신종 인플루엔자 대유행에 대한 우리나라의 대응방안 (The Preparedness Plan for Influenza Pandemic)

  • Influenza A viruses periodicall y cause worldwide epidemics, or pandemics, with high rates of illness and death. A pandemic can occur at any time, with the potential to cause serious illness, death and social and economic disruption throughout the world. Historic evidence suggests that pandemics occurred three to four times per century. In the last century there were three influenza pandemics. The circumstances still exist for a new influenza virus with pandemic potential to emerge an d spread. The unpredictability of the timing of the next pandemic is underlined by the occurrence of several large outbreaks of highly pathogenic avian influenza since the early 1980s. In 1999, the World Health Organization published the Influenza pandemic plan. The role of WHO and guidelines for national and regional planning. And in 2005, WHO revised the global influenza preparedness plan for new national measures before and during pandemics. This document outlines briefly the Korean Centers for Disease Control's plan for responding to an influenza pandemic. According to the new pandemic phases of WHO, we set up the 4 national levels of preparedness and made guidelines for preventing and control the epidemics in each phase. And also we described the future plans to antiviral stockpiles and pandemic vaccine development.

쭈쭈가무시병(Tsutsugamushi disease)으로 진단된 환자를 승마갈근탕가미방, 조중익기탕 및 Doxycycline으로 치료한 치험 2예 (Two Cases of Tsutsugamushi Disease Which Were Treated Sengmagalgentang-gamibang, Chojungikitang and Doxycycline)

  • We experienced two cases of Tsutsugamushi disease which occurred in October, 2000. The patients, who were over 70 years old and living in a rural area, visited Wonkwang University Oriental Chonju Medicine Hospital because of suffering fever, chill, skin rash, abdominal discomfort, and general weakness for a duration of seven days. The diagnosis was confmned as Tsutsugamushi disease by clinical findings such as eschar and high antibody titers on Reverse Passive Hemagglutination(RPHA). As for treatment, we used Sengmagalgentang-gamibang during the acute and Chojungikitang during convalescent phases of the illness. The patients were treated with Doxycycline of western medicine. The patients improved within 3 days of admission and were discharged within one week.

암환자와 가족원의 스트레스와 대처방법 차이 (The Comparison of the Stress and Coping Methods of Cancer Patients and Their Caregivers)

  • Purpose: The purpose of this study was to identify the stress and the coping methodes in the cancer patients and their caregivers. Method: The stress method was measured by V AS(Visual Analogue Scale). The coping methodes was measured using the modified Ways of Coping Questionnaire. The phases of patient illness consisted of Ist(initial) stage, and 2nd(recurred) stage and 3rd(terminal) stage based on Lewandowski & Jones(1988) method. The data were collected by a survey of convenience sampling of 257 cancer patients and 196 of their caregivers from two hospitals in Seoul. The data were analyzed using paired t-test, unpaired t-test. Result: The stress level of cancer patients was lower than their caregivers. The cancer patients used emotion-focused coping mode than problem-focused coping mode. The caregivers problem-focused coping mode over emotion-focused coping mode. In the problem-focused coping mode, the caregivers significantly used two coping strategies that were ‘positve cope’, ‘information seeking’ more than patients. In emotion-focused coping mode, the caregivers significantly used one coping strategies that was ‘wish’ more than patients. The patients tended to used two coping strategies that were ‘blame’ and ‘emotion expression’ more than the caregivers. Conclusion: Further study needs to be done to positively identify these coping methods and develop interventions to assist patients and their caregivers.

암환아 가족의 가치관, 강인성과 적응과의 관계 (Relationships of Family Value, Vamily Hardiness and Hamily Adaptation in Family who has a Child with Cancer)

