Intravenous administration of mesenchymal stem cells (IV-MSC) protects the ischemic rat brain in a stroke model, but the molecular mechanism underlying its therapeutic effect is unclear. We compared genomic profiles using the mRNA microarray technique in a rodent stroke model. Rats were treated with $1{\times}10^6$ IV-MSC or saline (sham group) 2 h after transient middle cerebral artery occlusion (MCAo). mRNA microarray was conducted 72 h after MCAo using brain tissue from normal rats (normal group) and the sham and MSC groups. Predicted pathway analysis was performed in differentially expressed genes (DEGs), and functional tests and immunohistochemistry for inflammation-related proteins were performed. We identified 857 DEGs between the sham and normal groups, with the majority of them (88.7%) upregulated in sham group. Predicted pathway analysis revealed that cerebral ischemia activated 10 signaling pathways mainly related to inflammation and cell cycle. IV-MSC attenuated the numbers of dysregulated genes in cerebral ischemia (118 DEGs between the MSC and normal groups). In addition, a total of 218 transcripts were differentially expressed between the MSC and sham groups, and most of them (175/218 DEGs, 80.2%) were downregulated in the MSC group. IV-MSC reduced the number of Iba-$1^+$ cells in the peri-infarct area, reduced the overall infarct size, and improved functional deficits in MCAo rats. In conclusion, transcriptome analysis revealed that IV-MSC attenuated postischemic genomic alterations in the ischemic brain. Amelioration of dysregulated inflammation- and cell cycle-related gene expression in the host brain is one of the molecular mechanisms of IV-MSC therapy for cerebral ischemia.
Cerebral ischemia, the most prevalent form of clinical stroke, is a medical problem of the first magnitude. Substantial efforts are being made to develop drugs which will protect the brain from the neurodegeneration followed by an ischemic stroke. A key factor in this process is the development of animal models that mimic the neuropathological consequences of stroke. Recently, there is increasing an evidence that free radical is involved in the mechanisms of ischemic brain damage. We investigated the macro scale gene expression analysis on the global ischemia induced by 4-vessel occlusion in Wister rats. The recent availability of microarrays provides an attractive strategy for elaborating an unbiased molecular profile of large number of genes during ischemic injury. This experimental approach offers the potential to identify molecules or cellular pathways not previously associated with ischemia. Ischemia was induced by 4-vessel occlusion for 10 minutes and reperfused again. RNA from sham control brain and time-dependent ischemed brain were hybridized to microarrays containing 4,000 rat genes. 589 genes were found to be at least 2 fold regulated at one or more time points. These survey data provide the foundation studies that should provide convincing proof for ischemia and oxidative stress on gene expression.
Objectives: This case study reports on the effects of Korean medicine on a corticobasal syndrome with apraxia. Methods: We used acupuncture, bee-venom, pharmaco-acupuncture and herbal medicine to treat a corticobasal syndrome patient during hospitalization for 32 days. We observed changes in the UPDRS score, hand movement by opening and closing of hands, the making of a tower with 10 blocks and writing. Results: The hand movement count was improved from 2 to 10. The time needed for making a tower with 10 blocks decreased from 68 to 50 sec, and the number of stroke when writing the Korean word '철' decreased. In addition, the UPDRS score was decreased after treatment from 27 to 24. Conclusion: This clinical case study suggests that Korean medicine treatment could be used in the treatment of corticobasal syndrome.

Purpose: This study was conducted to provide background information on nursing interventions to further enhance the quality of nursing practice and related professions, based on those performed for stroke patients. Methods: The analysis was performed in light of 84 researches papers on nursing intervention published between 1990 and 2010, and based on NIC(Nursing Interventions Classification) and NOC(Nursing Outcomes Classification). Results: 1. The quasi-experimental design was used as the most primary form of research design across 69 papers that constitute 82% of the total. 2. The number of nursing intervention methods identified throughout 84 research papers was 144. Based on the NIC that 90(62.5%) of those interventions fell into the physiological basic domain while 53(36.8%) belonged to the behavioral domain. 2) Interventions on activity and exercise management, physical comfort promotion, patient education conducted by class level of NIC were 40(27.78%), 34(23.61%), and 31(21.53%) respectively. 3) Outcomes of mobility, psychological well-being, energy maintenance, health & life quality measured by class of NOC among 317 dependent variables 79(24.92%), 64(20.19%), and 63(19.87%) respectively. Conclusion: Most interventions were classified as belonging to few particular domain types, which triggers needs for the development and application of multidisciplinary intervention methods through a more collective approach.
Objective: This study conducted in order to investigate the correlations between fall experience, balance, mobility, and confidence. We examined the difference between fall experience, and Berg Balance Scale (BBS), Timed-Up-and-Go test (TUG), Tinetti balance assessment (Tinetti balance [TiB], Tinetti gait [TiG]), and Activities-specific Balance Confidence (ABC) scale scores to see how fall experience, balance, mobility, and confidence of the persons with stroke affects their balance. Design: Cross-sectional study. Methods: Forty-one subjects participated in this study. The BBS includes 14 items, consisting of a 5-point scale from 0 to 4, totaling up to 56 points. The Timed Up and Go-Alone (TUGA) was used to measure the average time to take a 3 m round-trip by getting up and down from a 46-cm high chair with an armrest on a flat floor. The Timed-Up-and-Go-Cognitive (TUGC) was performed by counting backwards and the Timed Up and Go-Manual (TUGM) is performed by holding a cup full of water. The total score for the TiB is 16 points, and the TiG is 12 points, making a total of 28 points. There are 16 items total for the ABC scale. Results: According to the fall experience, BBS, the TUGA and TUGC values were significantly higher in the inexperienced group compared to the experienced group (p<0.05). The number of falls was significantly correlated with BBS, TUGA, TUGC, TUGM, TiB, TiG, TiB+TiG (p<0.05). Conclusions: This study supports that falls experience is strongly related to balance, mobility, and confidence. Optimal balance training programs for fall prevention is still insufficient and must be developed.

