Quality of Life and Chemotherapy-related Symptoms of Turkish Cancer Children Undergoing Chemotherapy

  • This cross-sectional and descriptive study was designed to determine symptoms emerging due to chemotherapy treatment and their effects on children's quality of life. The research was carried out between February 2008 and February 2009 at the pediatric oncology clinics in four hospitals, focusing on 93 patients receiving chemotherapy. A survey form, the Pediatric Quality of Life Inventory (PedsQL 4.0) and the Memorial Symptom Assessment Scale (MSAS) were used as data collection tools. Chi-square and Student t tests were performed for data analysis. Some 51.6% of the children were aged 13-15 years old, and 51.8% were boys and 50.5% were diagnosed as having solid tumors. There were significant relations between: antimetabolite chemotherapeutics and feeling irritable and worrying (p=0.001, p=0.030); vinkoalkaloid and numbness/tingling in hands/feet (p=0.043); antracyclines and lack of energy and skin changes (p=0.021, p=0.004); and corticosteroids and lack of appetite, nausea and sadness (p=0.008, p=0.009, p=0.009). Several symptoms such as feeling sad, worrying and feeling irritable caused a significant decrease in the total domain of quality of life scores (p=0.034, p=0.012, p=0.010, respectively). Chemotherapeutic drugs can cause symptoms that can seriously affect quality of life in children.

A Case Report of Long-term Acupuncture Treatment in Bortezomib Induced Peripheral Neuropathy (보르테조밉으로 유발된 말초신경병증에 대한 장기간의 침치료 증례 보고)

  • Objectives : We present a successful administration of acupuncture in alleviating persistant peripheral neuropathy due to bortezomib, a potent therapeutic agent of mutlple myeloma. Methods : A patient, experiencing numbness, coldness, tingling and weakness in her feet and hands after bortezomib treatment, was administered acupuncture for 32 weeks, 3 times/week for first 4 weeks and 2 times/week the last 28 weeks, in bilateral acupuncture points, ST36, ST40, EX-LE10, LI11, TE5, and EX-UE9. Responses were assessed at the end of every 4 weeks with Eastern Cooperative Oncology Group(ECOG) grading system, National Cancer Institute - Common Toxicity Criteria(NCIC-CTC) v4.0, Numeric Rating Scale(NRS) 0-10, and Neuropathy Pain Scale (NPS). Results : ECOG was improved from 2 to 1, NCIC-CTC from 2 to 1, NRS from 8 to 1, and NPS from 41 to 5 through 32 weeks of acupuncture treatment. Conclusions : It is conceivable that acupuncture can be a help to relieve bortezomib induced peripheral neuropathy.

Two Cases of Neurotoxin Tetramine Poisoning Following Ingestion of Buccinum Striatissinum (물레고동 섭취 후 발생한 테트라민 중독 2례)

  • Some carnivorous gastropods have heat stable tetramine toxins in their salivary glands. This toxin is an autonomic ganglionic blocking agent that enables them to catch the prey easily by paralyzing their targets. Acute tetramine toxin poisoning in humans from eating whelks has been well described based on numerous cases, but is rare in Korea. Symptoms of tetramine poisoning include eyeball pain, blurred vision, headache, dizziness, muscular twitching, tingling of hands and feet, weakness, paralysis and sometimes collapse. Gastrointestinal symptoms, such as abdominal pain, nausea, and vomiting can also occur. However, intoxication is self-limiting and patients will usually recover in about 24 hours. Herein, we report 2 cases of tetramine poisoning after ingestion of Buccinum striatissinum as meat and soup.

Compression Neuropathy of Superficial Radial nerve by Variation of Course of Radial Artery in Distal Forearm - A case report - (원위 전완부에서 요골 동맥의 주행 변이로 발생된 표재성 요골 신경 압박증 - 1례 보고 -)

  • A 57-year-old male suffered from tingling and paraesthesia on radial side of right hand. We carried out exploration, the process being done, a radial artery passing between the two branches of bifurcated superficial radial nerve was found by accident. Then we performed dissection of the radial artrery a little more distally. It was passing above the extensors of thumb to anstomical snuff box. We treated it with ligating the radial artery, which was done a little above the bifurcated site of superficial radial nerve after angiography of upper extremity. While we were following this procedure, we noticed that symptoms had improved a lot. To our knowlege, neuropathy, by itself of superficial radial nerve by the radial artery with anatomical variation has never been reported and therefore we report this case.

Laboratory Findings and Subjective Symptoms of Car Repair Workers (자동차 정비공의 혈액 및 뇨검사 소견과 자각증상에 대한 조사)

  • To evaluate the effects on health by exposure to low dose organic solvents, the author analyzed the air concentration of mixed organic solvents (toluene, xylene, butylacetate) at worker's breathing zone during painting, some laboratory findings of blood (WBC, RBC, Hb, Hct, SGOT, SGPT, TC, TG, HDL-C, LDL-C, IgG, IgM, IgA and IgE) and urine (hippuric acid, urinary protein, urobilinogen), surveyed the subjective symptoms those were obtained from 35 male workers exposed to mixed organic solvents, and 25 male workers not exposed to organic solvents who worked in car repair workshops in Taejon area from December 1, 1995, to February 29, 1996. The results were as follows: 1. The mean concentration of urinary hippuric acid of car painting worker group (organic solvent exposure group) was 0.76$\pm$ 0.21 g/l, which is significantly higher than that of non-exposed group. 2. In hematologic findings, the values of RBC, TC, LDL-C, IgG and IgE in the exposure group were significantly lower than those of the nonexposed group, but SGOT and SGPT in the exposure group were significantly higher than those of the non-exposed group. 3. Urinary hippuric acid levels showed positively correlated with toluene, urobilinogen and HDL-C levels, but those were negatively correlated with RBC, LDL-C, IgM levels. 4. Rates of the subjective symptoms such as "dizziness", "appetite loss", "weight loss", "palpitation", "chest tightness", "sore throat and eye discomfort", "tingling sense and acrodynia", "illusion or hallucination" and "decreaased motor power" were significantly higher in the exposure group than those of the non-exposed group.

