Primary Malignant Melanoma in the Pineal Region

  • A 59-year-old male patient had 5-month history of gait disturbance and memory impairment. His initial brain computed tomography scan showed $3.5{\times}2.8cm$ sized mass with high density in the pineal region. The tumor was hypointense on T2 weighted magnetic resonance images and hyperintense on T1 weighted magnetic resonance images with heterogenous enhancement of central portion. The tumor was totally removed via the occipital transtentorial approach. Black mass was observed in the operation field, and after surgery, histopathological examination confirmed the diagnosis of malignant melanoma. Whole spine magnetic resonance images and whole body 18-fluoro-deoxyglucose positron emission tomography could not demonstrate the primary site of this melanoma. Scrupulous physical examination of his skin and mucosa was done and dark pigmented lesion on his left leg was found, but additional studies including magnetic resonance images and skin biopsy showed negative finding. As a result, final diagnosis of primary pineal malignant melanoma was made. He underwent treatment with the whole brain radiotherapy and extended local boost irradiation without chemotherapy. His preoperative symptoms were disappeared, and no other specific neurological deficits were founded. His follow-up image studies showed no recurrence or distant metastasis until 26 weeks after surgery. Primary pineal malignant melanomas are extremely rare intracranial tumors, and only 17 cases have been reported since 1899. The most recent case report showed favorable outcome by subtotal tumor resection followed by whole brain and extended local irradiation without chemotherapy. Our case is another result to prove that total tumor resection with radiotherapy can be the current optimal treatment for primary malignant melanoma in the pineal region.

The metabolic syndrome and body composition in childhood cancer survivors

  • Purpose: Long-term survivors of childhood cancer appear to have an increased risk for the metabolic syndrome, subsequent type 2 diabetes and cardiovascular disease in adulthood compared to healthy children. The purpose of this study was to investigate the frequency of the metabolic syndrome and associated factors in childhood cancer survivors at a single center in Korea. Methods: We performed a retrospective review of medical records of 98 childhood cancer survivors who were diagnosed and completed anticancer treatment at Samsung Medical Center, Seoul, Korea between Jan. 1996 and Dec. 2007. Parameters of metabolic syndrome were evaluated between Jan. 2008 and Dec. 2009. Clinical and biochemical findings including body fat percentage were analyzed. Results: A total of 19 (19.4%) patients had the metabolic syndrome. The median body fat percentage was 31.5%. The body mass index and waist circumference were positively correlated with the cranial irradiation dose (r=0.38, P<0.001 and r=0.44, P<0.00, respectively). Sixty-one (62.2%) patients had at least one abnormal lipid value. The triglyceride showed significant positive correlation with the body fat percentage (r=0.26, P=0.03). The high density lipoprotein cholesterol showed significant negative correlation with the percent body fat (r=- 0.26, P=0.03). Conclusion: Childhood cancer survivors should have thorough metabolic evaluation including measurement of body fat percentage even if they are not obese. A better understanding of the determinants of the metabolic syndrome during adolescence might provide preventive interventions for improving health outcomes in adulthood.

Dosimetry for Total Skin Electron Beam Therapy in Skin Cancer (피부암치료를 위한 전자선 전신피부 치료방법과 선량분포 측정)

  • Increasing frequency of skin cancer, mycosis fungoides, Kaposi's sarcoma etc, it need to treatment dose planning for total skin electron beam (TSEB) therapy. Appropriate treatment planning for TSEB therapy is needed to give homogeneous dose distribution throughout the entire skin surface. The energy of 6 MeV electron from the 18 MeV medical linear accelerator was adapted for superficial total skin electron beam therapy. The energy of the electron beam was reduced to 4.2 MeV by a $0.5\;cm\times90\;cm{\times}180\;cm$ acryl screen placed in a feet front of the patient. Six dual field beam was adapted for total skin irradiation to encompass the entire body surface from head to toe simultaneously. The patients were treated behind the acryl screen plate acted as a beam scatterer and contained a parallel-plate shallow ion chamber for dosimetry and beam monitoring. During treatment, the patient was placed in six different positions due to be homogeneous dose distribution for whole skin around the body. One treatment session delivered 400 cGy to the entire skin surface and patients were treated twice a week for eight consecutive weeks, which is equivalent to TDF value 57. instrumentation and techniques developed in determining the depth dose, dose distribution and bremsstrahlung dose are discussed.

