Comparative study on patterns and symptoms of disharmony(病機病症) between the internal organs(藏府) and external bodily form(身形) (藏府와 身形의 病機 및 病症에 대한 比較考察)

  • All living things including human being consist of soul(spirit) and body. Soul is the root of a life and body frames it. I wrote this paper to tell how internal injury due to endogenous etiological factors and affection due to exegenous pathogenic factors, affect the internal organs and the external bodily from. This paper begins with description of the patterns of disharmony of the internal organs. General disorders of each Five-Jang(五藏) an be classified into two types of soul and body. The Liver and the Heart which lead changing to Yang(陽) of vital energy, have close relationship with spiritual symptoms because spirit is related to Yang. The Lung, Kidneys, the Spleen which lead changing to m(陰) of vital energy, mainly connected with physical disorders because body is relatively close to m. The Five-Jang are ruled by the Five-Phases(五行) system and cause troubles with Oche(五體) and the nine body orifice. Otherwise the main function of the Six-Bu(六府) is to receive food, absorb the usuable portions, and transmit and excrete waste. Therefore they can cause such problems as abdominal pain, distention, difficulty in urination, and constipation. The spleen is responsible for sending Grain-Ki)穀氣) 내 that is closely connected with the six-Bu. The Gall bladder takes care of control of giving out spirit. That's why it presents many symptoms related to the spirit that is ruled by the Five-Jang. Patterns of disharmony of external bodily form is influenced by the state of Maridians. Bodily forms get divided into many parts by the function of six-meridians(六經) to which they belong. Six-meridians have their own function related to excretion, related to excretion, retention, and balance(開闔樞). If local bodily froms get affected by pernicious influences, the Meridians to which they are attached will lose harmony and connot fulfil their own functions. Because the meridian symtem unifies all parts of the body, the whole body will be under the influence of the affection although local parts of the bodily forms get affected. Therefore the symptoms of disharmony should be considered in a synthetic view. There are however, also cases which should be focused on the very meridian channels affected. Disorder within a Meridian generates derangement in the pathway make it not to benefit the muscles and skin belong to it. Because the meridians connect the interior organs with the exterior bodily form, they are influenced by each other.

A Case Report of a Patient with Weight Loss Diagnosed with Pattern of Liver-Spleen Disharmony (간비불화증(肝脾不和證)으로 변증된 체중감소 환자 치험례)

  • Objectives: The aim of this report is to suggest that severe weight loss diagnosed with a pattern of liver-spleen disharmony can be effectively treated with Korean Medicine (acupuncture and herbal medication). Methods: This is a clinical case report of a patient with severe weight loss diagnosed with a pattern of liver-spleen disharmony. The patient was treated with Samryungbeakchul-san (Canlingbaizhu-san), Hyangsayangyi-tang (Xiangshayangwei-tang), and Haewoolhwajoong-tang (Jieyuhezhong-tang) two to three times a day and with acupuncture and moxibustion two to three times a week. Results: After the short-term treatments described above, the patient was gradually recovering from severe weight loss and many accompanying symptoms, such as diarrhea, insomnia, insufficient food intake, and others. Conclusions: The results show the possibility that severe weight loss diagnosed with a pattern of liver-spleen disharmony could be effectively treated with Korean Medicine (acupuncture and herbal medication).

Association between Emotional Labor and Levels of Depression among Health Professionals (의료기관 종사자의 감정노동과 우울수준의 관련성)

  • Objectives: We focused on investigating the effects between symptoms of depression and emotional labor among health professionals. Methods: A cross-sectional survey design was used. The participants were 365 health professionals. The survey was composed of questionnaires related to general characteristics and work-related characteristics. Korean emotional labor scale-24(K-ELS@24) and Patient Health Questionnaire(PHQ-9) data was collected from July 1, 2015 to August 31, 2015. Results: The prevalence for symptoms of depression among the survey participants was 79.2%. Organizational support(${\beta}=.387$, P<0.05), emotional disharmony(${\beta}=.238$, P<0.05), customer conflict(${\beta}=.125$, P<0.05) and emotional effort(${\beta}=.122$, P<0.05) showed a significant association with symptoms of depression. Conclusions: These results suggest that emotional disharmony, organizational support and organizational surveillance are related to symptoms of depression. Managing organizational support on the emotional labor scale is the most important factor in controlling symptoms of depression.

