Two Clinical Case Reports of Ipsilateral Facial Palsy and Conjugate Gaze Palsy caused by Foville Syndrome & Foville-Millard-Gubler Syndrome (동측 안면마비와 측방주시마비를 일으키는 Foville Syndrome & Foville-Millard-Gubler Syndrome 치험 각 1례)

  • Objective : Foville syndrome & Foville-Millard-Gubler syndrome is a disease characterized by facial palsy, ipsilateral conjugate gaze palsy and ipsilateral internal strabismus. There has never been a clinical report of this disease in oriental medicine. Since gradual improvement has been experienced with oriental medical treatments based on facial palsy and paralytic strabismus in this case, it is worthwhile to introduce this disorder and report the case referring to clinical record. Methods : 75-year-old female patient and 45-year-old male patient suffering right facial palsy, right conjugate gaze palsy and right internal strabismus were treated with acupuncture, herbal medicine, pharmacopuncture and electroacupuncture. Result : Concurrence of right facial palsy, right conjugate gaze palsy and right internal strabismus was diagnosed with Foville syndrome or Foville-Millard-Gubler syndrome which is generally caused by the impairment of PPRF, abducens and facial nerves on the affected side and showed improvement with oriental medical treatments. Conclusion : More accurate diagnosis, more aggressive treatments and further researches are all required on this disease afterwards.

Intralobar Puhnonary Sequestration - Report of one case - (폐 내엽형 격절부:1례 보고)

  • We experienced a case of intralobar pulmonary sequestration preoperatively confirmed. The 10 years old male patient was admitted beacuse of recurrent episode of coughing and production of purulent sputum. the chest X-ray showed a dense mass containing a large cyst with air-fiuid level in right lower lung field. An aortogram was performed and revealed that the sequestrated portion of the RLL was supplied by an aberrant large artery arising directly from the lower thoracic aorta. After division and ligation of the aberrant artery in pulmonary ligament, the right lower lobectomy was performed and the patient`s postoperative course was uneventful.

Primary Lipoma of the Heart -A Case Report- (심장내 발생한 원발성 지방종 1례 보고)

  • Primary cardiac lipoma is an extremely rare condition. We reports a case of primary lipoma located mainly in the right atrium of the heart in a 58-year-old male patient. The initial presenting symptoms were dyspnea followed by rapidly progress!ve congestive heart failure. Echocardiogram revealed huge mass on right atrium with stalk arising from septum. Under cardiopulmonary bypass the mass was removed and revealed characteristic findings of lipoma on microscope. The patient was recovered without any problem. We would like to describe this case of rare tumor with the review of literatures.

  • A congenital agenesis of right diaphragm which is very rare diaphragmatic anomaly was experienced in a 7-year old male patient who was very well until receiving traffic accident on Nov. 1, 1977. Congenital agenesis of diaphragm have been reported only 3 cases in the world, and no one have been reported in Korea. The surgical result of this patient was successful and the review of the literature was made.

  • 'Crossed occlusion' is the condition in which occlusal intercuspation is lost when several teeth on upper and lower jaw remain. This report describes a clinical case in which a patient had two upper-left posterior teeth and two lower-right posterior teeth; typically known as left-right crossed occlusion. Considering the patient's general condition and financial situation, the treatment plan included placement of two implants on each jaw against the remaining teeth using surgical guide. To find out the ideal position of implants, digital diagnostic wax-up was preceded by superimposing the cast and cone beam computed tomography image, which was aided with radiographic stents. The consequent surveyed implant bridge provided stable vertical stop for fabrication of the implant assisted removable partial dentures. The patient was satisfied with the functionality and esthetics of definitive prosthesis.

