio increased, and glutamic acid was positively correlated to the additional ratio. The total free amino acids increased as the additional ratio increased, but cysteine levels were high at 5% and 7% additional ratio. The textural characteristics of the bread, springiness and cohesiveness were lower than the control, but gumminess, brittleness, and hardness were higher than the control. The adhesiveness was realized at 7% additional ratio.
