Definition and Difference between Dose Equivalent and Equivalent Dose in Radiation Dose Measurement and Evaluation (방사선량의 측정, 평가에서 선량당량(dose equivalent)과 등가선량(equivalent dose)의 정의 및 차이)

  • In its recent recommendation No. 60(1990), ICRP has newly introduced several terminology which had not existed in its prior recommendation No. 26(1977). Of these, a newly defined quantity 'Equivalent Dose' replacing the 'Dose Equivalent' of the ICRU concept has been recommended to be adopted in the radiation protection programme. However, since the committee still uses the 'Dose Equivalent' and 'Equivalent Dose' in its several publications, it is likely to provoke unnecessary confusions and misuses in applying these two quantities. In this paper were described the definition and difference between these two quantities to help in understanding of these two quantitites among the person involved in the radiation protection activities.

Adipose tissue-derived mesenchymal stem cells reduce endometriosis cellular proliferation through their anti-inflammatory effects

  • Objective: Endometriosis is a chronic debilitating inflammatory condition characterized by the presence of endometrial tissues outside the uterine cavity. Pelvic soreness and infertility are the usual association. Due to the poor effectiveness of the hormone therapy and the high incidence of recurrence following surgical excision, there is no single effective option for management of endometriosis. Mesenchymal stem cells (MSCs) are multipotent stromal cells studied for their broad immunoregulatory and anti-inflammatory properties; however, their efficiency in endometriosis cases is still a controversial issue. Our study aim was to evaluate whether adipose tissue-derived MSCs (AD-MSCs) could help with endometriosis through their studied anti-inflammatory role. Methods: Female Wistar rats weighting 180 to 250 g were randomly divided into two groups: group 1, endometriosis group; established by transplanting autologous uterine tissue into rats' peritoneal cavities and group 2, stem cell treated group; treated with AD-MSCs on the 5th day after induction of endometriosis. The proliferative activity of the endometriosis lesions was evaluated through Ki67 staining. Quantitative estimation of interferon γ, tumor necrosis factor-α, interleukin (IL)-6, IL-1β, IL-10, and transforming growth factor β expression, as well as immunohistochemical detection of CD68 positive macrophages, were used to assess the inflammatory status. Results: The size and proliferative activity of endometriosis lesions were significantly reduced in the stem cell treated group. Stem cells efficiently mitigated endometriosis associated chronic inflammatory reactions estimated through reduction of CD68 positive macrophages and the expression of the proinflammatory cytokines. Conclusion: Stem cell therapy can be considered a novel remedy in endometriosis possibly through its anti-inflammatory and antiproliferative properties.

Comparative Study of the Effective Dose from Panoramic Radiography in Dentistry Measured Using a Radiophotoluminescent Glass Dosimeter and an Optically Stimulated Luminescence Detector

  • Accurate measurement of the absorbed dose and the effective dose is required in dental panoramic radiography involving relatively low energy with a rotational X-ray tube system using long exposures. To determine the effectiveness of measuring the irradiation by using passive dosimetry, we compared the entrance skin doses by using a radiophotoluminescent glass dosimeter (RPL) and an optically stimulated luminescence detector (OSL) in a phantom model consisting of nine and 31 transverse sections. The parameters of the panoramic device were set to 80 kV, 4 mA, and 12 s in the standard program mode. The X-ray spectrum was applied in the same manner as the panoramic dose by using the SpekCalc Software. The results indicated a mass attenuation coefficient of $0.008226cm^2/g$, and an effective energy of 34 keV. The equivalent dose between the RPL and the OSL was calculated based on a product of the absorbed doses. The density of the aluminum attenuators was $2.699g/cm^3$. During the panoramic examination, tissue absorption doses with regard to the RPL were a surface dose of $75.33{\mu}Gy$ and a depth dose of $71.77{\mu}Gy$, those with regard to the OSL were surface dose of $9.2{\mu}Gy$ a depth dose of $70.39{\mu}Gy$ and a mean dose of $74.79{\mu}Gy$. The effective dose based on the International Commission on Radiological Protection Publication 103 tissue weighting factor for the RPL were $0.742{\mu}Sv$, $8.9{\mu}Sv$, $2.96{\mu}Sv$ and those for the OSL were $0.754{\mu}Sv$, $9.05{\mu}Sv$, and $3.018{\mu}Sv$ in the parotid and sublingual glands, orbit, and thyroid gland, respectively. The RPL was more effective than the OSL for measuring the absorbed radiation dose in low-energy systems with a rotational X-ray tube.

