  • Atlantoaxial rotatory fixation (AARF) in adult is a rare disorder that occurs followed by a trauma. The patients were presented with painful torticollis and a typical 'cock robin' position of the head. The clinical diagnosis is generally difficult and often made in the late stage. In some cases, an irreducible or chronic fixation develops. We reported a case of AARF in adult patient which was treated by immobilization with conservative treatment. A 25-year-old female was presented with a posterior neck pain and limitation of motion of cervical spine after a traffic accident. She had no neurological deficit but suffered from severe defect on the scalp and multiple thoracic compression fractures. Plain radiographs demonstrated torticollis, lateral shift of odontoid process to one side and widening of one side of C1-C2 joint space. Immobilization with a Holter traction were performed and analgesics and muscle relaxants were given. Posterior neck pain and limitation of the cervical spine' motion were resolved. Plain cervical radiographs taken at one month after the injury showed that torticollis disappeared and the dens were in the midline position. The authors reported a case of type I post-traumatic AARF that was successfully treated by immobilization alone.

  • Primary hemangioperiycytoma is quite rare tumor of the lung and arising from pericyte in external layer of capillaries. Most cases are developed in 4th and 5th decade, are asymptomatic, and have malignant otential. On chest radiography, primary pulmonary hemangiopericytoma shows lobulated, well demarcated, homogeneous soft tissue density. Microscopically, it consisits of numerous vascular spaces of variable size and shape separated by aggregates of tightly packed oval to spindle-shaped cells. Treatment of choice is surgical excision. We report a case of primary pulmonary hemangiopericytoma in a 16-year-old man who had well demarcated homogeneous mass in the superior segment of left lower lobe, but had no symptom. He had undergone left lower lobectomy. He has been followed up for 8 months but has no sign of relapse or metastasis yet.

  • Computed tomography with virtual bronchoscopy shows inner shape of trachea and major bronchi with three-dimensional images. Virtual bronchoscopy can be used in cases of suspected foreign body aspiration. Virtual bronchoscopy shows the exact location of the obstructive lesion before conventional bronchoscopy. If the chest radiography is normal or suggests aspirated foreign body, virtual bronchoscopy can be considered before conventional bronchoscopy. We experienced a case of false-tooth foreign body extracted under bronchoscopy. In this case, virtual bronchoscopy was made retrospectively giving useful information about shape and orientation regarding foreign body.

Feline Diffuse Iris Melanoma in a Cat

  • A 4-year-old spayed female British shorthair cat was referred for abnormal pigmentation on the right iris. The pigmentation was mainly located in the medial portion of the iris. No abnormalities except iris were detected in a full ophthalmic examination. There was no evidence of metastasis after thoracic radiography and abdominal sonography. Enucleation was performed on the right eye and it was sent for a histopathological evaluation. It was confirmed as early stage of feline diffuse iris melanoma (FDIM) with involvement of iris stroma.

A Case Report: Diagnostic Imaging of a Chondrosarcoma of the Rib in a Dog (개에서 늑골 연골육종에 대한 영상학적 진단증례)

  • An 11-year-old female, Golden retriever dog with a history of solid mass on the chest wall was referred to Veterinary Medical Teaching Hospital, Seoul National University. The mass was firm on palpation. A soft tissue opacity mass with calcified foci around the right 7th rib and extrapleural patterns around the right 5th rib and 7th rib was shown on thoracic radiographs. The mass of the 7th rib has a mixed-echo pattern with a strong acoustic shadowing and internal vascular signals on ultrasonography. On CT scan, the mass showed contrast enhancement effect, expansive pattern of intrathoracic and extrathoracic legion around costochondral junction. The 7th rib appeared mildly lytic. The mass of the 5th rib had a soft tissue swelling without bone lysis. The mass was diagnosed as a sarcoma by fine needle biopsy. Therefore, the mass was surgically removed. Subsequent histopathological study found the mass was chondrosarcoma.

