Effects of Cross Training with Bilateral Ankle Dorsiflexor Strengthening Exercise on the Muscle Activity of the Paralytic Tibialis Anterior, Balancing Ability, and Gait Function in Patients with Chronic Stroke: A Preliminary Randomized, Controlled Study

  • PURPOSE: This study examined the effects of bilateral ankle dorsiflexors-strengthening exercise on the paralytic tibialis anterior activity, balance ability, and gait function of patients with chronic stroke. METHODS: Nineteen patients with chronic stroke were assigned randomly to the experimental and control groups. All participants received general physical therapy for 60-minutes per session, five times a week, for 6 weeks. In addition, the experimental group (n = 9) performed bilateral ankle dorsiflexion muscle-strengthening training three times a week, 30 minutes per session, for six weeks. The control group (n=10) performed the paraplegic ankle dorsiflexion muscle- strengthening training in the same manner. Before and after the intervention, the paralytic tibialis anterior muscle activity, timed up and go test (TUG), and 10m walking test (10 MWT) were performed. RESULTS: Both groups showed significant improvement in the post-intervention muscle activity of the paralytic tibialis anterior, TUG, and 10MWT compared to that before the intervention (p<.05), but the differences between the two groups were not significant (p >.05). CONCLUSION: Bilateral ankle dorsiflexors strengthening exercise is an effective cross-training method to improve the muscle activity of the paraplegic tibialis anterior, balance ability, and walking function in chronic stroke patients.

Correlation of Curved Walking Ability with Straight Walking Ability and Motor Function in Patients with Hemiplegia

  • Purpose: In real life there are both straight-paths and curved-paths. To evaluate walking ability of both kinds, a figure-8 walking test (F8WT) was developed. The aim of this study was to validate the measure in hemiplegic patients with walking difficulties and to identify correlations of curved walking ability with straight walking ability, motor function, and walking performance ability. Methods: Twenty subjects participated in this study. Curved walking was measured by a F8WT. Straight walking ability was measured by a 10-meter walking test (10MWT). Dynamic balancea bility was measured by timed up and go (TUG) tests. Walking performance ability was measured using a modified motor assessment scale (MMAS). Motor function was measured by the Fugl-Meyer assessment (FMA) scale. Data were analyzed using Pearson correlation analysis. Linear regression analyses were performed to explore other functional tests in mobility ability by F8WT time, 10MWT (dependent variable). Results: There was a significant positive correlation of F8WT time with 10MWT and TUG. There was a significant negative correlation of F8WT time with MMAS and FMA-coordination. There was a significant positive correlation of 10MWT with TUG. There was a significant negative correlation of 10MWT with MMAS and FMA-coordination. The F8WT time for curved walking ability was attributed to 10MWT for straight walking ability as 94% level of contribution. Conclusion: The results suggest that the F8WT is a good instrument for measuring walking ability because there is a robust correlation of F8WT time with 10MWT, TUG, MMAS, and FMA-coordination in hemiplegic patients who, after stroke, have a mobility deficiency.

리듬청각자극을 동반한 기능적 전기자극 보행훈련이 뇌졸중 환자의 균형과 보행능력에 미치는 영향 (Effects of Functional Electrical Stimulation Gait Training with Rhythmic Auditory Stimulation on Balance and Gait Ability of Stroke Patients)

  • PURPOSE: The present study was conducted to investigate the effects of functional electrical stimulation gait training with rhythmic auditory stimulation on balance and gait ability in stroke patients. METHODS: In this blinded randomized controlled study, 26 stroke patients were assigned to either experimental group (n=13) consisting of 30 min of gait training 5 days per week for 4 weeks while performing functional electrical stimulation gait training with rhythmic auditory simulation, or a control group (n=13) performing the same gait training program, also consisting of 30 minutes 5 days a week and lasting for 4 weeks, but without functional electrical stimulation and rhythmic auditory stimulation. At baseline and after the 4 week intervention, balance was measured using the timed up and go test (TUG). Gait velocity was measured using the 10-meter walk test (10 MWT) and gait ability was assessed using the functional gait assessment (FGA). RESULTS: After the intervention, the experimental group showed statistically significant differences in gait velocity and ability (10 MWT, FGA) (p<.05). Between-group differences were statistically significant in gait velocity and ability (10 MWT, FGA) (p<.05). CONCLUSION: The findings suggest that functional electrical stimulation gait training with a rhythmic auditory stimulation gait training program may help improve gait ability in stroke patients.

만성 뇌졸중 환자에게 측방 보행 훈련이 균형과 보행에 미치는 영향 (Effects on Balance and Gait for Chronic Stroke Patients with Side Walking Training)

  • Purpose : The purpose of this study is to investigate the impact on the ability to walk, balance after side walking training of hemiplegic patients caused by stroke. Method : The subjects were training before stroke onset whether more than one year elapsed 15 patients with chronic stroke patients, and Berg balance scale(BBS) and Timed up and go test(TUG), Functional reaching test(FRT), 20m walking time 200m walking time were measured and recorded. Training period, a total of three weeks, and training frequency circuit training times 10 minutes per training, 5-minute break, the 10-minute training total 25-minute training was conducted. Gait line of 3m to be based on the patient's side walking, and the risk of falling compared to the presence of the experimenter trained under was carried out. Result : 1. TUG, 2. 20m walking time, 3. 200m walking time 4. FRT, 5. All showed significant improvement in BBS. Judging from the results, the side walking training conducted three weeks due to chronic stroke hemiplegic patient's ability to balance and showed a positive effect on the improvement of walking ability. Conclusion : Accordingly, it was more effective to train hemiplegic patients with chronic stroke on side walking.