Calculation of Dose Conversion Coefficients in the Anthropomorphic MIRD Phantom in Broad Unidirectional Beams of Monoenergetic Photons (MIRD 인형팬텀의 넓고 평행한 감마선빔에 대한 선량 환산계수 계산)

  • The conversion coefficients of effective dose per unit air kerma and equivalent dose per unit fluence were calculated by MCNP4A code for antero-posterior(AP) and postero- anterior(PA) incidence of broad, unidirectional beams of photons into anthropomorphic MIRD phantom. Calculations have been performed for 20 monoenergetic photons of energy ranging from 0.03 to 10 MeV. The conversion coefficients showed a good agreement with the corresponding values given in the draft publication of joint task group of ICRP and ICRU within 10%. The deviations may arise from the differences of geometry in the MIRD phantom and the ADAM/EVE phantoms, and the differences in the codes and cross-section data used. Inclusion of a specific oesophagus model results in effective dose slightly different(5% at most) from the effective doses obtained by adopting the equivalent doses for the thymus or pancreas. Deletion of the ULI from the remainder organ appeared not to be significant for the cases of photon dosimetry covered in this study.

  • In the field of 'Human Vibration', it has been interested subjects to make equal sensation curves related to translational and rotational direction of whole-body, hand-transmitted and head-transmitted vibration, etc. When we consider the vibration of a vehicle, the main factor is vertical whole-body vibration. Until now, most of equal sensation curves used to derive frequency weighting function had been made using Western people. However, because of the inherent differences (for example, characteristic and shape of body parts, muscular and cellular tissue) between the Western people and the Oriental people, equal sensation curves based on Oriental people might be required. Also, the weight differences between the samples which consist of average-weighted and over-weighted group might cause the difference of equal sensation curves. So, in this study, 20 male Korean people were used to find equal sensation curves subject to vertical whole-body vibration on seated posture. Among 20 males, an over weighted group consisted of 10 male persons and an average weighted group was the others. Integrating and analyzing the data of two groups, some of non-parametric tests such as 'The Wilcoxon Signed Rank Test' and 'The Mann Whitney U test' were used.

Absorbed and effective dose in direct and indirect digital panoramic radiography (직.간접디지털 파노라마 방사선촬영시 흡수선량과 유효선량)

  • Purpose : We evaluated the absorbed doses to the organs and calculated the effective doses when using the digital panoramic radiography. Materials and Methods : The absorbed dose averages in major organs of oral and maxillofacial region were measured using the Dental head phantom (CIRS Co., USA), $^nLi_2B_4O_7$ TLD chip and UD-716AGL dosimeter (Matsushita Electric Industrial Co., JPN) when performing indirect and direct digital panoramic radiography. Effective doses were calculated from correspond to ICRP 2007 recommendations for two panoramic radiography. Results : The absorbed dose average on indirect and direct digital panoramic radiography was highest in parotid glands as measured 1259.6 mGy and 680.7 mGy respectively. Absorbed dose average in another organs were high in order of esophagus, submandibular gland, tongue and thyroid gland on both types of digital panoramic radiography. The absorbed dose average was higher on indirect type than direct one (p<0.05). The effective dose was higher on indirect type than direct one as measured 13.28 mSv and 8.70 mSv respectively. Conclusion : The absorbed doses in salivary gland and oral mucosa were high. However, thyroid gland also demands the attention on radiography due to high tissue weighting factor in spite of the low absorbed dose.

