재팽창성 폐부종 (Reexpansion Pulmonary Edema)

  • Reexpansion pulmonary edema following pneumothorax, atelectasis, massive pleural effusion are clinically uncommon, but sometimes life threatening progression. Reexpansion pulmonary edema is usually ipsilateral but rarely contralateral or both. Reexpansion pulmonary edema was occurred when chronically collapsed lung is rapidly reexpanded by evacuation of large amounts of air or fluid. The pathogenesis of the reexpansion pulmonary edema is unknown but is probably mutifactorial. The etiological factors of the reexpansion pulmonary edema are chronicity of the lung collapse, technique of the reexpansion, airway obstruction, loss of the surfactant, and pulmonary artery pressure changes. In the treatment of the chronically collapsed lung, physician must be remembered the possible events, and to prevent of the complication.

  • The knowledge of motility disorders of the gastrointestinal tract has increased over the past decades. The development of newer therapies for bowel motility disorders has been disappointingly slow. Prokinetic agents are medications that enhance coordinated gastrointestinal motility and transit of material in the gastrointestinal tract. These agents are pharmacologically and chemically diverse. However, life-threatening adverse effects of prokinetic agents such as cisapride was present. In this review, pharmacologic effects and use of prokinetic agents in children was introduced.

Hepatic Hemangioma Rupture Caused by Blunt Trauma

  • Hepatic hemangioma is the most frequently occurring benign tumor of the liver. Hepatic hemangioma rupture is a rare phenomenon, which can lead to life-threatening conditions. Here, we report a case of hepatic hemangioma rupture caused by blunt trauma. Explorative laparotomy was performed due to unstable vital signs and abdominal massive hemoperitoneum revealed on computed tomography. We detected arterial bleeding from a hepatic hemangioma and performed primary suture of the liver and postoperative angiographic embolization.

Fatal Rhabdomyolysis in a Patient with Head Injury

  • Rhabdomyolysis is a rare but potentially life-threatening disorder caused by the release of injured skeletal muscle components into the circulation. The authors report a case of severe head injury, in which a hyperosmolar state and continuous seizure complicated by severe rhabdomyolysis and acute renal failure evolved during the course of treatment resulted in a fatal outcome despite intensive supportive treatment. Our bitter experience suggests that rhabdomyolysis should be born in mind in patients with severe head injury who may develop hyperosmolar state and continuous seizure.

  • Cryptosporidium parvum causes a life-threatening diarrhea in acquired immunodeficiency syndrome (AIDS) patients. THe sporozoite stage of C. parvum has been known to be a target in treating cryptosporidiosis in AIDS patients as it is an extracellular stage. A sporozoite was ultrastructurally observed. It has a creascent shape with a rounded posterior end and a tapering body. The compact nucleus was located at the posterior end. A monoclonal antibody was produced, which recognized a 43 kDa of sporozoite antigens in a western blot analysis and showed the surface labeling in immunofluorescence.

  • Acute renal failure is the generic term for an abrupt and sustained decrease in renal function resulting in retention of nitrogenous and non nitrogenous waste product. This may results in life threatening consequences including volume overload, hyperkalemia, and metabolic acidosis. Acute renal failure is both common and carries high mortality rate, but as it is often preventable, identification of patients at risk and and appropriate management are crucial. This review summarized the most recent information on definition, epidemiology, clinical causes and management of acute renal failure in pediatric patients.

  • Strongyloidiasis has been rdcognized as one of the life-threatening parasitic infections in the immunocompromised patients. We report an intestinal infection case of Strongyloides stercoralis in a 61-year-old man. Rhabditiform larvae were detected in the stool examination and developed to filariform larvae having a notched tail through the Harada-Mori filter paper culture.The patient received five courses of albendazole therapy but not cured of strongyloidiasis.

  • Life-threatening cardiac arrhythmia is a frequent complication of open heart surgery. There are many causes of postoperative cardiac arrhythmias. Electrolyte imbalance such as hypokalemia and acidemia are major causes of ventricular arrhythmias. Infrequently, however, antiarrhythmic agents and /or hypomagnesemia induce[s] a ventricular arrhythmia such as "torsade de pointes" by increasing the repolarization time of myocardium, Recently, we have experienced two cases of "Torsade de pointes" associated with hyp-omagnesemia after replacement of mitral valve and one of whom after use of procainamide. Intravenous infusion of magnesium immediately and successfully abolished the torsade de pointes in both cases.intes in both cases.

재팽창 폐부종 치험 2례 보고 (Re-expansion Pulmonary Edema -2 Cases Report-)

  • Re-expansion pulmonary edema following pneumothorax or hemothorax is clinically uncommon but occasionally life threatening. Clinical details are given of two patients.Ipsilateral pulmonary edema were developed after chest tube insertion due to spontaneous pneumothorax in case I and after evacuation of postoperative hemothorax in case II. The patients were treated with frequent bronchial toilet.The administration of colloid solution and diuretics was effective. The possible mechanisms underlying the edema are discussed.Both increased time of collapse and suction tended to correlate with reexpansion edema.The present two cases provided evidence for longstanding lung collapse and immediate application of suction.
