The effect of temporomandibular joint movement on tinnitus (턱의 운동이 이명에 미치는 영향)

  • Purpose: A growing number of studies have been providing evidence for neural connections between the auditory and somatosensory systems that might be a critical part of the mechanisms underlying certain forms of tinnitus. The aim of this study was to investigate the effect of temporomandibular joint (TMJ) movements on tinnitus. Methods: One hundred sixty-three tinnitus patients participated in this study. All patients underwent a thorough audiological examination including pure-tone audiometry, tinnitus handicap inventory, and evaluation of tinnitus loudness, frequency and severity on a visual analog scale. Somatic testing consisting of nine forceful jaw muscle contractions was performed to evaluate the effect of TMJ movements on modulation of tinnitus. Results: 66.9% of patients had unilateral tinnitus. Somatic testing modulated tinnitus loudness in 125 ears (57.6 %) of 217 ears tested. An increase in tinnitus loudness was observed more often than a decrease. Loudness was most commonly increased by opening and clenching jaw. Tinnitus could be induced by opening, clenching and deviating jaw to the left. Conclusion: Our study showed evidence that TMJ movements may aggravate and even evoke tinnitus and somatic testing can be used for evaluating if stimulation of the TMJ induces or aggravates tinnitus.

A Case Report of Tinnitus Occurred in the Sudden Sensory Neural Hearing Loss Treated with Korean Medical Treatments (돌발성 난청 환자에서 발생한 이명의 한방 치험 1예)

  • Objectives : The purpose of this study is to show the clinical effect of Korean Medical Treatments for Tinnitus with Sudden Sensory Neural Hearing Loss. Methods : The patient was treated by Korean Medical Treatments for 10weeks. The effect of treatments on tinnitus was measured with Visual Analogue Scale(VAS) and Korean Tinnitus Handicap Inventory(K-THI). And the effect of treatments on sudden hearing loss was measured by VAS. Results : VAS of Tinnitus was decreased from 10 to 3 points and K-THI score was decreased from 70 to 26 points. VAS of hearing loss was decreased from 9 to 8 points. Conclusions : Korean Medical Treatments are effective on Tinnitus.

  • 1. According to causes of attack and symptoms, tinnitus is divided into two categories; deficiency and excess. Causes of excess syndrome of tinnitus were wind fire in the liver and gallbladder, phlegm fire, blood stagnation, and heat in meridian system and the causes of deficiency syndrome of tinnitus were qi deficiency or blood deficiency after an illness or delivery, yin deficiency of liver and kidney, and deficiency of sea of the marrow. 2. Tinnitus was related to the vicera and bowels, especially to liver, gallbladder and urinary bladder. 3. In regard of method of treatment, tonify kidney, nourish heart, clear the liver and discharge heat are used according to visera and bowel theory. Clear phlegm and downbear fire are used for phlegm fire. Tonify spleen and kidney is used for ancestral vessel deficiency. Dispel wind and dissipate fire can be used according to theory of five elements' motion and six kinds of natural factors. 4. The basal meridian of acupuncture and moxibustion treatment were the channels of Shaoyang.. 5. Regarding neuropsychiatric aspect of tinnitus, sudden anger and depression of mind were the main mechanism of disease and liver fire was the main cause. The prescriptions for neuropsychiatric tinnitus were Dangguiyonghuehwan, and Yongdamsagantang.

  • Tinnitus is a symptom frequently encountered in the E.N.T. clinic and variously described as buzzing, ringing and thumping sound, etc., but the cause, pathogenesis, specific treatment remains to be established through close investigation. In fact, it is not easy to evaluate the treatment efficiency of tinnitus reasonably. However, in this study. we clinically investigated 47 patients who visited E.N.T. department of Kyung Won University Hospital from May. 1996 to May. 1999 and belonged to specific bianzheng(辨證) types of tinnitus classified by bianzheng(辨證), a kind of diagnostic method in order to estimate efficiency of the treatment and give aids to establishing more effective and recognizable treatment method. The result is as follows: 1) In bianzheng(辨證) types of tinnitus, Piweixuruo(脾胃虛弱) was most frequently found, in descending order, Shenjingkuisun(腎精虧損), Ganhuoshangra(肝火上擾), Tanhuoyongjie(痰火壅結). 2) In the treatment efficiency, the type of Piweixuruo(脾胃虛弱) showed the highest, in descending order, Ganhuoshangrao(肝火上擾), Tanhuoyongjie(痰火壅結) and Shenjingkuisun(腎精虧損). 3) The treatment efficiency is shown higher in patients with tinnitus included in other sysmptoms than those with a simple symptom of tinnitus. especially in case of tinnitus accompanied by vertigo, we could get much higher treatment efficiency more than we had expected.

