Determination of plasma C16-C24 globotriaosylceramide (Gb3) isoforms by tandem mass spectrometry for diagnosis of Fabry disease (패브리병(Fabry) 진단을 위한 혈장 중 Globotriaosylceramide (Gb3)의 탠덤매스 분석법 개발과 임상 응용)

  • Purpose : A simple, rapid, and highly sensitive analytical method for Gb3 in plasma was developed without labor-ex tensive pre-treatment by electrospray ionization MS/ MS (ESI-MS/MS). Measurement of globotriaosy lceramide (Gb3, ceramide trihex oside) in plasma has clinical importance for monitoring after enzyme replacement therapy in Fabry disease patients. The disease is an X-linked lipid storage disorder that results from a deficiency of the enzyme ${\alpha}$-galactosidase A (${\alpha}$-Gal A). The lack of ${\alpha}$-Gal A causes an intracellular accumulation of glycosphingolipids, mainly Gb3. Methods : Only simple 50-fold dilution of plasma is necessary for the extraction and isolation of Gb3 in plasma. Gb3 in diluted plasma was dissolved in dioxane containing C17:0 Gb3 as an internal standard. After centrifugation it was directly injected and analyzed through guard column by in combination with multiple reaction monitoring mode of ESI-MS/MS. Results : Eight isoforms of Gb3 were completely resolved from plasma matrix. C16:0 Gb3 occupied 50% of total Gb3 as a major component in plasma. Linear relationship for Gb3 isoforms w as found in the range of 0.001-1.0 ${\mu}g$/mL. The limit of detection (S/N=3) was 0.001 ${\mu}g$/mL and limit of quantification was 0.01 ${\mu}g$/mL for C16:0 Gb3 with acceptable precision and accuracy. Correlation coefficient of calibration curves for 8 Gb3 isoforms ranged from 0.9678 to 0.9982. Conclusion : This quantitative method developed could be useful for rapid and sensitive 1st line Fabry disease screening, monitoring and/or diagnostic tool for Fabry disease.

A Case Study on Recreating Simcheongjeon in Class A Literary Therapeutic View (문학치료학적 관점에서 본 <심청전> 재창작 수업사례 연구)

  • This paper describes the procedure of recreating the classics which introduced in author's lecture on "Classics and Creation." The work and result of this paper is based on the activities in the Korean language and literature lecture which the author was leading at university A in Seoul in 2014. This paper aims to identify aspects of introspection that college students underwent during the procedure of recreating. The subjects for analysis were the works created by college students participating in the class through three presentations. In section 2, it is discussed three major issues regarding the process of recreating classics. One is "presentation of the background for the selection of the work to recreate and the way of implementation". Second is "analysis of the original works and case studies on recreated works". Third is "recreation of works and meta writing". Those activities were conducted as part of the lecture. In section 3, it is investigated that the pattern of introspection of college students. It focused on analyzing the attitudes of the students toward life based on their selection, analysis, and recreation of classic works. Moreover, their self-assessment of the recreated works was examined as well. It is noted that the attitudes and tendency of students when faced with problems if any. While the students in the class selected diverse works for recreating, this paper compared and analyzed the works and introspective aspects of two students who recreated Simcheongjeon. Therefore, aspects of creativity could be identified based on the pattern of their recreation, which participants subtly describe the relationship with their father in recreated works. In accordance with the analysis, it is required to select more works for an effective class on recreation of classic literature and investigate a more systematic and diverse methodology for doing so. However, since the class for recreation of classic literature uses the narrative of the original work, it would reduce the burden on creation. In addition, it is possible to scrutinize in-depth problems of participant's general issue in life by using classic literature as a medium for introspection. Moreover, it was identified that a public presentation of these works enabled exploring self-narration on more objective aspects and collecting feedback from others.

  • This study aims to examine the usage of and satisfaction with neighborhood parks and university campuses and compare through the RPRS analysis of them to figure out how university campuses, as substitutive green spaces, play the role of curative environment for neighboring people. This study relied on two methods: a descriptive method of researching publications and related scholastic writings and an empirical study of researching how the park and campus has been utilized and researching whether the residents are satisfied with the currently existing space and the environment or not, and finally researching how the university campus open space as a curative environment impacts the satisfaction and utilization through reliability analysis. As a result, contribution of healing environment has a relatively high average contribution of 4 or more points, respectively. These results suggest that urban people think that a relationship between man and nature is very important in the overabundance of stress in modern society. It shows that neighborhoods are higher than university campuses in the explanatory of the Attention Restoration Theory, but the campus is higher than the neighborhood park in similarity of configuration. The result of RPRS analysis shows that the therapeutic qualities of the landscape may be very useful to evaluate a college campus open space in "the Revised Perceived Restoration Scale." The results showed that, in future studies, it will be a great help by widespread use of pre-evaluation of the characteristics of restorative environments in the planning, design and other works. To study the impact of the curative features on evaluating future research locations, the location should provide a variety of purposes and motives. At the same time, it should provide specific design ideas to design-related designers by using perceived restoration scale.

