Triple antibiotics와 관련한 치아 변색의 예방 (Prevention of tooth discoloration associated with triple antibiotics)

  • Regenerative endodontics has a potential to heal a partially necrotic pulp, which can be beneficial for the continued root development and strengthening of immature teeth. For this purpose, triple antibiotic mixture of ciprofloxacin, metronidazole, and minocycline was recommended as intracanal medicament in an attempt to disinfect the root canal system for revascularization of a tooth with a necrotic pulp. However, discoloration of the tooth was reported after applying this. This case shows the idea for preventing the tooth discoloration using a delivery syringe to avoid the contact between the clinical crown and the antibiotics.

하악 영구 견치의 의원성 매복에 대한 증례 보고 (IATROGENIC IMPACTION OF LOWER LEFT PERMANENT CANINE : CASE REPORT)

  • 상악 영구 견치의 매복에 비해 하악 영구 견치의 매복은 매우 드물게 일어난다. 치아가 매복된 경우 장애물 존재 여부와 매복 위치와 방향, 맹출 가능한 공간의 유무, 치근의 형성 단계 등의 요소에 따라 매복 치아의 발거 혹은 장애물의 제거 후 주기적 관찰, 맹출이 지연된 치아의 외과적 노출, 교정적 견인 등의 방법으로 치료하게 된다. 본 증례는 하악골의 골절 부위에 시행된 골간 강선 결찰과 미니 플레이트에 의해 하악 좌측 견치가 매복되어 있었던 경우로 CT 검사 결과 하악 정중부위 플레이트의 스크류가 맹출을 방해하고 있는 것으로 판단되어 전신 마취 하에 플레이트를 제거하였다. 이후 자연적인 맹출을 기대하였으나, 맹출력이 부족하여 교정적 견인으로 양호한 결과를 얻었기에 보고하는 바이다.

디지털 방식의 인접면 접촉강도 측정장치의 개발 및 평가 (EVALUATION AND DEVELOPMENT OF DIGITAL DEVICE FOR MEASURING PROXIMAL TOOTH CONTACT TIGHTNESS)

  • Statement of problem: The proper contact relation between adjacent teeth in each arch plays an important role in the stability and maintenance of the integrity of the dental arches. Proximal contact has been defined as the area of a tooth that is in close association, connection, or touch with an adjacent tooth in the same arch. Purpose: The aim of this study was to develop a digital device for measuring the proximal tooth contact tightness by pulling a thin stainless steel strip (2mm wide, 0.03mm thick) inserted between proximal tooth contact. Material and method: This device consists of measuring part, sensor part, motor part and body part. The stainless steel strip was connected to a stain gauge. The strain gauge was designed to convert the frictional force into a compressive force. This compressive force was detected as a electrical signal and the electrical signal was digitalized by a A/D converter. The digital signals were displayed by a micro-processor. The pulling speed was 8mm/s. Results: For testing reliability of the device in vivo, two healthy young adults (A, B) participated in this experiment. The tightness of proximal tooth contact between the second premolar and the first molar of mandible (subject A) and maxilla (subject B) was measured fifteen times for three days at rest. We double-checked the accuracy of the device with a Universal Testing Machine. Output signals from the Universal Testing Machine and the measuring device were compared. Regression analysis showed high linearity between these two signals. In vivo test, no significant differences were found between measurements. Conclusion: This device has shown to he capable of producing reliable and reproducible results in measuring proximal tooth contact. Therefore, it was considered that this device was appropriate to apply clinically.

Prevalence and patterns of tooth agenesis among patients aged 12-22 years: A retrospective study

  • Objective: This study aimed to establish the prevalence and patterns of nonsyndromic tooth agenesis in patients referred to a tertiary health care facility. Methods: The intraoral records and panoramic radiographs of 9,874 patients aged 12-22 years were evaluated. The study group included 716 patients (371 male, 345 female) with non-syndromic agenesis of at least one tooth (except the third molars). The study data were assessed using descriptive statistics, chi-square test, and Mann-Whitney U test, while patterns were evaluated using a tooth agenesis code (TAC) tool. Results: A total of 1,627 congenitally missing teeth, were found in patients with non-syndromic tooth agenesis, with an average of 2.27 missing teeth per patient. The prevalence of tooth agenesis was 7.25%, and the most commonly missing teeth were the left mandibular second premolars (10.17%). The age group comparison revealed no significant difference in the median number of missing teeth per patient according to the cutoff values for ages between 12 and 22 years. When the missing teeth were examined separately according to quadrants, 114 different tooth agenesis patterns (upper right quadrant = 28, upper left quadrant = 27, lower left quadrant = 31, and lower right quadrant = 28) were identified, and 81 of these patterns appeared only once. Conclusions: This study highlights the benefits of applying the TAC tool in a large sample population. The application of the TAC tool in such studies will enable the development of template treatment plans by determining homogenous patterns of tooth agenesis in certain populations.

