교모면적과 교합접촉양태 간의 관계에 대한 연구 (A Study on the Relation between Occlusal Wear Area and Occlusal Contact Patterns)

  • The aim of the present study was to investigate a relation between occlusal wear area and occlusal contact patterns. For the purpose, occlusal wear area were measured in 58 dental students and in 129 patients with temporomandibular disorders(TMDs) from dental casts. Teeth used in this study were from canine to second molar on both sides in upper arch, totally ten. Occlusal wear area on casts was marked by pencil and photocopies, and then, the area was measured with planimeter. Occlusal relation was clinically examined with regard to Angle's classification, chewing side preference, lateral guidance pattern and bruxing and/or clenching habit. T-Scan, electronic occlusal contact analyzer, was used to record occlusal contact number, contact force, contact time and occlusal balance that is TLR(total left-right statistics) during tooth contact. All measurement were repeated 3 times and the average value was used for data processing. The obtained results were as follows : 1. Mean value of occlusal wear area did not differ significantly between dental students and patients. 2. There ws not significant difference in wear area between chewing side and non-chewing side in both groups. 3. Occlusal wear area was significantly increased with age in both groups. 4. Three subgroups divided by Angle's classification did not show any difference in occlusal wear area among them, but three subgroups divided by lateral guidance pattern showed slightly significant difference between canine guide subgroup and group function subgroup in patients. Occlusal wear ares\a in group function subgroup wear larger than canine guide subgroup. 5. Mean value of wear area in patients with bruxing and/or clenching habit did not differ from those in patients without such habit. 6. Correlationship among items related to occlusal contact pattern were highly consistent and significant in dental students and only one item significantly correlated with occlusal wear area was tooth contact time.

근전도 측정시 검사시간에 따른 신뢰도 연구 (Study on the Reliability of Electromygraphic Examine according to the Measurement Timing in a Day)

  • The purpose of this study was to investigate the reliability of electromyographic examination in a day and detect the objective measurement timing of electromyography. 18 normal students who were in dental collage of C national university were selected for this study. They had no symptoms on temporomandibular joint area, no restorations and missing tooth on dentition. Their molar relationship was normal or class I of Angle's classification. Electromyography had been taken every one hour from 9:00 A.M. to 5:00 P.M. in the state of clenching and rest by using Bioelectric processor EM2 interfaced with mandibular kinesiograph K-6 diagnostic system. We compared and analyzed the variances of electromyography values of each subjects. The obtained results were as follows : There were no significant differences on variability of electromyography values in each state clenching and rest. In the aspect of this circumstances, the use of electromyography could be a good objective procedure to diagnose the temporomandibular disorders and to detect the effect to the treatment of temporomandibular disorders at any time in a day from 9:00 A.M. to 5:00 P.M.

인위적인 이갈이가 교근의 Silent Period에 미치는 영향에 관한 연구 (THE EFFECT OF EXPERIMENTAL BRUXISM ON THE MASSETERIC SILENT PERIOD)

  • The purpose of this study was to investigate the duration of the silent period of the masseter muscle in normal subjects after experimental bruxism. The material for this study consisted of 9 male subjects with an age range of 22-24 years who showed normal occlusion clinically and were free from any history or symptoms of temporomandibular joint dysfunction. A jaw-jerk was stimulated by tapping the mandibular symphysis during maximal voluntary clenching with a solenoid-driven hammer. And three separate silent period records were made from the left masseter muscle at the following times: at normal state, after experimental bruxism, 1 week after bruxism. The following conclusions were obtained: 1. The average value of the duration of masseteric silent period during maximal voluntary clenching was $33.62{\pm}1.86msec$ when tap force of approximate 650g was applied at normal state. 2. After experimental tooth grinding for 30 minutes, the duration of masseteric silent period increased to $46.29{\pm}4.40msec$. 3. Comparing the duration of silent period measured at normal state with that measured after 1 week, the duration of silent period showed reproducibility.

