Comparison of the Effect of Superficial Acupuncture and Deep Acupuncture on Finger Extensor Muscle Function of Normal Person : Randomized controlled trial (정상인의 지신근 근육기능에 대한 천자와 심자의 효과 비교 : 무작위 배정 임상연구)

  • Objectives : The purpose of this study is to compare the effect of superficial acupuncture to deep acupuncture on normal muscle function. Methods : In Group A, 12 healthy subjects had been treated by deep acupuncture(10mm). In Group B, 12-healthful subjects had been treated by superficial acupuncture(5mm). Each session took 10 minutes. Before and after the treatment, outcomes were assessed by Algometer for muscle tenderness, dynamometer for grasping power and Meridian-Electromyograph(MEMG) for extensor muscles. Results : The Contraction power of finger extensor muscles assessed by MEMG had decreased significantly in both groups. The muscle fatigue of finger extensor muscles assessed by MEMG had increased significantly in Deep acupuncture group. Grasping power assessed by dynamometer had no significant change in both groups. Conclusions : According to above results, superficial acupuncture is more effective on reducing muscle fatigue.

The Clinical Study of Muscle Energy Techniques in Elector Spinae Muscle through Meridian Electromyography on Subjects (일반인에서의 근에너지 기법 시술 전과 후의 척추기립근 경근전도 변화)

  • Objectives : To evaluate the clinical utility of Muscle Energy Techniques(MET) in Elector Spinae Muscle on subjects. Methods : We compared electrical activity between a before MET and a after MET in Elector Spinae Muscle on subjects in same group(n=26) in dynamic flexion-reextension state during five seconds. We analyzed amplitudes and areas of electrical activity and Asymmetry Index(AI) and Median Edge Frequency(MEF). Results : 1. After MET in Elector Spinae Muscle on subjects were lower electrical activity than before MET in Elector Spinae Muscle on subjects but it is not a pointless observation(p<0.05). 2. AI of the after MET in Elector Spinae Muscle on subjects significantly decreased compared with before MET in Elector Spinae Muscle on subjects(p<0.05). 3. MEF of the after MET in Elector Spinae Muscle on subjects decreased compared with before MET in Elector Spinae Muscle on subjects but it is not a pointless observation(p<0.05). Conclusions : According to above results, there is clinical effect MET on subjects.

Sustantial Observation on Foot Taeyang Meridian Muscle in Human Lower Limb from a Anatomical Viewpoint

  • Objective : This study was carried to identify the anatomical component of FTMM(Foot Taeyang Meridian Muscle) in human lower limb, and further to help the accurate application to real acupuncture. Methods : FTM at the surface of the lower limb was labelled with latex. And cadaver was stripped off to demonstrate muscles, nerves and the others and to display the internal structures of FTMM, being divided into outer, middle, and inner layer. Results : FTMM in human lower limb is composed of muscles, nerves, ligaments etc. The internal composition of the FTMM in human lower limb are as follows : 1) Muscle : Gluteus maximus. biceps femoris, semitendinosus, gastrocnemius, triceps calf, fibularis brevis tendon, superior peroneal retinacula, calcaneofibular ligament, inferior extensor retinaculum, abductor digiti minimi, sheath of flexor tendon at outer layer, biceps femoris, semimembranosus, plantaris, soleus, posterior tibialis, fibularis brevis, extensor digitorum brevis, flexor digiti minimi at middle layer, and for the last time semimembranosus, adductor magnus, plantaris, popliteus, posterior tibialis, flexor hallucis longus, dorsal calcaneocuboidal ligament at inner layer. 2) Nerve : Inferior cluneal nerve, posterior femoral cutaneous n., sural cutaneous n., proper plantar branch of lateral plantar n. at outer layer, sciatic nerve, common peroneal n., medial sural cutaneous n., tibial n. at middle layer, and for the last time tibial nerve, flexor hallucis longus branch of tibial n. at inner layer. Conclusions : This study proves comparative differences from already established studies from the viewpoint of constituent elements of FTMM in the lower limb, and also in the aspect of substantial assay method. We can guess that there are conceptional differences between terms (that is, nerves which control muscles of FTMM and those which pass near by FTMM) in human anatomy.

