Objectives : The objective of this study was to determine the relationship between past smoking and the risk factors for metabolic syndrome. Methods : From January 2007 to December 2007, a total of 3,916 over thirty years old male health screen examinees were divided into the nonsmoking, smoking, ex-smoking groups. The diagnosis of metabolic syndrome was based on the criteria of the NCEP ATP III(Executive Summary of The Third Report of The National Cholesterol Education Program). Metabolic syndrome was defined as the presence of three or more of the following: a blood pressure $\geq$ 130/85 mmHg, a fasting glucose level $\geq$ 110 mg/dL, a HDL-C (High Density Lipoprotein Cholesterol) level < 40 mg/dL, a triglyceride level $\geq$ 150 mg/dL and, a waist circumference men $\geq$ 102 cm, but a waist to hip ratio > 0.90 was used as a surrogate for the waist circumference. Results : After adjustment for age, alcohol consumption and, exercise in the smokers, for the ex-smokers compared with the nonsmokers, the odds ratio (OR) of a lower HDL cholesterol level (<40 mg/dL) was 1.29 (95% CI=1.03-1.61) in the smokers, the ORs of a higher triglyceride level were 1.35 (95% CI=1.09-1.66) in the ex-smokers and, 2.12 (95% CI=1.75-2.57) in the smokers, and the OR of a waist to hip ratio was 1.25 (95% CI=1.03-1.52) in the ex-smokers. When there were over three components of metabolic syndrome in the ex-smokers and smokers as compared with the nonsmokers, the odds ratio against the risk of metabolic syndrome were 2.39 (95% CI=1.00-6.63) and 2.37 (95% CI=1.02-6.46), respectively. Conclusions : The present study suggested that there is an association of smoking with metabolic syndrome in men.

Background: The purpose of this study was to compare the clinical safety and effect with and without additional submucosal midazolam to oral chloral hydrate and hydroxyzine when used for pediatric conscious sedation in a clinical dental environment. Methods: Thirty one cases of pediatric conscious sedations were performed in this study. Selection criteria included good health (ASA I), under 6 years old, 20 kg of body weight, uncooperative behavior and the need for sedation to receive dental treatment including anesthesia and restorative or surgical procedure for at least two teeth. In each visit, patients were randomly assigned into one of two groups; CH group: chloral hydrate (60 mg/kg), hydroxyzine (1 mg/kg), CH-M group: chloral hydrate (60 mg/kg). hydroxyzine (1 mg/kg) and submucoal midazolam (0.1 mg/kg). 50% nitrous oxide-oxygen was maintained during the sedation period Sedations were monitored using a pulse oximeter for estimating pulse rate (PR) and percutaneous oxygen saturation ($SpO_2$). Behavior response rated using Houpt's scale and need of restraint was assessed every 2 minutes through 30 minutes of operative procedure reviewing the videotape recording. Evaluation of overall behavior success was performed using modified overall behavior rate of Houpt's scale. Data was analyzed using t-test. Results: PR and $SpO_2$ for both groups remained within the normal values. The mean scores for sleep and movement of CH-M group were higher than those of CH group (P < 0.05). There were no significant difference in mean score for crying between two groups. The mean scores of overall behavior of CH-M group was higher than those of CH group (P < 0.01). Reinstraint of CH-M group was less required than that of CH group (P < 0.05). Conclusions: Oral chloral hydrate (60 mg/kg) and hydroxyzine (1 mg/kg) combined with submucosal injection of midazolam was safer and showed more improved sedation effect than oral chloral hydrate (60 mg/kg) and hydroxyzine (1 mg/kg) without midazolam for sedation of pediatric dental patients.
Thirty-six plots (each size 100m2) have been set up and surveyed to investigate the plant community structure and the ecological density of Pinus densiflora for. eracta(Chunyang-type) community in Seobyeok-ri, Chungyang-myeon, bonghwa-gun, Kyeongsangbuk-do. Four communities, Community I (large DBH class), Community II (large DBH class), Community III (middle DBH class), Community IV(small DBH class), were classified into by mean DBH and mean height. Pinus densiflora for. eracta dominate in canopy layer, Fraxinus sieboldiana and Quercus mongolica dominate in understory layer, Rhododendron Schlippenbachii and R. mucronulatum dominate in shrub layer. It turned out that thefour communiteis had low species diversity(0.4320~0.9487; unit: 400$m^2$) and high similarity. By the result of ecological density analysis. the mean basal area was proportionated to mean DBH (cm) size. By the result of simple regression analysis between mean DBH(cm), mean distance(m), and the number of individual were as follow: Ecological distance(m) = 0.0934$\times$ DBH(cm) +0.6117, Number of individual=242.47$\times$ DBH(cm)$^{-1.009}$, Ecological distance=9.643$\times$No. of individua $l^{-0.7016}$. In addition to four communities were suitable to the growth of Tricholoma matsutake because average species were about 30~50 years old, litter layer was 0.5~2.5cm and the ratio of coverage shrub was 20% .
