The effects of selenium on fetal growth and development in CD-1 mice exposed with mercury for the gestation period (임신 중 수은을 섭취한 CD-1 마우스 태아의 성장발육과 기형발생에 미친 셀레늄의 효과)

  • Teratogenic and embryotoxic effects of mercury have been reported, however, there is little information about possible antidotes against mercury exposure during gestation. In order to evaluate therapeutic effects of selenium as an antidote against mercury poisoning, pregnant CD-1 mice were exposed to methylmercury chloride(20ppm) through the drinking water with treatment of sodium selenite (1.0mg, 2.0mg or 3.0mg/kg b.w., subcutaneously) or BAL(5.0mg/kg b.w., subcutaneously) under the single or combination base as the therapeutic agents from day 6 to 15 of gestation. Fetal growth parameters such as body weight and crown-rump length in the mice exposed to mercury, were reduced as was placental weight compared to those in the control. Treatment of selenium(alone, combination with BAL) reduced the harmful effects induced by mercury on the fetal growth parameters even though no specific relationship between dose and therapeutic effect. The incidence of dead fetuses/resorptions and malformed fetuses(especially cleft palate) was also increased in the mercury only treated group. Selenium treatment demonostrated reduced the incidence of abnormal fetuses under the exposure of mercury. Relative maternal organ weights(liver, kidney, spleen) were increased significantly but relative brain weight was decreased as evidenced by decreased in the mercury treated mice compared to that in the control. A subtle indication of maternal mercury toxicity evidenced by changes of relative maternal organ weights, decreased water and feed consumption were also prevented efficiently by selenium treatment. The present study suggests that methylmercuric chloride is embrytoxic and teratogenic in CD-1 mice when exposured during organogenesis and that selenium administration may have therapeutic application for the treatment of mercury poisoning although more applicable study in human should be performed with caution in the future.

Relationship between Personality Profiles by HEXACO Personality Structure Model and Depression and Anxiety Symptoms in Patient with Depression or Anxiety Disorders (성격의 6요인(HEXACO) 모형에 의한 우울 및 불안장애 환자 성격 특성과 우울 및 불안 증상 간의 관계)

  • Objective : This study used the HEXACO personality structure model to evaluate the relationship between personality profiles and symptoms in patients with depression and/or anxiety disorders. Methods : Total 123 patients with depression or anxiety disorders completed Beck depression inventory (BDI), state-trait anxiety inventory (STAI), and HEXACO-personality inventory. Correlation analysis and independent t-test were performed for comparison between HEXACO and BDI or STAI scores. Results : Depressive symptoms were negatively correlated with extraversion, conscientiousness, agreeableness and openness to experiences. Both severity of state and trait anxiety were positively correlated with emotionality, and negatively correlated with conscientiousness and agreeableness. Moreover, state anxiety was negatively correlated with honesty-humility, and trait anxiety was negatively correlated with extraversion. The depression group showed significantly higher emotionality and lower extraversion, conscientiousness, openness to experiences, and agreeableness compared to the non-depression group. In a comparison of the HEXACO domain scores between the non-anxiety and anxiety group, honesty-humility and agreeableness in the state anxiety group and extraversion, conscientiousness, and agreeableness in trait anxiety group were significantly lower. Conclusion : Theses results suggest that the depression or anxiety symptoms were highly correlated with personality profiles. Therefore, evaluating the personality profiles in these patients may be helpful for directing appropriate therapeutic plans.

The Relationship between Clinical Grading of Carpal TunnelSyndrome and Sensory nerve Conduction Velocity (수근관 증후군의 임상증상정도와 감각신경전도검사와의 관계)

  • Background: Carpal tunnel syndrome (CTS) is a common condition characterized by entrapment neuropathy of the median nerves. Clinical manifestations are the most important findings for diagnosis and assessment of therapeutic effects. But, objective indicators, such as electrophysiological findings, are also valuable supplementary tools. This study investigated the relationship between clinical grading and sensory nerve conduction velocity (SNCV) of median proper palmar digital nerve (MPPDN) in CTS patients. Method: This study was done on 90 upper limbs of 53 patients with CTS (men: 6, women: 47, age: 26~69 years, mean age; 52 years). Each SNCV of MPPDN was recorded with bar electrode using antidromic method. Each SNCV was compared with clinical grading of CTS. The clinical grades of CTS were designated as follows; group 1 is mild symptoms, 2 is moderate symptoms, and 3 is severe and longstanding symptoms. Result: In thumb, the SNCV of MPPDN was not different significantly between 3 groups (p=0.817). In the index finger, the SNCV was the fastest in the group 1, but faster in group 3 than in group 2 (p=0.001). In the middle and ring fingers, SNCV was decreased in higher clinical grading groups (middle finger: p=0.015, ring finger: p=0.044). Conclusion: SNCV of MPPDN of middle and ring finger correlated with the clinical grading of CTS. SNCV of index finger was the fastest in group 1. But SNCV of thumb did not correlate with the clinical grading of CTS.

