Outcomes of Carotid Endarterectomy according to the Anesthetic Method: General versus Regional Anesthesia

  • Background: The surgical strategies for carotid endarterectomy (CEA) vary in terms of the anesthesia method, neurological monitoring, shunt usage, and closure technique, and no gold-standard procedure has been established yet. We aimed to analyze the feasibility and benefits of CEA under regional anesthesia (RA) and CEA under general anesthesia (GA). Methods: Between June 2012 and December 2017, 65 patients who had undergone CEA were enrolled, and their medical records were prospectively collected and retrospectively reviewed. A total of 35 patients underwent CEA under RA with cervical plexus block, whereas 30 patients underwent CEA under GA. In the RA group, a carotid shunt was selectively used for patients who exhibited negative results on the awake test. In contrast, such a shunt was used for all patients in the GA group. Results: There were no cases of postoperative stroke, cardiovascular events, or mortality. Nerve injuries were noted in 4 patients (3 in the RA group and 1 in the GA group), but they fully recovered prior to discharge. Operative time and clamp time were shorter in the RA group than in the GA group (119.29±27.71 min vs. 161.43±20.79 min, p<0.001; 30.57±6.80 min vs. 51.77±13.38 min, p<0.001, respectively). The hospital stay was shorter in the RA group than in the GA group (14.6±5.05 days vs. 18.97±8.92 days, p=0.022). None of the patients experienced a stroke or restenosis during the 27.23±20.3-month follow-up period. Conclusion: RA with a reliable awake test reduces shunt use and decreases the clamp and operative times of CEA, eventually resulting in a reduced length of hospital stay.

Short-term Outcomes of Pylorus-Preserving Gastrectomy for Early Gastric Cancer: Comparison Between Extracorporeal and Intracorporeal Gastrogastrostomy

  • Purpose: This study aimed to compare the surgical and oncological outcomes between totally laparoscopic pylorus-preserving gastrectomy (TLPPG) with intracorporeal anastomosis and laparoscopy-assisted pylorus-preserving gastrectomy (LAPPG) with extracorporeal anastomosis. Materials and Methods: A retrospective analysis was performed in 258 patients with cT1N0 gastric cancer who underwent laparoscopic pylorus-preserving gastrectomy using two different anastomosis methods: TLPPG with intracorporeal anastomosis (n=88) and LAPPG with extracorporeal anastomosis (n=170). The following variables were compared between the two groups to assess the postoperative surgical and oncological outcomes: proximal and distal margins, number of resected lymph nodes (LNs) in total and in LN station 6, operation time, postoperative hospital stay, and postoperative morbidity including delayed gastric emptying (DGE). Results: The average length of the proximal margin was similar between the TLPPG and LAPPG groups (2.35 vs. 2.73 cm, P=0.070). Although the distal margin was significantly shorter in the TLPPG group than in the LAPPG group (3.15 vs. 4.08 cm, P=0.001), no proximal or distal resection margin-positive cases were reported in either group. The average number of resected LN was similar in both groups (36.0 vs. 33.98, P=0.229; LN station 6, 5.72 vs. 5.33, P=0.399). The operation time was shorter in the TLPPG group than in the LAPPG (200.17 vs. 220.80 minutes, P=0.001). No significant differences were observed between the two groups in terms of postoperative hospital stay (9.38 vs. 10.10 days, P=0.426) and surgical complication rate (19.3% vs. 22.9%), including DGE (8.0% vs. 11.8%, P=0.343). Conclusions: The oncological safety and postoperative complications of TLPPG with intracorporeal anastomosis are similar to those of LAPPG with extracorporeal anastomosis.

Effect of vitamin D deficiency in Korean patients with acute respiratory distress syndrome

  • Background/Aims: Vitamin D modulates innate and adaptive immune responses, and vitamin D deficiency is associated with increased mortality in hospitalized patients with pneumonia. We evaluated the prevalence of vitamin D deficiency in Korean patients with acute respiratory distress syndrome (ARDS) and its effect on the clinical outcomes of ARDS. Methods: We retrospectively analyzed the data of 108 patients who had a measured serum level of 25-hydroxy vitamin D3 ($25(OH)D_3$) at the time of diagnosis with ARDS. The clinical outcomes were evaluated based on $25(OH)D_3$ levels of 20 ng/mL and stratified by quartiles of $25(OH)D_3$ levels. Results: The mean age of patients was 59.4 years old; 77 (71.3%) were male. Vitamin D deficiency was found in 103 patients (95.4%). The mean $25(OH)D_3$ level was $8.3{\pm}7.0ng/mL$. Neither in-hospital mortality (40.0% vs. 68.0%) nor 6-month mortality (40.0% vs. 71.8%) significantly differed between groups. There were no significant differences in $25(OH)D_3$ level between survivors ($8.1{\pm}7.6ng/mL$) and non-survivors ($8.5{\pm}6.8ng/mL$, p = 0.765). There were no trends toward a difference in mortality among quartiles of $25(OH)D_3$ levels. However, $25(OH)D_3$ levels were inversely related with length of hospital stay and intensive care unit stay among in-hospital survivors. Conclusions: Vitamin D deficiency was prevalent in Korean patients with ARDS. However, levels of vitamin D were not associated with mortality. A large, prospective study is needed to evaluate the effects of vitamin D deficiency on clinical outcomes of ARDS.