The primary purpose of this study was to examine the variables as related to self-determination in preschoolers. In order to meet the purpose of this study preschoolers' self-determination, their playfulness, self-regulation, and self-esteem as well as goodness of fit between mother and child were taken into account as study variables of interest. Furthermore, the mediating effect of preschoolers' self-esteem in the effects of study variables on their self-esteem was examined. Three hundred thirty seven preschooler(aged 6-7 years old)-mother pairs who attended public subsidized child care facilities, located in Seoul and Gyeongi province were sampled. The data were obtained from mother-filled surveys, and on-site observations from both head teachers of the children as well as trained researchers. The obtained data were analyzed by using SPSS 21.0, and structural equation model (SEM) was tested with AMOS 21.0. The main results of this study were in the following. First, it was proven that self-determination of the preschoolers' influenced directly or indirectly on their self-esteem, their playfulness, and self-regulation, as well as goodness of mother-child fit. Also, the result pertaining to the mediational effects of child's self-esteem in the effects of study variables of interest on the child's self-determination were revealed. Along with results of this study, there is a strong need to empower the preschoolers' self-determination through improvement of their self-esteem in early childhood education and care as well as in family settings.
Purpose: The purpose of this study was to evaluate the result of anterior cruciate ligament (ACL) reconstruction using a fourstrand single semitendinous tendon to decrease the donor site morbidity due to harvest both semitendinosus and gracilis tendon. Materials and Methods: Thirty seven consecutive patients who had underwent ACL reconstruction using four-strand single semitendinosus tendon were evaluated. Mean age was 28.6 years old. Male was 34, female 3 patients. Time from injury to surgery was 5.4 months. Combined injuries were 10 meniscus injuries, 3 medial collateral ligament injuries and 1 osteochondral injury. Mean follow-up period was 16 months(12~18 months). Clinical evaluation was done using range of motion, Lachman test, pivot-shift test, Lysholm score & KT-2000 arthrometer. Results: All patients showed the normal range of motion of mean 150..at follow-up. Lachman test and pivot-shift test was negative in 35 cases. Lysholm score was improve from 84 to 92. Two cases had residual laxity due to poor compliance. Mean anterior translation compared to contralateral side by KT-2000 arthrometer improved from 6.7 mm preoperatively to 2.1 mm at follow-up. Conclusion: Reconstruction of the anterior cruciate ligament with use of a four-strand single semitendinosus tendon autograft showed good clinical results.
Background: Meralgia paresthetica(MP) which is characterized by paresthesias and sensory impairment without motor weakness in the anterolateral aspects of the thigh is produced by compression of the lateral femoral cutaneous nerve(LFCN). Even though the diagnosis of MP is mostly based on the clinical symptoms, electrophysiologic study is mandatory to confirm the disease objectively. It has been known that Somatosensory evoked potential(SSEP) study of LFCN is a simple and very useful method to evaluate MP, so we studied SSEP of LFCN in normal adults and offer normal values. Materials and Methods: Thirty six normal adults(23 males and 13 females) ages from 21 to 73 years old($mean{\pm}SD$:$42.06{\pm}15.74$) were studied SSEP of LFCN bilaterally. The stimulation site was anterolateral aspect of thighs and the recording site was Cz'. Results: The mean values($mean{\pm}SD$) of $LP_0$, $SP_0$, $LN_1$ and $SN_1$ of all subjects were 35.10(${\pm}2.42$), 33.80(${\pm}2.4$), 43.68(${\pm}1.88$) and 42.16(${\pm}2.12$) and the mean values($mean{\pm}SD$ of $DP_0$, $DN_1$ and DA(${\mu}V{\pm}SD$ were 1.30(${\pm}1.14$), 1.52(${\pm}1.38$) and 0.32(${\pm}0.33$). Conclusion: For the diagnosis of MP. comparison of latency difference between both sides is more reliable than simple value of latency itself because of individual differences of body types. According to our results. the latency difference should be less than 2 msec and the amplitude difference was less than 1.6 times in normal adults.

Objective : Sleep disorder caused by stress or disease to elderly currently. We tried to make clear constant exercise according to exercise intensity would have effect on sleep disorder in elderly using EEG and melatonin. Methods : Thirty subjects were over 65 years old who lived a senior facility in some parts. They were able to communicate and understand the purpose of the study. They also expressed their intension to participate actively in experiment. They should have alert consciousness and orientation about time, place, and people. Sleep disorder was assessed via below 6 hr total sleep time and Pittsburge Sleep Quality Index, which was satisfied with both of criterion at the same time. Exercises composed of low intensity walking, moderate aerobic exercise, and high intensity resistance strength. We used QEEG 8-System (LAXTHA Inc. KOREA) to check wave type and Polysomnograpy (Compumedics, Australia) to test quality of sleep. and Histologic features were observed by TTC (triphenyltetrazolium chloride) staining and H & E (Hematoxylin & Eosin) staining. Results : There was a significant sleep index and change of melatonin after aerobic exercise. There was a significant SOL, S1, S2, and SWS in aerobic exercise, but there was a significant SWS in walking and resistance strength. There was a significant change of delta wave especially in frontal and temporal region between exercises. Conclusion : They had different effects according to type of exercise, when elderly who have chemical, behavioral change of circadian rhythms did exercise consistently. Aerobic exercise had more effect on sleep disorder than other exercise. Therefore, we may supply proper exercise to elderly and high quality of life.