  • The purposes of the study were to develop an instrument for family value and to identify the relationships of family value, family hardiness, and family adaptation by appling the family value scale to family with cancer children. The study was conducted in three phases. 1) A survey was conducted from July 20 to August 20, 1999 and 18 items of general family value scale was modified from the data of 153 fathers and 164 mothers. 2) In-depth interviews were made with 29 parents of cancer children from April 20, 1998 to May 20, 1999 to develop family value scale with cancer children, and 12 statements were developed. 3) The final survey was conducted from July 18, 2000 to August 30, 2000 and the data from 309 parents of children who are diagnosed as cancer, 18 or less years of age, and treated either hospitalized or at the outpatient clinics were analyzed to identify the relationships of the concepts. The data analysis utilized SAS 6.12 and LISREL 8 for descriptive statistics, correlation, and Regression for path analysis. The study findings are as follows. The psychometric testing of general family value scale was Cronbach's alpha = 0.78. The reliability of the family value scale with cancer children showed the reliability as Cronbach's alpha = 0.73. Demographic characteristics showing significant correlations were cancer children's age, period of illness, period after completing treatment, mother's age, mother's education level, monthly income, payment type, confidence with health professional, and severity of children's illness. The correlation coefficients among major variables showed that family stressor was positively related with family strains(r=0.33, p<.001), and negatively related with family hardiness(r=-0.21, p<.001). Family strains was negatively related with family hardiness(r= -0.41, p<.001) and family adaptation(r=-0.46, p<.001). Correlations of family hardiness was positive with family value with cancer children(r=-0.31, p<.001), and negative with general family value(r=-0.16, p<.01). Family hardiness was positively related with family adaptation(r=0.35, p<.001). The causal relationship between study variables showed that family strains predicts general family value(γ=0.12, t=2.02), family value with cancer children predicts family hardiness(γ=0.31, t=6.30), family strains predicts family hardiness(γ=-0.40, t=-7.70), family value with cancer children predicts family adaptation(γ=-0.23, t=-4.11), and family hardiness predicts family adaptation(γ=0.43, t=7.78).

한국 성인 남녀에서 지방섭취제한 행동 변화단계에 따른 영양상태와 건강관련 행위에 대한 신념 비교 (Comparison of Nutritional Status and Beliefs on Health Behavior Regarding Stages of change in Dietary Fat Reduction among Korean Men and Women)

  • This study describes the application of the stages of change construct to fat intake by examining the associations of the stages of change with nutritional status and beliefs on health behavior. Data were obtained from apparently healthy 596 adults(326 females and 270 males) residing in large cities. Stages of change assessed by an algorithms based on 6 items were designed each subject into one of the 5 stages: precontemplation(PC), contemplation(CO), preparation(PR), action(AC) and maintenance(MA). Beliefs on health behavior were assessed by self efficacy as well as 4 belief scales from the Locus of Illness Control(LIC) developed using factor analysis such as internal disease cure and prevention and external disease cure and prevention. Energy and fat intakes were measured by a 39 item short form food frequency questionnaire. Regarding the 5 stages of change, MA stage comprised the largest group(37.9%), followed by Ac(30.7%), PC(11.4%), CO(10.4%) and PR(9.6%). Subjects who were females, older or healthier were more likely to belong to either AC or MA. Stage assignment of individuals was corroborated by their nutritional variables. Those in PC had the most energy and fat and those in MA ate the least for females. BMI was higher in PR than any other stages for both males and females. Those in PC were distinctive in that they were more externally oriented in terms of health control showing higher scores on external disease prevention(for males) and external disease cure(for females), and lower score on internal disease cure. On the other hand, those in MA received the highest scores on internal disease prevention and self efficacy, which suggested that they were more internally oriented. Canonical discriminant function analysis indicated that the 5 stages were importantly discriminated by BMI, self efficacy, internal disease prevention and external disease prevention for males and by fat intake, self efficacy and external disease cure variables for females. The results of our study confirm differences in stages of change in fat intake in terms of nutritional status and beliefs on heath behavior and indicate the need for taking these phases of change into account in nutrition advice. 마리화나 남용환자의 역동적인 발달요인과 문제해결 (Dynamic Developmental Factors and their Problem Solving of Patients that Abuse Marihuana)

  • This study was made on patients who were Hospitalized due to sickness caused by the abuse of marihuana at the Psychiatric Section of a hospital during the period from Feb. to Oct., 1974. The following conclusion was obtained by reviewing the literature with respect to the nursing, and problem solving of those patients. 1. It appears that marihuana is a comparatively mild intoxicant in ordinary preparation without causing physical dependence or tolerance. 2. According to the status of men who are marihuana smokers, approximately 20% of them were college students, those preparing to retake college entrance examinations, non-employed and pharmacists. The men belong mostly to the middle foreigner′s mistress of higher, income bracket, and in the case of women, most of the smokers were US. servicemen entertainers. 3. Dynamic developmental factors: Case 1. : The patient had a characteristic, hysterical and emotionally unitable character, and was of low intelligence, In addition, to this already existing problem, the added uses of marihuana caused a mental illness to develope. Case 2 : The character, was reserved and introspective, her creative power and sentiment was fading and his ability of self-control was weakened. She used the smoking of marihuana to get rid of her own feeling of inferiority complex and tensions coming from interpersonal relationships. Case 3 : The patient was unconditionally resistive to the authoritativeness of superiors and irresponsible in his relationship with women, in his attitude concerning sex in general. He smoked marihuana because he felt become peace-loving and get enchanted experience through smoking it. 4. The points of issue appearing from the above case; (1) Movement of anti-social feelings against the "established system" by the youngsters. (2) Family problem. (3) Shamelessness, loss of motivation, disorderly attitude toward the sex, (4) Worries concerning the future. (5) Lack of knowledge concerning smoking of cigarette and marihuana. Chronic use of marihuana made, those youngman who had originally been ambitions to achieve something in life, lazy, inefficient, unable to make long-term plans, are such weak mined persons that they did not try to overcome problems when encountered. This will pose a great and important question in the mental health of the society, 5. Treatment and Problem sieving According to the literature, we will have to place importance upon hospitalized treatment The phases of treatment were divided into five parts. (1) Prehospital phase (2) Withdrawal Phase (3) Rehabilitation phase (4) Transitional phase (5) After-care phase The experiments have proved that there was much progress in the recovery of patients through environment therapy, supportive therapy and group psychotherapy. This was the above mentioned 5 phases of treating process in accordance with the weekly schedule of the hospital. It was thought that the patients would require prolonged after care management even after they were released from the hospital and that they will also require periodic visit, to the hospital and doctor′s interview with their family. In conclusion, the question of the young generation and marihuana smoking is becoming a great social problem in which their resistances to the "established system" and society is growing in the from of antiestablishment movements. In our country, the smoking of marihuana is gradually developing, therefore, I think, that it would be a very fortunate thing for us, if this report could be helpful for the motivation of further study on the questions of the young generation and its problems.