A Multilevel Analysis about the Impact of Patient's Willingness for Discharge on Successful Discharge from Long-term Care Hospitals (퇴원 의지가 요양병원의 성공적 퇴원에 미치는 영향에 대한 다수준 분석)

  • Background: Since November 2019, long-term care hospitals have been able to provide patients with discharging programs to support the elderly in the community. This study aimed to identify both patient- and hospital-level factors that affect successful community discharge from long-term care hospitals. Methods: A multilevel logistic regression model was performed using hospitals as a clustering unit. The dependent variable was whether a patient stayed in the community for at least 30 days after discharge from a long-term care hospital. As for the patient-level independent variables, an agreement between a patient and the family about discharge, length of hospital stay, patient category, and residence at discharge were included. The number of beds and the ratio of long-stay patients were selected for the hospital-level factors. The sample size was 1,428 patients enrolled in the discharging program from November 2019 to December 2020. Results: The number of patients who were discharged to the community and stayed at least for 30 days was 532 (37.3%). The intraclass correlation coefficient was 22.9%, indicating that hospital-level factors had a significant impact on successful community discharge. The odds ratio (OR) of successful community discharge increased by 1.842 times when the patients and their families agreed on discharge. The ORs also increased by 3.020 or 2.681 times, respectively when the patients planned to discharge to their own house or their child's house compared to those who didn't have a plan for residence at discharge. The ORs increased by 1.922 or 2.250 times when the hospitals were owned by corporate or private property compared to publicly owned hospitals. The ORs decreased by 0.602 or 0.520 times when the hospital was sized over 400 beds or located in small and medium-sized cities compared to less than 200 bedded hospitals or located in metropolitan cities. Conclusion: The results of the study showed that the patients' and their family's willingness for discharge had a great impact on successful community discharge and the hospital-level factors played a significant role in it. Therefore, it is important to acknowledge and support long-term care hospitals to involve active in the patient discharge planning process.

  • DK 754S, a forage corn hybrid is bred by Dekalb plant Genetics Co.. Garst 8255 by Garst Seeds Co., and GW 737 by Crosbyton Co. in US, respectively. These have been tested in Suwon and Chonan for 3 years to test their regional yield trial. In result, we were able to confirm the excellence of above cultivars and these were selected as new recommended cultivars April of 2002 by NACF. The characteristics of these cultivars are as follows ; 1. DK 7545 a. This cultivar is mid-late maturing, high quality, high yielding forage corn hybrid. Its mean tasseling date is the 11th of July. It takes 78 days from emergence to tasseling date, which is almost same with Kwanganok. The culm length and the ear height are 231 cm and 127 cm. It is relatively strong to lodging. b. The color of ear is yellow, the number of ear row is nineteen. DK 7545 shows strength to H maydis and MBSDV has occurred between 0% and 4.1% in Chonan test area but it still does not affect yielding at all. It is relatively resistant to corn borer and its stay green is almost same level comparing to a check hybrid, Kwanganok. c. Dry matter yield of this cultivar is 19.6 tons, TDN is 13.4 tons per a ha, therefore these hybrids are increased by from 6 to 9% comparing to Kwanganok. The percent ear among total dry matters yield is 43.7%. 2. Garst 8255 a. This hybrid is mid-late maturing, high quality, high yielding forage corn like as Kwanganok. Its the mean tasseling date and the period from emergence to tasseling date are almost same with a check hybrid, Kwanganok. The Culm length and the ear height are 259 cm, and 146 cm which are tall. It resists to lodging and shows excellence of stay green. b. Garst 8285 resists to H maydis, and MBSDV, and corn borer as much as Kwanganok has. c. Dry matter yield of this cultivar is 21,735 kg. TDN is 14,627 kg per a ha, therefore this is increased by 21%, 16%, respectively, comparing to a check hybrid, Kwanganok. The percent ear among total dry matters yield is 44.2%. 3. GW 737 a. This hybrid is mid-late maturing, high quality, high yielding forage com hybrid like as Kwanganok. Its mean tasseling date is 13th of July which is 2 days late comparing with a check hybrid, Kwanganok and the period from emergence to tasseling date is 80 days. b. The culm length and the ear height are 274 cm and 150 cm, which are tall. It resists to lodging moderately and shows excellence of stay green. c. GW 737 resists to H maydis and MBSDV very well, corn borer moderately resistance. d. Dry matter yield of this cultivar is 18,025kg, TDN 15,164kg per a ha, therefore this hybrid remarkably increased by 26%, 20%, respectively, comparing to a check hybrid, Kwanganok.