This study was conducted to select superior provenances of Scots pine (Pinus sylvestris L.) well adapted to Korean environment for timber production. In 1976, twenty-two provenances of Scots pine were introduced from Sweden and the seeds were sown in seed beds in March. After one year, the seedlings were transplanted to nursery beds. The resulting 1-1 seedlings of 22 provenances were planted at Whaseong in 1978. Randomized complete block design with 3 replications were used for test plantation. Each provenance was planted with 20-tree row plot in each block and at a spacing of $1.8{\times}1.8m$. The growth performance of each provenance was monitored up to 33-years after planting. There were significant differences among provenances in volume growth. F3001 provenance showed the best volume growth of 33-years after planting ($0.160m^3$), which was 2.2 times greater than that of the lowest provenance W2027 ($0.072m^3$). The ranking of provenances was stabilized after 14 years. Comparing to reference tree species, Japanese red pine (Pinus densiflora), all Scots pine provenances showed poor growth performance. In other words, volume growth of Japanese red pine at age 28 and 33 were 2.1 and 3.3 times greater than that of Scots pine, respectively. Moreover, survival rate of Scots pine was lower than that of Japanese red pine. Based on these results, it was suggested that Scots pine was not suitable to Korean environments. The cause of maladaptation of Scots pine and the implications of introduction breeding were discussed.
Background: Thirty children ranging from 3 to 15 years of age underwent cardiac valve replacement at Dongsan Medical Center from 1982 to 1997. Material and Method: There were 16 boys and 14 girls. The mean age was 12.1. The underlying pathological cause for valve replacement was congenital heart disease in 17 children and acquired heart disease in 13. The valve replaced was mitral in 15 children, aortic in 11, tricuspid in 3, and combined aortic and mitral in 1. Twenty-one mechanical and 10 tissue valves were placed: primary mechanical valve have been utilized since 1985. Eight of ten patients with tissue valves have had successful second valve replacements 4 to 11 years after the initial operation. Result: The operative mortality was 6.7%, but mortality was higher among patients less than 5 years of age and patients who had previous cardiac operations. Of the 28 operative survivors, 4 patients were lost to follow-up: the remaining patients were observed for a total of 2091 patient/months(mean 74.7 months, maximum 187 months). There was one late death from dilated cardiomyopathy after mitral valve replacement in 7 year-old patient with atrioventricular septal defect. After the operation, all patients with mechanical valves were placed on a strict anticoagulant regimen with Coumadin. The actuarial survival rate was 96% at the end of the follow-up. No instance of thromboembolism or major bleeding were observed in the survivors. Conclusion: These results indicate that valve replacement can be performed with low mortality in children, and with satisfactory long-term survival.

Purpose : The purpose of this study was to investigate the incidence, location, type and treatment of meniscal injury associated with acute anterior cruciate ligament (ACL) tears Material and Methods: From Mar. 1997 to Feb. 2002, we reviewed 73 cases of acute ACL tear. There were 66 males and 7 females. The average age at the time of surgery was 33 year old (range, 16-62 years). The common causes of meniscal injury were sports injuries (45 cases) and traffic accidents (19 cases). In 45 sports injuries, thirty four cases were soccer injuries. All patients had undergone arthroscopic evaluation. We analyzed incidence, type, location, and treatment of meniscal injuries, especially those of soccer injuries. Results : Foully eight (57 sites) out of 73 had meniscal tears : Of these injuries, tears of lateral meniscus (26 cases, $54.2\%$) were more common than medial meniscus. Posterior horn of the meniscus was the most common location (52 cases, $91.2\%$) and radial tears were the most common type (15 cases, $26.3\%$). Thirty eight cases $(66.7\%)$ were treated by partial menisectomy. In soccer injuries, twenty eight (33 sites) out of 34 had meniscal tears. and tears of lateral meniscus (17 cases, $60.7\%$) were more common. Posterior horn of the meniscus was the most common location (30 cases, $90.9\%$) and radial tears were the most common type (9 cases, $27.3\%$). Twenty two cases $(66.7\%)$ were treated by partial menisectomy. Conclusions : Lateral meniscal tears in the setting of an acute ACL insufficiency are more common than medial meniscal tears. The findings in the this study would be helpful in clinical diagnosis and subsequent treatment decisions in patients with an acute ACL tears.