만성 신 질환 청소년의 자기관리 경험 (Self-Management Experiences of the Adolescents with Chronic Kidney Disease)

  • Purpose: The aim of this study was to develop a substantive theory on self-management conducted by the adolescents with chronic kidney disease from their lived experience. Methods: Data was collected through in-depth interviews from May to December in 2015 with thirteen adolescents with chronic kidney disease. The data collected were analyzed on the basis of Strauss and Corbin's grounded theory. Results: The core of the category found in this study was "overcoming the unstable sense of self- control and integrating disease experience into their life". The causal conditions triggering the central phenomenon were "restriction in daily life" and "manifestation and aggravation of symptom". The central phenomenon in the experience of self-management within the adolescents with chronic kidney disease was "unstable sense of self control". The intervening condition for unstable self control were "micro system support" and "motivational resources". This study found that the adolescents with chronic kidney disease followed a series of strategies when they faced the central phenomenon, including; passive coping, reappraisal of illness, active coping, compliance with treatment, controlling physical activity, and adjusting school life. With these strategic approaches, the adolescents with chronic kidney disease could maintain their active lifestyles and achieve their health behaviors. The process of self-management by these adolescents passed through four phases; limited experience caused by diseases, effort for normalization, reorganizing their daily lives, and integration with daily lives and self-management. Conclusion: This Study explored the process and experience of self-management of adolescents with chronic kidney disease. These findings can be used for basis for developing substantive theory and nursing intervention strategy for adolescents with chronic kidney diseases.

입원한 암환자 가족원의 스트레스, 대처방법 및 신체증상간의 상관관계 (Correlation Between Stress, Coping Patterns and Physical Symptom of Cancer Patient's Caregiver)

  • The purpose of this study was to identify correlation of stress, coping patterns and physical symptoms in cancer patient's caregiver. The stress was measured by VAS(Visual Analogue Scale). The coping methods were measured using the modified Ways of Coping Questionnaire by Yang (1998) and the actual physical symptoms were investigated. The phases of patient illness consisted of 1st (initial) stage, and 2nd (recurred) stage and 3rd (terminal) stage based on literature (Lewandowski & Jones, 1988). The data were collected by a survey conducted from March to July, 2000 and which included 196 cancer patients' caregivers from two hospitals in Seoul. The data were analyzed using paired t-test, unpaired t-test, ANOVA, Scheffe test and Pearson correlation coefficient. The results were as follows: 1. The average of caregivers' stress scores was 62.5. Problem-focused coping methods were significantly used more than emotion-focused coping methods by the cancer patients' caregiver. The mean number of caregivers' physical symptom was 1.03. 2. There were significantly high level of stress in women, those who were more than 60 years old, those who had a low education level, those who had no job, those who are patients' wives' and those who are terminal patients' caregiver. There were significantly low levels of coping in women, those who were more than 60 years old, those who had low education levels, those who had no job and those who are patients' wives. There were significantly higher number of physical symptoms in women, and those who have no job. 3. Caregivers' stress was significantly correlated to problem-focused coping methods (r=-.21, p=.006), and physical symptom (r=-.28, p=.0001). In conclusion, attempts to develop nursing interventions for cancer patients' caregiver in women, those who are more than 60 years old, with a low education level, have no job, and are cancer patients' wives could have an improvement on positive coping methods and provide relaxation from stress in the patients' experience.