  • Objectives : We search the contents about Bi disease in Huang Di Nei Jing(黃帝內經), to analyze the significance of Bi disease, etiopathogenisis and pathogenesis of Bi disease, treatment of Bi disease. Methods : We find that the key feature of Bi disease is joint pain induced by impatency of Qi and blood. Exterior and interior etiological factors are involved in, such as exogenous evil of cold and dampness, emotional disorders, intemperance of taking food, dysfunction of yingqi and weiqi, strong or weak constitution, etc. Results : The important pathogenesis are invaded by exogenous evil because of deficiency, disharmony of yingqi and weiqi and disharmony of five viscera. The key points of treatments are the individual concerned therapy and climate concerned therapy, selecting the acupoint according to the differentiation of symptoms and signs. Conclusions : The combined therapy should be used such as acupuncture and moxibustion, hot application of medicine, massage, Daoyin, outside apply, etc. These supply the theory foundation for etiological factor, pathogenesis, syndrome and treatment, and to direct the diagnosis and treatment of Bi disease later generations.

An Expert Survey for Developing the Pattern Diagnosis Instrument of Acne (여드름 변증 유형 도구 개발을 위한 전문가 설문조사 연구)

  • Objective : The purpose of this study was to develop a Pattern Diagnosis Instrument of Acne in traditional Korean Medicine. Methods : Data was collected by structured survey papers from 20 professors of The Korean Oriental Medical Ophthalmology & Otolaryngology & Dermatology Society(Delphi method). We investigated importance rates of the symptoms of each Pattern in the survey papers, then calculated Weighted Value of the symptoms. Results : Nine objective symptoms and nine subjective symptoms were selected by investigated importance rate in the survey from 20 professors of The Korean Oriental Medical Ophthalmology & Otolaryngology & Dermatology Society. In Wind-Heat pattern(WHP, 風熱型), inflammatory lesion of acne was the most important symptom. Oily skin, nodule and effect of menstruation are the most important symptoms in Dampness-Heat pattern(DHP, 濕熱型), Phlegm-Stasis pattern(PSP, 痰瘀型) and Disharmony of the thoroughfare and conception vessels pattern(DTCVP, 衝任不調型) each. Conclusions : The results might provide backgrounds and methods for clinical care of acne and follow-up study.

Reliability and Validity Analysis of the Instrument of Oriental Medical Evaluation for Hwa-Byung (화병 한의 평가도구의 신뢰도, 타당도 평가)

  • Objectives: This study was performed to evaluate the reliability and validity of the instrument of oriental medical evaluation for Hwa-byung. Methods: 97 participants diagnosed with Hwa-byung divided into 5 pattern identifications by the instrument on pattern identifications for Hwa-byung were tested by the instrument of oriental medical evaluation for Hwa-byung twice a week. Results: The intraclass correlation coefficient of the test-retest analysis of oriental medical evaluation for Hwa-byung had over moderate agreement. The intraclass correlation coefficient of test-retest analysis of evaluation index of symptoms had moderate agreement for the total symptoms, and fair agreement for the physical and mental symptoms. The validity analysis of the instrument represented a positive correlation in the pattern identification of stagnation of liver Gi, malfunction of gallbladder due to phlegm stagnation, and disharmony between heart and kidneys. Conclusions: The results showed that the reliability analysis of the instrument of oriental medical evaluation for Hwa-byung showed over moderate agreement, and validity analysis represented a positive correlation except for the pattern identification of flare-up of liver fire and deficiency of both Gi and blood.

  • Review is made on the sweat in 'Donguibogam' from the view point of Hyungsang medicine. The conclusions are as follows: Clinically sweet is an important indicator for the condition and constitution of the patients. Sweat should be taken into consideration in diagnosis and treatment. Sweat is one of the symptoms that appears when there is a disharmony among Jung(精), Ki(氣) and Shin(神). The cause and phase of sweat varies with every patient. Dam type persons (膽體) are inclined to have night sweat, while Bangkwang type persons(膀胱體) are apt to have day sweat.

