프로스트 시에 나타나는 치유적 기능으로서의 불확실성 (The Uncertainty as the Healing Function in the Poetry of Robert Frost)

  • 이 연구의 목적은 프로스트 시에 나타나는 치유적 기능으로서의 불확실성을 고찰해 보는 것이다. 시치료에서 다루고 있는 치료적 기능 중 하나는 삶을 새로운 시각으로 바라보게 하는 것인데 프로스트 시는 이런 면을 불확실성으로 드러내고 있다. 프로스트의 불확실성은 그의 독특한 시론과 이원론 철학에 기초를 두고 있어 이 점을 연구하고 몇 편의 시 작품들을 통해 불확실성의 양상을 좀 더 구체적으로 분석하였다. 연구결과 프로스트는 시를 통해 우리사회가 지닌 비논리성, 애매성, 예측불가능, 그리고 일상에서의 두려움과 외로움과 불안을 드러내고 삶의 복잡한 문제를 있는 그대로 보여줌으로써 이를 극복할 수 있는 치유적 수단을 제시하고 있음을 알 수 있었다. 따라서 불확실성을 담고 있는 프로스트의 시가 정신건강 회복과 치유와 성장의 가능성을 제공한다고 볼 수 있다. 앞으로 영시의 치료적 기능에 대한 실용적인 심도 있는 연구가 필요하다.

Motor imagery on upper extremity function for persons with stroke: a systematic review and meta-analysis

  • Objective: The purpose of this review was to investigate whether motor imagery training has an effect on the recovery of upper extremity function in individuals with hemiparetic stroke or not. Design: A systematic review and meta-analysis. Methods: PubMed and three other databases were searched up to December 18th, 2018 and randomized controlled trials (RCTs) evaluating motor imagery training on upper extremity function in persons with a diagnosis of hemiparetic stroke were included. This review selected the following information from each study: surname of the first author, published year, nation, population, intervention, therapeutic intensity of intervention, therapeutic comparison, outcome measures, additional therapy, summary of results, and descriptive statistics of outcome measures. Results: This review selected seventeen RCTs with 487 stroke survivors and the following intervention methods: six motor imagery training with additional therapeutic technology, two motor imagery training with additional modified constraint-induced therapy, four mirror therapy, and five motor imagery training. Ten RCTs were eligible for meta-analysis after systematic review. The motor imagery group were more effective than the control group based on the Fugl-Meyer assessment (3.43; 95% confidence interval [CI], 1.65 to 5.22; heterogeneity [$chi^2=8.03$, df=8, $I^2=0%$], test of overall effect Z=3.76; test for subgroup differences [$chi^2=2.56$, df=2, $I^2=21.8%$]) and the Action Research Arm Test (1.32; 95% CI, -8.12 to 10.76; heterogeneity [$Tau^2=70.74$, $chi^2=15.22$, df=3, $I^2=80%$], test of overall effect Z=3.76). Conclusions: The results of this review suggests that motor imagery shows positive effectiveness on improving upper extremity function in persons with hemiparetic stroke.

재활승마에 대한 재활전문인력의 인식도 조사 (A Survey on the Recognition of Rehabilitation Specialists on Therapeutic Horse Riding)

  • 장애인을 위한 치료의 수단으로서 승마에 대한 관심이 높아지고 있는데 승마는 근육, 관절, 심폐 기능 등의 신체 조건을 향상시키고, 긴장을 완화시키며, 말과 같은 큰 동물을 통해 자신감을 키워주는 운동이다. 다양한 재활승마의 치료효과는 미국의 작업치료 및 물리치료협회 등을 포함한 많은 의학 전문가들에 의해 인정받고 있다. 반면 우리나라는 구체적인 질환별 치료적 효과나 임상적인 치료에 쓰기에 과학적 토대에 관한 연구는 아직 미비한 실정이다. 본 연구는 재활과 관련된 전문인력들의 재활승마에 대한 인식도를 조사함으로써 임상으로서의 가능성을 알아보고자 하였다. 재활승마에 대한 인지도와 사용경험은 전반적으로 낮았으나 효과성의 기대감이나 재활승마의 교육이 가정될 경우 활용하고자 하는 의사는 비교적 높았다. 이러한 결과를 토대로 향후 재활승마가 임상적 치료방법으로 인정될 가능성을 확인할 수 있다.

레크리에이션 치료 프로그램이 치매 노인들의 인지기능과 균형감각에 미치는 영향 (The Effect of Therapeutic Recreation Program on Cognitive Functions and Balance in Elderly People with Alzheimer's Dementia)

  • 본 연구는 레크리에이션 치료 프로그램이 치매 노인들의 인지기능과 균형감각에 미치는 영향을 조사 분석하고자 실시되었다. 연구대상은 알츠하이머형 치매를 진단받은 노인 16명을 대상으로 6 개월 동안 레크리에이션 치료 프로그램을 제공하였으며, 실험군과 대조군 두 그룹의 프로그램 제공전과 후의 인지기능과 균형감각의 변화를 분석하였다. 연구결과 레크리에이션 치료 프로그램은 알츠하이머형 치매 노인의 인지기능과 균형감각을 유지시키거나 증진시키는 것으로 보고되었다. 연구 대상자 모집과 손실 및 레크리에이션 치료 프로그램 기간과 같은 변수 통제의 어려움을 제한점으로 볼 수 있으나 레크리에이션 치료와 같은 비약리적 방법의 개발과 효과성 검증의 측면에서 치매 노인의 재활치료 연구에 기여하였다고 판단된다.