Numerical computations were performed to investigate the effects of pintle stroke, altitude, and bore on the performance of an external pintle thruster. Results show that under-expansion flow occurs always, independent of pintle stroke. An external pintle thruster shows good performance in that it is capable of good amount of thrust control, while aerodynamic loads are increased due to shock waves on the pintle support. When altitude is increased to 20 km, the nozzle exit velocity, Mach number, thrust as well as aerodynamic loads are increased. Bore increases aerodynamic load 5.9%, and therefore pintle shape without bore is preferred for lower aerodynamic load of a pintle in order to actuate the pintle.

The performance of a direct-injection diesel engine often depends on the strength of swirl or squish, the shape of combustion chamber, the number of nozzle holes, etc. This is natural because the combustion in the cylinder was affected by the mixture formation process. Since the available duration to make the mixture formation of air-fuel is very short, it is difficult to make complete mixture. Therefore, an early stage of combustion is violent, which leads to the weakness of noise and vibration. In this paper, the combustion process of a common-rail diesel engine was studied by employing two kinds of pistons. One has several grooves with inclined plane on the piston crown to generate swirl during the compression and expansion strokes in the cylinder in order to improve the atomization of fuel. The other is a toroidal piston, generally used in high speed diesel engines. To take photographs of flame and flaming duration, a four-stroke diesel engine was remodeled into a two-stroke visible single cylinder engine and a high speed video camera was used.

Shipping Along with the aging of the total population, its importance is greater cause of stroke, which is one of the diseases are listed cerebral perfusion is about 80%,.ie that the flow of blood flowing to the brain is reduced as the cause. One of the most typical prophylaxis and treatment of these stroke is to enhance cerebral perfusion. However, since the method of enhancing conventional cerebral perfusion have been conducted mainly in invasive method, giving a burden to the patient, the problem of inducing a number of complications were noted. Therefore, in this paper, an attempt to supplement the shortcomings of such invasive brain perfusion increase method, to provide a device to enhance the non-invasively brain perfusion.
Purpose: We aimed to provide effective emergency medical services (EMS) response strategies for coping with high acuity patients during the pandemic by analyzing the influence of the COVID-19 and social distancing on EMS. Methods: In this retrospective observational study, we analyzed the distribution of high acuity patients transported by Daejeon 1-1-9 EMS during the COVID-19 pandemic period, between February 1, 2020 and October 31, 2020 and the same period in 2019, as well as the level 3 social distancing enforced period, between July 27, 2020 and October 31, 2020. Results: The EMS dispatches decreased by 17% during the observed COVID-19 pandemic period compared to the same period in 2019. The number of cases with cardiac arrest and positive prehospital stroke scale rose by (p<.001). Patients with cardiac arrest, trauma, and positive prehospital stroke scale increased by during the level 3 social distancing period. Conclusion: Unlike the decreased EMS call volume and patient transports during the COVID-19 pandemic, cardiac arrest cases and the severity of high acuity patients tended to increase. We suggest that EMS systems should contrive a response strategy considering the collateral effect of major epidemics on the incidence rate of high acuity patients.
A clinial observation was done on 12 cases of paralytic ileus patients, treated by Bo-Ryu Enema(保留灌腸), who were hospitalized from May 1, 1995 to October 31, 1996 at the Department of Oriented Internal Medicine II, Oriental Medicine Hospital. Taejon University. The results were as follows; 1. The ratio between male and female was 1 : 1.4. The distribution of age. 70' years or over, 60', 50' years generation were revealed in turn. 2. In classiffication of human coporeal constitution, Soeumin(少陰人) were 9 cases(75.0%), Taeumin(太陰人) 2 cases (16.7%), Soyangin(少陽人) 1 case (8.3%). 3. In distribution of disease on admission, Stroke sequela was the most number with 7 cases(58.3%), Stroke 3 cases(25.3%). Hypertensive encephalopathy and Brain tumor were 1 case, each other. 4. The effect of treated by Bo-Ryu Enema was as follows: Each of Excellent(良好) and Good(好戰) were 6 cases(50%) but. Fair(別無好戰) and Poor(惡化) were no case.