Evaluation of Cancer Incidence Rate using Exposure Dose to Surrounding Normal Organs during Radiation Therapy for Prostate Cancer (전립선암의 방사선 치료 시 주변 정상장기 피폭선량을 이용한 암발생확률 평가)

  • The purpose of this study was to evaluate the cancer incidence rate and provide basic data by measuring the photoneutron dose generated during intensity-modulated radiation therapy and volumetric modulated arc therapy used in radiation therapy for prostate cancer. The optically stimulated luminescence albedo neutron dosimeter for neutron measurement was placed on the Rando phantom in the abdomen and thyroid and photoneutron dose generated was measured. As a result of the study, intensity-modulated radiation therapy (7 portal) was measured to be higher than volumetric rotational radiation therapy in both abdominal and thyroid locations. When the cancer incidence rate was evaluated using the nominal risk coefficient of ICRP 103, the cancer incidence rate due to exposure to the colon and thyroid during intensity-modulated radiation therapy was 9.9 per 1,000 people, and volumetric rotational radiation therapy for 1,000 people. It was 3.5 per person. Based on the principle of ALARA (As low as reasonably archievable), it is considered to be a guideline for minimizing the exposure dose to normal organs in the establishment of a radiation treatment plan.

Measurement and Analysis of Pediatric Patient Exposure Dose Using Glass dosimeter and a PC-Based Monte Carlo Program (Glass dosimeter와 PCXMC Program을 이용한 소아피폭선량 측정 및 분석)

  • Exposed dose of young child should be managed necessarily. Young child is more sensitive than adult of a Radioactivity, especially, and lives longer than adult. Must reduce exposed dose which follows The ALARA(As Low As Reasonably Achievable)rule is recommended by ICRP(International Commission on Radiological Protection)within diagnostic useful range. Therefore, We have to prepare Pediatric DRL(Diagnostic Reference Level) in Korea as soon as possible. Consequently, in this study, wish to estimate organ dose and effective dose using PCXMC Program(a PC-Based Monte Carlo Program), and measure ESD(Entrance surface dose)and organ dose using Glass dosimeter, and then compare with DRL which follows EC(European Commission)and NRPB(National Radiological Protection Board). Using glass dosimeter and PCXMC programs conforming to the International Committee for Radioactivity Prevention(ICRP)-103 tissue weighting factor based on the item before the organs contained in the Chest, Skull, Pelvis, Abdomen in the organ doses and effective dose and dose measurements were evaluated convenience. In a straightforward way to RANDO phantom inserted glass dosimeter(GD352M)by using the hospital pediatric protocol, and in a indirect way was PCXMC the program through a virtual simulation of organ doses and effective dose were calculated. The ESD in Chest PA is 0.076mGy which is slightly higher than the DRL of NRPB(UK) is 0.07mGy, and is lower than the DRL of EC(Europe) which is 0.1mGy. The ESD in Chest Lateral is 0.130mGy which is lower than the DRL of EC(Europe) is 0.2mGy. The ESD in Skull PA is 0.423mGy which is 40 percent lower than the DRL of NRPB(UK) is 1.1mGy and is 28 percent lower than the DRL of EC(Europe) is 1.5mGy. The ESD in Skull Lateral is 0.478mGy which is half than the DRL of NRPB(UK) is 0.8mGy, is 40 percent lower than the DRL of EC(Europe) is 1mGy. The ESD in Pelvis AP is 0.293mGy which is half than the DRL of NRPB(UK) is 0.60mGy, is 30 percent lower than the DRL of EC(Europe)is 0.9mGy. Finally, the ESD in Abdomen AP is 0.223mGy which is half than the DRL of NRPB(UK) is 0.5mGy, and is 20 percent lower than the DRL of EC is 1.0mGy. The six kind of diagnostic radiological examination is generally lower than the DRL of NRPB(UK)and EC(Europe) except for Chest PA. Shouldn't overlook the age, body, other factors. Radiological technician must realize organ dose, effective dose, ESD when examining young child in hospital. That's why young child is more sensitive than adult of a Radioactivity.