뇌졸중 환자의 균형 증진을 위한 체감형 전자게임 훈련에 적용되는 되먹임 방식에 따른 효과 (The Effect of Feedback on Somesthetic Video Game Training for Improving Balance of Stroke Patients)

  • PURPOSE: The purpose of this study is to assess the difference in the effect of provision of feedback on knowledge of performance and knowledge of result in the training using somesthetic video game aimed at enhancement of balance of hemiparalysis patients due to stroke. METHODS: 20 stroke patients participated in the study. The participants were randomly divided into 2 groups, namely, the knowledge of performance feedback group (KP group, n=10) and the knowledge of result feedback group (KR group, n=10). Both groups received somesthetic video game training 5 times (30 minutes each) a week for total of 4 weeks. The KP group received feedback on the patterns of movement in execution of somesthetic video game. The KR group received feedback on the scores acquired following execution of somesthetic video game. Verification of the significance of the data was performed through paired t-test and independent t-test. RESULTS: Both groups displayed significant reduction in the movement of center of pressure (COP) and Timed up and Go (TUG), and significant increase in the Berg Balance Scale (BBS) following the training. Although the movement of COP was reduced for the KP group in comparison to the KR group, it was not statistically significant, and there was significant reduction in TUG and significant increase in BBS. CONCLUSION: The above results illustrate that provision of feedback on knowledge of performance is more effective than feedback on knowledge of result in somesthetic video game training for the purpose of enhancement of balance in stroke patients. Therefore, provision of feedback on knowledge of performance is necessary in somesthetic video game training for stroke patients.

고유수용성 감각 증진 운동이 뇌졸중 환자의 균형 능력에 미치는 영향 (The effects of proprioceptive exercises on balance ability after stroke)

  • Purpose : The purpose of this study was to investigate the effects of proprioceptive exercise (PE) using a trampoline and a balance board on a balance ability after stroke. Method : Sixteen chronic stroke patients participated. Participants were randomly assigned to the PE group or control group (8 experimental, 8 control). All of participants were in-patients at local rehabilitation centre and had been receiving a traditional rehabilitation program, five days a week. The PE group have additionally undergone for four weeks, three days a week, the PE using a trampoline and a balance board under supervision by a physical therapist but control group was not received any additional program except the traditional rehabilitation program. The position sense test used to assess a proprioceptive sense at a knee joint. The Berg Balance Scale (BBS) and the Timed Up & Go (TUG) test to measure the balance ability were carried out before and after the training. Result : After the training the error of position sense at knee joint of PE group significantly decreased compared to the control group. The PE group demonstrated a significant improvement in the scores of the BBS and TUG. Conclusion : The present study suggests that the PE program using a trampoline and balance board may become a useful tool for enhancing a balance ability in chronic stroke patients through the ennced proprioceptive position senses.

지팡이 손잡이 형태가 편마비 환자의 균형 회복에 미치는 영향 (The Effects The Type of Canes-Handle Affects in Recovering-Balance of Hemiplegic patients)

  • Purpose: We compared T-type and I-type canes on postural balance in 28 hemiplegic patients. Methods: Subjects were allocated randomly into two groups: a T-shape cane group (n=14) and an I-shape cane group (n=14). Before the test, subjects were trained by a physical therapist to walk with a cane for 6 weeks. The Main Outcome Measures were measured as maximal sway velocity, sway path, sway area, and partial weight bearing using a Balance Performance Monitor (BPM) and ambulation velocity using a 'Timed up and go test'. We also measured the maximal ambulation velocity. Results: The distribution of weight bearing on the affected side without the cane was 35% in the I-shape cane group and 36% in the T-shape cane group. After training, weight bearing on the affected side increased by 45% in the I-shape cane group and 40% in the T-shape cane group. With the cane held in the hand, weight bearing on the affected side in the T-shape cane group decreased by 3%. Conclusion: The I-shaped cane increased static standing balance, including hemiplegic side weight bearing. Therefore, I-shape canes can improve the balance of hemiplegic patients.