Changes in the Laterality of Functional Connectivity Associated with Tinnitus: Resting-State fMRI Study

  • Purpose: One of the suggested potential mechanisms of tinnitus is an alteration in perception in the neural auditory pathway. The aim of this study was to investigate the difference in laterality in functional connectivity between tinnitus patients and healthy controls using resting state functional MRI (rs-fMRI). Materials and Methods: Thirty-eight chronic tinnitus subjects and 45 age-matched healthy controls were enrolled in this study. Connectivity was investigated using independent component analysis, and the laterality index map was calculated based on auditory (AN) and dorsal attention (DAN), default mode (DMN), sensorimotor, salience (SalN), and visual networks (VNs). The laterality index (LI) of tinnitus subjects was compared with that of normal controls using region-of-interest (ROI) and voxel-based methods and a two-sample unpaired t-test. Pearson correlation was conducted to assess the associations between the LI in each network and clinical variables. Results: The AN and VN showed significant differences in LI between the two groups in ROI analysis (P < 0.05), and the tinnitus group had clusters with significantly decreased laterality of AN, SalN, and VN in voxel-based comparisons. The AN was positively correlated with tinnitus distress (tinnitus handicap inventory), and the SalN was negatively correlated with symptom duration (P < 0.05). Conclusion: The results of this study suggest that various functional networks related to psychological distress can be modified by tinnitus, and that this interrelation can present differently on the right and left sides, according to the dominance of the network.

A Clinical Case Study on the Tinnitus with Sudden Sensorineural Hearing Loss (돌발성 난청을 동반한 이명 환자 1례에 대한 증례보고)

  • Tinnitus is noise from ear or head without any external sound stimulation and can cause hearing difficulties, psychologenic disturbances or many difficulties in everyday life. the etiology has yet been discussed and the management is also very difficult. Sensorineural hearing loss is defined as a sudden hearing impairment which was develope over a period of hours to days. I report one tinnitus case which accompanied with sudden sensorineural hearing loss. In the early and acute stage which judged as Dam-Wha, herbal treatment with Tong-myeong-ri-gi-tang was efficacious on the tinnitus. In the convalescent stage and judged as Sin-heo, herbal treatment with Ja-sin-tong-i-tang was efficacious on the tinnitus that accompanied with sudden sensorineural hearing loss.

Lamivudine- and Adefovir-Induced Tinnitus in a Patient with Hepatitis B: A Case Report (B형 간염 환자에서 Lamivudine과 Adefovir 병용처방으로 유발된 이명의 증례보고)

  • Lamivudine and adefovir are medications used to treat hepatitis B. We observed the occurrence of tinnitus after administering lamivudine and adefovir to a 49-year-old hepatitis B patient for two months. The patient had no comorbidities and no history of ear diseases, including tinnitus, and was not taking any other medications. In general, neither lamivudine nor adefovir are known to induce tinnitus as an adverse reaction. A literature search revealed that this is the first case in which tinnitus occurred after lamivudine and adefovir were administered to a hepatitis B patient. Therefore, we believe that this case is clinically valuable and decided to report it.

  • Objective : The purpose of this paper is to report the effect of Daehamhyung-tang(DHHT) on tinnitus with panic disorder from the disease pattern identification diagnostic system based on Shanghanlun provisions. Methods : According to a disease pattern identification diagnostic system based on Shanghanlun provisions, the patient was diagnosed with 'Taeyang-byung Gyeolhyung'. He took DHHT for 82 days. This paper evaluated the results of treatment on tinnitus by Tinnitus Handicap Inventory(THI) and Verbal Numercial Rating Scale(VNRS). To assess the mental state of anxiety, this paper used Hamilton Anxiety Rating Scale(HAM-A) Results : The THI score was declined from 58 to 6 And the VNRS score was declined from 6 to 1. HAM-A score was declined from 28 to 3. Conclusions : This case study suggests that DHHT has effectiveness on a patient suffers from tinnitus with panic disorder and diagnosed with 'Taeyang-byung Gyeolhyung'.

Recent Trends in Oriantal Medical Treatment for Tinnitus (이명(耳鳴)의 한방치료에 대한 최신 연구 동향)

  • Objectives : This study is to research the trend of the oriantal medical treatment for tinnitus and to establish the hereafter direction for the study on oriantal medical treatment for tinnitus. Methods : We reviewed the domestic articles published last eleven years(2000- 2010). Results : We have searched 21 articles in 5 journals of korean oriental medical ophthalmology &dermatology, korean acupuncture & moxibustion, korean oriental medicine, oriental physiology & pathology, sasang constitutional medicine. 1. There were the most research done 2006. They have 5 articles. In other years there were 3 or less articles. 2. The pattern of study was as follow: the consideration of documents were 5, clinical trials were 6, case studies were 8 and the others were 2. the rate of case studies were increased. 3. After mid 2000s, there were more studies of new research method appearing. Conclusions : The oriental medical treatment already effective for tinnitus. But we need more studies about various kind of oriental medical treatment for tinnitus.

Transcutaneous Electrical Nerve Stimulation System for Treating Tinnitus based on the Cortex-M4 Microcontroller

  • Among the methods of treating tinnitus, the transcutaneous clectrical nerve stimulation (TENS) method of treating by electrical stimulation is common. However, there is a problem that surgical operation is required to stimulate the vagus nerve (VN) main trunk near most of the bronchus. Alternatively, we found that the same effect could be achieved by electrically stimulating the vagus nerve VN branch (Arnold's nerve) distributed in the outer ear. The TENS system for stimulation of vagus nerve has been developed, but it has not been able to implement to stimulate as a parameter optimized for the patient by simultaneously playing the sound of eliminating the tinnitus frequency. Therefore, in this paper, it is important to develop a safe and practical TENS device for tinnitus treatment based on a 32-bit microprocessor that simultaneously applies non-invasive and notched sounds and to develop optimal treatment methods for treating tinnitus.