이갈이의 진단 및 치료 (The Diagnosis and Treatment of Bruxism)

  • 이갈이는 이를 갈거나 악무는 것을 포함하는 부기능적인 행위를 말하며 광범위하게는 주간 및 야간에 나타나는 것을 모두 포함한다. 이갈이의 원인으로는 기본적으로 말초성 요인과 중추성 요인으로 나누어 살펴볼 수 있는데 현재까지의 연구 결과를 종합하여 볼 때 중추성 요인이 주로 작용하는 것으로 알려져 있다. 이러한 이갈이는 교모, 치경부 미세파절, 교근 비대, 교근 및 측두근의 통증, 턱관절의 통증 및 움직임의 제한, 치아 및 수복물 파절, 치수염, 외상성 교합 등을 유발 할 수 있고 특히 이악물기의 경우 협점막 압흔이나 협점막 백선 또는, 혀의 측면에 압흔 등을 유발한다. 이갈이의 정확한 진단을 위해 구강내 장치, 근전도, 수면다원검사 등을 이용하며 미국수면장애학회의 경우 이갈이의 임상 진단 기준을 제시하고 있다. 그러나 아직까지 이갈이의 명확한 원인이 밝혀져 있지 않기 때문에 치료법의 선택은 신중하여야 한다. 이갈이가 중등도 이상이고 임상 증상이나 징후를 유발하는 경우에 이갈이를 관리하기 위한 방법으로는 위험 요인의 조절, 구강내 장치, 보툴리눔 독소 주사, 약물 치료, 바이오피드백 등이 있으며 구강내 장치 요법이 현재로서 가장 합리적인 방법이다. 이갈이 치료와 교근비대에 대한 심미적인 개선을 같이 원하는 환자에서는 보툴리눔 독소 주사 요법이 이용될 수 있다.

The Effect of Psychological Factors on Sleep Disturbances in Young Adults

  • Purpose: The purpose of this study was to assess influences of psychological factors on sleep disturbances in young adults through the revised version of the Minnesota Multiphasic Personality Inventory (MMPI-2). Methods: Two hundred and ninety nine college students in Gyeonggi-do completed the MMPI-2 and a questionnaire related to sleep disturbances and collected data were analyzed by logistic regression. Results: The odds of tooth grinding increased significantly with the increase of T-score of hysteria (Hy) scale. The increase of T-score of hypomania (Ma) and social introversion (Si) scale significantly contributed to the risk of insomnia. The occurrence of insomnia increased significantly as T-score of somatic complaints (RC1) and psychoticism (PSYC) scale increased. The odds of tooth grinding increased significantly as T-score of anxiety (ANX) and family problems (FAM) scale increased. The occurrence of insomnia decreased significantly with the increase of T-score of type A behavior (TPA) scale. The increase of T-score of dominance (Do) scale significantly contributed to the risk of tooth clenching. The odds of tooth grinding decreased significantly as T-score of MacAndrew alcoholism-revised (MAC-R) scale increased, whereas it increased significantly as T-score of addiction potential scale (APS) increased. No scale of MMPI-2 significantly affected the occurrence of snoring. Conclusions: T-scores of Hy, ANX, FAM, Do, MAC-R, and APS scales on MMPI-2 affected the risk of bruxism. T-scores of Ma, Si, RC1, PSYC, and TPA scales on MMPI-2 influenced the occurrence of insomnia. The psychometric instrument such as MMPI-2 is helpful in understanding and managing bruxism and insomnia.

저작근의 피로가 치아접촉안정성 및 저작근활성에 미치는 효과 (The Effect of Masticatory Muscle Fatigue on the Occlusal Contact Stability and Masticatory Muscle Actibities)

  • The authors performed a experimental study to evaluate the effects of masticatory muscle fatigue on tooth contact and masticatory muscle activity in 26 normal healthy women. The experimental masticatory muscle fatigue was induced by unilateral biting of 5kg force on mandibular first molar. The results were as follows : 1. The initial symptom related to muscle fatigue pain appeared in 85.19 seconds of isometric contraction and the endurance time of isometric contraction was 203.15 seconds. 2. The pain occurred more frequently in masseter region than in temporal region. In masseter pain the incidence was almost equal between both sides, whereas the temporal pain was more in contralateral side. 3. The spontaneity and the symmetry of tooth contact during maximum clenching were reduced after isometric unilateral biting. 4. After induction of experimental muscle fatigue, the EMG activities of masseter muscles of both sides and ipsilateral temporal muscle showed the tendency of decreasing activities. 5. The asymmetry indicies of masseter and temporal muscles were reduced after isometric bilateral biting.

