  • Myositis ossificans is a solitary benign ossifying lesion in the soft tissue. Occurring predominantly in muscles, a similar lesion may occasionally be found in subcutaneous tissues, tendons, fascia and periosteum. It is often misdiagnosed as an extraskeletal osteosarcoma because of its rapid growth. Therefore, it is sometimes called pseudomalignant myositis ossificans. Myositis ossificans rarely occurs in the distal portion of the finger. We present a case of myositis ossificans of finger and review the the clinical, radiographical, and histological presentation, as well as the appropriate therapeutic management.

  • We present a case of unresectable cancer of the base of tongue treated with hypofractionated 3D conformal radiotherapy and concomitant chemotherapy. Based on the excellent tumour response in this radiotherapy regimen and international experience in short course treatments we shortly reviewed, we propose that this therapeutic approach could be considered in a curative setting for patients unsuitable for the a standard long course radiochemotherapy schedule.

  • Few recent reports have indicated that Mycobacterium massiliense causes various infections including respiratory infection. However, there is scarce information on the clinical significance, natural history of the infection, and therapeutic strategy. This report describes a case of an immunocompetent old man infected by M. massiliense that causes acute respiratory failure. In light of the general courses of non-tuberculous mycobacterium infections, rapid progression and fatality are very rare and odd. In addition, we discuss the biological and pathological properties of M. massiliense with the review of cases reported previously including our fatal one.

  • Rupture of the aorta following blunt trauma of the thorax may occur more frequently than has generally been recognized. Actual complete transection of the aorta usually results in immediate death but varying degrees of lesser disruption permit increased chance of survival. Chronic traumatic aneurysms are so designated following a period of 3 months from the traumatic incident. The exact time required for the acute process and hematoma to convert into an aneurysm is unknown. Because a thoracic surgeon encounters so few of these aneurysms, it is difficult for him to arrive at sound dicta of management from his personal experience. It is not surprising that controversy exists concerning the therapeutic approach to the aneurysm which is asymptomatic and is discovered months or years after the causative injury. In the hope of improving the surgical treatment of patients with this condition, we reported one case with chronic traumatic aneurysm of the descending thoracic aorta.

  • Chyloperitoneum or chylous ascite after trauma is a rare condition. It can develop after direct injuries of lymphatic vessels or cisterna chyli. Though isolated chyle duct injury has sometimes been reported, chyloperitoneum is generally accompanied by various kinds of damage to other intraabdominal organs. There's still no established therapeutic protocol regarding the treatment of chyloperitoneum when it is accompanied by the serious injuries of intraabdominal organs. We describe a 66-year-old male with serious intraabdominal organ injuries after blunt trauma. In our case, chyloperitoneum developed due to the injuries to the mesenteric lymph vessels and compression of cisterna chyli by hematoma around aorta.

  • Benign hyperplasia of hilar lymph glands is rare. Most of the lesions were intrathoracic cavity. The lesions were discovered most often on routine roentgenograms of the chest or because. of pressure symptoms or the presence of a palpable mass if outside the thorax. Diagnosis is. made after removal of gland, a procedure which may also have therapeutic value. They have been divided into 2 histol0gic types: the hyaline-vascular lesions, which were, most numerous, were characterized by small hyaline-vascular follicles and interfollicular capillary proliferation ;the plasma cell lesions were characterized by large follicles with! intervening sheets of plasma cells. We experienced one case of benign hyperplasia of lymph gland in left hilum, which were. most numerous, characterized by small hyaline-vascular follicles and interfollicular capillary proliferation-hyaline-vascular type. This 29 years old male patient was treated by right upper lobectomy with excision of lesion. The postoperative courses was uneventful.

  • Aconitum is a genus of various species of flowering plants that belongs to the Family Ranunculaceae. Most Aconitum sp. have extremely toxic alkaloid substances such as aconitine, mesaconitine and hypaconitine. Among these substances, aconitine can cause fatal cardiotoxicity by activating sodium channels followed by calcium channels in myocardial cells. Even though there have been various therapeutic plans suggested comprising antidotes based on diverse case reports and studies, there is no confirmatory treatment protocol for aconite poisoning. Here, we report an aconite poisoning patient who had refractory ventricular tachyarrhythmia that did not respond to intravenous amiodarone therapies even though they were sustained for over 2 hours, but showed successful recovery following intravenous fat emulsions (IFE) therapy.

  • The mortality of patients with brain abscess has decreased significaltly. This has been attributed to improved diagnostic imaging, the evolution of neurosurgical techniques and understanding of intracranial pressure pathophysiology, greater critical care understanding, and newer antibiotics. However, the mortality associated with intraventricular rupture of brain abscess remained consistently high at or above 80% once identified. A case of intraventicular rupture of thalamic abscess with good quality of survival is presented based on aggressive 4-component therapeutic plan used. The four components are 1) extraventricular drainage for 6 weeks, 2) lavage of the ventricular system using closed irrigation system, 3) intravenous antibiotics, 4) intraventricular gentamicin and vancomycin, twice and once daily, respectively.

  • Evidence based physical therapy interventions for this patient was applied in 15 clinical sessions. Considering the patient's two main impairments, two physical therapy interventions were delivered with WOMAC index outcome measurement. From this case report, I observed that a combined physical therapy interventions consisting of manual therapy, therapeutic exercise, and TENS Unit in patients with knee osteoarthritis may result in decreased pain, stiffness and increased physical function.

  • Wei symptom(?證) is symptom that reveals muscle relaxation without contraction and muscle relaxation occurs in the lower or upper limb, in severe case, leads to death. This is the clinical report about the Wei symptom(?證)-patient doubt as Transverse Myelitis and Conversion Disorder. The patient was treated by acupuncture, moxibustion, herb medication(十全大補湯), electriccal stimulation theraphy, Bee Venom acupuncture, and had significant improvement in Wei symptom(?證). these results suggest the surface temperature differ remarkably from before being treated. The temperature is measured by using Digital Infrared Thermal Imaging(DITI). The results suggest that oriental medicare is an effective treatment for Wei symptom. We expected that therapeutic value of treatment of both leg Paralysis in the oriental medicine will be higher if more clinical studies and researches are accomplished.
