We experienced a case of leiomyoma on proximal thoracic esophagus. The patient, a 42-year-old female, noted the insidious onset of dysphagia and chest discomfort for 6 months. Esophagogram showed a smooth filling defect in upper third of thoracic esophagus, T1-T4 level and esophagoscopy revealed a firm mobile mass about 6cm in length with normal overlying mucosa. The lesion was approached the Rt. posterolateral thoracotomy through 4th ICS and enucleated of tumor without difficulty. The patient`s hospital course was uncomplicated.

The five patients with leiomyoma of esophagus were treated from 1976, to 1990. The patients were 4 men and 1 woman whose ages ranged from 28 to 53 years. One of them was asymptomatic, two had mainly dysphagia and the others complained indigestion and epigastric discomfort. There was no relationship between the severity of symptoms and the size of tumor. The preoperative diagnosis was made by esophagoscopy and esophagogram, and all of them were treated by thoracotomy and enucleation. There was no postoperative complication and the results were excellent.

Traumatic rupture of the main bronchus is comparatively very rare. With the advent of widespread mechanization and high-speed era, the incidence of traumatic rupture of the tracheobronchial tree has been increased considerably. Rupture of the bronchus is an unusual result of nonpenetrating trauma to the chest. Early recognition of bronchial rupture and emergency thoracotomy and management is essential for reducing of morbidity, mortality and late complications. We experienced 3 cases of bronchial rupture caused by nonpenetrating chest trauma with or without rib fracture. Patients were suffered from dyspnea and chest pain. After closed thoracostomy, corrective surgery was performed. Postoperative courses were uneventful and discharged without any complication.

We reported a successful right pneumonectomy using video assisted thoracoscopy for endobronchial tuberculosis. Thoracoscopic surgery has many advantages in spite of its technical difficulties. These advantages include good cosmetic effects, short hospital stay, decreased postoperative morbidity, and good postoperative pulmonary function.The patient was young unmarried female, and her right lung was nearly total obstructed by endobronchial tuberculous dissemination and secondary pulmonary infection. The procedure was a little complicated by inadequate placements of the thoracoscopic ports and 5cm length utility thoracotomy incision. Minimal pain killer was needed only two days after operation and scar of operative wound was very satisfying. Postoperative course was unusual and the patient was discharged 7 days after operation.

Though leiomyoma is the most common benign esophageal tumor, it is rare in the upper third of esophagus. Recently, we experienced two cases of esophageal leiomyoma in the upper third of esophagus. The patient were a 53 year old female and a 40 year old male. On esophagoscopy, masses were found at 20\ulcorner23 cm, 22\ulcorner26 cm from upper incisor with normal overlying mucosa. Enucleation was done through a right posterolateral thoracotomy without complications.

Dumbbell neurogenic tumor is a rare disease, arising about 10 % of mediastinal neurogenic tumor. We report a 31 year old man who was suffered from paresthesia and weakness of lower extremities for about 8 months. Chest x-rays revealed a dumbbell shaped mass shadow in left lower posterior mediastinum. MRI demonstrated a well defined dumbbell shaped mass in the mediastinum of T7 level, including spinal cord compression by the extended tumor into the spinal canal and enlarged intervertebral foramen. The patient underwent one-stage combined resection of the tumor through the thoracotomy and laminectomy, simultaneously. The tumor was confirmed as neurilemmoma. The postoperative course was smooth and uneventful.

The strictured esophagus is not removed usually in corrosive injury. But corrosion carcinoma and the late complications such as esophago-bronchial fistula, chronic mediastinal abscess, diverticulum can occur in case that strictured & inflamed esophagus be remained. Recently transhiatal esophagectomy is being done with low mortality and morbidity. So we are reporting 2 cases of esophageal stricture in young patients which were treated with transhiatal esophagectomy and esophagocologastrostomy. All of 2 cases were successfully treated and recovered. Postoperative esophageal function tests showed the mild reflux but it did not count clinically.

This is a case report of superior vena cava syndrome due to thyroglossal duct cyst, which was surgically treated. The patient was 61 year old male who progressively complained exertional dyspnea for about 5 months and right ptosis, facial flushing, and nasal stuffiness for about 1 month before admission. The CT scan of the thorax revealed the right paratracheal cystic mass that compressed and displaced the trachea to leftward and SVC to rightward. The resection of the cystic mass was performed through the right posterolateral thoracotomy. The pathologic result was compatible with thyroglossal duct cyst. The postoperative status of the patient was uneventful.

Embryonal carcinoma of the mediastinum is a very uncommon disease. This is a report of an embryonal carcinoma in the mediastinum found in a 25 years old Korean male patient who had been suffering from chest pain and intractable coughing for 6 months. 5 weeks prior to this admission hemoptysis and high fever were followed. Right exploratory thoracotomy was performed under the impression of a mediastinal tumor, but found to be unresectable. Irradiation therapy was tried, but no response was observed. Patient expired on 78th day postoperatively.

Postraumatic lung hernia is a rare occurrence. A number of cases reported in the literature have been treated with early thoracotomy to repair partial protruded lung and pleura to prevent strangulation and incarceration. We present a case of a 45-year-old patient of left posttrumatic lung hernia, in which closed digital reduction was successful. The strategy of the management approach could be established by further accumulated experience.