  • In this research, the sensor and control system for self detecting masticatory muscle relaxation appliances. A strain gauge is used to measure a strength of tooth clenching force. A bridge circuit and voltage amplifier is designed to amplify measured signals and RF transmitter and receiver is also designed to communicate inner and outer mouth device. The experiments are performed to show the effectiveness of designed system.

두부자세에 따른 두경부 근활성 및 교합접촉양태의 변화 (Effects of Head Posture on Resting EMG Activity of Craniocervical Muscles and on Occlusal Contacts)

  • This study was performed to investigate influence of the changes of head posture on resting electromyographic (EMG) activity in anterior temporalis, masseter, sternocleidomastoid muscle and trapezius, and on status of occlusal contacts. For this study twenty-nine patients with temporomandibular disorders(TMD) and thirty dental students without any masticatory symptoms were selected as patients group and control group, respectively. EMG activity($\mu$V) at rest was observed in four kind of head postures such as natural or normal head posture(NHP), forward head posture(FHP), upward head posture(UHP), downward head posture(DHP), and in NHP and FHP, EMG activity with flat occlusal splint was also checked. BioEMG$^\textregistered$(Bioelectromyograph, Bioresearch Inc., USA) was used to record EMG activity in the above four muscles with eight locations on both sides. The author used T-Scan$^\textregistered$(Tekscan Co., USA) system to investigate the changes of oclusal contats on clenching in the four head postures about number, force, time(duration) and total left-right statistis(TLR, occlusal stability crossing left-right dental arch on clenching). For taking in upward or downward head posture, head was inclined $10^{\circ}$ upward or downward and CROM$^\textregistered$ (cervical-range-of motion, Performance attainment Inc., USA) was used to maintain same posture during the procedure. The results obtained were as follows : 1. For resting EMG activity, anterior temporalis did not show any difference by change of head posture, but masseter and sternocleidomastoid muscle showed higher value of EMG activity in FHP and UHP, and trapezius showed higher value of EMG activity in FHP and DHP. 2. EMG activity of trapezius was higher than that of any other muscles in NHP, FHP, and DHP, but in UHP, the activity was the lowest reversely. 3. Patients group showed higher EMG activity than control group did in all the muscles in NHP. And significant difference between the two groups were also observed in anterior temporalis in FHP, in sternocleidomastoid muscle in UHP, and in sterno-cleidomastoid muscle and trapezius in DHP with higher activity in patients group. 4. There was no change of EMG activity in NHp with splint, but EMG activity in anterior temporalis and masseter was decreased in FHP with splint. 5. In general, status of occlusal contacts was not changed with head posture in all subjects, and difference between patients group and control group was only noted for number and force of tooth contact in UHP and DHP with more value in control group. 6. Correlationship between EMG activity and number ad force of tooth contacts was shown negatively with regard to masseter in NHP, and trapezius in UHP and DHP.

실험적으로 유발되는 근피로가 근통증 및 교합양상에 미치는 영향 (Effect of Experimental Muscle Fatigue on Muscle Pain and Occlusal Pattern)