The Impacts of Muscle Energy Technique (MET) and Transcutaneous Electrical Nerve Stimulation (TENS) Performed on Elector Spinae Muscle of Acute Low Back Pain Patients on Pain and Meridian Electromyography (경근추나요법과 경피전기자극치료가 급성 요통 환자의 통증 및 척추기립근 경근전도에 미치는 영향)

  • Objectives To evaluate the clinical usefulness between muscle energy techniques (MET) and transcutaneous electrical nerve stimulator (TENS), we performed both on elector spinae muscle of acute low back pain patients. Methods After performing MET and TENS, we compared both in terms of electrical activity. We performed MET or TENS on elector spinae muscle of acute low back pain patients in each group (n=15,15). After performing MET or TENS, we analyzed root mean square (RMS), median edge frequency (MEF) and asymmetry index (AI). Results 1. After performing MET on elector spinae muscle of acute low back pain patients, RMS was significantly decreased compared with before (p<0.005). 2. After performing TENS on elector spinae muscle of acute low back pain patients, RMS was significantly decreased compared with before (p<0.005). 3. After performing MET on elector spinae muscle of acute low back pain patients, asymmetric index was significantly decreased compared with before (p<0.05). Conclusions According to above results, performing MET on elector spinae muscle of acute low back pain patients has effect in terms of RMS and asymmetric index. And performing TENS on elector spinae muscle of acute low back pain patients also has similar effect in terms of RMS but has not in terms of asymmetric index.

  • Until now, there have been no study about kinematic analysis of trunk with meridian muscular system. The purpose of this article is to contribute to the knowledge of meridian muscles of trunk that are related with breathing and basic movements. We research into respiration mechanics analysis and basic trunk movement analysis. And we suggest that the studies of the therapy that apply meridian muscles in trunk and respiration abnormality are more necessary.

The Effect of Using Massage Chair with Acupoint and Meridian Muscle Massage Program on Back Pain: A Single Arm Pre-post Comparison Study (경혈 및 경근 안마 프로그램이 적용된 안마의자 사용이 요통에 미치는 영향)

  • Objectives : The purpose of this study is to investigate the effect of acupoint and meridian muscle massage given by automatic massage chair. Methods : We recruited 30 patients with back pain who met inclusion criteria. Two weeks were given as a waiting period, and the massage intervention was applied 12 times for 4 weeks. The programmed automatic massage chair was used to provide effective stimulus on acupoints and meridians for 20 minutes. We used Visual analog scale(VAS) and Korean Oswestry Disability Index(KODI) to quantify pain and function. We also recorded patients' all of vital signs and abnormal reactions during the trial to asses to assess safety. Results : Two participants dropped out due to personal reasons and 28 participants completed the clinical trial. We found that the degree of back pain (VAS, KODI) decreased significantly after the trial. As a result, VAS score decreased from $6.44{\pm}0.85$(Visit 2) to $4.59{\pm}1.52$ (Visit 14)(p<0.001), and KODI score was also reduced from $14.79{\pm}5.13$(Visit 14) to $9.04{\pm}4.52$ (Visit 2) (p<0.001). Also, we did not find any adverse effects in this study. Conclusions : We found that the acupoint and meridian muscle massage can effectively decrease back pain. This proves that the massage program designed by Korean medicine can be a good option to patients with back pain. However, further study with controlled design is still needed to verify its effectiveness.

The Experimental Study of the Effects of Continuous Traction Therapy in Meridian Sinews Therapy (경근 치료방법 중 지속적 견인요법의 효과에 관한 실험적 연구)