Butein Disrupts Hsp90's Molecular Chaperoning Function and Exhibits Anti-proliferative Effects Against Drug-resistant Cancer Cells

  • Hsp90 shows great promise as a therapeutic target due to its potential to disable multiple signaling pathways simultaneously. In this study, we discovered that a natural product, butein moderately inhibited the growth of drug-resistant cancer cells (A2780cis and H1975), and brought about the degradation of oncogenic Hsp90 client proteins. The study demonstrated that butein would be a therapeutic lead to circumvent drug-resistance in cancer chemotherapy. The structure-based screening, synthesis, and biological evaluation of butein are described herein.

  • Tumor necrosis factor-$\alpha$ (TNF-$\alpha$) exhibits cytotoxicity towards various tumor cells in vitro and induces apoptotic necrosis in transplanted tumors in vivo. It also shows severe toxicity when used systemically for the treatment of cancer patients, hampering the development of TNF-$\alpha$ as a potential anticancer drug. In order to understand the structure-function relation of TNF-$\alpha$ with respect to receptor binding, we selected four regions on the bottom of the TNF-$\alpha$ trimer that are in close contact with the receptor and carried out mutagenesis studies and computational modeling. From the study, various TNF-$\alpha$ muteins with a high therapeutic index were identified. These results will provide a structural basis for the design of highly potent TNF-$\alpha$ for therapeutic purposes. By conjugating TNF-$\alpha$ muteins with a high therapeutic index to a fusion partner, which targets a marker of angiogenesis, it could be possible to develop TNF-$\alpha$ based anticancer drugs.

Mitochondrial dysfunction and Alzheimer's disease: prospects for therapeutic intervention

  • Alzheimer's disease (AD) is a multifactorial neurodegenerative disease and has become a major socioeconomic issue in many developed countries. Currently available therapeutic agents for AD provide only symptomatic treatments, mainly because the complete mechanism of the AD pathogenesis is still unclear. Although several different hypotheses have been proposed, mitochondrial dysfunction has gathered interest because of its profound effect on brain bioenergetics and neuronal survival in the pathophysiology of AD. Various therapeutic agents targeting the mitochondrial pathways associated with AD have been developed over the past decade. Although most of these agents are still early in the clinical development process, they are used to restore mitochondrial function, which provides an alternative therapeutic strategy that is likely to slow the progression of the disease. In this mini review, we will survey the AD-related mitochondrial pathways and their small-molecule modulators that have therapeutic potential. We will focus on recently reported examples, and also overview the current challenges and future perspectives of ongoing research.

Effects of Therapeutic Climbing Training on the Balance and Gait Ability in Chronic Stroke Patients

  • Background: Therapeutic climbing training, which originated in Germany, is a wall-hanging rock climbing-based therapy to increase the body's coordination through movement of the upper and lower limbs against gravity. However, there are no studies examining the effectiveness of therapeutic climbing training to treat balance and gait ability in patients with chronic stroke. Objectives: To investigate therapeutic climbing training program on balance and gait in patients with chronic stroke. Design: Pretest-posttest control group design. Methods: Fourteen patients with chronic hemiplegic stroke participated. Participants were randomized into the therapeutic climbing training group (TCTG, n=7) and the standard rehabilitation program group (SRPG, n=7) group. All subjects participated in the same standard rehabilitation program consisting of 60 minutes 5 times a week for 6 weeks. TCTG participated additionally in the therapeutic climbing program consisting of 30 minutes sessions 3 times a week for the same 6 weeks. Berg balance scale (BBS), Gaitview Measure, Timed up and go test (TUG) were measured. Results: In the TCTG, revealed a statistical difference in BBS between the groups; in the difference of plantar pressure ratio in the static standing position revealed a statistical difference between the groups after training; the balance ability in the one-leg standing tests increased significantly; the time in TUG decreased significantly after training in both groups; The changes in the difference of dynamic plantar pressure ratio were reduced significantly in the TCTG. Conclusion: Therapeutic climbing training contribute to improve balance and walking function in patients with chronic stroke.

The Potential of Renal Progenitor Cells in Kidney Diseases: Preclinical Findings

  • The kidney is a highly complex organ, and acute or chronic renal diseases can occur with various complications such as diabetes and hypertension. So far, no target specific treatment is available in acute or chronic renal failure, necessitating the development of alternative therapeutic strategy. Recent experimental findings suggest that the renal function and structure can be restored after being treated with various sources of stem/progenitor cells. In this review, we discuss up-to-date findings of the potential of renal progenitor/stem cells in alleviating renal injuries with a focus on preclinical studies. We also review cellular mechanisms underlying the therapeutic function of these cells.

Pharmacological and medical applications of Panax ginseng and ginsenosides: a review for use in cardiovascular diseases

  • Panax ginseng, also called Asian or Korean ginseng, has long been traditionally used in Korea and China to treat various diseases. The major active ingredients of P. ginseng are ginsenosides, which have been shown to have a variety of therapeutic effects, including antioxidation, anti-inflammatory, vasorelaxation, antiallergic, antidiabetic, and anticancer. To date, approximately 40 ginsenoside components have been reported. Current research is concentrating on using a single ginseng compound, one of the ginsenosides, instead of the total ginseng compounds, to determine the mechanisms of ginseng and ginsenosides. Recent in vitro and in vivo results show that ginseng has beneficial effects on cardiac and vascular diseases through efficacy, including antioxidation, control of vasomotor function, modulation of ion channels and signal transduction, improvement of lipid profiles, adjustment of blood pressure, improvement in cardiac function, and reduction in platelet adhesion. This review aims to provide valuable information on the traditional uses of ginseng and ginsenosides, their therapeutic applications in animal models and humans, and the pharmacological action of ginseng and ginsenosides.