A Study on the Acupuncture Methods of Joseon Dynasty Using Five Viscera Diagnosis (오장변증(五臟辨證)을 활용한 조선(朝鮮) 침법(鍼法) 연구(硏究))

  • Objective : The objective of this study is to verify the characteristics of acupuncture methods of Joseon Dynasty by looking into the relationship between five viscera diagnosis and acupuncture methods. Material & Method : In the process, I've reviewed the relationship between meridian/exterior and viscera/bowels, along with a thorough comparison of the academic tendency in acupuncture of Ming-China, Qing-China and Joseon. Result & Conclusion : The two fields of meridian and exterior, and viscera and bowels had been theoretically merged, and based upon that, foundation methods applying the five viscera diagnosis were designed. Joseon acupuncture exceeded the existing concept of viscera which simply related itself to the exterior meridian and exterior by integrating the concepts from the visceral manifestation theory. With this, large proportions of medicine related to the visceral manifestation theory were invited into acupuncture, expanding therapeutic boundaries for acupuncture treatment. A historical review on medical texts starts from the Hyangyakjipseongbang("鄕藥集成方") which familiarized the public with mainstream acupuncture knowledge up until the Song dynasty, followed by Uibangyuchwi("醫方類聚"), which sparked up interest on the acupuncture methods based on viscera and bowels. Donguibogam("東醫寶鑑") organized the medical theories up until then, building a foundation upon which viscera/bowel-based acupuncture was able to develop further. In Chimgugyeongheombang("鍼灸經驗方"), viscera/bowel-based acupuncture methods started to blossom, integrating the meridian and exterior theory with the viscera manifestation theory, which in turn provided various methods through five viscera diagnosis. In the Saamdoin acupuncture method(舍岩道人鍼法), diagnostic criteria moved on to the five viscera diagnosis, and new methods resulting from the inter-complimentary and inter-prohibiting relationships between the five phases were introduced, opening a new world of acupuncture.

Relationship of Job-stress, Work-family Conflict, and Mental Health according to Life-style of Married Office Worker (기혼 직장인의 라이프스타일에 따른 직무스트레스, 직장-가정 갈등 및 정신건강 간의 관계)

  • The purpose of this study was to investigate the relationship of job-stress, job-family conflict, and mental health according to lifestyles of married office worker. Data were collected by structured questionnaires form 185 married office worker by convenient sampling methods from May to August 2011. In our research outcomes, it found that job-stress was positively related to job-family conflict in the self-belief type, family-centralized type, heeding duty type and creative type. Job-family conflict was negatively related to mental health in the heeding duty type. And job-stress was positively related to metal health in the creative type and individual success pursuing type. These findings show that it is necessary to develop therapeutic psychosocial nursing intervention and to locate social-support-resources for reducing high job-stress, solving job-family conflict, and improving mental health in the married office worker.

Relationship between paravertebral muscle twitching and long-term effects of radiofrequency medial branch neurotomy

  • Background: To achieve a prolonged therapeutic effect in patients with lumbar facet joint syndrome, radiofrequency medial branch neurotomy (RF-MB) is commonly performed. The purpose of this study was to evaluate the prognostic value of paravertebral muscle twitching when performing RF-MB in patients with lumbar facet joint syndrome. Methods: We collected and analyzed data from 68 patients with confirmed facet joint syndrome. Sensory stimulation was performed at 50 Hz with a 0.5 V cut-off value. Patients were divided into 3 groups according to the twitching of the paravertebral muscle during 2 Hz motor stimulation: 'Complete', when twitching was observed at all needles; 'Partial', when twitching was present at 1 or 2 needles; and 'None', when no twitching was observed. The relationship between the long-term effects of RF-MB and paravertebral muscle twitching was analyzed. Results: The mean effect duration of RF-MB was 4.6, 5.8, and 7.0 months in the None, Partial, and Complete groups, respectively (P = 0.47). Although the mean effect duration of RF-MB did not increase significantly in proportion to the paravertebral muscle twitching, the Complete group had prolonged effect duration (> 6 months) than the None group in subgroup analysis. (P = 0.03). Conclusions: Paravertebral muscle twitching while performing lumbar RF-MB may be a reliable predictor of long-term efficacy when sensory provocation under 0.5 V is achieved. However, further investigation may be necessary for clarifying its clinical significance.

Effect of carbamazepine on tetrodotoxin-resistant Na+ channels in trigeminal ganglion neurons innervating to the dura

  • Migraine is a neurological disorder characterized by recurrent and disabling severe headaches. Although several anticonvulsant drugs that block voltagedependent $Na^+$ channels are widely used for migraine, far less is known about the therapeutic actions of carbamazepine on migraine. In the present study, therefore, we characterized the effects of carbamazepine on tetrodotoxin-resistant (TTX-R) $Na^+$ channels in acutely isolated rat dural afferent neurons, which were identified by the fluorescent dye DiI. The TTX-R $Na^+$ currents were measured in medium-sized DiIpositive neurons using the whole-cell patch clamp technique in the voltage-clamp mode. While carbamazepine had little effect on the peak amplitude of transient $Na^+$ currents, it strongly inhibited steady-state currents of transient as well as persistent $Na^+$ currents in a concentration-dependent manner. Carbamazepine had only minor effects on the voltage-activation relationship, the voltage-inactivation relationship, and the use-dependent inhibition of TTX-R $Na^+$ channels. However, carbamazepine changed the inactivation kinetics of TTX-R $Na^+$ channels, significantly accelerating the development of inactivation and delaying the recovery from inactivation. In the current-clamp mode, carbamazepine decreased the number of action potentials without changing the action potential threshold. Given that the sensitization of dural afferent neurons by inflammatory mediators triggers acute migraine headaches and that inflammatory mediators potentiate TTX-R $Na^+$ currents, the present results suggest that carbamazepine may be useful for the treatment of migraine headaches.