Impact of CYP2D6 Polymorphisms on Tamoxifen Responses of Women with Breast Cancer: A Microarray-based Study in Thailand

  • This study was designed to investigate the frequency of CYP2D6 polymorphisms and evaluate the association between genetic polymorphisms of CYP2D6 and tamoxifen therapeutic outcome in Thai breast cancer patients. We recruited 48 breast cancer patients who received adjuvant tamoxifen for evaluating CYP2D6 genetic polymorphisms using microarray-based technology. Associations between genotypes-phenotypes and disease free survival were analyzed. Median follow up time was 5.6 years. The mean age of the subjects was 50 years. The 3 common allelic frequencies were 43.8% ($^*10$), 36.5 ($^*1$) and 10.4% ($^*2$) which are related to extensive metabolizer (EM) and intermediate metabolizer (IM) with 70.8% and 29.2 %, respectively. No association between CYP2D6 genotypes and DFS was demonstrated. Nevertheless, exploratory analysis showed statistically significant shorter DFS in the IM group of post-menopause patients (HR, 6.85; 95%CI, 1.48-31.69; P=0.005). Furthermore, we observed statistically significant shorter DFS of homozygous $CYP2D6^*10$ when compared with heterozygous CYP2D6*10 and other genotypes (P=0.005). $CYP2D6^*10$ was the most common genotype in our subjects. Post-menopause patients with homozygous $CYP2D6^*10$ and IM have shorter DFS. To confirm this relationship, larger samples and comprehensively designed trials in Thailand are required.

Effects of tianeptine on symptoms of fibromyalgia via BDNF signaling in a fibromyalgia animal model

  • Previous reports have suggested that physical and psychological stresses may trigger fibromyalgia (FM). Stress is an important risk factor in the development of depression and memory impairments. Antidepressants have been used to prevent stress-induced abnormal pain sensation. Among various antidepressants, tianeptine has been reported to be able to prevent neurodegeneration due to chronic stress and reverse decreases in hippocampal volume. To assess the possible effect of tianeptine on FM symptoms, we constructed a FM animal model induced by restraint stress with intermittent cold stress. All mice underwent nociceptive assays using electronic von Frey anesthesiometer and Hargreaves equipment. To assess the relationship between tianeptine and expression levels of brain-derived neurotrophic factor (BDNF), cAMP response element-binding protein (CREB), and phosphorylated cAMP response element-binding protein (p-CREB), western blotting and immunohistochemistry analyses were performed. In behavioral analysis, nociception tests showed that pain threshold was significantly decreased in the FM group compared to that in the control group. Western blot and immunohistochemical analyses of medial prefrontal cortex (mPFC) and hippocampus showed downregulation of BDNF and p-CREB proteins in the FM group compared to the control group. However, tianeptine recovered these changes in behavioral tests and protein level. Therefore, this FM animal model might be useful for investigating mechanisms linking BDNF-CREB pathway and pain. Our results suggest that tianeptine might potentially have therapeutic efficacy for FM.

Predictive Factors for Switched EGFR-TKI Retreatment in Patients with EGFR-Mutant Non-Small Cell Lung Cancer

  • Background: Third-generation tyrosine kinase inhibitors of the epidermal growth factor receptor (EGFR-TKIs) have proved efficacious in treating non-small cell lung cancer (NSCLC) patients with acquired resistance resulting from the T790M mutation. However, since almost 50% patients with the acquired resistance do not harbor the T790M mutation, retreatment with first- or second-generation EGFR-TKIs may be a more viable therapeutic option. Here, we identified positive response predictors to retreatment, in patients who switched to a different EGFR-TKI, following initial treatment failure. Methods: This study retrospectively reviewed the medical records of 42 NSCLC patients with EGFR mutations, whose cancers had progressed following initial treatment with gefitinib or erlotinib, and who had switched to a different first-generation EGFR-TKI during subsequent retreatment. To identify high response rate predictors in the changed EGFR-TKI retreatment, we analyzed the relationship between clinical and demographic parameters, and positive clinical outcomes, following retreatment with EGFR-TKI. Results: Overall, 30 (71.4%) patients received gefitinib and 12 (28.6%) patients received erlotinib as their first EGFR-TKI treatment. Following retreatment with a different EGFR-TKI, the overall response and disease control rates were 21.4% and 64.3%, respectively. There was no significant association between their overall responses. The median progression-free survival (PFS) after retreatment was 2.0 months. However, PFS was significantly longer in patients whose time to progression was ${\geq}10months$ following initial EGFR-TKI treatment, who had a mutation of exon 19, or whose treatment interval was <90 days. Conclusion: In patients with acquired resistance to initial EGFR-TKI therapy, switched EGFR-TKI retreatment may be a salvage therapy for individuals possessing positive retreatment response predictors.

