  • Facial nerve palsy is not an uncommon disease encountered at the department of otolaryngology or pain clinic. It usually occurs following viral infection(Bell's palsy, BP). However, it is rare to develop bilaterally, and that not simultaneously. We experienced a rare case of bilateral facial nerve palsy. Patient first experienced left side paralysis, then right side paralysis approximately two months later. We treated the patient with serial bilateral stellate ganglion blocks(SGB). When left side paralysis improved, we performed unilatertal SGB for right side paralysis. Patient was also treated with intermittent electro-acupuncture stimulations, to right side first, then left side. Four months of treatment provided good results. This may be the first case, in Korea, of facial nerve paralysis bilaterally within a certain interval, treated with serial SGB and electro-acupuncture stimulation. However, the etiology of this bilateral BP was not fully confirmed as being a cause of viral origin.

  • Objectives This study was conducted to confirm anatomical information near the treatment areas of visceral chuna manual therapy and to secure stability and effectiveness during the treatment. Methods For 50 healthy adult men and women, a total of 13 ultrasound images were taken of the 7 treatment areas which are the representative treatment areas of visceral Chuna manual therapy. Results The treatment area of the bottom of the liver can be palpated around the right side ST19 and CV14. The treatment area of the gallbladder and the common bile duct can be palpated around the right side SP16 and ST20. The treatment area of the cardia and the pylorus can be palpated around the left side KI21, right side ST20, ST21, KI19, KI20, CV12, and CV13. The origin point of the mesentery root can be moved to the left and lower left from CV12 and can be palpated. The treatment area of the ileocecal valve and the cecum can be palpated around the right side SP14. The treatment area for the colic flexure can be palpated around the both side LR13. The treatment support point for the kidneys can be palpated around both side BL51. Conclusions It is thought that if the ultrasound image data from the meridians around the treatment site is used as an auxiliary, it will be more effective in terms of safety and effectiveness during the treatment of visceral Chuna manual therapy.

  • We present the case of a 60-year-old male with post-macrotrauma disc displacement and retrodiscitis, in which temporomandibular joint (TMJ) injection and manual therapy were used to alleviate his symptoms. He visited our clinic with complaints of pain and swelling in his right facial area and malocclusion of his right side teeth after being hit on the right side of his face five days earlier. During clinical and radiological examinations, the inflammatory state of the joint and disc displacement on the right side, which led to malocclusion, were noted. At the initial visit, we performed TMJ intracapsular injection and prescribed medications to control pain and inflammation. Simultaneously, manual manipulation was performed to relocate the disc. The same treatments were employed two days later. However, 10 days after the first visit, his symptoms did not mitigate substantially. We also performed magnetic resonance imaging (MRI), prescribed nortriptyline, and created a stabilization splint. MRI images depicted inflammatory disc displacement and joint effusion in the right TMJ. Based on the accurate diagnosis, we kept administering a stabilization splint, intra-articular injection, and medication. His signs and symptoms were alleviated 20 days after the initial visit and did not reoccur for the next 40 days.

  • Background: The purpose of this study was to identify the effects of cervical manipulation for improve blood velocity and flow in the subjects with asymmetric vertebral artery. Methods: Twenty-four subjects on asymmetric vertebral artery with right side have less blood flow than left side participated in this study for apply to non-specific cervical manipulation on lower portion. Measurement method were using duplex ultrasound with colour doppler imaging for blood velocity and flow on left and right vertebral artery. Results: Compared changes of blood velocity and flow on unilateral after the cervical manipulation, the left blood velocity decreased significantly (p<.01) and the right blood velocity increased significantly (p<.01). The left blood flow no significant changes and the right blood flow increased significantly (p<.01). Compared changes of blood velocity and flow on bilateral, the left and right blood velocity and flow made significantly difference on pre-manipulation (p<.01). However, both side no significantly difference on post-manipulation. Conclusions: These findings suggest that the non-specific cervical manipulation took effect for improve blood velocity and flow in the subjects with asymmetric vertebral artery. Therefore, therapeutic approaches for improve to asymmetric vertebral artery should be consider non-specific cervical manipulation.