The Effects of Therapeutic Approach of Patellofemoral Pain Syndrome with Asymmetrical Hip Rotation : Case Study (비대칭성 고관절 회전각을 지닌 슬개대퇴통증증후군 환자의 치료적 접근 - 사례연구)

  • Background: Patellofemoral pain syndrome is very common knee problem and altered hip rotation may play a role in patellofemoral pain. The purpose of this case study is to describe the manual therapy of and the therapeutic exercise for a patient with asymmetrical hip rotation and patellofemoral pain. Method: The patient was a 29 years old woman with an 3 month history of anterior right knee pain, without known trauma or injury. Prior to intervention, her score on the VAS was Max 6 to Min 4. Left hip internal rotation was less than right hip internal rotation, and manual muscle testing showed weakness of the left hip internal rotator and abductor muscles. The intervention consisted of manual therapy and therapeutic exercise for three times a weeks, two weeks for increasing right hip medial rotation, improving left hip muscle strength, and eliminating anterior right knee pain. Result: After intervention for 2weeks, passive left and right hip medial rotations were symmetrical, and her right hip internal rotator and abductor muscle grades were Good plus. Her VAS score was Max 2 to Min 0. Conclusion: Manual therapy and therapeutic exercise is effective in improving for patient had patellofemoral pain with pattern of asymmetrical hip rotation.

Delayed Tension Pneumocephalus Caused by Ventriculoperitoneal Shunt

  • The authors describe a rare case of tension pneumocephalus, caused by ventriculoperitoneal[V-P] shunting for communicating hydrocephalus. The patient had a history of a right frontal skull fracture and pneumocephalus after a traffic accident five months prior to the present presentation of gait disturbance and memory impairment. A CT scan showed hydrocephalus and a V-P shunt was put in place. On the fourth day after surgery, the mental status of the patient gradually deteriorated due to a tension pneumocephalus; this was treated by repairing a fistula in the frontal sinus and a dural defect. The patient's mental status improved and symptoms were completely recovered. We report a case of tension pneumocephalus following V-P shunt for hydrocephalus in a patient who sustained a right frontal skull fracture.

Thoracoscopic Pneumonectomy (흉강경을 이용한 전폐절제술)

  • We reported a successful right pneumonectomy using video assisted thoracoscopy for endobronchial tuberculosis. Thoracoscopic surgery has many advantages in spite of its technical difficulties. These advantages include good cosmetic effects, short hospital stay, decreased postoperative morbidity, and good postoperative pulmonary function.The patient was young unmarried female, and her right lung was nearly total obstructed by endobronchial tuberculous dissemination and secondary pulmonary infection. The procedure was a little complicated by inadequate placements of the thoracoscopic ports and 5cm length utility thoracotomy incision. Minimal pain killer was needed only two days after operation and scar of operative wound was very satisfying. Postoperative course was unusual and the patient was discharged 7 days after operation.

  • A 17 year old female patient with Ebstein`s anomaly received surgical treatment for WPW syndrome and AV nodal reentrant supraventricular tachycardia[SVT] Electrophysiologic study revealed that an anomalous pathway was located in the right posterolateral portion and antegrade dual AV nodal pathway responsible for AV nodal reentrant tachycardia. The patient was underwent surgery on February 18, 1987. Intraoperative mapping was used to define the location of accessory pathway. The accessory pathway was cryoablated through the epicardium. Simultaneously discrete cryoablation around the perinodal area was performed to prevent AV nodal reentrant SVT. The atrialized right ventricle of Ebstein`s anomaly was plicated with 11 pledget mattress sutures under the cardiopulmonary bypass. Two and half years after surgery, the patient has no evidence of WPW syndrome or supraventricular tachycardia.

Infection of Thyroid Cyst Occurring 1 Month after Fine-Needle Aspiration in an Immunocompetent Patient

  • Fine-needle aspiration (FNA) with ultrasonography is considered a minimally invasive and safe procedure. Complications of it are infrequent and occur immediately or within a few days after FNA. Such complications may occur mainly in patients with underlying problems. We here report a rare case of thyroid cystic nodule infection occurring 1 month after FNA in an immunocompetent patient and serial sonographic findings in this patient. A 33-year-old woman with a cystic nodule including partially isoechoic solid areas on the right thyroid gland complained of difficulty swallowing and painful sensations in the right neck 1 month after FNA. On follow-up examination, the cystic nodule and perithyroidal soft tissue were suspicious of infection. The possibility of infection after FNA should be considered even if the patient is immunocompetent in order to prompt evaluation and immediate management with empirical antibiotic therapy to avoid life-threatening complications.