  • Introduction : One of the important domains in OT is performance skills which include sensory perceptual skills, motor and praxis skills, emotional regulation skills, cognitive skills, and communication/social skills. All of these skills are support ed by integrated neurological processes. Body : Stereology robust tool when employed to investigate morphological changes in neurons, cortex area, and specific parts of brain involved in special brain function. Stereology is an interdisciplinary field focused or analyzing biological tissue with the three-dimensional interpretation of planer sections by using estimating method and mathematically unbiased sampling. With the unbiased stereological method based on probability theory, researchers can estimate morphological and anatomical changes in biological reference areas accurately and efficiently. Changes in anatomical and cytoarchitectural parameters, such as volume, number, and length, affect specific brain function related to the brain area. Occupational therapists provide treatment to improve functions for participation of occupation in neurological disorder. The functional improvements in neurological disorder reflect neurobiological changes because functional difficulties, such as motor cognitive disorder, are due to neurological disturbances. Thus, combination of two kinds of evidence, neurological changes and functional improvement, provide fundamental evidence for OT intervention in neurological disorder. Even though most of stereological studies are in animal model and in postmortem human because of practical and ethical issues, stereology provides fundamental knowledge to support OT theory and practice. Conclusion : Therefore, stereology informs translation from neuroscience to OT based on structure-function relationship in performance skills and experience-dependent neural plasticity.

Analysis of T Lymphocyte Subpopulation Change Defined by Monoclonal Antibodies Immediately after Radiotherapy (단클론항체를 이용한 방사선치료 직후의 T 임파구아형의 변화에 관한 연구)

  • We studied the T lymphocyte and its subpopulation percentage change in 40 patients immediately after the radiation therapy. Study population consisted of 12 patients treated at the site of head and neck region,14 patients treated at the site of thoracic region, and 14 patients treated at the site of pelvic region. Twenty two patients received radiotherapy as radical modality, and remaining 18 patients received radiotherapy as postoperative modality. Immediately after radiotherapy, total T lymphocyte (T1) percentage was decreased from $56.4\%$ to $55.2\%$, helper T cell (T4) percentage was decreased from $36.4\%$ to $34.1\%$, but suppressor T lymphocyte (T8) percentage was increased from $23.5\%$ to $25.4\%$. As a result, T4/T8 ratio was decreased from 1.57 to 1.39. This study suggested that immediate change after radiotherapy of the T lymphocyte and its subpopulation percentage was not related to the treatment volume and the degree of helper T cell decrement was not pronounced by the radiation dose increment. Long-term follow-up study En larger scale is needed to determine long term changing pattern in T lymphocyte subpopulation and its relationship to the prognosis of patients.

Relationship between Eradication of Helicobacter pylori Infection and Clarithromycin Resistance (Helicobacter pylori 감염의 치료와 Clarithromycin 내성간의 연관성)

  • H. pylori strains were isolated from antral biopsies taken during upper endoscopy in 114 dyspeptic patients with no previous therapy against H. pylori. Rapid urease test, PCR amplification of SSA and cagA gene for H. pylori detection, and Western blot for CagA expression detection were performed. H. pylori infected patients were treated with omeprazole, clarithromycin (a macrolide), and amoxicillin. At 6 weeks after the discontinuation of therapy, the bacterial eradication rate was determined by endoscopy. The resistance rate to clarithromycin and amoxicillin was $20.2\%$ and $0.0\%$, respectively. The clarithromycin resistance was mainly caused by the A2142G mutation in the 23S rRNA gene of H. pylori. MICs of clarithromycin for the A2142G mutant isolates were significantly higher than MICs for the A2143G mutant isolates. H. pylori eradication was obtained in all patients with clarithromycin-susceptible isolates but not in patients with clarithromycin-resistant isolates (P = 0.0001). These results did not appear to be biased by any differences in CagA expression. The resistance of H. pylori to clarithromycin included in the therapeutic regimens is the most important reason for treatment failure. H. pylori antimicrobial susceptibility testing of the gastric biopsy culture should be performed before choosing the first triple therapy in infected patients and the increase in prevalence of clarithromycin resistance in Korea was problematic.

Correlations Between Concentrations of Negative Ions and Forest Type, and Site Factors at National Forests in Yangyang-gun (양양군 국유림 지역의 음이온 농도와 임상, 숲 관련인자와의 상관)

  • To investigate the variation of anion concentrations in relation to forest type, forest-related factors, forest survey and anion concentrations measurement were carried out on the 52 quadrats located at the national forest areas in Yangyang-gun, Kangwon-do. The relationship between anion concentrations and forest-related factors were correlated. Total mean of anion concentration was 2,405 ea/$cm^3$, which was slightly higher than general figures 2,000ea/$cm^3$. Highly significant positive correlations were observed between anion concentrations and soil moisture contents, and also between anion concentrations and altitude. Significant positive correlations were observed between anion concentrations and topography. Anion concentrations were significantly different among soil moisture levels. Mean anion concentration were highest at 4,524.4ea/$cm^3$ in on wet sites, and followed slightly wet sites and moderate sites. Mean anion concentrations was highest on valley sites at 3,372.4ea/$cm^3$, followed by slope and ridge areas. Mean anion concentrations measured before noon was highest at 3,133.4ea/$cm^3$, and measurements during 12:00~15:00 and after 15:00, showed relatively low anion concentrations. Mean anion concentrations was highest at mixed deciduous forests at 3,503.9ea/$cm^3$, followed by oak forests, pine forests, and pine-oak forests. Results of this study are expected to be used in planning therapeutic forests.