A Study on the Incidence of Side Effects according to the Number of Beams in Intensity-modulated Radiation Therapy for Prostate Cancer using 15 MV (15 MV를 이용한 전립샘암 세기조절 방사선치료 시 빔의 개수에 따른 부작용 발생률에 관한 연구)

  • In this study, we analyzed the incidence of side effects of photoneutron dose according to the number of beams during intensity-modulated radiotherapy of prostate cancer using 15 MV. The radiation treatment plan design for intensity-modulated radiation therapy for prostate cancer was established with a prescription dose of 220 cGy per dose and a total of 7260 cGy for 33 treatments. The linear accelerator used in the experiment is Varian's True Beam STx (Varian, USA). Photoneutron dose was generated by using 15 MV energy in the planning target volume (PTV). The treatment plan was designed with IMRT 5, 7, and 9 portals using the Eclipse System (Varian Ver 10.0, USA). An optically stimulated luminescence albedo neutron dosimeter (Landauer Inc., USA) was used to measure photoneutron dose. IMRT 5 portals, 1.7 per 1,000, 7 portals, 1.8 per 1,000, 9 portals, 2.0 per 1,000 were calculated as the probability of experiencing side effects on the thyroid gland due to photoneutron dose. This study studies the risk of secondary radiation exposure dose that can occur during intensity-modulated radiation therapy, and it is considered that it will be used as useful data in relation to stochastic effects in the future.

Evaluation of Organ and Effective Dose using A PC-Based Monte Carlo Program in AEC Mode and Fix Mode for the whole spine antero-posterior radiography (전 척추 전.후 방향 검사 시 AEC Mode와 Fix Mode에서 PC-Based Monte Carlo Program을 이용한 장기선량 및 유효선량 평가)

  • There are AEC mode and fix mode to exposure when the whole spine antero-posterior radiography is done by using DR equipment. This study compared the utility of fix mode to AEC mode, by evaluating organ dose and effective dose and by examining the quality of radiographic image. GE DEFINIUM 8000 and ART-200X Rando Phantom manufactured by Flukebiometical were used for this study. The Rando phantom was set in front of wall detector of X-rays equipment. AEC mode was set at 80kVp and Fix mode was set at 80kVp, 25mAs, 32mAs, 40mAs, and 50mAs. Whole spine AP image were aquired by combining C, T-L and L-S spine images obtained through 3 exposures. When obtaining C, T-L and L-S spine images, were checked for Air kerma (mGy) value calculated by UNFORS Xi meter attached at the phantom surface of center of radiation field. The effective and organ doses were compared by PCXMC program (PC-Based Monte Carlo Program). The quality of obtained radiographic image was evaluated visually by 3 radiologists using resolution chart. When the effective doses was calculated based on tissue weighting factor of ICRP-103, 1.278mSv was measured by AEC mode, and Fix mode measured 0.405mSv at 25mAs, 0.518mSv at 32mAs, 0.649mSv at 40mAs, and 0.810mSv at 50mAS. In addition, the organ dose measured with esposure at 25mAs by Fix mode was almost equivalent to the organ dose by AEC mode, at the esophagus, thyroid, oral mucosa, salivaly glands located at the cervical spine part, while the organ dose by Fix mode was in general lower than the organ dose by AEC mode at the other organs. When Fix mode at 32mAs, 40mAs, and 50mAs was compared to AEC mode for organ dose in 26 organs, AEC mode had higher measurement in 21 organs but not for than brain, trachea, thyroid, oral mucosa, and salivaly glands which are located at the cervical spine part. The image quality evaluated by resolution test chart was much higher with AEC mode than the quality with Fix mode at all exposure conditions. However, while the image quality of cervical spine exposured at 50mAs by Fix mode was lower than the quality of AEC mode, thoraco-lumbar spine and lumbo-sacral spine were calculated and the quality was similar to AEC mode. Scoliosis occurs mainly at thoraco-lumbar and lumbo-sacral spine, not at cervical spine. Compared to AEC mode, Using the appropriate protocol (80kVp, 50mAs) of fix mode for whole spine AP radiography was thought to be useful because the image quality of the thoraco-lumar and lumbo-sacral spine was similar on AEC mode, Also organ and effective doses can be decreased with Fix mode. Therefore, It is considered that fix mode can be used properly with AEC mode for whole spine AP radiography when considering patient's body posture.