점진적 앉은 자세에서의 PNF운동이 소뇌 손상환자의 동적 균형능력에 미치는 효과 -단일 사례 연구- (Effects of Using PNF in Progressive Sitting Position on Dynamic Balance in a Patient with a Cerebellar Injury -Single Subject Design-)

  • Purpose: The purpose of this study was to investigate the effects of using proprioceptive neuromuscular facilitation (PNF) exercise in the progressive sitting position on the dynamic balance ability of a patient with a cerebellar injury. Methods: The subject had ataxia due to cerebellar injury. The subject participated in a PNF bilateral scapular pattern exercise with stabilizing reversal technique during a progressive sitting position session as well as baseline for 20 minutes a day for 4 weeks. In the first session, PNF exercises were performed at a height of 40 cm for 10 minutes, and in the second session they were performed at 50 cm for 10 minutes from a lower center of gravity (COG) to a higher COG sitting position. We used the Berg Balance Scale (BBS), Five-Times-Sit-to-Stand Test (FTSST), and the Timed Up and Go Test (TUGT) to measure the subject's dynamic balance ability every two days through the entire session. Results: After participating in the program, the subject's dynamic balance ability improved compared to the first baseline, as measured by BBS (2 points increased), FTSST (5.3 sec decreased), and TUGT (2 sec decreased). The increase was also maintained in the second baseline session. Conclusion: PNF exercise using bilateral scapula patterns with a stabilizing reversal technique helps to enhance the dynamic balance ability of a cerebellar injury patient.

테라테인먼트 테이핑과 발바닥 운동이 대학생의 평발 개선에 미치는 영향 (Effects of Teratainment Taping and Footpad Exercise on the Improvement of Flat Foot in a University Student)

  • 본 연구는 테라테인먼트 테이핑과 발바닥 운동이 대학생의 평발 개선에 미치는 영향을 알아보았다. 발배뼈 하강 검사를 통해 유연성 평발 테이핑군(13명), 유연성 평발 발바닥 운동군(14명), 강직성평발 테이핑군(12명), 강직성 평발 발바닥 운동군(13명)으로 나누었다. 테라테인먼트 테이핑과 발바닥 운동을 1시간 적용 후, 발배뼈의 높이 변화, 균형 능력, 자세 안정성을 평가하였다. 발배뼈 높이 변화는 중재 전-후의 높이를 측정하고, 균형 능력은 timed up & go test를 측정하였으며, 자세 안정성 평가는 어깨넓이 두발지지, 두발모음 자세, 발뒤꿈치-발끝서기 자세에서 안정성 한계를 평가하였다. 본 연구의 연구결과, 발배뼈 높이 변화에서 유연성 평발 테이핑군의 왼발과 오른발, 유연성 평발 운동군의 오른발, 강직성 평발 태이핑군의 오른발에서 중재 전-후 비교에서 통계학적으로 유의한 차이가 있었으며(p<0.5), 균형 능력 평가에서는 군간과 군내의 중재 전-후 비교에서 통계학적으로 유의한 차이는 없었다. 자세 안정성 평가에서는 유연성 평발 운동군과 강직성 평발 테이핑군의 중재 전-후 비교에서 발뒤꿈치- 발끝서기 자세에서 통계학적으로 유의한 차이가 있었으며(p<0.5), 중재 후 비교에서 유연성 평발 운동군이 강직성 평발 운동군에 비해 발뒤꿈치-발끝서기 자세에서 통계학적으로 유의한 차이가 있었다(p<0.5). 본 연구결과를 통해 테라테인먼트 테이핑과 발바닥 운동이 대학생의 평발 개선에 유의한 영향을 주는 것을 알 수 있었다. 따라서 평발의 개선을 위해 테라테인먼트 테이핑과 발바닥 운동의 적용을 권장한다.

비디오 게임을 이용한 가상현실 운동 프로그램이 노인의 근력, 균형 및 보행에 미치는 영향 (The Effect of a Virtual Reality-based Exercise Program Using a Video Game on the Muscle Strength, Balance and Gait Abilities in the Elderly)

  • 노화로 인한 근력, 균형 및 보행능력의 감소가 노인의 신체활동 저하를 일으킨다. 가상현실운동은 노인의 기능적 균형과 움직임을 회복하는데 사용되고 있다. 본 연구의 목적은 비디오게임을 이용한 가상현실운동프로그램(VR)이 노인의 근력, 균형 및 보행능력에 미치는 영향을 알아보는 것이다. 48명의 노인을 대상으로 VR군(남자 11명, 여자 14명, 나이 68.4세)과 대조군(남자 10명, 여자 13명, 나이 67.6세)으로 무작위 추출하였다. VR군은 가상현실 운동을 8주 동안 주 2회 실시하였으며 대조군은 특별한 운동을 실시하지 않았다. 가상현실운동프로그램은 준비운동(10분), 가상현실프로그램(40분), 정리운동(10분)으로 구성되었으며 가상현실프로그램은 상하지의 움직임뿐만 아니라 시청각피드백을 제공하는 플레이스테이션 아이토이플레이로 수행되었다. 운동 전·후의 변화를 비교하기 위하여 하지의 근력, 힘판을 이용한 정적균형능력, 기능적 팔 뻗기와 일어나 걸어가기 검사로 구성된 동적균형능력과 10m 보행검사와 6분 보행검사로 이루어진 보행능력을 사전, 사후로 측정하였다. VR군은 하지 근력과 정적균형능력, 동적균형능력, 보행능력에서 모두 유의하게 향상되었다(p<.05). 결론적으로 가상현실운동프로그램이 노인의 근력, 균형 및 보행능력의 개선에 효과가 있는 것으로 나타났다.
