A Study on the Effects of Chewing Side Preference on the Pattern of Occlusal Contacts (저작습관에 따른 교합접촉의 변화양태에 관한 연구)

  • The purpose of his study was to investigate the variations of occlusal contact pattern according to chewing side preference. The author selected 59 dental students (mean age 23.6 years) who had no signs and symptoms of masticatory disorders and divided into two groups, that is, bilateral chewing side group and unilateral chewing side group, respectively. For recording, T-Scan System(Teksan Inc., USA) was used and the recorded occlusal contacts were examined as to the number of occlusal contacts, points distribution in dental arch, time sequence and force snapshot. The obtained results were as follow s: 1. Total number of occlusal contacts were more in unilateral chewing side group and there were significant difference in number between right and left side or between chewing and less-chewing side in all two groups. 2. All items related to time sequence showed no statistical significant difference between two groups in any case. 3. Unilateral chewing side group had more occlusal contact force than bilateral group, especially in chewing side. From the above finding, the author considered that there was occlusal unbalance in unilateral chewing side group. 4. Chewing side preference would possibly have more effects on the occurrence rate of anterior tooth contact that the rate of prolonged their contact but proved.

TREATMENT OF PRIMARY TUBERCULOUS OSTEOMYELITIS OF THE MANDBLE : A CASE REPORT (하악골에 발생한 원발성 결핵성 골수염의 치험례)

  • This is an extermely rare disease and a case of tuberculous osteomyelitis of mandible in a 45-year-old woman was reported. The patient was visited for complaining slight facial swelling on left side and purulent discharge from extracted socket. Roentgenograhic examination of the mandible revealed bone destruction on left side of mandibular body. Destructive lesion was removed with saucerization and specimen from the involved soft tissue and bone were sent for microscopic examination. This case proved to be primary tuberculous osteomyelitis caused by the absence of primary focus. After 3 months follow-up check, we noticed good prognosis of bone and soft tissue healing.

Lateral Medullary Syndrome Caused by Prone Position for Spine Surgery

  • We report a rare case of Wallenberg's lateral medullary syndrome caused by prone position for spine surgery. A 48-year old man developed Wallenberg's syndrome characterized by involuntary myoclonic movements, ataxia on his left side, hyperalgia and cold sensation on his right side after prone position for general anesthesia for the spinal stenosis L3-L4, L4-L5. Brain computed tomography scan was immediately performed and showed negative findings, but magnetic resonance image [MRI] demonstrated brain infarction on the left medulla. Emergent heparinization was performed and his motor power and sensation returned to normal and discharged with stable and satisfactory recovery after 16 days.

Numerical analysis of flow and settling efficiency in a sedimentation basin (수치모의를 통한 침사지에서의 흐름 및 침사효율 해석)

  • This paper has assessed the flow patterns and settling efficiency in the sedimentation basin using the particle tracking method of the CFD code and has reached the following conclusions: In the original design where no baffle is installed in the sedimentation basin, a large recirculating area where the flow stagnates is created in the right side of the sedimentation basin, with most of the particles moving to the left side of the sedimentation basin following the flow. This biased flow structure in the sedimentation basin reduces the residence time of particles and thereby undermines settling efficiency. The biased flow toward the left side of the sedimentation basin is alleviated by installing a baffle in the sedimentation basin, promptly reducing the fast flow of over 0.7 m/s in the inlet of the sedimentation basin to the rate below 0.2 m/s. In this paper's simulation conditions, if a one-sided baffle is to be installed in the sedimentation basin, placing it 15 meters away from the basin's inlet leads to the best settling efficiency; it has also been analyzed that installing a two-sided baffle-rather than a one-sided one-is a better option in terms of settling efficiency. The highest settling efficiency of 96.2% is achieved when the underwater length of the two-sided baffle is set at 8 meters.

The Impact of Cervical Stretching Exercise and Cervical Traction on Cervical Pain and Muscle Activity among Patients with Cervical Hypolordosis

  • The purpose of this study is to provide fundamental clinical data for the treatment plan and rehabilitation of patients with cervical hypolordosis by comparing the cervical headache and muscle activity after cervical stretching exercise and cervical traction, which are generally applied to patients with cervical hypolordosis. The research subjects included 20 patients without gender division who were diagnosed with cervical hypolordosis. After applying cervical stretching exercise and cervical traction for six weeks, cervical headache and the activity of the muscles around the cervical vertebra(upper trapezius muscle, sternocleidomastoid muscle, splenius capitis muscle, and anterior temporal muscle) were investigated and the following results were obtained. In a comparison of the within group intervention effects of the two groups, cervical pain statistically significantly decreased in the cervical stretching exercise group. According to the results of analyzing the change of muscle tension in the upper trapezius muscle, both the cervical traction group and showed a statistically significant within group difference in the left and right side. According to the results of analyzing the change in the muscle tension of the splenius capitis muscle, both groups showed a statistically significant within group difference in the left and right side. In a between-group comparison, a statistically significant difference in the right side was observed. These results confirm that cervical vertebra traction and cervical stretching exercise decrease the cervical headache and muscle activity of the upper trapezius muscle and the splenius capitis muscle among patients with cervical hypolordosis.