  • Objectives : Meridian sinew theory was introduced in Miraculous Pivot, Huangdi's Internal Classic, to explain in relation with locations of meridian sinews, causes, mechanisms, and treatment of diseases. The meridian sinews are understood to include muscles, tendons and ligaments, or muscles in the superficial body made up with muscles, ligaments, tendons, fascia etc. This theory shows the similarity or organic relationship between the meridian sinews and muscles. From the Hippocrates(460-385 BC) ages, traction therapy was used as a treatment method on muscular diseases such as low back pain, scoliosis, etc in western medicine. The effects of traction therapy, however, were unclear so that this study was purposed to illustrate the effectiveness of continuous traction therapy and to develop meridian sinews treatment. Methods : We made 2 hypotheses to explain the cause of scoliosis occurrence, muscles contraction and relaxation. As the hypothesis, we made the spinal model having 3 joints with wood and rubber bands. Each of the three joints in the spinal model represents the case of normal(NT; control), contraction(AT 1)and relaxation(AT 2) condition, and distance between the vertebrae joints was measured. Results : Under normal circumstance models, the normal type 1(NT 1; muscle relax state) and normal type 2(NT 2; muscle contract state) all joints were being towed equally. But in an unusual contracted situation, regardless of the relationship of joint area, contracted part of joint was not released. And in a relaxed situation, regardless of joint areas, released parts of joint were further released. These observation results mean that the effects of traction might be different from the purpose of traction therapy of Hippocrates. Conclusions : To explain the effect of traction therapy for scoliosis, the spinal cord model and scoliosis model were made. After vertebral bodies were pulled with different tensile forces, we compared the observed length of the each joints pulled. The results suggested that there were no effects of traction in objected parts with traction method from Hippocrates' design, continuous traction method. Moreover, it may worsen the symptom in worst case. Of course, our results are just the result of experimental models and clinical results may be different. More careful studies, therefore, are required.

Analysis of Osteopathic Manipulation and Study on Relationship with Chuna Manual Therapy for Meridian Sinew System (정골의학적(Osteopathic) 수기요법 분석 및 경근추나와의 관련성 연구)

  • Objectives : The aim of this study is to analyze the actual investigation and classification of osteopathic manipulation by investigation of the various literature of the inside and outside of the country, and to present the application plan of osteopathy in chuna manual therapy for meridian sinew system. Methods : I referred to the domestic and foreign books about osteopathy and chiropractic. In order to investigate domestic dissertations, I searched 4 Korean medical databases and 4 Korean medical journals of osteopathy. Search terms used were osteopathy, osteopathic, fascia, proprioreceptor, mechanoreceptor, muscle spindle, golgi tendon organ, osteopathic manipulation technics. And I classified all the searched studies into principle and region and etc. In order to investigate foreign dissertations, I search 'NCBI pubmed'. Search terms used were osteopathy, osteopathic technique, osteopathic manipulative technique. Results : 1. Osteopathy do not regard the systems which compose the human body in individual territory, but regard whole. It is diagnosis, prevention and medicine which treats 2. Osteopathic manipulation techniques are classified into direct techniques, indirect techniques, and compound techniques. 3. Osteopathic manipulation techniques are classified into fascia, muscle, ligament-joint in applied region. 4. I could search clinical cases in domestic and foreign study. I found cases about myofascial release technique(MFR), postisometric relaxation(PIR), proprioceptive neuromuscular fascilitation(PNF), muscle energy technique(MET), joint mobilization in domestic studies, and strain-counterstrain technique(SCS), MET, AK in foreign studies. Conclusions : Osteopathic manipulation techniques can be used in diagnosis and treatment of meridian muscle theory, because osteopathy and the oriental medicine have many similarities in theoretical background. So osteopathic manipulation technique can be useful in oriental medicine treatment techniques.

A study on the interrelation between the twelve-Meridian Muscle and Muscles (십이경근(十二經筋)과 근육(筋肉)과의 관계(關係)에 대한 연구)

  • The Oriental Medicine is described with so many terms of its own theory causing misunderstand of the concept which is expressed with same term used in modern medicine. Muscular system is also used in the both medicines, the Oriental Medicine and modern medicine. For the purpose of resolving the misunderstand of using the medical terms, we referred to a large number of literature for the muscular system in both medicine. Although there are few references concerning about systematic components in the Oriental Medicine, among the concepts of the Oriental Medicine, there are comparatively many approaches to the Meridian muscular system, a muscular system related with the Meridian, Therefore, there are many similarities and differences in the interrelation between the muscular system which was stated at the time of the concept of the Meridian system was developed and anatomical muscular system in the modern medicine. As a result of survey the references, we found out that anatomical muscular system is limited to visual compartments, whereas the Meridian muscular system is covering not only visual components but also the relation with internal organs. We conclude that there are conceptual differences in the kyung-keun system in the past and anatomical muscular system in the present.
