Background: Thoracic outlet syndrome(TOS) is caused by the compression of neurovascular structures that supply to the upper extremities. Only a few reports have been published in Korea, and this study attempts to investigate the clinical aspects and results of the patients who underwent surgical treatment. Material and Method: This study consist of 16 patients who underwent operations for thoracic outlet syndrome from May, 2002 to October, 2004. The surgical indications were confined to patients with: 1) symptom too severe to perform ordinary daily life because of pain, paresthesia, edema of upper extremities, 2) no improvement after proper physical therapy, 3) definite finding of compression confined by radiologic examinations (MRI, angiography, etc), and 4) no other diseases such as cervical intervertebral herniation, myositis, neurologic diseases below the brachial plexus. The surgical approaches were by transaxillary approaches in 12 cases, supraclavicular approaches in 2 cases, and infraciavicular approaches in 2 cases. Result: There were 15 males and one female with an average age of 23.9 years (range:19$\∼$39). Rib anomalies were observed in four cases (25.0$\%$), but the others had no abnormal ribs. Right lesions were found in eight cases (50.0$\%$), left lesions in five cases (31.3$\%$), and bilateral lesions in three cases (18.7$\%$). The follow-up period was 9$\∼$26 months and recurrence rate was 12.5$\%$ (2/16). Complications were one case of ulnar nerve palsy, one case of persistent pain despite radiologic improvement and three cases of wound dehiscence due to fat necrosis and hematoma. Conclusion: Although the choice of treatment in patients with TOS has been disputed, patients who have no response with proper physical therapies can benefit from the surgical treatment which may help patients to return to normal daily activity in shorter period of time.

A 21 year old male patient was admitted for pain, edema and a tingling sensation in his right arm. He underwent venography and an MRI scan, and he was diagnosed with Paget-Schroetter syndrome. Thoracoscopic first rib re-section was performed after failure of thrombolytic therapy, The symptoms were relieved after the operation and he was discharged with warfarin medication. First rib resection is generally performed through the standard transaxillary or supraclavicular route. We report here on a successful thoracoscopic first rib resection.
Objectives : We investigated the possibility of cervical spine misalignment caused by scalenus anticus syndrome to find out how it affects cervical spine misalignments. Methods : 28 patients with scalenus anticus syndrome (sample group), along with 21 participants without neck pain (control group), who attended the Dept. of Oriental Rehabilitation Medicine, College of Oriental Medicine, Dong-guk University from the 20th of June to the 30th of November, 2006 were investigated. After researching misalignments through neutrality, flexion and extension lateral x-ray examination views, we measured the difference of each length of cervical spine misalignment. We analyzed the relationships among the neutrality lateral, flexion lateral and extension lateral positions. Results : We found with statistical significance that there were differences in length of cervical spine misalignments between the sample and control groups. Furthermore, we found that C3 and C4 vertebra bodies were shown in lateral neutrality position, only C3 vertebra body in flexion lateral position, and C2, C3, and C4 vertebra bodies in extension lateral position. Conclusions : It is considered that scalenus anticus syndrome could increase cervical spine misalignment which could be a factor in causing cervical spine disease.
Thirteen patients with cyanotic cardiac malformations having more complex intracardiac defects, hemodynamics and operative procedures than ones in Tetralogy of Fallot undertaken total surgical corrections from July 1981 to August 1985. The cases of corrective surgery for complex cardiac malformations were 3.9% of all congenital cardiac malformations and 12.6% of cyanotic cardiac malformations. Six patients died within 30 days after surgery. So operative mortality was 46%; Transposition of the great arteries, two of 4 patients, due to low cardiac output syndrome and tracheal bleeding ; Univentricular heart, one of 3 patients, due to bleeding; Corrected transposition of the great arteries, one of 2 patients, due to acute heart failure; Tricuspid atresia, one of 2 patients, due to low cardiac output syndrome; Double outlet right ventricle, one of single patient, due to respiratory failure. The cases of surgical correction for complex cardiac malformations are progressively increasing in numbers. The more accurate evaluation of anatomical condition and hemodynamics in preoperative diagnosis, studies on applicable surgical procedure and perioperative care of patients are necessary in the improvement of clinical and surgical results.