Feeding Habits of Pacific Cod (Gadus macrocephalus) in the Coastal Waters off Gadeok-do, Korea (가덕도 주변해역에 출현하는 대구(Gadus macrocephalus)의 식성)

  • The feeding habits of Pacific cod (Gadus macrocephalus) were studied based on the examination of stomach contents of 192 specimens collected from November, 2005 to January, 2006 in the coastal waters off Gadeok-do, Korea. The size of Pacific cod ranged from 35 to 82 cm in standard length (SL). Pacific cod mainly consumed shrimps such as Eualus spathulirostris, Crogon hakodatei and C. affinis and fishes. Its diet also included small quantities of cephalopods, amphipods, hermit crabs and crabs. Individuals between 35 cm and 45 cm SL mainly consumed shrimps. The portion of shrimps decreased with increasing fish size, and this decrease was paralleled with increased consumption of fishes and cephalopods. In this study we found Syngnathus schlegeli in the stomach contents of many Pacific cod specimens. S. schlegeli is a typical seagrass fish species which inhabits in seagrass beds, This fact means that Pacific cods stay in the seagrass beds during spawning period. Therefore migration of Pacific cod to coastal waters off Gadeok-do during spawning period seems to be a survival strategy for its larvae and small juveniles to stay in seagrass beds which provide with abundant foods and shelters to many commercial fish species.

Influence of Living Abroad on People in General and Their Clothing Style: A Case Study of Koreans (해외체류경험이 소비자의 일반적 특성과 의복 스타일에 미치는 영향: 한국인의 경우)

  • As globalization has been accelerating in recent years, more people have opportunities to live abroad for an extended period of time, which is known to have strong influence on consumers. The main purposes of this study were to examine if and how sojourning experience could bring changes to people in general and in clothing styles with focus on Korean adults. The researchers conducted in-depth interviews on a total of thirty-six women and twenty-one men who lived in foreign countries more than two years and have been back in Korea longer than one year. The major results indicated that in addition to personalities, age, length of stay, activities during stay, gender, and cultural distances played important roles in bringing long-term changes on people. Re-established self-identity, changes in values, and openness toward diversity were major common changes observed in the interviewees. In addition, they became educated consumers with good knowledge of diverse brands and prices, and emphasized value as opposed to price. Changes in clothing styles were noted mainly among women. They placed high importance on having their own style, coordination and appropriateness, which were consistent with the general changes. Influences of different cultures were also noted.

Nonoperative Management of Blunt Liver Trauma (둔상성 간 손상환자의 비수술적 치료)

  • Background: The management of hepatic injuries has changed dramatically during the past two decade after the technologic breakthroughs in radiologic imaging techniques. Recently, the non-operative management of blunt hepatic trauma has become the standard of care in hemodynamically stable patients. We reviewed our experience of the non-operative management of blunt hepatic trauma. And the purpose of this study was to examine the prognostic factors and indicators affecting the decision for treatment modality of emergent hepatic trauma. Methods: The medical records of 84 patients who were treated for blunt hepatic injury at Masan Samsung Hospital from January 2002 to December 2003. The patients were divided two groups, non-operative(Non-OP) and operative(OP), according to the treatment modality. The two groups were compares for age, sex, mechanism of injury, grade of liver injury scale, combined injury, systolic blood pressure, pulse rate, hemoglobin, hematocrit, WBC count, S-GOT, S-GPT, ALP, transfusion amount during initial 24 hours, amount of infused crystalloid fluid, length of ICU stay, length of ward care, morbidity and mortality. The grade of the liver injury were determined by using the organ injury scale(OSI). Results: Among the 84 patients, 46 cases(54.8%) were managed non-surgically, and 3 cases of Non-OP group were treated by transarterial embolization. Between the two groups, there were significant difference in age, injury grade, combined injury, hemoglobin, hematocrit, initial systolic blood pressure, amount of infused crystalloid fluid, amount of transfusion during the first 24 hours, and length of ICU care, morbidity and mortality.(p<0.05) The overall mortality rate was 8.3%, but 2.2% mortality in the non-operative group. Conclusion: Non-operative management may be considered as a first choice in hemodynamic stable patients with blunt liver trauma. The reliable indicators affecting the treatment modality of blunt hepatic trauma were systolic BP, Hb, Hct, amount of infused crystalloid fluid, amount of transfusion during the first 24 hours, liver injury grade and combined injury. Strict selection of treatment madality and aggresive monitoring with intensive care unit were more important.

