Identification of Biomarkers for Diagnosis of Gastric Cancer by Bioinformatics

  • Background: We aimed to discover potential gene biomarkers for gastric cancer (GC) diagnosis. Materials and Methods: Genechips of 10 GC tissues and 10 gastric mucosa (GM, para-carcinoma tissue, normal control) tissues were generated using an exon array of Affymetrix containing 30,000 genes. The differentially expressed genes (DEGs) between GC tissues and normal control were identified by the Limma package and analyzed by hierarchical clustering analysis. Gene ontology (GO) and pathway enrichment analyses were performed for investigating the functions of DEGs. Receiver operating characteristics (ROC) analysis was performed to measure the effects of biomarker candidates for diagnosis of GC. Results: Totals of 896 up-regulated and 60 down-regulated DEGs were identified to be differentially expressed between GC samples and normal control. Hierarchical clustering analysis showed that DEGs were highly differentially expressed and most DEGs were up-regulated. The most significantly enriched GO-BP term was revealed to be mitotic cell cycle and the most significantly enriched pathway was cell cycle. The intersection analysis showed that most significant DEGs were cyclin B1 (CCNB1) and cyclin B2 (CCNB2). The sensitivities and specificities of CCNB1 and CCNB2 were both high (p<0.0001). Areas under the ROC curve for CCNB1 and CCNB2 were both greater than 0.9 (p<0.0001). Conclusions: CCNB1 and CCNB2, which were involved in cell cycle, played significant roles in the progression and development of GC and these genes may be potential biomarkers for diagnosis and prognosis of GC.

위상이동 형상측정법을 이용한 도재 소성시의 도재 수축률의 측정 (Measurement of Porcelain Shrinkage After Firing Using the Phase-Shifting Profilometry)

  • To compare several porcelains made by various manufacturers in shrinkage after firing and investigate the effect of condensation on shrinkage, specimens were prepared and the volume of each body was measured by the phase-shifting profilometry. Baseplate wax was cut by $2.5cm{\times}2cm$ and cast in nonprecious metal(Verabond, U.S.A.), then any surfaces of specimens were abrased and polished on the SiC abrasing papers, preparing 120 specimens. Specimens were divided into six groups according to the porcelain used, and the porcelain used in each group were as follows. Group I : Ceramco dentin porcelain Group B : Creation dentin porcelain Group III : Creation margin porcelain Group IV : Vintage margin porcelain Group V : Vita dentin porcelain Group VI : Vintage dentin porcelain Porcelain was built up on the metal plates using a small spoon and then solution matching to each porcelain was added. The six groups are subdivided into a and b. In subgroup a, only excessive solution was absorbed with tissue and in subgroup b, porcelain was condensed sufficiently. When build-up was completed, the shape was measured using the phase-shifting profilometry. After that, specimens were fired in the furnace programed for each porcelain and then their changed shape were measured again. Using the difference between the two above measurements, the ratio of shrinkage was calculated. Obtained results were as follows ; 1. Regardless of condensation, the volume of fired specimens were not different significantly between the two subgroups a and b in the same group. 2. The ratios of shrinkage were significantly higher in the groups porcelain built-up was condensed than in the groups not condensed 3. The ratios of shrinkage were in the range of 36.81-27.19% in the groups porcelain built up was condensed and 44.52-37.54% in the other groups not condensed.

Association of a Pyruvate Kinase M2 (PKM2) Polymorphism with Back Fat Thickness in Berkshire Pigs

  • Pyruvate kinase M2 (PKM2) is a key regulatory enzyme in the glycolytic pathway. It is one of four pyruvate kinase isoenzymes that widely differ in their occurrence according to tissue type. PKM2 is expressed in differentiated tissues, such as fat tissues, lung, as well as normal proliferating cells, embryonic cells, and tumor cells. The objective of this study was to investigate the association of single nucleotide polymorphisms (SNPs) in the PKM2 gene with meat quality traits in Berkshire pigs. We detected a SNP (g.34341 A>G) in the 3'UTR region of the PKM2 gene in 670 Berkshire pigs through DNA sequencing. Three genotypes, AA, AG, and GG, were found for this SNP, but based on an association analysis with meat quality traits, genotype AA was significantly associated with thicker back fat than genotype GG (p=0.027). Therefore, the g.34341 A>G polymorphism in the 3'UTR region of the porcine PKM2 gene could be applied in pig breeding programs to improve back fat thickness.