Twenty-seven patients with double-outlet right ventricle underwent complete intracardiac repair between 1978 and 1983, June, at Seoul National University Hospital . Although definite aorto-mitral discontinuity was discovered in 20 patients, both great arteries arose wholly or mostly from the right ventricle in all cases. There were 17 cases with subaortic VSD, 6 with subpulmonic, 2 with doubly-committed, and 2 with non-committed VSD. Pulmonary stenosis was present in 21 patients. Intraventricular baffle repair was applied in 23 patients. Three patients required extracardiac conduit to establish continuity between right ventricle and pulmonary artery, and modified Fontan operation was performed in one patient. Over-all mortality rate was 37.0%, but recently 4 of 15 died [26.7%]. One late death occurred from infective endocarditis. Incremental risk factors were small patient size, subpulmonic or non-committed VSD, presence of PS, coronary artery anomalies, associated valvular lesion and other complicated anomalies. However, great artery relationship, restrictive VSD and transannular patch were not risk factors. No instances of complete heart block occurred. Of the survivors, all showed complete or in complete right bundle branch block and in one patient intermittent ventricular tachycardia had developed. The important cause of death was low cardiac output syndrome.
Thirty-two patients with a cyanotic cardiac malformations having more complex intracardiac defects than ones in a tetralogy of Fallot underwent complete intracardiac repairs in a-full-year period from July 1981 to June 1982. Twenty-two patients [68.8%] died within 30 days after surgery: Transposition of the great arteries, seven of 10 patients; Double-outlet right ventricle, four of 6 patients; Tricuspid atresia, four of 6 patients; Single ventricle, all of 4 patients; Pulmonary atresia, two of 3 patients; Double-outlet left ventricle, none of 2 patients; and Truncus arteriosus, one of a single patient. All deaths occurred with a low cardiac output syndrome or a failed off-bypass, and they were almost always accompanied with other grave postoperative complications. The complex intracardiac anatomy itself was one of the risk factors by making a complete intracardiac repair of the defects difficult in a small heart. The reconstruction of the right ventricular outflow carried a difficulty in balancing an adequate relief of the obstruction with an avoidance of making too much pulmonary valvular insufficiency as well. On the other hand, the presence of an elevated pulmonary arterial pressure and a high pulmonary vascular resistance was also the factors affecting the postoperative surviv als. The importance of detailed knowledge of intracardiac anatomy and hemodynamics from the careful preoperative evaluation of the patient was discussed along with the necessity of technical refinement of the correction.
We reviewed our experiences on 33 patients who underwent a bidirectional cavopulmonary shunt[BCPS from February 1992 to July 1994. There were 19 male an 14 female patients, and their weight ranged from 4.4 to 13.3 Kg[mean weight 8.4 $\pm$2.9 Kg . The age ranged from 2 to 55 months [mean age 16.7 $\pm$15.5 months . Their diagnosis included single ventricle group in 16, unbalanced ventricles in 8 whose associated anomalies were double outlet right ventricle, transposition of great arteries and total anomalous pulmonary venous return, tricuspid atresia in 7, hypoplastic left heart syndrome in 1 who underwent a Norwood procedure and double outlet right ventricle with pulmonic stenosis and tricuspid stenosis in 1 who underwent biventricular repair. Among them 10 patients had received other palliative operation before [Norwood procedure 1, pulmonary artery banding 3, modified Blalock-Taussig shunt 6 . The BCPS operations were performed under the cardiopulmonary bypass. 16 patients underwent unilateral BCPS and 17 patients who had bilateral SVC underwent bilateral BCPS. Three patients whose associated anomalies were interruption of IVC underwent total cavopulmonary shunt. There were 5 operative deaths [mortality rate 15.1 % and 2 late deaths. The risk factor for the operation was high mean pulmonary artery pressure [p value<0.05 . The survivors showed good postoperative course and their postoperative oxygen saturation was increased significantly compared to that of preoperative status[p value<0.05 .Conclusively, BCPS operation is effective and safe palliative procedure for the many cyanotic complex congenital anomalies with decreased pulmonary blood flow especialy for the patients who have the high risk factors for Fontan operations.