Fertility preservation for patients with hematologic malignancies: The Korean Society for Fertility Preservation clinical guidelines

  • Although the survival rate of hematologic malignancies in young patients is very high, cytotoxic therapies such as chemotherapy and total body irradiation therapy can significantly reduce a patient's reproductive capacity and cause irreversible infertility. Early ovarian failure also commonly occurs following additional cancer treatment, bone marrow transplantation, or autologous transplantation. Because the risk of early ovarian failure depends on the patient's circumstances, patients with a hematologic malignancy must consult health professionals regarding fertility preservation before undergoing treatments that can potentially damage their ovaries. While it is widely known that early menopause commonly occurs following breast cancer treatment, there is a lack of reliable study results regarding fertility preservation during hematologic malignancy treatment. Therefore, an in-depth discussion between patients and health professionals about the pros and cons of the various options for fertility preservation is necessary. In this study, we review germ cell toxicity, which occurs during the treatment of hematologic malignancies, and propose guidelines for fertility preservation in younger patients with hematologic malignancies.

  • Intraocular melanoma is the most common primary occular malignancy in adult above the age of 20. Before treatment of intraocular melanoma, it is essential to do complete work-ups including LFT (LDH, SGPT), chest X-ray, whole body bone scan. Also, liver-spleen and brain scan will be done if clinically indicated. Though, malignant melanoma is radioresistant, however, show rapid tumor shrinkage after irradiation with total tumor dose of 7,000 to 8,000 rads in 7-8 weeks.

Late Biological Effect of Gamma Radiation and the Effect of Bu-Zhong-Yi-Qi-Tang on Radiation-Induced Late Biological Effect in Mice (감마선 조사 마우스의 만성장해와 보중익기탕 투여 효과)

  • We performed this study to determine the late biological effect of gamma radiation and effect of Bu-Zhong-Yi-Qi-Tang (BZYQT), a prescription of traditional Oriental medicine, on radiation-induced late biological effect (survival, hematological change, carcinogenesis) of mice irradiated with 3 Gy of gamma-radiation. There were little difference in body weights between normal and irradiated mice. Survival rate were decreased in irradiated mice and the survival rate and mean survival time of the groups treated with BZYQT were far better than the irradiation control group. A significant elevation of total leukocyte or lymphocyte counts was seen at week 4 and 12 of the group treated with BZYQT. Stimulated recovery by the extract from BZYQT was also observed in thrombocyte. Main gross findings of irradiated mice were appeared as enlargement of spleen, thymus and liver, tumorous nodules of lung and cyst or mass of ovary. Microscopically, there were various findings including hematopoietic and lymphoid tumor, lung cancer, ovarian cancer and cancer of other lesions. BZYQT reduced the incidence of tumor development. Further studies are needed to characterize better the protective nature of ingredients and active compounds.

Superior Vena Cava Syndrome -2 Bypass Graft Cases- (상공정맥 증후군 -Dacron & Nylon 환치수술 2예-)

  • Superior Vena Cava Syndrome: Dacron and Nylon graft between the left innominate vein and the right atrial appendage. Two cases with typical superior vena cave syndrome treated by by-pass graft between the left innominate vein and the right atrial apepndage were presented. One of them was a 58 year old farmer who suffered from marked swelling of the neck and upper half of body, the other was a 50 years old government employee who had acutely progressive symptoms of superior vena cave obstruction. Both of cases revealed that [1] cubitel venous pressure was markedly increased. [2] tumors were noted in the posterior mediastinum by laminography. [3] preoperative cavogram showed the occlusion of superior vena cava and marked collaterals. Dacron and Nylon graft were inserted between the left innominate vein and the right atrial appendage. Postoperatively, the symptoms were relieved markedly, showing edema free face and decreased cubital venous pressure. Postoperative cavogram showed patent graft. Histologically the first case was diagnosed as squamous cell carcinoma and the second as undifferentiated carcinoma, originated probably from bronchus. Total doses of 3150 r X-ray irradiation and 5000 mg of 5-FU were administered in each cases. The first case expired 11 months postoperatively without recurrence of superior vena cave obstruction symptom and the second case is living now without obstruction signs, 4 months after by-pass operation.