Management of Defects on Lower Extremities with the Use of Matriderm and Skin Graft

  • Background The reconstruction of large skin and soft tissue defects on the lower extremities is challenging. The skin graft is a simple and frequently used method for covering a skin defect. However, poor skin quality and architecture are well-known problems that lead to scar contracture. The collagen-elastin matrix, Matriderm, has been used to improve the quality of skin grafts; however, no statistical and objective review of the results has been reported. Methods Thirty-four patients (23 male and 11 female) who previously received a skin graft and simultaneous application of Matriderm between January 2010 and June 2012 were included in this study. The quality of the skin graft was evaluated using Cutometer, occasionally accompanied by pathologic findings. Results All 34 patients showed good skin quality compared to a traditional skin graft and were satisfied with their results. The statistical data for the measurement of the mechanical properties of the skin were similar to those for normal skin. In addition, there was no change in the engraftment rate. Conclusions The biggest problem of a traditional skin graft is scar contracture. However, the dermal matrix presents an improvement in skin quality with elastin and collagen. Therefore, a skin graft along with a simultaneous application of Matriderm is safe and effective and leads to a significantly better outcome from the perspective of skin elasticity.

두경부편평세포암종에서 Gleevec의 효과 (Effect of Gleevec on Head and Neck Squamous Cell Carcinoma)

  • Purpose: The serine/threonine kinase Akt was described to inhibit apoptosis in cancer. This study was to examine the effect of Gleevec on head and neck squamous cell carcinoma(HNSCC) through the mechanism of Akt. Experimental Design: Gleevec was introduced into the HNSCC cell lines UMSCC10B, HN12 and HN30 in a range of concentrations. Cell viability was assessed by clonogenic survival analysis. Targets of Gleevec(PDGFR, c-Kit, and c-Abl) were evaluated by Western blot. HNSCC tissue samples were stained for PDGFR, c-Kit and phosphorylated Akt. Akt phosphorylation following Gleevec treatment was assessed using Western blot. Akt siRNA was used to as the positive control. Results: Colony forming efficiency decreased with an increase in concentration of Gleevec. Expressions of PDGFR, c-Kit, and c-Abl were observed in HNSCC cells. Immunohistochemistry confirmed high expression of PDGFR, c-Kit, and p-Akt in human HNSCC tissues. Akt kinase activity was significantly inhibited with increasing concentration of Gleevec in HNSCC cells, and near complete dephosphorylation of Akt was observed at $6{\mu}M$ of Gleevec in the UMSCC10B and HN30 cell lines. Conclusions: Gleevec at clinically comparable concentrations caused a dose dependant decrease in HNSCC survival. The decreased cell survival was related to the inhibition of Akt kinase activity and dephosphorylation of Akt. Akt signaling pathway may be a relevant target for Gleevec in treating HNSCC.

Flexor Tenorrhaphy Using Absorbable Suture Materials

  • Background Nonabsorbable sutures are favorable for repairing flexor tendons. However, absorbable sutures have performed favorably in an animal model. Methods Two-strand sutures using the interlocking modified Kessler method with polydioxanone absorbable sutures 4-0 were used to repair completely ruptured flexor tendons in 55 fingers from 41 consecutive patients. The medical records of average 42 follow up weeks were analyzed retrospectively. The data analyzed using the chi-squared test, and Fisher's exact test was used for postoperative complications. The results were compared with those of other studies. Results Among the index, middle, ring, and little fingers were injured in 9, 17, 16, and 13 fingers, respectively. The injury levels varied from zone 1 to 5. Of the 55 digits in our study, there were 26 (47%) isolated flexor digitorum profundus (FDP) injuries and 29 (53%) combined FDP and with flexor digitorum superficialis injuries. Pulley repair was also conducted. Concomitant injuries of blood vessels and nerves were found in 17 patients (23 fingers); nerve injuries occurred in 5 patients (10 fingers). Two patients had ruptures (3.6%), and one patient had two adhesions (3.6%). Using the original Strickland criteria, all the patients were assessed to be excellent or good. Also, fibrosis and long-term foreign body tissue reactions such as stitch granuloma were less likely occurred in our study. Compared to the Cullen's report that used nonabsorbable sutures, there was no significant difference in the rupture or adhesion rates. Conclusions Therefore, this study suggests that appropriate absorbable core sutures can be used safely for flexor tendon repairs.