Trial of a Synthetic Absorbable Staple Line Reinforcement for Preventing Recurrence after Performing Video-assisted Thoracoscopic Bullectomy for the Treatment of Primary Pneumothorax (일차성 기흉 치료에서 흉강경 기포 절제술 시 재발 방지를 위한 Staple Line 보강재 사용)

  • Background: Postoperative recurrence is a major problem after performing video-assisted thoracic surgery for the treatment of primary pneumothorax. This study was designed to evaluate the efficacy and safety of a bioabsorbable staple line reinforcement (GORE $SEAMGUARD^{(R)}$) for preventing recurrence after bullectomy. Material and Method: From January 2000 to December 2004, 300 patients underwent video assisted thoracoscopic surgery for the treatment of primary penumothorax. 143 patients were treated with bioabsorbable staple line reinforcement (Group A) and 142 patients were treated with stapling of the bullae (Group B). Mechanical pleural abrasion was performed in all the patients. The operating time, the duration of the indwelling chest tube, the length of the hospital stay and the number of recurrences after operation were compared between the groups. Result: No operative deaths occurred. The conversion rate to an open procedure was 5% (15/300). Comparison with these groups (Group A versus Group B) showed the following results: the operating time ($49.6{\pm}25.6$ vs $51.8{\pm}30.4$ minutes, respectively, p=0.514), the duration of an indwelling chest tube ($5.8{\pm}2.5$ vs $7.2{\pm}3.3$ days, respectively, p<0.005), the hospital stay ($10.9{\pm}4.3$ vs $12.5{\pm}4.3$ days, respectively, p<0.005) and the number of recurrences (14 (9.8%) vs 10 (7.0%), respectively, the over all rate: 8.4% p=0.523), and the mean follow up period. ($48.1{\pm}36.6$ vs $36.5{\pm}24.4$ months, respectively). Conclusion: There were advantages to use synthetic absorbable staple line reinforcement over the usual method for the treatment of primary pneumothorax in regard to the duration of an indwelling chest tube and the hospital stay, but here was no significant difference between the groups for postoperative recurrence.

Critical Pathway for Operable Gastric Cancer (위암수술 환자에서의 Critical Pathway의 개발과 적용)

  • Purpose: Critical pathways (CP), also known as clinical pathways, are management plans that display goals for patients and have led to improved outcomes for many disease entities. This study was aimed at developing a critical pathway for the surgical treatment of gastric cancer patients and evaluating its usefulness. Materials and Methods: A CP was developed and implemented by a team of surgeons, nurses, nutritionists, and administrative officials. Among the 117 patients who received curative gastrectomies for gastric cancer at Kangnam St. Mary's Hospital, The Catholic University of Korea, between October 2003 and August 2004, 26 patients were treated according to the CP. We evaluated its usefulness by comparing the clinical characterisctics, postoperative progress, hospital stays, and costs between the CP and the non-CP groups. Patient satisfaction was also surveyed with questionnaires. Results: Of the initial 26 patients in the CP group, two were excluded from the final evaluation; one patient had a duodenal stump leakage, and the other had a gastric stasis postoperatively. In 8 patients, protocol violation occurred; six patients refused to be discharged on the $7^{th}$ postoperative day, one patient who had an gastric staisis postoperatively stayed for 2 additional days, and one patient who needed ICU care stayed for 4 additional days. The drop-out rate was $7.7\%$ (2/26), and the variance rate was $30.8\%$ (8/26). The mean hospital stay was 11.3 days ($10\~15$ days) for the CP group compared with 17.5 days ($9\∼68$ days) for the non-CP group, resulting in a difference of about 6 days (P<0.05). The mean hospital stays after surgery were 10.3 days ($7\∼68$ days) and 8.3 days ($7\∼12$ days) for the non-CP and the CP groups, respectively, but the difference was statistically not significant (P>0.05). The mean charge during the hospital stay was higher in the non-CP group ( $\\$ 6,292,200) than in the CP group ( $\\$ 4,863,685). The charge per hospital day was higher in the CP group ( $\\$ 430,414) than in the non-CP group ( $\\$ 359,554). Patient satisfaction was higher in the CP group than in the non-CP group. Conclusion: By developing and applying a critical pathway in the surgical treatment of stomach cancer patients, we could reduce the length of hospital stay as well as the cost. A multi-centered prospective study to establish a standard treatment pathway and to demonstrate its effectiveness is needed in the future.