  • The effect of immune activation by Echinacea purpurea was investigated by measuring total immunoglobulin (Ig) G, IgM. and the radioprotective effect of immune activation by Echinacea purpurea was investigated by measuring T lymphocyte subsets in the peripheral blood of mice following whole body irradiation. Echinacea purpurea activated macrophages to stimulate $IFN-{\gamma}$ production in association with the secondary activation of T lymphocytes, resulting in a decrease in IgG and IgM production. Cytokines released from macrophages in mouse peripheral blood after Echinacea purpurea administration activated helper T cells to proliferate. In addition, activated macrophages in association with the secondary T lymphocyte activation increased $IFN-{\gamma}$ production and stimulated proliferation of cytotoxic T cells and suppressor T cells, indicating the activation of cell-mediated immune responses.

Vitamin E Modulates Radiation-induced Oxidative Damage in Mice Fed a High-Lipid Diet

  • The Vitamin E (VE) effect was examined on oxidative damage to DNA, lipids, and protein in mice that were fed various levels of lipid diets after total body irradiation (TBI) with X-rays at 2 Gy. No increase of 8-hydroxydeoxyguanosine (8OHdG) by TBI was observed in the +VE group; however, in the case of the -VE group, a significantly higher 8OHdG level was observed in the high-lipid group than in the low- or basal-lipid group. In the groups with TBI, the concentration of thiobarbituric reactive substances (TBARS) only significantly increased in the high-lipid (-VE) group. These changes in TBARS, due to TBI, were not detected in other groups. The contents of protein carbonyls only increased in the (-VE) group. The contents of protein carbonyls was significantly different between the (+VE) and the (-VE) groups, regardless of the lipid levels. The concentrations of GSH, vitamins C and E in the liver were lower, and the concentration of non-heme iron in the liver was higher in the high-lipid group than in the low- and basal-lipid groups. These concentrations in the high-lipid group were significantly different between the (+VE) and the (-VE) groups. These results strongly suggest that mice that are fed a high-lipid diet are susceptible to TBI-induced oxidative damage. Also, decreases in the GSH levels and an increase in the iron level are involved in the mechanism of this susceptibility.

Effect of Adrenaline Against TotalBody Gamma-irradiation on Serum Protein (${\gamma}$-선 전신조사시 혈청단백질에 미치는 Adrenaline의 영향)

  • 가물치 (Ophicephalus argus CANTOR) ♂♀를 재료로 하여 두 실험군으로 나누어서 대조군에 부치사량인 25-${\gamma}$의 137Cs-${\gamma}$ 선을 조사하고 실험군에는 250${\gamma}$의 137Cs-${\gamma}$선을 조사 즉후 adrenaline (0.5cc/80g$\pm$18%)을 매일 투여하여 약 23$^{\circ}C$에서 사육하면서 혈청단백질의 전기영동상과 총혈청단백질의 변화를 1, 4, 7, 10 , 13 및 1일구간으로 분리 즉 시일적 경과로 조사하였다. 한편 가물치에서의 ${\gamma}$선 치사량은 310${\gamma}$이다. 1) 양군에서 일반적으로 1, 4, 7, 10 및 13 일구에 현저한 albumin 분설의 감소가 나타났다. 즉 대조군인 adrenaline 비투여 대조군에서 a1 및 a2 globulin 분설의 증가와 실험군이 adrenaline 투여 대조군에서 a1 및 $\beta$-globulin 분설의 증가에 다른 albumin 분설의 감소가 나타났다. 따라서 A/G 비고 역시 시일적 경과에 따라 감소된 치를 나타내었다. 그러나 특히 adrenabine 투여 대조군의 1 일간에서 A/G비가 현저하게 감소의 지체성을 나타내었다. 2) 총혈청단백질량은 양군에서 일반적으로 1, 4 및 7일구에서 현저하게 감소되나 13 미치 16 이구에서 가물치의 정상치로 근접된 치를 나타내었다. 그러나 특히 adrenaline 투여 조사군에서 4 및 7일구에서 현저하게 감소의 지체성을 나타내었다. 3) 상기의 결과로 미루어보다 명백하게 혈청단백질에 미치는 adrenaline 처리의 방어효과가 ${\gamma}$선 조사후 특히 초기상태에서 현저하게 초래된다고 생각도니다.