Study on histological features and Bmp4 expression pattern during tooth formation and replacement in Xenopus laevis

  • This study explores the histological features and Bmp4 expression patterns in the replaced tooth germ of Xenopus laevis. Tooth germ formation starts from the dental placode through epithelial-mesenchymal interactions, involving various signaling pathways such as Fgf, Shh, Bmp, and Wnt. In mice, Bmp4 expression in the dental placode inhibits Pax9 expression in the dental mesenchyme. Although absent in the presumptive dental lamina of birds and toothless mammals, Bmp4 remains conserved in reptiles and fish owing to gene duplication. However, its expression in amphibian tooth germs is poorly understood. Three-month-old X. laevis were employed in this study. Initially, samples underwent paraffin embedding and were sectioned into 5 or 12 ㎛ ribbons for H&E staining and in situ hybridization, respectively. Results revealed teeth appearing in two maxillary rows: the labial side, with prefunctional and functional teeth, and the lingual side, with replaced tooth germs behind functional teeth. Enameloid was observed between the inner dental epithelium and dental mesenchyme at the cap or early bell stages, whereas enamel and dentin formed during the late bell or mineralization stages from the replaced tooth germ. Bmp4 expression was evident in the inner dental epithelium (ameloblasts), dental papilla (odontoblasts), stellate reticulum, and Hertwig's epithelial root sheath. Overall, these findings highlight the conservation of Bmp4 expression in X. laevis tooth development.

구개측 매복된 상악 견치의 교정적 치험례 (ORTHODONTIC TREATMENT OF THE PALATALLY IMPACTED MAXILLARY CANINE)

  • An impacted tooth is defined pathologically as a tooth that remains under the mucosa of inside bone without eruption of the crown after a specific period of eruption. Clinically, the term includes those teeth, even before eruption period, that are not expected to erupt due to shape, position and alignment of tooth and lack of space. Canine is prone to impaction more than other teeth because it has the longest time to develop and a complex route from the place of formation to the site of eruption. The impaction incidence of maxillary canine is repoted 0.92$\sim$3.3% (Ferguson, 1990). In 1995 Orton reported that the incidence was 0.92$\sim$2.2% and palatal impaction was more frequent than labial impaction(85%:15%). In 1969 Johnston presented it was more common to woman than to man(3:1). The etiology includes systemic disease such as endocrine disorder, cleidocranial dysostosis, irradiation, Crouzon syndrome, ricketts, facial hemihypertrophy and hereditary and local problems such as ectopic position of the tooth, distance of tooth from its place of eruption, malformation of the tooth, presence of supernumerary teeth, trauma of tooth germ, infection of tooth germ, displacement of tooth germ or tooth by a neoplasm, ankylosis, overretention of deciduous predecessor, lack of space for the tooth in the dental arch and mucosal barrier due to gingival fibrosis. The maxillary canine is especially important as it has the longest root, provides guidance for lateral movement of the mandible and masticatory function and assumes an important role esthetically as it is located at mouth angle. If left untreated, it may cause migration and external, internal resorption of adjacent teeth, loss of arch length, formation of dentigerous cyst or tumors, infection and referred pain as well as malposition of the tooth. Therefore, periodic examination of the development and eruption of the maxillary canine is especially important in a growing child. This case study presents the results of treatment of palatally impacted maxillary canine utilizing surgical exposure and orthodontic tooth movement on patients visiting SNUDH dept. of pediatric dentistry.