Relations between the Emotional Labor and Depression Symptoms of Female Workers in Cosmetics Sales - With a focus on spiritual resilience - (화장품 판매직 여성근로자의 감정노동과 우울증상과의 관련성 -정신적 회복탄력성을 중심으로-)

  • The purposes of this study were to examine the depression level of sales workers at a cosmetics company in South Korea and investigate the effects of emotional labor, job stress, and resilience on their depression symptoms. A structured questionnaire was distributed to 189 workers at the cosmetics company to assess their general characteristics, diseases diagnosed by the doctor, depression, emotional labor, job stress, and resilience. Univariate analysis and multiple logistic regression analysis was conducted to examine connections among depression symptoms, emotional labor, job stress, and resilience. The findings show that the depression symptom rate of the cosmetics sales workers was 32.3%. The multiple logistic regression analysis results of the high risk group in depression symptoms show that depression had significant relations with resilience(OR : 3.84, CI : 1.68-8.77), diseases diagnosed by the doctor(OR : 3.82, CI : 1.73-8.47), emotional disharmony(OR : 2.51, CI : 1.06-5.96), and job demand(OR : 2.12, CI : 0.93-4.85). The low risk group of emotional labor was corrected in age, doctor's diagnosis, and job stress with the reference value to analyze connections with depression symptoms according to the degree of resilience. The analysis results show that depression symptoms increased by 2.35 times(CI : 0.73-7.53) when they had high emotional labor and good resilience, by 3.74 times(CI : 1.17-11.97) when they had low emotional labor and bad resilience, and by 10.39 times(CI : 3.34-32.28) when they had high emotional labor and bad resilience. These findings raise a need to run a program to increase resilience, reduce emotional disharmony through emotional labor management, and take health management measures to control physical illness and lower job stress so that cosmetics sales workers can manage their depression symptoms.

A comparative study on pattern identification by OMS-prime of Hwa-Byung group and Hwa-Byung with Major Depression double diagnosis group (화병과 화병 ${\bullet}$ 주요우울증 중복진단군의 OMS-prime을 통한 변증유형 비교연구)

  • The objective of tills study is to identify the difference of somatic characteristics between Hwa-Byung and Major Depression by comparing the pattern identification of Hwa-Byung group and Hwa-Byung with Major Depression group(double diagnosis) Method: According to Hwa-Byung Diagnostic Interview Schedule(HBDIS) and SCID, 17 patients as diagnosed Hwa-Byung and 20 patients as diagnosed Hwa-Byung with Major Depression group(double diagnosis) were recruited. and by depression scale like Hamilton Rating Scale for Depression(HRDS) & Montgomery-Asberg Depression Rating Scale(MADRS), we excluded patients complaining moderate & severe depression among Hwa-Byung group and excluded patients showing mild depression among Hwa-Byung with Major Depression group. After this evaluation, we analysed and compared the pattern identification of both groups by OMS-prime. Result: 1. There were no significant differences of demographic data between both groups. 2. In the result of 'analysis on pattern identification' for all participant used by OMS-prime, most frequent pattern was deficiency of Yin and Yang of the heart(49%). 3. In the result of 'analysis on most correlated pattern identification' used by OMS-prime, for Hwa-Byung group was deficiency of Yin and Yang of the heart(45%) and the next were disharmony of the liver and spleen (20%), generation of phlegm due to stagnation of Gallbladder(15%) deficiency of Qi and Yin of the heart(l0%), And for Hwa-Byung with Major Depression group(double diagnosis) was deficiency of Yin and Yang of the heart(53%), the next were generation of phlegm due to stagnation of Gallbladder(18%), and deficiency of Qi and blood of the heart(l2%), 4. In the result of 'analysis on significant pattern identification' used by OMS-prime, for Hwa-Byung group was deficiency of Yin and Yang of the heart(20%) and the next were disharmony of the liver and spleen(15%), generation of phiegm due to stagnation of Gallbladder(15%), deficiency of Qi and Yin of the heart(14%), And for Hwa-Byung plus Major Depression group(double diagnosis) was deficiency of Yin and Yang of the heart(18%), the next were deficiency of Qi and Yin of the heart(18%), deficiency of Qi and blood of the heart(l0%), generation of phiegm due to stagnation of Gallbladder(18%), Conclusion: Hwabyung is syndrome that have many different symptoms, but there is no difference between Hwa-Byung group and Hwa-Byung with Major Depression group(double diagnosis) on the side of symptoms. Therefore, Hwabyung could be a new model for research on depression in Korean.