A Case Report on Herpes Zoster (대상포진 환자 치험 1례)

  • Clinical symptoms of herpes zoster include red rash, burning pain, tingling or extreme sensitivity in the affected area, usually limited to one side of the body. There might be a fever or a headache. The pain of acute herpes zoster may be severe, but it is usually temporary. Some of old patients are likely to develop postherpetic neuralgia. We treated a 73 year-old male patient who had severe pain and a headache after acute herpes zoster. In the point of Differentiation of Syndromes(辯證), this subject was diasgnosed as wind-heat syndrome(風熱證) and was administered Bangpungtongsung-san. For the purpose of making the pain easier, we used the western medication as well. After six days of treatment, pain and the other symptoms improved. Besides postherpetic neuralgia dissipated. Based on this experience, both oriental medicine and western medicine have a good effect on acute herpes zoster.

  • Objectives : This study was progressed a research with the aim of using it as basic data necessary for a plan in order to increase qualitatively medical level for patients who feel anxiety about dental treatment, and of offering basic data for improving the whole medical services by grasping conditions and satisfaction level with laser therapy in patients who visited dental clinic. Methods : The statistical analysis was carried out t-test and ANOVA by using SPSS(PASW statistics) 18.0. When there is statistically significant difference in analysis of variance, post-test was carried out by using Duncan multiple range test. Results : In therapy kinds of using laser, gingival treatment was the largest with 37.0%. It was the highest in likely seeming to be not painful with 32.9% as for a decisive factor, which came to receive laser therapy, and in having been enough to endure even if having been slightly tingling as for pain level. Conclusions : This study can be considered to have significance in changing recognition of medical cure by surveying patients satisfaction with using laser as a method for slightly solving elements of fear and anxiety about dental treatment and to have its value as basic data for seeking improvement plan for diverse medical services in the future.

Painful legs and moving toes syndrome in a 16-year-old girl

  • Painful legs and moving toes (PLMT) syndrome is characterized by spontaneous movements of the digits and pain in one or both lower extremities. Of the reported cases, a majority of the patients was female, and the mean age of onset was 58 years. Only one pediatric case has been reported so far. Herein, we report the first adolescent case of PLMT in Korea. A 16-year-old girl complained of tingling pain in the left leg and involuntary movement of the ipsilateral great toe one month after a second untethering surgery. Three years ago, she had undergone untethering surgery to correct lipomeningomyelocele at the S2 level of the conus medullaris. At that time, she was diagnosed with polyradiculopathy at the left L5 level with axonal involvement. We diagnosed her with PLMT syndrome and prescribed gabapentin. Her symptoms diminished within a day. Complete relief from involuntary movement of the toe was achieved within four months. PLMT is a rare syndrome but it should be considered in the differential diagnosis of children and adolescents with limb pain and spontaneous movement in their toes.

Cauda Equina Syndrome and Common Peroneal Nerve Palsy after Spinal Anesthesia -A case report- (척추마취후 발생한 마미증후군과 총비골신경마비 -증례 보고-)

  • Although spinal anesthesia has long been considered a safe technique, it is not without risk or side effect. Cauda equina syndrome is a rare but serious complication of spinal anesthesia. We have experience a case of cauda equina syndrome after spinal anesthesia. A twenty year old healthy male patient complained of pain, numbness, tingling sensation and motor weakness on his right lower extremity 8 hours after subarachnoid blockade. On the following day, the patient was noted to have a right L1 to S2 radiculopathy. Magnetic Resonance Imaging results were unremarkable. The patient sprained his ankle while trying to move down from the bed, so short leg splint was applied. Then he had additional right common peroneal nerve injury from the splint. His neurologic symptoms improved gradually thereafter, and three months postoperatively his electromyogram revealed improving stage from right common peroneal nerve palsy.

The Therapeutic Value of Ultrasound-Guided Irrigation in Sialodochitis: A Case Report

  • In the present case study, we reported a female patient with sialodochitis fibrinosa in which a conservative ultrasound (US)-guided irrigation method was used to relieve her symptoms. A 30-year-old female patient visited Kyung Hee University Dental Hospital with chief complaints of persistent swelling and discomfort in the left facial area. Her primary symptom was facial swelling that was accompanied by neuropathological symptoms, such as itching, tingling, and warmth that usually continued for 3 to 4 days. During clinical examination of orofacial area, mild swelling and fever were noted in the left face including parotid region, and mild induration could be felt at the corresponding site; sialadenitis of the left parotid gland was tentatively diagnosed. Herein, we performed intraductal irrigation of the left parotid gland in three times per month and prescribed some medication. Thus, her signs and symptoms have been improved, and she did not experience a recurrence for 12-month follow-up to date. This study can support the usefulness of the US-guided irrigation method to treat the sialodochitis fibrinosa without remarkable side effects.