Phenomenological Research on Recovery Lived Experience of Stroke Inpatients (뇌졸중 입원 환자들의 회복체험에 관한 현상학적 연구)

  • The components that influence recovery were investigated to understand the recovery lived experience of stroke inpatients. Using the phenomenological research methodology reported by Giorgi, 3 conversation sessions were performed on 12 stroke inpatients. The conversations were recorded through agreement, in which the data were analyzed according to the scientific phenomenological methods. Sixteen summarized meaning units were integrated to deduct 10 main meanings and 6 themes. The themes of the restoration experience included the burden of help, performance of independent roles, self-overcoming, return to society, psychological support, and enhanced motivation for recovery. This can be used as data to predict the difficulty that stroke patients experience during hospitalization and propose a direction of intervention for restoration. Rehabilitation experts must provide intervention for the restoration of stroke patients based on the formation of a therapeutic relationship to strengthen the psychological support and motivation, and make strategies for self-overcoming and a proper cooperation relationship with their family.

Multidimensional Relationship between Auditory Verbal Hallucinations and PANSS Factors of Psychopathology in the Patients with Schizophrenia (조현병 환자의 언어성 환청과 정신병리의 PANSS 요인들 간의다차원적 관계)

  • Objectives This study was aimed to examine the multidimensional relationship between auditory verbal hallucinations (AVHs) and Positive and Negative Syndrome Scale (PANSS) factors of psychopathology in the patients with schizophrenia. And we explored the differences between assessments to hallucination by the clinicians and patients. Methods 82 patients with schizophrenia who were assessed by the Hamilton Program for Schizophrenia Voices Questionnaire (HPSVQ), Psychotic Symptom Rating Scale-Auditory Hallucination (PSYRATS-AHS), and the PANSS were recruited. Hwang's five-factor model of PANSS, items and total scores of hallucination scales, Kim's and Haddock's factor models of hallucination were applied to examine the correlations between psychopathology and AVHs. AVH-positive patients was 50 in PANSS-HPSVQ group and 24 in PANSS-PSYRATS-AHS. These two groups were separately analyzed. Results Among the five factors of the PANSS, negative and depression/anxiety factors were correlated with the total scores of HPSVQ and PSYRATS-AHS, and positive and autistic preoccupation factors were correlated only with the total score of PSYRATS-AHS. The activation factor was correlated with none of the total scores of HPSVQ/PSYRATS-AHS. These correlation patterns of a total score of HPSVQ/PSYRATS-AHS were same in the emotional factor of HPSVQ and physical factor of PSYRATS-AHS respectively. In the items which showed significant correlations, correlation coefficients of PANSS-PSYRATS-AHS group ranged between 0.406-0.755 and those of PANSS-HPSVQ ranged between 0.283-0.420. Conclusions This study suggested that the psychopathological domains of schizophrenia were differentially correlated with AVHs and the assessment of AVHs by clinicians and patients showed substantial differences which should be integrated into the therapeutic interventions.

Factors Influencing the Somatization of Elementary School Students (초등학생의 신체화 증상에 관한 영향요인)

  • Purpose: This study was conducted to investigate factors affecting the somatizationof 4th grade elementary school students in South Korea. Methods: The research used cross-sectional data of 2,031 first grade elementary school students that participated in 4th-year Korean Children and Youth Panel Survey (KCYPS). Data analysis, which included descriptive statistics, t-tests, one-way ANOVA, Pearson's correlation, and hierarchical regression,was performed using SPSS/WIN 23.0. Results: Aggression (r=0.60, p<.001) and depression (r=0.69, p<.001) showed significant positive correlation, while affection (r=-0.22, p<.001), reasonable explanation (r=-0.28, p<.001), peer relationship (r=-0.06, p=.001), and teacher relationship (r=-0.20, p<.001) were negatively correlated with somatization of elementary school students. Hierarchical regression analysis revealed that depression (${\beta}=0.52$, t=25.52, p<.001), aggression (${\beta}=0.27$, t=13.30, p<.001), reasonable explanation (${\beta}=0.05$, t=2.99, p=.002), and affection (${\beta}=0.04$, t=2.13, p=.031) were significant predictors of somatization. This regression model has an explanatory power of 52.8%. Conclusion: To reduce somatization, it is necessary to develop a therapeutic intervention to change student emotions into a positive coping mechanism and strategy. The results of this study will contribute to the development of programs and practices for healthy school life.