A Case of B Cell Lymphoma Presenting with Multiple Osteolysis in a Juvenile Golden Retriever (다발성 골 융해를 동반한 림프종 증례)

  • A 1.5-year-old male Golden Retriever was presented with worsening lameness of two month duration. Abnomral findings of blood works and serum chemistry included anemia, thrombocytopenia, hypercalcemia and hyperglobulinemia. Radiography revealed osteolysis of polyostotic regions including right femur and tibia, bilateral ilium, and spinous processes from the 13th thoracic vertebra to 5th lumbar vertebra. Enlarged multiple lymph nodes and mixed echo pattern of muscular region ventral to vertebra were observed with ultrasonography. Because concentrations of both parathyroid hormone and parathyroid hormone related peptide were all within reference ranges, humoral hypercalcemia by tumor was ruled out and extensive osteolysis was considered as the cause of hypercalcemia. Based on radiographic and ultrasonographic study, lymphoma, multiple myeloma and osteomyelitis were included in differential diagnosis. Fungal serologic test was negative. Monoclonal gammopathy was not found on serum protein electrophoresis. Cytological and histopathological examinations of the lytic lesions revealed neoplastic lymphoid proliferation, and B cell type clonal expansion was detected by polymerase chain reaction for the antigen receptor gene rearrangement. The case was diagnosed as B cell lymphoma involving polyostotic regions.

Diagnosis of an Asymptomatic Iridociliary Tumor using Ultrasound Biomicroscopy in a Dog (초음파 생체 현미경을 이용한 증상이 없는 홍채모양체 종양의 진단 1 증례)

  • A 10-year-9-month-old spayed female Shih Tzu was presented with ocular discharge and corneal opacity to Veterinary Medical Teaching Hospital of Seoul National University. Complete ophthalmic examination revealed bilateral keratoconjunctivitis sicca accompanied by the severe corneal neovascularization and edema in the right eye (OD). Ultrasound biomicroscopy incidentally showed an iridociliary mass located at 6~7 o'clock in the OD. No evidence of metastasis was observed on thoracic and abdominal radiography. Enucleation was selected and performed on the OD, considering the risk of metastasis or recurrence. Non-invasive and pigmented iridociliary adenoma was confirmed on histopathological evaluation.

Evaluation of ST2 and NT-proBNP as cardiac biomarkers in dogs with heartworm disease

  • This study compared serum concentrations of suppression of tumorigenicity 2 (ST2) and N-terminal pro-B-type natriuretic peptide (NT-proBNP) between healthy and heartworm- infected dogs. Eighteen heartworm-infected dogs and five healthy dogs were included in the study. Dogs were diagnosed and categorized by history, clinical signs, and blood assay, thoracic radiography, echocardiography, and commercial ELISA kit results. Serum samples were sent to the IDEXX reference laboratory for NT-proBNP measurement. ST2 was examined by using a canine interleukin 33 receptor ELISA kit with the quantitative sandwich ELISA method. The severely infected group showed significant elevation of NT-proBNP concentration over those of the control (P=0.03) and mildly infected (P=0.04) group. There were no significant difference in ST2 concentrations among the three groups. The usefulness of NT-proBNP as a cardiac biomarker in dogs with severe heartworm disease was confirmed by the results of this study. Further investigations to assess ST2 as a cardiac biomarker are warranted.

Mediastinal Cavernous Hemangioma (상대정맥에 발생한 해면상 혈관종 -1례 보고-)

  • Mediastinal hemangioma is a rare tumor and only few cases have been reported in the literatures. We have experienced one case of cavernous hemangioma occuring at the superior vena cana. The patient was a thrity-five year old female with no specific symtoms except palpable cystic mass in the right cervical area. A routine chest radiography showed an upper mediastinal mass. Computed Tomography showed about 4$\times$5 cm sized cystic mass communicating to the superior vena cava 2 cm above of the veno-atrial junction. After the evaluation, surgical excision was performed and the pathologic diagnosis was confirmed to cavernous hemangioma. Postoperative course was uneventful and the patient has been followed up without any problems.

Intratracheal Stenting Under the Aid of Transcutaneous Cardiac Pacing in a Dog with Severe Bradycardic Conduction Defect (중증 서맥성 전도장애 개에서 피부경유 심박 조절 하에 기관내 스텐트 장착)

  • A 10-year-old intact female Yorkshire terrier dog (weighing 3 kg) was referred with the primary complaint of severe dyspnea, cyanosis and occasional syncopal episode. Physical examination revealed irregularly irregular bradycardia with persistent pulse deficits. The 12-lead surface ECG showed high grade atrioventricular blocks. Thoracic radiography revealed severe intrathoracic collapse. The dog was successfully treated with an intraluminal self-expanding stent ($COOK^{(R)}$) under the aid of transcutaneous cardiac pacing.