대본청 앵무(Psittacula eupatria )로부터 PCR에 의한 avian polyomavirus 최초 검출 (First detection of avian polyomavirus by PCR from Alexandrine Parakeet (Psittacula eupatria) in Korea)

  • In early April 2014, a month-old Alexandrine Paraqeet (Psittacula eupatria) that was raised in a domestic aviary located in Gyungju-si, Korea was suddenly died and submitted to Animal Disease Intervention Center, Kyungpook National University in order to diagnose the causative agent. In post-mortem examination, the bird had abnormally developed feathers on the neck and abdomen region and subcutaneous hemorrhages on the neck and cheek adjacent to the beak. At necropsy, the bird had hemorrhage on the muscle of the femoral region, ascites, multi-focal hemorrhages on the epicardium, and diffuse hemorrhages on the sub-serosa of proventriculus and gizzard, suggesting typical avian polyomavirus (APV) infection. The partial large tumor (T) antigen gene of APV was detected by PCR from tissues of the heart, lung, liver, kidney, proventriculus and feathers of the APV-suspected birds. However, other pathogenic virus-specific nucleic acid common with psittacine birds such as avian bornavirus, psittacine beak and feather disease virus and psittacid herpesvirus were not detected from the mixed tissue samples of the bird, indicating this case is due to single infection of APV. Nucleotide sequence analysis of the partially amplified large T antigen DNA was confirmed to have 99~100% homology with that of the previously reported APV strains. This case report describes the first detection of APV in Alexandrine Paraqeet in Korea.

광배근 유리 피판술을 이용한 두피 및 두개골 결손의 재건 (Reconstruction of Scalp and Calvarial Defects Using Latissimus Dorsi Myocutaneous Free Flap)

  • Purpose: Reconstruction of scalp and calvarial defects should provide both aesthetic and functional aspects. The inelastic nature of the scalp and previous surgery or radiation preclude the use of primary closure or a local flap. With development of microsurgical technique, a free tissue transfer is a good option. We use the latissimus dorsi myocutaneous free flap for reconstruction. Materials and Methods: A review of all latissimus dorsi free flap reconstructions performed in nine patients from 2009 to 2012 was conducted. There were six males and three females, ranging in age from seven to 69 years, and nine different regions, including five temporal regions, two occipital regions, and two frontoparietal regions. The flaps ranged in size from $9.0{\times}10.0cm$ to $14.0{\times}15.0cm$. Recipient vessels available for microanastomosis were most often the superficial temporal vessels and two patients had anastomoses to the external carotid artery and internal jugular vein. Results: All flaps survived postoperatively. With a median follow-up period of 14 months, no major complications were noted. However, two patients developed minor wound dehiscence, and a hematoma was observed in one patient. Conclusion: We performed the latissimus dorsi myocutaneous free flap reconstruction, which is one of the most popular reconstructive methods. The latissimus dorsi myocutaneous free flap reconstruction has been proven successful in our patients with satisfactory results. During the long term follow-up period, even though depressions were observed on the defect area in some patients, they were treated successfully with cranioplasty. Therefore, we recommend the latissimus dorsi myocutaneous free flap for reconstruction of scalp and calvarial defects.

활액성 육종의 증례 보고 (A CASE REPORT OF SYNOVIAL SARCOMA)

  • A malignant sarcoma of soft tissue that arises near but not in a joint, most often in an adolescent or young adult, is typically slowgrowing, and may escapes notice until it causes pain. The diagnosis of this tumor can be suspected by X-ray or imaging, made by biopsy, and confirmed by chromosome studies. Cytogenic studies reveal a translocation between the X chromosome and chromosome 18 in the tumor cells. The most common treatment for the tumor is surgery to remove the entire tumor, nearby muscle, and lymph nodes. Synovial sarcoma tends to recur locally and to involve regional lymph nodes. Distant metastasis occurs in half cases. The well-documented and adequately followed cases are currently lacking and this report described an instance of Synovial sarcoma good result after treatment and review of literature.

근위 경골의 결합조직형성 섬유종 치료 후 발생한 골육종 - 1예 보고 - (Osteosarcoma Arising from Desmoplastic Fibroma of Proximal Tibia - A Case Report -)

  • 결합조직형성 섬유종은 조직 소견상 연부 조직의 유건종(desmoid tumor)과 유사한, 비교적 드문 종양으로 대부분 장관골에 발생한다. 그러나 양성 종양이지만 골소파 및 골이식 치료후에는 국소 재발이 빈번한 것으로 알려져 있다. 더욱이 재발을 하게 되면, 아주 드물지만 골 육종으로 악성 변화되는 경우가 보고되고 있다. 본 교실에서는 17세 남자 환자에서 근위 경골에 발생한 결합 조직 섬유종에 대해 소파술 및 골 이식술을 시행 후, 11년 후에 골육종으로 악성 변화된 결합조직형성 섬유종 1례를 경험하였기에 보고하는 바이다.