Alteration of cellular events in tooth development by chemical chaperon, Tauroursodeoxycholic acid treatment

  • Several factors, including genetic and environmental insults, impede protein folding and secretion in the endoplasmic reticulum (ER). Accumulation of unfolded or mis-folded protein in the ER manifests as ER stress. To cope with this morbid condition of the ER, recent data has suggested that the intracellular event of an unfolded protein response plays a critical role in managing the secretory load and maintaining proteostasis in the ER. Tauroursodeoxycholic acid (TUDCA) is a chemical chaperone and hydrophilic bile acid that is known to inhibit apoptosis by attenuating ER stress. Numerous studies have revealed that TUDCA affects hepatic diseases, obesity, and inflammatory illnesses. Recently, molecular regulation of ER stress in tooth development, especially during the secretory stage, has been studied. Therefore, in this study, we examined the developmental role of ER stress regulation in tooth morphogenesis using in vitro organ cultivation methods with a chemical chaperone treatment, TUDCA. Altered cellular events including proliferation, apoptosis, and dentinogenesis were examined using immunostaining and terminal deoxynucleotidyl transferase dUTP nick end labeling assay. In addition, altered localization patterns of the formation of hard tissue matrices related to molecules, including amelogenin and nestin, were examined to assess their morphological changes. Based on our findings, modulating the role of the chemical chaperone TUDCA in tooth morphogenesis, especially through the modulation of cellular proliferation and apoptosis, could be applied as a supporting data for tooth regeneration for future studies.

Dentin Matrix Block의 치조골 복원 능력에 관한 임상적 연구 (Clinical Study on the Alveolar Bone Repair Capacity of Dentin Matrix Block)

  • In the oral and maxillofacial area, bone defects are created by various reasons and demand for bone grafts, while dental implant implantation has been increased consistently. To solve these problems, there has been development of autogenous tooth-bone graft material (AutoBT$^{(R)}$, Korea Tooth Bank Co., Korea), and we have collected ground reasons to substitute free autobone graft with this material in clinical use. This autogenous tooth-bone graft material is produced in powder type and block type. Block type is useful in esthetic reconstruction of the defect site and vertical and horizontal augmentation of alveolar bone because this type has high strength value, well maintained shape and is less absorbed. Therefore, the author of this study gained favorable result by grafting the block type autogenous tooth-bone graft material after dental implant implantation on the bone defects of the mandibular molar extraction site. Moreover, the author represents this case with literature review after confirming bone remodeling on the computed tomography image and by histological analysis.

Autotransplantation: A biological treatment alternative for a patient after traumatic dental injury

  • Traumatic dental injury is considered a public dental health problem because of a high childhood incidence, high treatment costs, and prolonged treatment time. Although management guidelines for traumatized teeth have been outlined, tooth loss following trauma is occasionally unavoidable. Here, we describe the successful interdisciplinary management of a traumatized central incisor in an 11-year old boy that was extracted because of a poor prognosis and restored by the autotransplantation of an immature donor tooth into the site. The patient underwent orthodontic treatment in order to close the donor site space and bring the autotransplanted tooth to an ideal position. Postorthodontic treatment radiographs and photographs revealed an esthetic and functional natural tooth replacing the lost tooth. The findings from this case suggest that autotransplantation offers unique advantages as a treatment modality for the restoration of missing teeth, particularly in growing children.

자탈형 콤바인 탈곡부 설계요인(設計要因)의 적정화(適正化)를 위한 연구(I) -급동축(扱胴軸) 토오크 파형의 추정(推定)- (Optimizing the design factors of the head-fed type combine(I) -Estimation of the threshing drum torque curve-)

  • The threshing action of the head-fed type threshing unit occurs mainly by the impact between threshing tooth and grains. It may be therefore the most fundamental step to calculate the time and order of the occurrance of impact by the tooth for predicting the performance of threshing unit. The threshing teeth arrangement was defined by length and diameter of threshing dram, number of spiral arrays, number of threshing teeth by kind per one spiral array, number of windings of spiral array around the threshing drum, delay angle of impact line. The linear equations for locus of left and right margin of paddy bundle, spiral array, impact line on the development figure of the threshing drum were expressed by fastors of the threshing teeth arrangement. In the computer program, the teeth which inflict impact were searched successively along the impact line. Searching range and impact condition were defined by the relation between four linear equations. If the impacting tooth was found, time and the kind of threshing tooth was derived from the coordinate of the threshing tooth. At this time the unit torque curve was accumulated on the array of computer memory. At last the completed torque curve of threshing drum shaft was described on the computer screen. Remarkably the peack valae and fluctuation of torque curve was decreased by adopting the delay angle of impact line.