Eleven shoulders (9 patients) with refractory scapulothoracic dysfunction were treated with scapulothoracic arthrodesis between 2000 and 2006. Refractory shoulder dysfunction included facioscapulohumeral muscular dystrophy in five shoulders (3 patients), refractory scapular winging with long thoracic nerve palsy in one shoulder, scapular winging caused by serratus anterior palsy with trapezius dysfunction in one shoulder, post-surgical thoracic outlet syndrome due to medial clavicle resection in two shoulders, refractory scapular winging with spinal accessory nerve injury in one shoulder, and chronic trapezius rupture caused by cervical spine surgery in one shoulder. The mean active flexion was improved from 82 degrees preoperatively to 112 degrees postoperatively. The mean Constant score was improved from 27.2 points to 68.0 points. Two shoulders (1 patient) that had facioscapulohumeral muscular dystrophy had broken wires due to nonunion, and one patient had a reactive pulmonary effusion. In ten of the eleven shoulders, the patients were satisfied with their results. The scapulothoracic arthrodesis can cause significant pain relief and functional improvement in refractory scapulothoracic and/or shoulder dysfunction. By selecting patients that present with appropriate indications, and using experienced surgical technique through complete preoperative evaluation, we can diminish the complication rate and make good clinical outcomes.
Objectives: The objective of this study was to compare the effects of Sweet Bee Venom(Sweet BV) Therapy between the hand paresthesia patients with Osteoporosis and without Osteoporosis. Methods: This study was carried out to established the clinical criteria of hand parethesia. The patients who had past history of diabeics, neuropathy induced by alcohol or drug and was positive on Myofacial Pain Syndrome Theory were excluded. 32 patients who had hand paresthesia related with unknown-reason was selected by the interview process. And the effects of treatment were analyzed using VAS score before treatment, after treatment, after 1 month and after 3 months. Results and conclusion: After treatment, While Osteoporosis group decrease from $64.81{\pm}7.81$ to $27.21{\pm}7.32$, Non-Osteoporosis group decrease from $58.76{\pm}1.43$ to $24.74{\pm}3.81$ by VAS scores. and After 3 months, While Osteoporosis group increase from $27.21{\pm}7.32$ to $54.96{\pm}9.40$, Non Osteoporosis group increase from $24.74{\pm}3.81$ to $32.43{\pm}5.57$. Non-Osteoporosis group was accordingly more effective than Osteoporosis group after 3 months. So Sweet BV therapy for hand numbness patients without Osteoporosis was e effective than patients with Osteoporosis.
Objectives & Methods : I investigated 45 literature of Oriental and Western medicine about the treatment of pain in shoulder and arm. Result and Conclusion : 1. The etiological causes of Pain in Shoulder and Arm based on literatures of Oriental medicine are attack of wind-heat on the lung, wind cold, damp-heat struggle between the vital energy and pathogenic factor and six pathogenic factors. And all these causes are the conception of blockage syndrome, Qi and blood stagnating in meridian system. 2. The treatment of Pain in Shoulder and Arm based on Oriental medicine is mainly composed of both medical therapy for Bi syndrome due to pathogenic wind, deficiency of both Qi and blood, consumption of the liver and the spleen, and also acupuncture and moxibustion treatment by selection for acupoint. And those treatments are for treating etiology. And also there are treatments using the meridian system and Twelve Muscle Region and Ashihyeol for the purpose of treating the symptoms. 3. The etiological causes of Pain in Shoulder and Arm based on literatures of Western medicine are degenerative cut of tendon and nerve symptoms caused by tendonitis, bursitis, calcification, ruptured cervical disc and thoracic outlet syndrome. 4. The treatment of Pain in Shoulder and Arm based on Western medicine is for alleviation of pain, such as giving an anodyne, steroid products, local anesthetic injection and stretching and strengthening the muscles.