The Strength of the Lower Trapezius in Violinists With Unilateral Neck Pain

  • Violinists tend to position the neck asymmetrically to hold the violin between the chin and the left shoulder. Asymmetrical neck posture may induce unilateral neck pain. Previous studies have suggested that individuals with unilateral neck pain exhibit reduced muscle strength of the lower trapezius, but no study has investigated violinists with unilateral neck pain. To this end, we recruited 18 violinists with unilateral neck pain for the present study in which the side on which neck pain was experienced, pain duration, and intensity were recorded. Lower trapezius strength was measured bilaterally in each subject using a handheld dynamometer. Significant differences in lower trapezius strength were evident between the ipsilateral and contralateral sides of neck pain (p<.05). No significant association between neck pain intensity or duration, and the extent of a deficit in lower trapezius strength, was evident (both p>.05). The association between the sides of weakened lower trapezius strength and neck pain was significant (p<.05). In conclusion, violinists with unilateral neck pain exhibited significantly less lower trapezius strength on the ipsilateral compared to the contralateral side of the pain. Unilateral neck pain more frequently involved the left side of the neck, which is used to stabilize the violin during playing. Thus, our study suggests that a possible relationship exists between muscle weakness in the lower trapezius and neck pain.

Visibility of the mandibular canal and the mental foramen in panoramic radiography (파노라마 방사선사진에서 하악관과 이공의 관찰되는 정도)

  • Purpose : To determine the head position that the superior border of the mandibular canal as well as mental foramen can be more clearly visualized in panoramic radiography Materials and Methods : Ten dry mandibles were radiographed bilaterally using PM 2002 CC panoramic machine. A 20 mm thick aluminium filter was added to the slit collimator to obtain radiographs with acceptable density. The specimens were tilted by 2, 4, 6, 8, and 10 degrees downward with and without radiopaque markers. Radiopaque markers were inserted into the mandibular canals and the mental canals of each side of the specimens to serve as reference image when assessing the radiographs. The visibility of the mandibular canal and the mental foramen was estimated by 4 observers on all radiographs. The obtained results were analyzed statistically. Results: Mandibular canals were significantly more clearly visible in the radiographs with 4 and 6 degree downward position on both sides (P<0.05). Mental foramens were significantly more clearly visible in the radiographs with 4, 6, and 8 degree downward on right side and 6 degree downward on left side (P<0.05). There was not significant difference between right and left sides. Conclusion: Panoramic radiographs with 4 to 6 degree downward tilting could be valuable in locating the mandibular canal as well as the mental foramen.

A Feature of Producing the Symptom Complex Related to Pain and Discomfort by the Experimental Isometric Unilateral Canine Biting in Adult Females within Normal Masticatory Function (정상 성인 여성에 있어 실험적 견치부 등척성편측교합에 의한 동통성 증상 발현 현상)

  • In order to evaluate the effect of unilateral tooth clenching, the author observed a feature of producing the symptom complex related to pain and discomfort by the isometric unilateral canine biting under force of 5kg(FS) in Korean 31 adult females within normal masticatory function who were divided into the bilateral, the right and the left group according to their habitual sides of mastication, and analyzed the observed data statistically. The obtained results were as follows: 1. The duration of the isometric unilateral canine biting maintained before the occurrence of symptom complex related to pain and discomfort 66.72 seconds at right side and 39.50 seconds at left side in a group of subjects with bilateral habitual mastication. And the difference of the duration between of unilateral biting side was almost significant (P<0.05). 2. The most frequent region of occurrence of symptom complex related to pain and discomfort by the isometric unilateral canine biting was the contralateral superficial masseter muscle in a group of subjects with bilateral habitual mastication.

Differences in Electric Potential of Meridian System -Comparing Electrical Potentials of Patients with Facial Hemiparalysis- (구안와사 환자의 12경맥 전위측정 연구)

  • Objective : Assuming that the characteristic of meridian system has been similar to this of electric potentials in human body and that measurements of electric potential at well(井穴) and sea (合穴) points in branches of the twelve meridians(WSBTM) will be representative of measurements of the twelve meridians, to measure the electric potentials of 13 patients with Facial Hemiparalysis (구안와사<口眼蝸斜>, FH), to find out the characteristic of meridian system in patients with AK. Methods : Electric potentials of well and sea points in the meridians in twenty one patients with the pain in the lion diagnosed as FH were repeatedly measured by physiograph(PowerLab). Measurements of those electrical potentials were analyzed by factor analysis. Results : The electric potentials of WSBTM at the left side were divided into five factors. On the other hand those at the right side Were divided into four factors. Conclusions : In conclusion, their electrical potentials at the left and right side were each other five and four factors. Thus electrical potentials of well and sea points might be the representative meridian to show their characteristics.

The Clinical Observation on 47 Cases of Patients with Blepharospasm Treated by Wisunghanggyeok(胃勝寒格) (위승한격(胃勝寒格)을 시술한 안검경련환자 47례에 대한 임상적 고찰)

  • Objectives : This study was performed to observe 47 cases of patients with blepharospasm treated with Wisunghanggyeok(胃勝寒格). Methods : For 28 months from Sep. 2006 to Dec. 2008, we researched 47 blepharospasm patients treated with Wisunghanggyeok(胃勝寒格) who visited the author's clinic. Results & Conclusion: 1. It shows more female patients than male patients, and mostly patients in their fifties. 2. The lesion is shown more on the right side than on the left side, and the left side blepharospasm is cured more effective. 3. The causes of hemifacial spasm are mainly stress and overwork, and there is a 85% improvement of 39 cases from these two causes. 4. The disease period is mainly within 1 week, and that period is the most effective time. 5. The number of treatment is mainly under 5 times, and the effect of treatment over 4 times for week is more available. 6. There are 59.57% of excellent improvement and 23.4% of good improvement, and these make 82.97% improvement of 47cases.