  • 치과에 내원하는 주된 이유 중의 하나는 통증이며 이런 통증 치료시에 장시간의 개구는 저작근에 근육문제를 야기할 수 있다. 장시간의 근피로를 유발할 수 있는 치과진료실내에서의 치료로 인하여 두통, 저작근의 통증, 개구 장애, 저작 곤란 등을 유발할 수 있으며, 통증을 치료하기위하여 내원한 환자로서는 예상하지 못한 다른 국면의 통증에 맞부딪치게 된다. 물론 이악물기등의 악습관 그리고 과도한 껌저작등으로 인해서도 임상적으로 근쇠약감이 호소하며 이에 대해서는 실험적 근피로유도후에 근전도등을 이용하여 연구가 이루어져왔다. 근쇠약감 및 근피로로 인하여 교합양상의 변화를 주관적으로 호소하며 이 상태에서의 근육의 근전도 평가가 이루어져 왔던 것에 비하여 교합의 변화에 대해서는 평가가 미진하였다. 과도한 이악물기나 개구 및 껌 저작으로 인한 근피로가 교합양상에 미치는 변화를 평가하고 감각신경 변화로 인하여 근통증에 영향을 미치는 조사하고자 측두하악장애 증상이 없고 정상 범주의 교합을 가졌으며 교정치료나 3개 이상의 보철 치료를 받지 않은 지원자 총 24명(평균나이 25.7세)을 대상으로 하였다. 근 피로를 실험적으로 유발하기 위하여 이악물기, 장시간 개구 상태 유지, 주저작측으로 껌 저작을 시행하여 근피로를 느끼도록 하였다. 본 연구에서는 전기적 자극을 이용하는 뉴로미터를 이용하여 근통증시에 주저작측 삼차신경의 감각신경을 측정하였고, 근통증을 느끼는 시점에 근쇠약감이나 저작근 기능 장애로 인한 교합상태의 변화를 평가하기 위하여 T-scan II system을 이용하여 교합접촉, 교합력 등을 조사하여 다음의 결과를 얻었다. 1. 근피로는 장시간 개구 후에 가장먼저 느꼈으며 근통증을 느끼는 시간은 이악물기 후에 가장 빨리 나타났다. 근피로, 통증을 느끼는 시점에서의 근피로, 통증의 정도는 이악물기 후에 가장 높았다 2. 최초접촉시의 접촉시간 접촉력은 차이가 없었으며 교두간 접촉위에서 껌저작후 접촉시간, 장시간 개구 후와 껌저작후에 접촉력이 감소하였다. 최대접촉위에서는 장시간 개구후와 껌저작후에 접촉력이 감소하였다. 최종접촉위에서는 모든 실험적 근피로후에 접촉시간이 감소하였고 접촉력은 껌저작후에 감소하였다. 3. 실험적 근피로가 유발된 근육에서의 감각 변화는 보이지 않았다. 4. 접촉수는 실험적 근피로 유발후에 최초접촉위 최대 접촉위 그리고 최종접촉위에서 감소되는 양상을 보였다. 5. 접촉수의 주저작측에 따른 비교에서 좌측으로 껌을 저작한 군은 접촉수에 좌우측 차이를 보이지 않았으며, 우측으로 껌을 저작한 군에서는 좌측이 많았다. 결론적으로 구강내외에 발생하는 일련의 사건은 임상적으로 근쇠약감을 호소하는 경우 교합양상에 변화를 초래 할 수 있음을 확인 할 수 있었으며, 장시간의 개구나 이악물기, 과도한 저작등의 악구강계에 영향을 미칠 수 사건은 특히 치과치료 시에 교합 변화에 주의를 요함을 확인할 수 있었다.

과도한 치아 마모 환자의 3D 프린팅 교합안정장치를 이용한 수직 교합 고경 증가를 동반한 고정성 보철물 전악 수복 증례 (Full mouth rehabilitation with fixed prostheses by increased vertical occlusal dimension using 3D printed splint in a patient with excessive tooth wear)

  • 전치부의 심한 교모는 측방운동 중 구치부를 교모로부터 보호하는 전방유도의 상실을 촉진하며, 구치부의 상실은 교합의 부조화 및 수직고경의 감소를 초래한다. 또한 다수의 치아상실과 치아 마모로 인해 교합이 붕괴된 환자 치료시에는 정확한 임상 및 방사선 검사, 진단용 왁스업을 이용하여 수직고경 상실에 대한 여부를 파악하는 것이 중요하다. 본 증례의 환자는 44세 여성환자로 이갈이와 이악물기 습관으로 전체적인 치아 마모 및 구치부 상실로 저작기능 및 발음, 심미적 불편감을 호소하였으며, 보철적 치료를 통해 저작기능 회복 및 심미적 안모 개선을 주소로 내원하였다. 과도한 치아 마모와 상, 하악 구치부의 결손으로 감소한 수직교합고경 및 전치부의 심한 수직피개를 보였다. 모형 분석과 진단납형을 통해 수직고경 증가를 결정하였으며, 고정성 보철물로 전악수복을 계획하였다. 안정된 치료위로의 적응을 위해 CAD (Computer aided design)소프트웨어로 디자인한 3D(3-Dimensional) 프린팅 교합안정장치를 이용하여 단계적으로 수직고경에 대한 적응도를 확인하였으며, 이 후 임시치아로 수복하여 4개월의 적응기간 후 전체 치열은 금속도재관 및 임플란트로 수복하여 심미적 및 기능적으로 만족스러운 결과를 얻었기에 이를 보고하고자 한다.