  • This study was performed to investigate the reproductibility of eccentric mandibular movements according to preferred chewing side, range of mouth opening, type of lateral guidance and involvement of temporomandibular disorders. 50 patients with temporomandibular disorders and 65 dental students without any signs and symptoms were randomly selected for this study as the patients group and the control group, respectively. For recording and observation of eccentric mandibular movement trajectory, BioEGN$^\textregistered$ of Biopak$^\textregistered$ system (Bioresearch Inc., USA) was used. Each eccentric movement to anterior, right and left side was performed three times similar to the movement pattern for Pantronic Reproducibility Index. mandibular path was analyzed by three dimensional positional change and the three paths from one direction were compared with one another. From this, reproducibility index of one-directional lateral movement could be calculated, and total reproducibility index, named BioEGN reproducibility index(BERI), was also computed from three-directional eccentric movement likewise. BioEGN reproducibility Index could have four value of score by small or large scale, and by outgoing or incoming movement. The data were analyzed by SAS/stat program and the results obtained were as follows: 1. Right side chewing subjects showed more consistent pattern In reproducibility index in comparison between patients group and control group than left chewing subjects have done, and reproducibility was low in patients group. However, there was no difference between the two stoups in bilateral chewing subjects. 2. There were no difference in reproducibility index between preferred chewing side and contralateral side in unilateral chewing subjects whereas reproducibility index in left side on outgoing movement were higher than in right side in bilateral chewing subjects. 3. Difference in total reproducibility index(BERI) between canine guidance group and non-canine guidance group were not observed though difference in reproducibility index on lateral movement were observed in part. 4. There were no difference in reproducibility index between affected side and contralateral side in unilaterally affected patients, and between unilaterally affected patients and bilaterally affected patients in patients group. 5. Highly significant positive correlationship were shown among the four 쇼pes of total reproducibility index(BERI) in total subjects, and range of clinical mouth opening was negatively correlated with BEBI on outgoing movements and with index on outgoing movement to preferred side.

  • This paper presents experimental observation of asymmetric electrohydrodynamic flow generated around a pair of symmetric coplanar electrodes. Electrodes are attached on the bottom of the cavity containing a dielectric liquid, i.e., a mixture of dodecane and 0.5% wt Span80. In the first experiment, an AC voltage of 1500 V is applied with the frequency varying in the range 10~500 hz and the left electrode being grounded. The flow patterns show that the center line of vortices is unexpectedly tilted to the left side. If the right side electrode is grounded, the center line is tilted to the right side. The magnitude of the fluid velocity shows an irregular variation with the frequency in the range 10 Hz~100 Hz, beyond which it simply decays. In the second experiment, we applied fixed AC with 1000 V and 60 Hz superposed by DC voltage varying in the range -1000 V ~ +1000 V. The center line of the flow pattern is tilted to the right side with positive DC voltage and to the left side with negative DC. We have managed to show that the flow pattern can be symmetric with a suitable combination of DC and AC, e.g., DC 850 V plus AC 1000 V with the frequency 10 Hz.

  • In this paper, analysis results on the failure of slope behind a Plant Complex of Gimhae due to typhoon Rusa in 2002 are introduced. The left side of the slope was reinforced by soil nails and the right side of the slope was going to construct slope reinforcement works. In the slope failure, the damage area is about $34,000m^2$, the lower width of slope failure is about 230m, the upper width of slope failure is about 50m, and the height of slope failure is about 120m. The elevation of a bedrock in the right side of the slope was lower than the left side of the slope. Due to the depth of weathered soils and weathered rocks in right side of the slope was thick, it will be expected that the effects of pore-water pressure during the rainfalls are large. For the analysis of the failure mechanism, 3-dimensional numerical analysis was carried out by FLAC-3D.

  • The patient was 47-year-old male who had suffered from aphasia and hemiplegia of the right side, but mental state was alert. On physical examination, BP was 130/80 mmHg in the right arm, but not checked in the left arm. The pulses of the left common carotid, brachial, and radial arteries were not palpable. The pulses of the right femoral, popliteal, and dorsalis pedis arteries were weakly palpable. Brain CT Scan revealed cerebral infarction of the left hemisphere. Aortogram showed occlusion of the left common carotid, and the right internal carotid and common iliac arteries. Subclavian steal phenomena were observed in the delayed aortogram. Double extra-anatomic bypasses; Axillo-Axillar bypass and Femora-Femoral bypass, were performed in the local anesthesia at two stages, because of risk of major operation under general anesthesia. Postoperatively, all pulses except for pulse of the left common carotid artery were equally palpable. On discharge, the hemiplegia of the right side was improved and able to walk with assistance.

  • We could summerize the concept of the right and left mentioned in Nai-Gyung and some literature of oriental medicine as follow At first the right and left is a directional right and left which implys left-liver-right-lung(左肝右肺), left-yang-right-eum(左陽右陰), secondly it is also the road of eumyang which implys man-left-woman-right(男左女右), left-blood-right-ki(左血右氣), left-sinsu-right-myungmon(左腎水右命門), and left-right of pulse. left-liver-right-lung(左肝右肺), left-yang-right-eum(左陽右陰) and man-left-woman-right(男左女右) are expressions of the movement of yang which is a core of chang, at the same time, left-blood-right-ki(左血右氣), left-sinsu-right-myungmon(左腎水右命門), and left-right of pulse are expressions of the eum in response to the movement of yang. (go up and down of water and fire) Finally, both a directional right and left and a road of eumyang mean a road of going up and down of eumyang so, this is very important index in circulation and keeping ballance of eumyang (ki-Hyul:氣血) in the human body therefore, we can treat a disease with acupuncture in the use of this charater, for example when a disease occurs in the left side, we can treat it with acupuncture in the right side, the same applys to right.
